| 1 | Unexpected complication of arteriovenous fistula of the left common carotid to internal jugular vein following central venous catheterization显示文摘Incidence of inadvertent arterial puncture secondary to central venous catheter insertion is not common with an arterial puncture rate of<1%.This is due to the advancements and wide availability of ultrasound to guide its insertion.Formation of arteriovenous fistula after arterial puncture is an unexpected complication.Till date,only five cases(including this case)of acquired arteriovenous fistula formation has been described due to inadvertent common carotid puncture.The present case is a 26-year-old man sustained traumatic brain injuries,chest injuries and multiple bony fractures.During resuscitative phase,attempts at left central venous catheter via left internal jugular vein under ultrasound guidance resulted in inadvertent puncture into the left common carotid artery.Surgical neck exploration revealed that the catheter had punctured through the left internal jugular vein into the common carotid artery with formation of arteriovenous fistula.The catheter was removed successfully and common carotid artery was repaired.Postoperatively,the patient recovered and clinic visits revealed no neurological deficits.From our literature review,the safest method for removal is via endovascular and open surgical removal.The pull/push technique(direct removal with compression)is not recommended due to the high risk for stroke,bleeding and hematoma formation. | Tan Chor Lip Henry Tan Jih Huei Mohamad Yuzaidi Lenny Suryani Safri K.Krishna Imran Alwi Rizal Md Idris Mohamad Azim Hanafiah Harunarashid | 2020 | Chinese Journal of Traumatology2020，23，1： | 5 |
| 2 | Analysis on fused deposition modelling performance显示文摘Fused deposition modelling (FDM) is one of rapid prototyping (RP) technologies which uses an additive fabrication approach.Each commercially available FDM model has different types of process parameters for different applications.Some of the desired parts require excellent surface finish as well as good tolerance.The most common parameters requiring setup are the raster angle,tool path,slice thickness,build orientation,and deposition speed.The purpose of this paper is to discuss the process parameters of FDM Prodigy Plus (Stratasys,Inc.,Eden Prairie,MN,USA).Various selected parameters were tested and the optimum condition was proposed.The quality of the parts produced was accessed in terms of dimensional accuracy and surface finish.The optimum parameters obtained were then applied in the fabrication of the master pattern prior to silicone rubber moulding (SRM).These parameters would reduce the post processing time.The dimensional accuracy and surface roughness were analyzed using coordinate measuring machine (CMM) and surface roughness tester,respectively.Based on this study,the recommended parameters will improve the quality of the FDM parts produced in terms of dimensional accuracy and surface roughness for the application of SRM. | Nur Saaidah Abu BAKAR Mohd Rizal ALKAHARI Hambali BOEJANG | 2010 | Journal of Zhejiang University-Science A(Applied Physics & Engineering)2010，11，12： | 3 |

| 18 | Critical evaluation of contemporary management in a new Pelvic Exenteration Unit: The first 25 consecutive cases显示文摘AIM To critically appraise short-term outcomes in patientstreated in a new Pelvic Exenteration(PE) Unit.METHODS This retrospective observational study was conducted by analysing prospectively collected data for the first 25 patients(16 males, 9 females) who underwent PE for advanced pelvic tumours in our PE Unit between January 2012 and October 2016. Data evaluated included age, co-morbidities, American Society of Anesthesiologists(ASA) score, Eastern Cooperative Oncology Group(ECOG) status, preoperative adjuvant treatment, intra-operative blood loss, procedural duration, perioperative adverse event, lengths of intensive care unit(ICU) stay and hospital stay, and oncological outcome. Quantitative data were summarized as percentage or median and range, and statistically assessed by the χ~2 test or Fisher's exact test, as applicable.RESULTS All 25 patients received comprehensive preoperative assessment via our dedicated multidisciplinary team approach. Long-course neoadjuvant chemoradiotherapy was provided, if indicated. The median age of the patients was 61.9-year-old. The median ASA and ECOG scores were 2 and 0, respectively. The indications for PE were local y invasive rectal adenocarcinoma(n = 13), advanced colonic adenocarcinoma(n = 5), recurrent cervical carcinoma(n = 3) and malignant sacral chordoma(n = 3). The procedures comprised 10 total PEs, 4 anterior PEs, 7 posterior PEs and 4 isolated lateral PEs. The median follow-up period was 17.6 mo. The median operative time was 11.5 h. The median volume of blood loss was 3306 mL, and the median volume of red cell transfusion was 1475 mL. The median lengths of ICU stay and of hospital stay were 1 d and 21 d, respectively. There was no case of mortality related to surgery. There were a total of 20 surgical morbidities, which occurred in 12 patients. The majority of the complications were grade 2 Clavien-Dindo. Only 2 patients experienced grade 3 Clavien-Dindo complications, and both required procedural interventions. One patient experienced grade 4a Clavien-Dindo complication, requiring temporary renal dialysis without long-term disability. The R0 resection rate was 64%. There were 7 post-exenteration recurrences during the follow-up period. No statistically significant relationship was found among histological origin of tumour, microscopic resection margin status and postoperative recurrence(P = 0.67).
