| 1 | Hypertrophic cardiomyopathy: Can the noninvasive diagnostic testing identify high risk patients?显示文摘Hypertrophic cardiomyopathy(HCM) is the most common cause of sudden cardiac death(SCD) in the young, particularly among athletes. Identifying high risk individuals is very important for SCD prevention. The purpose of this review is to stress that noninvasive diagnostic testing is important for risk assessment. Extreme left ventricular hypertrophy and documented ventricular tachycardia and fibrillation increase the risk of SCD. Fragmented QRS and T wave inversion in multiple leads are more common in high risk patients. Cardiac magnetic resonance imaging provides complete visualization of the left ventricular chamber, allowing precise localization of the distribution of hypertrophy and measurement of wall thickness and cardiac mass. Moreover, with late gadolinium enhancement, patchy myocardial fibrosis within the area of hypertrophy can be detected, which is also helpful in risk stratification. | 15 | Rapunzel syndrome is not just a mere surgical problem: A case report and review of current management显示文摘Recurrent Rapunzel syndrome(RRS) is a rare clinical presentation with fewer than six cases reported in the Pub Med literature. A report of RRS and literature review is presented. A 25-year-old female was admitted to hospital with a 4-wk history of epigastric pain and swelling. She had a known history of trichophagia with a previous admission for Rapunzel syndrome requiring a laparotomy nine years earlier, aged 16. Psychological treatment had been successfully achieved for nine years with outpatient hypnotherapy sessions only, but she defaulted on her last session due to stressors at home. The abdominal examination demonstrated an epigastric mass. Computer tomography scan revealed a large gastric bezoar and features of aspiration pneumonia. The patient underwent emergency open surgical laparotomy for removal as the bezoar could not be removed endoscopically. The bezoar was cast in a shape that mimicked the contours of the stomach and proximal small bowel, hence the diagnosis of RRS. The patient was seen by a psychiatrist and was commenced on Quetiapine before discharge. | 18 | Active tracking of rejected dried blood samples in a large program in Nigeria显示文摘AIM: To study the impact of rejection at different levels of health care by retrospectively reviewing records of dried blood spot samples received at the molecular laboratory for human immunodeficiency virus(HIV) early infant diagnosis(EID) between January 2008 and December 2012.METHODS: The specimen rejection rate, reasons for rejection and the impact of rejection at different levels of health care was examined. The extracted data were cleaned and checked for consistency and then deduplicated using the unique patient and clinic identifiers. The cleaned data were ciphered and exported to SPSS version 19(SPSS 2010 IBM Corp, New York, United States) for statistical analyses.RESULTS: Sample rejection rate of 2.4%(n = 786/32552) and repeat rate of 8.8%(n = 69/786) were established. The mean age of infants presenting for first HIV molecular test among accepted valid samples was 17.83 wk(95%CI: 17.65-18.01) vs 20.30 wk(95%CI: 16.53-24.06) for repeated samples. HIV infection rate was 9.8% vs 15.9% for accepted and repeated samples. Compared to tertiary healthcare clinics, secondary and primary clinics had twofold and three-fold higher likelihood of sample rejection, respectively(P < 0.05). We observed a significant increase in sample rejection rate with increasing number of EID clinics(r = 0.893, P = 0.041). The major reasons for rejection were improper sample collection(26.3%), improper labeling(16.4%) and insufficient blood(14.8%). | 19 | Antimalarial activity of Ageratum conyzoides in combination with chloroquine and artesunate显示文摘Objective:To determine the suppressive and curative activity of aqueous leaf extract of Ageratum conyzoides(A.conyzoides) in combination with chloroquine and artesunate, respectively against Plasmodium berghei infection in mice.Methods:Using malaria(Plasmodium berghei) infected albino mice of both sexes,aqueous extracts of A.conyzoides in combination with chloroquine and artesunate were tested for antimalarial activity,respectively.Four-day suppressive test and Rane’s curative test were carried out.Results:Suppressive tests showed significant dose dependent reduction in parasitemia level produced by the extract-chloroquine and extract-artesunate combinations.Suppressive activities of both extract-drug combinations were greater than the individual drugs alone.Extract-chloroquine(100:5) produced the highest suppressive effect(98%suppression).Curative tests showed absolute survival in two extract-drug combinations.Two extract-drug combinations produced higher curative effects than the individual drugs alone.The highest dose combinations of extract-chloroquine(100:5) and extract-artesunate(100:5) produced absolute parasitemia clearance(cure) in the infected mice. | 20 | Sedimentary facies and textural characteristics of Cretaceous sandstones in the southern Bida Basin,Nigeria:Implication for reservoir potential and depositional environment显示文摘A study of sedimentary facies,mineral and textural characteristics of sections of conglomerates/pebbles and sandstone facies found within the Lokoja and Patti Formations in southern Bida Basin,Nigeria,is carried out to evaluate the paleo-environment and reservoir quality of the sandstones of the formations through field observations and textural and mineralogical(using X-Ray Diffractogram)analyses.Nine lithofacies are identified and grouped into three main facies associations.These sedimentary facies suggest deposition in both foreshore and estuaries.The mineral analysis(XRD)shows that the quartz content in sandstone facies of the Lokoja Formation ranges between 49%and 67%,and that of the Patti Sandstone between 43%and 56%,indicating a quartz dominance.The granulometric studies reveal that the Lokoja Formation comprises coarse-to medium-grained(-0.63 to 1.30φ)sandstone with moderatepoor sorting of 0.72e1.824.On the other hand,medium to fine-grained sandstones dominate the Patti Formation,which is averagely poorly sorted(1.29-1.54φ).The sandstones are coarsely skewed to finely skewed,with the kurtosis ranging from very platykurtic to leptokurtic.Morphological results of the pebbles from both Lokoja and Patti Formations indicate that the study areas are mainly characterized by the interplay of both wave and fluvial processes,which suggests that the environment of deposition of sandstone facies is predominantly of the fluvial to the shallow marine environment with the indication of tidal influence.Integrating sedimentary facies and textural results suggests subtidal sand ridge/foreshore,estuarine mudflat,and estuarine delta environments with fluvial influences.Evidence from the textural and mineral characteristics indicate that the sandstone facies of the Lokoja and Patti Formations are fairly matured and can act as potential hydrocarbon reservoirs in the formations.The facies,XRD mineral,and textural results have revealed potential hydrocarbon reservoir areas and paleo-depositional features of the Lokoja and Patti Formations. | Godwin Okumagbe Aigbadon Simon Dalom Christopher Ernest Orji Akudo Obinna Chigoziem Akakuru | 2022 | Energy Geoscience2022,3,3: | 0 |
