1Case of inappropriate ADH syndrome:Hyponatremia due to polyethylene glycol bowel preparation显示文摘Colonoscopic screening has been reported to reduce deaths from colorectal cancer.Adequate bowel preparation is essential for this and safety is an important issue in choosing the methods.Polyethylene glycol(PEG)is regarded as a safe method for cleansing,especially compared with oral sodium phosphate.Here,we present a case of hyponatremia caused by the syndrome of inappropriate antidiuretic hormone(ADH)syndrome after PEG precolonoscopic cleansing resulting in generalized tonic-clonic seizures.A 62-year-old women had ingested PEG for precolonoscopic bowel cleansing.While waiting for the colonoscopy,she developed a stuporous mentality and generalized tonic-clonic seizures,which did not correlate with brain magnetic resonance imaging.Her serum sodium level was 113 mEq per liter and laboratory analyses were consistent with inappropriate ADH syndrome.Her thyroid and adrenal functions were normal.There were no malignancies,infections,respiratory disorders or central nervous disorders and she had no history of taking either diuretics or other medications,which might have caused inappropriate ADH syndrome.She was treated with 3%hypertonic saline and showed a complete neurological recovery as her sodium levels recovered.Follow-up visits showed the patient to have a normal sodium level without neurologic deficits.This case shows that inappropriate ADH syndrome can be caused by PEG preparation,which implies that physicians have to be aware of the possible side effects of this colonic cleansing approach and mindful of the possible ensuing symptoms.Sun-Hye Ko Chul-Hyun Lim Jae-Young Kim Seung Hun Kang Myong Ki Baeg Hyun Jin Oh 2014World Journal of Gastroenterology2014,20,34:5
2Primary signet ring cell carcinoma of the appendix:A rare case report and our 18-year experience显示文摘Primary adenocarcinoma of the appendix is a rare ma-lignancy that constitutes < 0.5% of all gastrointestinal neoplasms. Moreover, primary signet ring cell carci-noma of the appendix is an exceedingly rare entity. We have encountered 15 cases of primary appendiceal cancer among 3389 patients who underwent appen-dectomy over the past 18 years. In the present report, we describe a rare case of primary signet ring cell carcinoma of the appendix with ovarian metastases and unresectable peritoneal dissemination occurring in a 67-year-old female patient. She underwent ap-pendectomy and bilateral salpingo-oophorectomy with a laparoscopy procedure. She then received palliative systemic chemotherapy with 12 cycles of oxaliplatin, 5-flurorouracil, and leucovorin (FOLFOX-4). 4Diabetic factors associated with gastrointestinal symptoms in patients with type 2 diabetes显示文摘AIM: To determine whether gastrointestinal (GI) symptoms are more frequent in type 2 diabetic patients and to examine which diabetic factors are associated with the symptoms. METHODS: Consecutive subjects with diabetes and age-/gender-matched normal controls were recruited for this study. GI symptoms were assessed using a structured questionnaire divided into two GI symptomcategories (upper and lower GI symptoms), and consisting of 11 individual symptoms. In the diabetic patient group, diabetic complications including peripheral neuropathy, nephropathy and retinopathy, glycosylated hemoglobin (HbA1c) level and diabetes duration were evaluated. RESULTS: Among the total 190 diabetic patients and 190 controls enrolled, 137 (72%) of the diabetic patients and 116 (62%) of the controls had GI symptoms. In the diabetic patient group, 83 (43%) had upper GI symptoms and 110 (58%) lower GI symptoms; in the control group, 59 (31%) had upper GI symptoms and 104 (55%) lower GI symptoms. This difference between the two groups was significant for only the upper GI symptoms (P = 0.02). Among the diabetic factors, the HbA1c level was the only independent risk factor for upper GI symptoms in the multiple logistic regression analysis (odds ratio = 2.01, 95% confidence interval: 1.02-3.95). 5A MEMS ultrasound stimulation system for modulation of neural circuits with high spatial resolution in vitro显示文摘Neuromodulation by ultrasound has recently received attention due to its noninvasive stimulation capability for treating brain diseases.Although there have been several studies related to ultrasonic neuromodulation,these studies have suffered from poor spatial resolution of the ultrasound and low repeatability with a fixed condition caused by conventional and commercialized ultrasound transducers.In addition,the underlying physics and mechanisms of ultrasonic neuromodulation are still unknown.To determine these mechanisms and accurately modulate neural circuits,researchers must have a precisely controllable ultrasound transducer to conduct experiments at the cellular level.Herein,we introduce a new MEMS ultrasound stimulation system for modulating neurons or brain slices with high spatial resolution.The piezoelectric micromachined ultrasonic transducers(pMUTs)with small membranes(submm membranes)generate enough power to stimulate neurons and enable precise modulation of neural circuits.We designed the ultrasound transducer as an array structure to enable localized modulation in the target region.In addition,we integrated a cell culture chamber with the system to make it compatible with conventional cell-based experiments,such as in vitro cell cultures and brain slices.In this work,we successfully demonstrated the functionality of the system by showing that the number of responding cells is proportional to the acoustic intensity of the applied ultrasound.We also demonstrated localized stimulation capability with high spatial resolution by conducting experiments in which cocultured cells responded only around a working transducer.Jungpyo Lee Kyungmin Ko Hyogeun Shin Soo-Jin Oh CJustin Lee Namsun Chou Nakwon Choi Min Tack Oh Byung Chul Lee Seong Chan Jun Il-Joo Cho 2019Microsystems & Nanoengineering2019,5,1:3
6Nanoimprint lithography for high-throughput fabrication of metasurfaces显示文摘Metasurfaces are composed of periodic subwavelength nanostructures and exhibit optical properties that are not found in nature.They have been widely investigated for optical applications such as holograms,wavefront shaping,and structural color printing,however,electron-beam lithography is not suitable to produce large-area metasurfaces because of the high fabrication cost and low productivity.Although alternative optical technologies,such as holographic lithography and plasmonic lithography,can overcome these drawbacks,such methods are still constrained by the optical diffraction limit.To break through this fundamental problem,mechanical nanopatteming processes have been actively studied in many fields,with nanoimprint lithography(NIL)coming to the forefront.Since NIL replicates the nanopattem of the mold regardless of the diffraction limit,NIL can achieve sufficiently high productivity and patterning resolution,giving rise to an explosive development in the fabrication of metasurfaces.In this review,we focus on various NIL technologies for the manufacturing of metasurfaces.First,we briefly describe conventional NIL and then present various NIL methods for the scalable fabrication of metasurfaces.We also discuss recent applications of NIL in the realization of metasurfaces.Finally,we conclude with an outlook on each method and suggest perspectives for future research on the high-throughput fabrication of active metasurfaces.Dong Kyo OH Taejun LEE Byoungsu KO Trevon BADLOE Jong G.OK Junsuk RHO 2021Frontiers of Optoelectronics2021,14,2:3

10Clinical correlations of infliximab trough levels and antibodies to infliximab in South Korean patients with Crohn's disease显示文摘AIM To investigate the clinical implications of infliximab trough levels(IFX-TLs) and antibodies to infliximab(ATI) levels in Crohn's disease(CD) patients in Asian countries. METHODS IFX-TL and ATI level were measured using prospectively collected samples obtained with informed consent from CD patients being treated at Asan Medical Center, South Korea. We analyzed the correlations between IFX-TLs/ATI levels and the clinical activity of CD(quiescent vs active disease) based on the CD activity index, C-reactive protein level, and physician's judgment of patients' clinical status at enrollment. The impact of concomitant immunomodulators was also investigated. RESULTS This study enrolled 138 patients with CD(84 with quiescent and 54 with active disease). In patients with quiescent and active diseases, the median IFX-TLs were 1.423 μg/mL and 0.163 μg/mL, respectively(P < 0.001) and the median ATI levels were 8.064 AU/m L and 11.209 AU/m L, respectively(P < 0.001). In the ATI-negative and-positive groups, the median IFXTLs were 1.415 μg/mL and 0.141 μg/mL, respectively(P < 0.001). In patients with and without concomitant immunomodulator use, there were no differences in IFX-TLs(0.632 μg/mL and 1.150 μg/mL, respectively; P = 0.274) or ATI levels(8.655 AU/mL and 9.017 AU/mL, respectively; P = 0.083).CONCLUSION IFX-TL/ATI levels were well correlated with the clinical activity in South Korean CD patients.
