| 1 | Single-stage laparoscopic management of acute gallstone pancreatitis:outcomes at different timings显示文摘BACKGROUND:Definitive therapy for gallstone pancreatitis requires eradication of gallstones with cholecystectomy and common bile duct(CBD) clearance.Current guidelines recommend this be done within the same admission and preferably by laparoscopic cholecystectomy and CBD exploration.We report our experience of laparoscopic single-stage management with cholecystectomy and intraoperative cholangiogram followed by laparoscopic bile duct exploration(LBDE) when necessary performed at three different stages.METHODS:From January 1998 to December 2012,134 patients(100 females and 34 males) underwent single-stage laparoscopic management of gallstone pancreatitis.Patients were classified according to the timing of surgery:'A',≤7 days from symptom onset(n=27); 'B',8 to 30 days(n=58) and 'C',>30 days(n=49).RESULTS:LBDE was performed in 30 patients with a success rate of 100%.CBD stones were found in 25 patients(A:22.2%,B:22.4%,C:12.2%).CBD stones were more common in patients undergoing surgery within 30 days of presentation than after this time point(P=0.35).Multiple choledocholithiasis was more frequent in patients treated within 7 days(P=0.04).The 30-day mortality after surgery was 0,with no conversion to an open approach.Overall complication rate was 11.9%,which did not differ significantly between patients treated within 7 days or after this time point(P=0.83).CONCLUSIONS:This study demonstrated the feasibility and reproducibility of single-stage laparoscopic management of acute gallstone pancreatitis,which has a low complication rate at any stage.Patients undergoing early treatment have a higher incidence of choledocholithiasis and multiple stones than those treated after 30 days,supporting the passage of stones with time. | Antonio Navarro-Sanchez Hutan Ashrafian Aggelos Laliotis Kamran Qurashi Alberto Martinez-Isla | 2016 | Hepatobiliary & Pancreatic Diseases International2016，15，3： | 5 |

| 3 | Efficacy and safety of treatment of hepatitis C virus infection in renal transplant recipients显示文摘AIM: To assess the efficacy and safety of combinedpegylated interferon and ribavirin therapy in hepatitis C virus (HCV) infection in renal transplant recipients. METHODS: This is a retrospective chart review of post renal transplant patients who were positive for anti-HCV and HCV-RNA, and who have received treatment with combination of pegylated interferon and ribavirin between October 2003 and December 2008. Only patients with stable graft function and absence of evidence of cirrhosis and who received the therapy for continuous 48 wk were included. Nineteen patients (13 male and 6 female) were identified and included. The patient's complete blood count, liver and kidney prof ile, and cal- culated glomerular f iltration rate (GFR) were monitored every 6-8 wk while on treatment. HCV-RNA was tested at 12 wk for early virological response, at 48 wk for end of treatment response (ETR), and then retested at 24, and 48 wk after completion of therapy for sustained virological response (SVR). Liver biopsies were obtained before treatment from all patients and graft kidney biopsies were performed as required. RESULTS: Of the entire cohort, 9 patients (47.4%) showed an ETR and 8 had SVR (42.1%). Of the 8 patients with abnormal alanine aminotransferase (ALT) levels at baseline, 78.9% had their ALT normalized (including the virological non responders). ALT was normal in all responders at the end of therapy and at 24 wk post therapy (100%). Only one patient (5.3%) developed an increase in creatinine and decline in GFR from baseline towards the end of treatment. This patient's kidney biopsy revealed borderline rejection. There was no impact on response by HCV-genotype, initial HCV RNA load, age or sex of the patient or duration post transplant before commencement of therapy. All patients tolerated treatment in the same way as non-transplant with no unusual or increased occurrence of side effects. | 5 | Effect of compression deformation on precipitation phase behavior of B-containing S31254 super austenitic stainless steel显示文摘Effects of compression deformation on the sigma-phase precipitation behavior of B-containing S31254 stainless steel after solution treatment were studied using the Gleeble compression test. The cold and hot processing characteristics of B-containing S31254 stainless steel were evaluated, and the results show that the speed of compression deformation increased the precipitation rate of the sigma phase, and the location of precipitation extended from the austenite grain boundary to the original hot rolling deformation area. During cold deformation at room temperature, the precipitation rate increased when the deformation reached at 40%. Deformation at 950℃ affected precipitation more obviously. At 1074 ℃, when the deformation reached 20%, the precipitated phases started increasing, and above this deformation range, precipitation began decreasing. Also, at 1074℃ the deformation accelerated the precipitation of sigma phase;but with deformation, there was a change in critical temperature for the sigma-phase excursion. The precipitation position of the sigma phase is strongly related to the area of the original hot rolling deformation.
