1Association between obesity-related adipokines and colorectal cancer:A case-control study and meta-analysis显示文摘AIM:To examine the association between obesityrelated adipokines(adiponectin,leptin,resistin,interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)and colorectal cancer(CRC)risk.METHODS:Serum levels of adipokines were measured in 100 CRC patients and age-and sex-matched controls for the data analysis.Unconditional logistic regression models were used for estimating ORs and95%CIs related to each adipokine.For the metaanalysis,studies published before July 2013 available on Medline/PubMed and EMBASE were retrieved.The analysis included a total of 17 relevant studies(including the present case-control study):nine studies on adiponectin and eight on leptin.The effect sizes of ORs and 95%CIs were estimated using RevMan 5.1.Heterogeneity was evaluated using Cochran’s Q-test and I2 statistics.RESULTS:Among the five adipokines,only resistin levels were significantly higher in cases than in controls(P<0.001).The case-control study results showed no association between adiponectin and CRC and a negative association between leptin and CRC.However,the results of the meta-analysis showed a significant inverse association between adiponectin and CRC(OR=0.91,95%CI:0.83-1.00,P=0.04)and no association between CRC and leptin.After stratification by study design,an inverse association between adiponectin and CRC was observed in prospective studies only(OR=0.90,95%CI:0.82-0.99,P=0.03),whereas the association between leptin and CRC was inconsistent(prospective studies:OR=1.14,95%CI:1.02-1.27,P=0.02 and retrospective studies:OR=0.47,95%CI:0.29-0.74,P=0.001).The associations of resistin and TNF-αwith CRC risk were positive,but no association was observed for IL-6.CONCLUSION:Our results suggest a negative association of leptin,positive associations of resistin and TNF-α,and null associations of adiponectin and IL-6with CRC.However,further studies with larger number of prospective approaches are needed.Rakhi Kumari Joshi Woo Jin Kim Sang-Ah Lee 2014World Journal of Gastroenterology2014,20,24:11
2Effect of Alstonia scholaris bark extract on testicular function of Wistar rats显示文摘Aim:To evaluate the antifertility effect of Alstonia scholaris bark extract in male rats.Methods:In male Wistar rats Alstonia scholaris bark extract was given by oral route at a dose of 200mg/day for60days,The fertility and testicular function were assessed by mating tests,sperm motility,sperm concentration,biochemical indices and testicular cell population dynamics.Results:Oral feeding with the extract at a dose of 200mg/day for the period of 60days did not cause body weight loss,while the weights of testes,epididymides,seminal vesicle and ventral prostate were significantly reduced.The production of step-19 spermatids was reduced by 79.6%in treated rats.The popu-lation of preleptotene and pachytene spermatocytes were decrased by61.9%and60.1%,respectively.Spematogo-nia and Sertoli cell population were also affected.The seminiferous tubule and Leydig cell nuclear area were reduced significantly(P<0.01)when compared to the controls..Reduced sperm count and motility yesulted in a total supprs-sion of fertility.A significant fall in the protein and sialic acid content of the testes,epididymides,seminal vesicle and ventral prostate as well as glycogen content of testes were also noticed.The fructose content in the seminal vesicle was lowered whereas the testicular cholesterol was elevated as compared with the controls.The following com-pounds were obtained from the extract with chromatographic separation over Si-gel column;α-amyrin,β-amyrin,lupiol acetate,venenative,rhazine and yohimbine,Conclusion:At the dose level employed,Alstonia scholaris bark extract has a significant antifertility effect in male rast;the primary site of action may be post meiotic germ cells(Step19spermatids).R. 9Effect of inflammatory bowel disease treatments on patients with diabetes mellitus显示文摘As medical care progresses and the number of patients with chronic conditions increases there is the inevitable challenge of managing patients with multiple comorbidities.Inflammatory bowel disease(IBD)is an umbrella term for are inflammatory conditions affecting the gastrointestinal tract,the two most common forms being Ulcerative Colitis and Crohn’s disease.These diseases,usually diagnosed in young adults,exhibit a relapsing and remitting course and usually require longterm treatment.IBD can be treated with a number of topical and systemic treatments.We conducted a review of the current published evidence for the effects these medications can have on diabetes mellitus(DM)and glycaemic control.Searches were conducted on medline and embase with a timeframe from 1947(the date from which studies on embase are recorded)to November 2020.Suitable publications were selected and reviewed.Current evidence of the impact of aminosalicylates,corticosteroids,thiopurines,and biologic agents was reviewed.Though there was limited evidence for certain agents,IBD medications have been shown to have an effect of DM and these effects should be considered in managing patients with dual pathologies.The effects of steroids on blood sugar control is well documented,but consideration of other agents is also important.In patients requiring steroids for Ulcerative Colitis,locally acting steroid agents delivered rectally may be preferred to minimise side effects in those with distal bowel Ulcerative Colitis.A switch to other agents should be considered as soon as possible in people with diabetes to limit the impact on glycaemic control.5-aminosalicylates appear to play a role in the reduction of hemoglobin A1c(HbA1c),although the literature suggests these may be falsely low readings.Consequently,monitoring of people with diabetes on these agents may require daily monitoring of capillary blood sugars rather than relying simply on HbA1c;for example fructosamine performed 3-6 monthly,although this risks missing the rise in readings.There is only limited evidence of the effects of thiopurines on diabetes and further investigation is needed into the possible relationship between them.However,given the current available evidence it may be preferable to commence patients with diabetes on thiopurines as soon as possible,whilst also monitoring for side effects such as pancreatitis.There appears to be more evidence supporting a link between tumor necrosis factor-αinhibitors and DM.Both infliximab and adalimumab have evidence suggesting that both can cause reduced blood sugar levels.Further studies on the effects of the various biological agents mentioned are required alongside any novel biologic therapy and the impact of dual biologic therapy in the future.Joshua Ashley Jack Bower Lauren O'Flynn Rakhi Kakad David Aldulaimi 2021World Journal of Diabetes2021,12,8:1
