| 1 | Retrospective study on mixed neuroendocrine non-neuroendocrine neoplasms from five European centres显示文摘BACKGROUND Mixed neuroendocrine non-neuroendocrine neoplasm(MiNEN)is a rare diagnosis,mainly encountered in the gastro-entero-pancreatic tract.There is limited knowledge of its epidemiology,prognosis and biology,and the best management for affected patients is still to be defined.AIM To investigate clinical-pathological characteristics,treatment modalities and survival outcomes of a retrospective cohort of patients with a diagnosis of MiNEN.METHODS Consecutive patients with a histologically proven diagnosis of MiNEN were identified at 5 European centres.Patient data were retrospectively collected from medical records.Pathological samples were reviewed to ascertain compliance with the 2017 World Health Organisation definition of MiNEN.Tumour responses to systemic treatment were assessed according to the Response Evaluation Criteria in Solid Tumours 1.1.Kaplan-Meier analysis was applied to estimate survival outcomes.Associations between clinical-pathological characteristics and survival outcomes were explored using Log-rank test for equality of survivors functions(univariate)and Cox-regression analysis(multivariable).RESULTS Sixty-nine consecutive patients identified;Median age at diagnosis:64 years.Males:63.8%.Localised disease(curable):53.6%.Commonest sites of origin:colon-rectum(43.5%)and oesophagus/oesophagogastric junction(15.9%).The neuroendocrine component was;predominant in 58.6%,poorly differentiated in 86.3%,and large cell in 81.25%,of cases analysed.Most distant metastases analysed(73.4%)were occupied only by a poorly differentiated neuroendocrine component.Ninety-four percent of patients with localised disease underwent curative surgery;53%also received perioperative treatment,most often in line with protocols for adenocarcinomas from the same sites of origin.Chemotherapy was offered to most patients(68.1%)with advanced disease,and followed protocols for pure neuroendocrine carcinomas or adenocarcinomas in equal proportion.In localised cases,median recurrence free survival(RFS);14.0 months(95%CI:9.2-24.4),and median overall survival(OS):28.6 months(95%CI:18.3-41.1).On univariate analysis,receipt of perioperative treatment(vs surgery alone)did not improve RFS(P=0.375),or OS(P=0.240).In advanced cases,median progression free survival(PFS);5.6 months(95%CI:4.4-7.4),and median OS;9.0 months(95%CI:5.2-13.4).On univariate analysis,receipt of palliative active treatment(vs best supportive care)prolonged PFS and OS(both,P<0.001).CONCLUSION MiNEN is most commonly driven by a poorly differentiated neuroendocrine component,and has poor prognosis.Advances in its biological understanding are needed to identify effective treatments and improve patient outcomes. | Melissa Frizziero Xin Wang Bipasha Chakrabarty Alexa Childs Tu V Luong Thomas Walter Mohid S Khan Meleri Morgan Adam Christian Mona Elshafie Tahir Shah Annamaria Minicozzi Wasat Mansoor Tim Meyer Angela Lamarca Richard A Hubner Juan W Valle Mairéad G McNamara | 2019 | World Journal of Gastroenterology2019,25,39: | 13 |
| 2 | Ibrutinib alleviated LPS-induced neuroinflam⁃mation and synaptic defects in mouse model of depression显示文摘OBJECTIVE Previous studies have demonstrated a close association between an altered immune system and major depressive disorders,and inhibition of neuroinflammation may represent an alternative mechanism to treat depression.Recently,the anti-inflammatory activ⁃ity of ibrutinib has been reported.However,the effect of ibrutinib on neuroinflammation-induced depression and its underlying mechanism has not been comprehensively studied.Therefore,we aimed to elucidate the potential anti-depres⁃sive role and mechanism of ibrutinib against neu⁃roinflammation-induced depression and synaptic defects.METHODS AND RESULTS Ibrutinib treatment significantly reduced lipopolysaccha⁃ride(LPS)-induced depressive-like behaviors and neuroinflammation via inhibiting NF-κB acti⁃vation,decreasing proinflammatory cytokine levels,and normalizing redox signaling and its downstream components,including Nrf2,HO-1,and SOD2,as well as glial cell activation mark⁃ers,such as Iba-1 and GFAP.Further,ibrutinib treatment inhibited LPS-activated inflammasome activation by targeting NLRP3/P38/caspase-1 signaling.Interestingly,LPS reduced the number of dendritic spines and expression of BDNF,and synaptic-related markers,including PSD95,snap25,and synaptophysin,were improved by ibrutinib treatment in the hippocampal area of the mouse brain.CONCLUSION Ibrutinib can allevi⁃ate neuroinflammation and synaptic defects,sug⁃gesting it has antidepressant potential against LPS-induced neuroinflammation and depression. | LI Wei-fen Tahir Ali HE Kai-wu LIU Zi-zhen Fawad Ali Shah REN Qing-guo LIU Yan JIANG An-long LI Shu-peng | 2021 | 中国药理学与毒理学杂志2021,35,9: | 10 |
| 3 | Carcinogenesis on the background of liver fibrosis: Implications for the management of hepatocellular cancer显示文摘Hepatocellular carcinoma(HCC) is now the second leading cause of cancer-related deaths globally and many patients have incurable disease. HCC predominantly occurs in the setting of liver cirrhosis and is a paradigm for inflammation-induced cancer. The causes of chronic liver disease promote the development of transformed or premalignant hepatocytes and predisposes to the development of HCC. For HCC to grow and progress it is now clear that it requires an immunosuppressive niche within the fibrogenic microenvironment of cirrhosis. The rationale for targeting this immunosuppression is supported by responses seen in recent trials with checkpoint inhibitors. With the impact of immunotherapy, HCC progression may be delayed and long term durable responses may be seen. This makes the management of the underlying liver cirrhosis in HCC even more crucial as studies demonstrate that measures of liver function are a major prognostic factor in HCC. | 4 | Transfer of Heavy Metals from Soils to Vegetables and Associated Human Health Risks at Selected Sites in Pakistan显示文摘Contamination of the food chain with heavy metals is considered as one of the major environmental pathways of human exposure to metals leading to potential health risks. This study aimed to investigate the concentrations of heavy metals such as copper(Cu),zinc(Zn), chromium(Cr), nickel(Ni), and manganese(Mn) in agricultural soils and food crops(fruit, leaf, and root vegetables), and their associated health risks to the local population in selected southern districts of Khyber Pakhtunkhwa Province, Pakistan. The concentrations of the selected metals in soil varied over a wide range, in the following decreasing order: Mn > Zn > Cr > Ni > Cu.The bioaccumulation of metals in vegetables was within the permissible risk limits, except for Cr which showed higher contamination in all the tested food crops. The trend of metal transfer factors for different vegetables was in the order of Cu > Ni > Cr > Mn >Zn, while the calculated daily intake of metals(DIM) in adults and children through consumption of food crops was in the decreasing order of Mn > Zn > Ni > Cr > Cu. The health risk index(HRI) values for the heavy metals for both adults and children were less than 1. | 5 | Lead isotope signatures of Pb-Zn sulfide mineralization in the Reshian-Lamnian area of Azad Jammu and Kashmir,Pakistan显示文摘The Reshian-Lamnian area within the Hazara-Kashmir syntaxis in Pakistan is composed mainly of the rocks of the Salkhala,Panjal and Murree formations.Base metal sulfide mineralization in the form of sphalerite and galena with lesser amounts of chalcopyrite and pyrite is present within the Salkhala Formation of the study area.Chemically all these ore phases are homogeneous in composition.The Pb isotopic composition of galena from the area suggests that there is very little or negligible variation in the ratios of 206Pb/204Pb,207Pb/204Pb and 208Pb/204Pb.Modal ages ranging from 509 to 562 Ma and the μ values of 10.71 to 10.93 have been calculated for the studied Pb-Zn mineralization.On the basis of field features,mineralogy and Pb-isotope signatures,it is concluded that the Pb-Zn sulfide mineralization in the Reshian-Lamnia area is pre-Himalayan in age and can be correlated with the Cambro-Ordovician(Pan-African) orogenic event. | Mohammad Tahir Shah Tazeem Khan Ahmad Khan | 2010 | Chinese Journal Of Geochemistry2010,29,1: | 4 |

| 7 | Pancreatic pseudocyst or a cystic tumor of the pancreas?显示文摘Pancreatic pseudocysts are the most common cystic lesions of the pancreas and may complicate acute pancreatitis, chronic pancreatitis, or pancreatic trauma. While the majority of acute pseudocysts resolve spontaneously, few may require drainage. On the other hand, pancreatic cystic tumors, which usually require extirpation, may disguise as pseudocysts. Hence, the distinction between the two entities is crucial for a successful outcome. We conducted this study to highlight the fundamental differences between pancreatic pseudocysts and cystic tumors so that relevant management plans can be devised. We reviewed the data of patients with pancreatic cystic lesions that underwent intervention between June 2007 and December 2010 in our hospital. We identified 9 patients(5 males and 4 females) with a median age of 40 years(range, 30–70 years). Five patients had pseudocysts, 2 had cystic tumors, and 2 had diseases of undetermined pathology. Pancreatic pseudocysts were treated by pseudocystogastrostomy in 2 cases and percutaneous drainage in 3 cases. One case recurred after percutaneous drainage and required pseudocystogastrostomy. The true pancreatic cysts were serous cystadenoma, which was treated by distal pancreatectomy, and mucinous cystadenocarcinoma, which was initially treated by drainage, like a pseudocyst, and then by distal pancreatectomy when its true nature was revealed. We conclude that every effort should be exerted to distinguish between pancreatic pseudocysts and cystic tumors of the pancreas to avoid the serious misjudgement of draining rather than extirpating a pancreatic cystic tumor. | 8 | HCV genotype distribution and possible transmission risks in Lahore, Pakistan显示文摘AIM: To investigate the prevalence of hepatitis C virus (HCV) genotypes and their association with possible transmission routes in the general population of Lahore, as the data exclusively related to this city is limited. METHODS: Complete data regarding patient's history, possible route of infection and biochemical tests was collected from the public hospital for 1364 patients. SPSS version 16 windows software was used for data analysis by univariate and multivariate techniques. RESULTS: Age range ≤ 40 years showed high prevalence of HCV infection. HCV genotype 3a was dominant (55.9%), followed by 1a (23.6%), 4a (12.5%), 3b (3.2%), untypable (2.5%), 4b (1.2%) and mixed type (1.2%). Blood transfusion, dental surgery and barber shops were the main risk factors for HCV transmission. Genotype prevalence was independent of age (P = 0.971) and gender (P = 0.122) while risk factors showed a significant association with age (P = 0.000) and genotypes (P = 0.000). We observed an independent association of risk factors and genotype 3a, while patients with genotype 1 and 4 were mostly infected due to dental surgery blood transfusion and barber shops. Risk factors of intravenous drug use and sexual exposure were exclusively found in ≤ 40 years age group. CONCLUSION: An increase in genotypes 1a and 4a suggest migration of people, possibly from Balochistan and the northern war-zone area. | 13 | Nitrate application affects root morphology by altering hormonal status and gene expression patterns in B9 apple rootstock nursery plants显示文摘Lateral roots(LRs)are critical for absorbing water,minerals,and nutrients.Nitrate is a vital signaling molecule for regulating LR growth.However,understanding how nitrate interacts with endogenous hormones and genes to coordinate LR development is a bottleneck.In this study,B9 apple rootstock nursery plants were cultured hydroponically and treated with different nitrate treatments(Control;T1,2.25 mM L^(−1);T2,9 mM L^(−1);T3,18 mM L^(−1);T4,36 mM L^(−1);and T5,72 mM L^(−1)),where T5 outperformed the other treatments.Nursery plants were subsequently treated with control and T5 to explore the underlying physiological and molecular mechanisms by which nitrate promoted LR growth.Plant height,stem diameter,root morphological parameters,endogenous hormones,and gene expression levels were measured.Results showed that T5 promoted LR growth by increasing the endogenous contents of indole-3-acetic acid(IAA)and gibberellic acid(GA),and by decreasing the contents of abscisic acid(ABA),jasmonic acid(JA),and zeatin riboside(ZR)at most time points.Furthermore,the relative expression levels of nitrate transporter genes were upregulated in the control group,and nitrate assimilation genes were upregulated in T5 nursery plants at various time points.In addition,the transcript abundances of auxin-related genes were higher in T5 nursery plants,which increased auxin contents.MdSHR,MdGATA1,and MdSCR1 expressions were induced,hence increasing LR growth.In contrast,higher auxin content raised MdWOX11 expression,which enhanced the MdLBD16 and MdLBD29 expression levels,thus prompting the transcripts of cell cycle-related genes:MdCYCD1;1 and MdCYCP4;1.Overall,the changes in hormonal content and gene expression levels followed superior LR growth by T5. | Muhammad Mobeen Tahir Xiaoyun Zhang Kamran Shah Faisal Hayat Shaohuan Li Jiangping Mao ke Li Yu Liu Yun Shao Dong Zhang | 2021 | Fruit Research2021,1,1: | 1 |

| 17 | Prognostic factors and survival after surgical resection of pancreatic neuroendocrine tumor with validation of established and modified staging systems显示文摘Background: Pancreatic neuroendocrine tumors(PNETs) display wide heterogeneity with highly variable prognosis. This study aimed to identify variables related to survival after surgical resection of PNET.Methods: A total of 143 patients were identified from a prospectively maintained database. Patient characteristics were analyzed and prognostic factors for overall survival and progression-free survival were evaluated. The WHO, ENETS and AJCC scoring systems were applied to the cohort, and their ability to predict patient outcomes were compared.Results: Multivariate analysis found that female gender, lymph node metastases and increasing WHO2010 grade to be independently associated with reduced overall survival(P < 0.05). Patients requiring multi-visceral resection or debulking surgery found to be associated with shortest survival. ROC analysis found the ENETS and AJCC scoring systems to be similarly predictive of 5-year overall survival. Modified Ki67 significantly improved its accuracy in predicting 5-year overall survival(AUROC: 0.699 vs 0.605;P < 0.01).Conclusions: Multi-visceral or debulking surgery is associated with poor outcomes. There seems to be no significant difference between enucleation and anatomical segmental resection.
