1Incidence and characteristics of HBV reactivation in hematological malignant patients in south Egypt显示文摘AIM:To investigate characteristics of hepatitis B virus(HBV)implicated in HBV reactivation in patients with hematological malignancies receiving immunosuppressive therapy.METHODS:Serum samples were collected from 53 patients with hematological malignancies negative for hepatitis B surface antigen(HBsAg)before the start of and throughout the chemotherapy course.HBV reactivation was diagnosed when the HBsAg status changed from negative to positive after the initiation of chemotherapy and/or when HBV DNA was detected by realtime detection polymerase chain reaction(RTD-PCR).For detecting the serological markers of HBV infection,HBsAg as well as antibodies to the core antigen(antiHBc)and to the surface antigen were measured in the sera by CEIA.Nucleic acids were extracted from sera,and HBV DNA sequences spanning the S gene were amplified by RTD-PCR.The extracted DNA was further subjected to PCR to amplify the complete genome as well as the specific genomic sequences bearing the enhancerⅡ/core promoter/pre-core/core regions(nt1628-2364).Amplicons were sequenced directly.RESULTS:Thirty-five(66%)of the 53 HBsAg-negative patients were found to be negative serologically for antiHBc,and the remaining 18(34%)patients were positive for anti-HBc.Five of the 53(9.4%)patients with hematologic malignancies experienced HBV reactivation.Genotype D1 was detected in all five patients.Four types of mutant strains were detected in the S gene product of HBV strains and were isolated from 3 patients with HBV reactivation:T/S120,L143,and I126.HBV DNA was detected in the pretreatment HBsAg-negative samples in one of the five patients with HBV reactivation.In this patient,sequences encompassing the HBV full genome obtained from sera before the start of chemotherapy and at the time of de novo HBV hepatitis were detected and it showed 100%homology.Furthermore,in the phylogenetic tree,the sequences were clustered together,thereby indicating that this patient developed reactivation from an occult HBV infection.CONCLUSION:Past infection with HBV is a risk factor for HBV reactivation in Egypt.Mandatory anti-HBc screening prior to chemotherapy in patients with hematological malignancies is recommended.Abeer Elkady Sahar Aboulfotuh Elsayed Mostafa Ali Douaa Sayed Nashwa M Abdel-Aziz Amany M Ali Shuko Murakami Sayuki Iijima Yasuhito Tanaka 2013World Journal of Gastroenterology2013,19,37:7
2Characteristics of escape mutations from occult hepatitis B virus infected patients with hematological malignancies in South Egypt显示文摘AIM To investigate the prevalence and virological characteristics of occult hepatitis B virus(HBV) infections in patients with hematological malignancies in South Egypt.METHODS Serum samples were collected from 165 patients with hematological malignancies to monitor titers of HBV DNA, hepatitis B surface antigen(HBs Ag), and antibodies to HBV core(anti-HBc) and surface antigens. Serum samples negative for HBs Ag and positive for anti-HBc were subjected to nucleic acid extraction and HBV DNA detection by real-time polymerase chain reaction. DNA sequences spanning the S region were analyzed in cases with occult HBV infection. In vitro comparative study of constructed 1.24-fold wild type and S protein mutant HBV genotype D clones was further performed. RESULTS HBV DNA was detected in 23(42.6%) of 54 patients with hematological malignancies who were HBsA g negative, but anti-HBc positive, suggesting the presence of occult HBV infection. The complete HBV genome was retrieved from 6 occult HBV patients, and P120 T and S143 L were detected in 3 and 2 cases, respectively. Site directed mutagenesis was done to produce 1.24-fold genotype D clones with amino acid mutations T120 and L143. The in vitro analyses revealed that a lower level of extracellular HBsA g was detected by chemiluminescence enzyme immunoassay(CLEIA) with the clone containing T120 mutation, compared with the wild type or the clone with S143 L mutation despite the similar levels of extracellular and intracellular HBs Ag detected by Western blot. Southern blot experiments showed that the levels of intracellular HBV DNA were not different between these clones. 9Immunomodulatory effect of garlic oil extract on Schistosoma mansoni infected mice显示文摘Objective:To assess the effect potency,and the immunomodulatory response of garlic oil extract in enhancing the host's immune system against the disorders caused by Schistosoma mamoni(S.mamoni) in mice at different stages of worm maturation.Methods:A total of 70 male CD-I Swiss albino mice were divided into 7 groups.Group Ⅰ:healthy control.Group Ⅱ:garlic oil group orally administrating 100 mg garlic oil extract /kg b.wt.3 d a week for 6 weeks.Group Ⅲ:infected with S.inansoni cercariae and left untreated for 42 d.Group Ⅳ:treated with garlic oil extract from day 1 to day 7 post infection(PI).Group Ⅴ:treated with garlic oil extract from day 14 till day 21 PI.Group Ⅵ:administrating garlic oil extract from day 35 until day 42 PI.Group Ⅶ received oil extract from the first day of infection for 42 d.Results:Garlic oil extract showed changes in the parasite tegument with a significant decrease in worm burden,hepatic and intestinal ova count with a decline in granuloma number and diameter.These alterations were accompanied with a reduction in serum TNF- α,ICAM-1,IgG and IgM after 7 and 42 d post S.mamoni cercarial infection.Conclusions:Results obtained confirmed the effect of garlic oil extract on the larval and mature stage of the parasite and in enhancing the host's immune system against the disorders caused by 5.mansoni in mice.Reem O.A.Kamel Nashwa A.El-Shinnawy 2015Asian Pacific Journal of Tropical Medicine2015,8,12:1

15Trends of central line-associated bloodstream infections in the intensive care unit in the Kingdom of Bahrain:Four years’experience显示文摘BACKGROUND The central venous line is an essential component in monitoring and managing critically ill patients.However,it poses patients with increased risks of severe infections with a higher probability of morbidity and mortality.AIM To define the trends of the rates of central line-associated bloodstream infections(CLABSI)over four years,its predicted risk factors,aetiology,and the antimicrobial susceptibility of the isolated pathogens.METHODS The study was a prospective case-control study,performed according to the guidelines of the Center for Disease Control surveillance methodology for CLABSI in patients admitted to the adult intensive care unit(ICU)and auditing the implementation of its prevention bundle.RESULTS Thirty-four CLABSI identified over the study period,giving an average CLABSI rate of 3.2/1000 central line days.The infection's time trend displayed significant reductions over time concomitantly with the CLABSI prevention bundle's reinforcement from 4.7/1000 central line days at the beginning of 2016 to 1.4/1000 central line days by 2018.The most frequently identified pathogens causing CLABSI in our ICU were gram-negative organisms(59%).The most common offending organisms were Acinetobacter,Enterococcus,and Staphylococcus epidermidis,each of them accounted for 5 cases(15%).Multidrug-resistant organisms contributed to 56%of CLABSI.Its rate was higher when using femoral access and longer hospitalisation duration,especially in the ICU.Insertion of the central line in the non-ICU setting was another identified risk factor.CONCLUSION Implementing the prevention bundles reduced CLABSI significantly in our ICU.Implementing the CLABSI prevention bundle is crucial to maintain a substantial reduction in the CLABSI rate in the ICU setting.Safaa Al-Khawaja Nermin Kamal Saeed Sanaa Al-khawaja Nashwa Azzam Mohammed Al-Biltagi 2021World Journal of Critical Care Medicine2021,10,5:0

17Community Detection Applications in Mobile Networks:Towards Data-Driven Design for Next Generation Cellular Networks显示文摘The existing radio access network(RAN)is facing many challenges to meet the very strict speed and latency requirements by different mobile applications in addition to the increasing pressure to reduce operating cost.Innovation and development in RAN have been accelerated to tackle these challenges and to define how next generation mobile networks should look like.The role of machine learning(ML)and artificial intelligence(AI)driven innovations within the RAN domain is strengthening and attracting lots of attention to tackle many of the challenging problems.In this paper we surveyed RAN network base stations(BSs)clustering and its applications in the literature.The paper also demonstrates how to leverage community detection algorithms to understand underlying community structures within RAN.Tracking areas(TAs)novel framework was developed by adapting existing community detection algorithm to solve the problem of statically partitioning a set of BSs into TA according to mobility patterns.Finally,live network dataset in dense urban part of Cairo is used to assess how the developed framework is used to partition this part of the network more efficiently compared to other clustering techniques.Results obtained showed that the new methodology saved up to 34.6%of inter TA signaling overhead and surpassing other conventional clustering algorithms.Ayman Gaber Nashwa Abdelbaki Tamer Arafa 2023Journal of Communications and Information Networks2023,8,2:0

19The Implication of microRNAs as non-invasive biomarkers in 179 Egyptian breast cancer female patients显示文摘Background:MicroRNAs(miRs)are small(19–25 nucleotides),non-protein coding RNAs that regulate gene expression,and thus play essential roles in cell cycle progression.The evidence has demonstrated that the expression of several miRs is dysregulated in human cancer.Methods:The study includes 179 female patients and 58 healthy women Patients were identified as luminal A,B,Her-2/neu,and basal-like,as well as classified into I,II,and III stages.Analysis of the expression fold change of miR-21 and miR-34a with molecular markers,including the oncogene Bcl-2(B-cell lymphoma 2)and the tumor suppressor genes BRCA1(breast cancer susceptibility gene 1),BRCA2(breast cancer susceptibility gene 2),and the tumor suppressor protein p53,was carried out for all patients,pre-and post-chemotherapy,and for all healthy women.Results:At diagnosis(pre-chemotherapy),miR-21 was up-regulated(p<0.001),while miR-34a was down-regulated(p<0.001).Post-chemotherapy,the expression of miR-21 decreased significantly(p<0.001),while the expression of miR-34a increased significantly(p<0.001).Conclusion:miR-21 and miR-34a may be helpful to non-invasive biomarkers to evaluate the response of breast cancer to chemotherapy.NADIA ZSHAABAN NASHWA KIBRAHIM HELEN NSAADA FATMA HEL-RASHIDY HEBATALLAH MSHAABAN NERMEEN MELBAKARY AHMAD SKODOUS 2022Oncology Research2022,30,6:0
