| 2 | Cytoreduction and hyperthermic intraperitoneal chemotherapy in the treatment of peritoneal carcinomatosis from pseudomixoma peritonei显示文摘AIM:To investigate the most important aspects of hyperthermic intraperitoneal chemotherapy (HIPEC) that has been accepted as the standard treatment for pseudomyxoma peritonei (PMP),with special regard to morbidity,overall survival (OS) and disease free survival (DFS) over 10 years. METHODS:Fifty-three patients affected by PMP underwent cytoreduction (CCR) and HIPEC with a 'semi-closed' abdomen technique in our institution. The peritonectomy procedure and completeness of CCR were classifi ed according to Sugarbaker criteria. Preoperative evaluation always included thoracic and abdominal CT scan to stage peritoneal disease and exclude distant metastases. Fifty-one patients in our series were treated with a protocol based on administration of cisplatinum 100 mg/m2 plus mitomycin C 16 mg/m2,at a temperature of 41.5℃ for 60 min. Anastomoses were always performed at the end of HIPEC. The mean duration of surgery was 12 h including HIPEC. Continuous monitoring of hepatic and renal functions and hydroelectrolytic balance was performed in the postoperative period. RESULTS:Twenty-four patients presented with postoperative complications:surgical morbidity was observed in 16 patients and 6 patients were re-operated. All complications were successfully treated and no postoperative deaths were observed. Risk factors for postoperative morbidity were considered to be gender,age,body surface,duration of surgery,Peritoneal Cancer Index (PCI) and tumor residual value (CC score). No statistically signifi cant correlation was found during the multivariate analysis:only the CC score was statistically significant. The OS in our experience was 81.8%,with a DFS of 80% at 5 years and of 70% at 10 years. CONCLUSION:In our experience,even if HIPEC combined with cytoreductive surgery involves a high risk of morbidity,postoperative complications can be resolved favorably in most cases with correct patient selection and adequate postoperative care,thus minimizing mortality. The association of CCR and HIPEC can be considered as the standard treatment for PMP. The OS and DFS results confi rm the validity of this combined approach for the treatment of this rare neoplasm. | 3 | Numerical simulations of water flow and contaminants transport near mining wastes disposed in a fractured rock mass显示文摘A numerical tool, called Hydro-Geosphere, was used to simulate unsaturated water flow and contaminants migration around an open pit filled with mining wastes. Numerical simulations had been carried out to assess the influence of various factors on water flow and solute transport in and around the surface openings including recharge, properties of the waste material and presence of fractures in the surrounding rock mass. The effect of the regional hydraulic gradient was also investigated. The analyses were conducted by simulating various 2D cases using experimentally obtained material properties and controlled boundary conditions. The effects of the hydrogeological properties of the filling material(i.e., water retention curve and hydraulic conductivity function), fracture network characteristics and conductivity of the joints were assessed. The results illustrate that fractures control water flow and contaminants transport around the waste disposal area. | 4 | Starring role of toll-like receptor-4 activation in the gutliver axis显示文摘Since the introduction of the term 'gut-liver axis', many studies have focused on the functional links of intestinal microbiota, barrier function and immune responses to liver physiology. Intestinal and extra-intestinal diseases alter microbiota composition and lead to dysbiosis, which aggravates impaired intestinal barrier function via increased lipopolysaccharide translocation. The subsequent increased passage of gut-derived product from the intestinal lumen to the organ wall and bloodstream affects gut motility and liver biology. The activation of the toll-like receptor 4(TLR-4) likely plays a key role in both cases. This review analyzed the most recent literature on the gut-liver axis, with a particular focus on the role of TLR-4 activation. Findings that linked liver disease with dysbiosis are evaluated, and links between dysbiosis and alterations of intestinal permeability and motility are discussed. | 8 | Bimanual microincision cataract surgery with implantation of the new Incise~? MJ14 intraocular lens through a 1.4 mm incision显示文摘AIM: To analyze the visual outcomes and the posterior capsule opacification(PCO) with the new Incise? MJ14 intraocular lens(IOL) implanted through a 1.4 mm clear corneal incision(CCI) in patients who underwent bimanual microincision cataract surgery(B-MICS). METHODS: Eighty eyes which underwent cataract surgery using B-MICS technique performed by the same experienced surgeon were included in the study: 40 eyes were implanted with an Incise? MJ14 IOL through a 1.4 mm CCI(group A)without enlargement of the main CCI, while 40 eyes were implanted with an Akreos? MI60 IOL with enlargement of the main CCI to 1.8 mm(group B). Best corrected visual acuity(BCVA), astigmatism and endothelial cell loss were evaluated before and after surgery at 7, 30 d and 6 mo.Anterior segment-optical coherence tomography(AS-OCT) of CCI was performed at 1, 3, 7, 30 d, 6 and 18 mo. PCO incidence was evaluated at 18 mo using EPCO 2000 Software. RESULTS: Mean BCVA improvement and endothelial cell loss were statistically significant at 18 mo in both groups with no difference between the two groups; no statistically significant difference in surgically induced astigmatism(SIA) was noticed in the two groups. At AS-OCT the only significant alterations in the CCI were endothelial gaping and local detachment of Descemet's membrane at 1 and 7 d after surgery; no statistically significant alterations were found at 1, 6 and 18 mo. PCO score at 18 mo was 0.03±0.07 for group A and 0.08±0.18 for group B(P=0.11) with no sign of central optic plate invasion in both groups.CONCLUSION: The implant of the new Incise? MJ14 IOL through a 1.4 mm CCI and B-MICS technique appeared to be a safe and effective procedure with rapid visual recovery. | 17 | Grazing Management Plans improve pasture selection by cattle and forage quality in sub-alpine and alpine grasslands显示文摘Over the last decades, the reduction of manpower for herd management has led to an increase of continuous grazing systems(CGS) in the Italian Alps, which allow cattle to roam freely. Under CGS, due to high selectivity, livestock exploit grasslands unevenly, over-and under-using specific areas at the same time with negative effects on their conservation. To counteract these effects, a specific policy and management tool(i.e. Grazing Management Plan) has been implemented by Piedmont Region since 2010. The Grazing Management Plans are based on the implementation of rotational grazing systems(RGS), with animal stocking rate adjusted to balance it with grassland carrying capacity. A case study was conducted on alpine summer pastures to test the 5-year effects produced by the implementation of a Grazing Management Plan in grasslands formerly managed under several years of CGS on 1) the selection for different vegetation communities by cattle, 2) the abundance of oligo-, meso-, and eutrophic plant species(defined according to Landolt N indicator value), and 3) forage yield, quality, and palatability. A total of 193 vegetation surveys were carried out in 2011 and repeated in 2016. Cows were tracked yearly with Global Positioning System collars to assess their grazing selectivity, and forage Pastoral Value(PV) was computed to evaluate forage yield, quality, and palatability. Five years after RGS implementation, cow selectivity significantly decreased and the preference for the different vegetation communities was more balanced than under CGS. The abundance of meso-and eutrophic species increased, whereas oligotrophic ones decreased. Moreover, the abundance of moderately to highly palatable plant species increased, whereas non-palatable plant species decreased, with a consequent significant enhancement of the PV.
