1Inflammatory mediators and microcirculatory disturbance in acute pancreatitis显示文摘BACKGROUND:Inflammatory mediators are not only initiation factors of acute pancreatitis(AP)but also key factors causing pancreatic hemorrhage and necrosis,which damage important organs such as the heart,brain,liver, kidney and lung.Microcirculatory disturbance in AP has attracted widespread attention.In order to provide a theoretical basis for clinical therapy of AP,it is very important to explore the effect of inflammatory mediators on microcirculatory disturbance in this disease. DATA SOURCES:In this review,the impact of inflammatory mediators on microcirculatory disturbance in AP was reviewed according to the literature,especially the articles indexed in PubMed and books published in China and reports from websites. RESULTS:At present,inflammatory mediation and micro- circulatory disturbance are the two major hypotheses to explain the development of AP.Although experimental studies have shown that inflammatory mediators induce or aggravate microcirculatory disturbance,the clinical application of these findings is still difficult because the inflammatory mediators are diverse and their research is not comprehensive and thorough. 2Experimental study of therapeutic efficacy of Baicalin in rats with severe acute pancreatitis显示文摘AIM: To observe the therapeutic efficacy of Baicalin in rats with severe acute pancreatitis (SAP) and explore its therapeutic mechanisms.METHODS: The SAP rat models were randomly divided into the model control group, Baicalin treatment group, octreotide treatment group and sham operation group. All groups were randomly subdivided into 3 h, 6 h and 12 h groups with 15 rats in each group. The survival, ascites volume and pathological changes of pancreas in all rats were observed at different time points after operation. The plasma amylase content and serum TNF-α, IL-6, malonaldehyde (MDA) and PLA2 contents were also determined. RESULTS: The survival was not obviously different between the treated groups, and was significantly higher in treated groups at 12 h compared to the model control group (P < 0.05, 15 vs 10). The ascites/body weight ratio at 3 h and 6 h was significantly lower in Baicalin treatment group compared to the model control group and octreotide treatment group (P < 0.05, 1.00 vs 2.02 and 1.43 and P < 0.001, 2.29 (1.21) vs 2.70 (0.80) and 2.08 (2.21), respectively). The contents of amylase, TNF-α, IL-6, MDA and PLA2 were significantly lower in the treated groups than in the model control group (P < 0.05, 4342 vs 5303, 5058 vs 6272 in amylase, P < 0.01, 21.90 vs 36.30, 23.80 vs 39.70, 36 vs 54.35 in MDA and 56.25 vs 76.10 in PlA2, or P < 0.001, 65.10 and 47.60 vs 92.15 in TNF-α, 3.03 vs 5.44, 2.88 vs 6.82, 2.83 vs 5.36 in IL-6, respectively). The pathological scores of pancreas in the treated groups were significantly lower than that in the model control group (P < 0.05, 9.00 vs 10.05, 6.00 vs 9.00, 8.00 vs 10.05), but no marked difference was found between the treated groups. CONCLUSION: The Baicalin injection has significant therapeutic effects on SAP rats, its effects are similar to those of octreotide. 3生长抑素联合复方丹参注射液治疗急性胰腺炎临床研究显示文摘目的:观察生长抑素联合复方丹参注射液治疗急性胰腺炎(AP)的临床疗效。方法:采用随机对照分组的临床研究方法,将2002年6月—2006年12月收治的62例AP患者分为对照组和观察组,每组31例。对照组在常规禁食、胃肠减压等治疗的基础上,先静脉注射生长抑素250μg,然后改用3 000μg加生理盐水500 ml以250μg/h的速度持续静脉滴注(静滴),连续7~14 d;观察组在对照组治疗基础上,予复方丹参注射液20 ml溶于生理盐水250 ml中静滴,每日1次,连续7~14 d。观察并比较两组患者的症状,腹部体征消失时间,血、尿淀粉酶,肝功能恢复正常时间,血钙浓度变化和多器官功能障碍综合征(MODS)发生率、病死率。结果:观察组治疗后血淀粉酶显著低于对照组,而血钙水平显著高于对照组,尤以治疗后3 d和5 d更为明显(P均<0.01)。观察组痊愈率(38.7%)和总有效率(90.4%)均明显高于对照组(16.2%和77.4%,P<0.01和P<0.05),而MODS的发生率(19.3%)和病死率(6.5%)均显著低于对照组(45.1%和16.2%,P均<0.05)。结论:生长抑素联合复方丹参注射液治疗AP疗效好于单纯生长抑素治疗。蒋龙元 周莉莉 吴满辉 杨正飞 余涛 黄子通 2007中国中西医结合急救杂志2007,14,4:21
4急性胰腺炎发病机理的研究进展显示文摘目的总结急性胰腺炎发病机理的研究进展。方法分析近年来国内、外有关急性胰腺炎发病机理的文献报道。结果实验数据表明,促炎性细胞因子和趋化因子的合成与释放对于局部损伤和炎性介质的全身播散起着重要作用,同时提示免疫系统,包括淋巴细胞和中性粒细胞的活化,在胰腺炎的发展过程中也起一定作用。另外,胰腺在一次发病之后可以完全恢复,也可以纤维化,星状细胞被认为在胰腺成纤维过程中起着极其重要的作用。结论急性胰腺炎的发病机理非常复杂,在疾病发作后能够决定其严重程度的因子仍然未知。余文 吴伟 张少辉 王尧华 2006中国普外基础与临床杂志2006,13,6:20
5Significance of platelet activating factor receptor expression in pancreatic tissues of rats with severe acute pancreatitis and effects of BN52021显示文摘AIM:To investigate the dynamic changes and significance of platelet activating factor receptor (PAF-R) mRNA and protein in pancreatic tissues of rats with severe acute pancreatitis (SAP) and effects of BN52021 (Ginkgolide B). METHODS:Wistar male rats were randomly assigned to the negative control group (NC group),SAP model group (SAP group),and BN52051-remedy group (BN group),and each of the groups was divided into 6 subgroups at different time points after operation (1 h,2 h,3 h,6 h,12 h,and 24 h) (n=10 in each). PT-PCR and Western blot methods were used to detect PAF-RmRNA and protein expression in pancreatic tissues of rats respectively. Pathological examination of pancreatic tissues was performed and the serum amylase change was detected. RESULTS:Serum amylase and pathological results showed the that SAP model was successfully prepared,BN52021 was able to decrease serum amylase,and the pathological ratings in BN group at 3 h,6 h,and 12 h significantly decreased compared with those in the SAP group (8.85 ± 0.39 vs 5.95 ± 0.19,9.15 ± 0.55 vs 5.55 ± 0.36,10.10 ± 0.65 vs 6.72 ± 0.30,P < 0.05). The result of PAF-mRNA showed dynamic changes in SAP and BN groups,which increased gradually in early stage,reached a peak at 3 h (0.71 ± 0.14 vs 0.54 ± 0.14,0.69 ± 0.13 vs 0.59 ± 0.04,P < 0.05),and decreased gradually later. There were significant differences at each time point except 1 h and 2 h,when compared with those in the NC group (0.71 ± 0.14 or 0.69 ± 0.13 vs 0.47 ± 0.10,0.38 ± 0.08 or 0.59 ± 0.04 vs 0.47 ± 0.09,0.25 ± 0.07 or 0.29 ± 0.05 vs 0.46 ± 0.10,0.20 ± 0.06 or 0.20± 0.04 vs 0.43 ± 0.09,P < 0.05),whereas there was no significant difference between BN and SAP groups at each time point. The result of PAF-R protein showed that the change of PAF-R protein in the SAP group and the BN group was consistent with that of PAF-R mRNA. There were significant differences at each time point except 1 h,when compared with those in the NC group (0.90 ± 0.02 or 0.80 ± 0.05 vs 0.48 ± 0.02,1.69 ± 0.06 or 1.58 ± 0.02 vs 0.48 ± 0.03,1.12 ± 0.10 or 0.98 ± 0.03 vs 0.49 ± 0.09,1.04 ± 0.14 or 0.87 ± 0.02 vs 0.52 ± 0.08,0.97 ± 0.16 or 0.90 ± 0.05 vs 0.49 ± 0.10,P < 0.05),whereas there was no significant difference between the BN group and the SAP group. CONCLUSION:PAF-R plays an important role in occurrence and development of SAP. 6Effect of BN52021 on NFκ-Bp65 expression in pancreatic tissues of rats with severe acute pancreatitis显示文摘AIM: To investigate dynamic changes and significance of expression of NF-κBp65 in pancreatic tissues of rats with severe acute pancreatitis (SAP), as well as BN52021 effects. METHODS: Wistar male rats were randomly divided into negative control group (NC group, n = 60), SAP-model group (SAP group, n = 60), and BN52021-treated group (BN group, n = 60), and each of the above groups was respectively divided into 6 subgroups at different time points after operation (1 h, 2 h, 3 h, 6 h, 12 h, and 24 h) (n = 10). By RT-PCR and Western blot, NF-κBp65 mRNA and its protein expression in pancreatic tissues of rats were detected respectively. RESULTS: The expression of NF-κBp65 mRNA dynamically changed in both SAP groups and BN groups. The mRNA level was higher in SAP groups than NC groups at 2 h, 3 h, 12 h, and 24 h after operation (P < 0.05), higher in BN groups than NC groups at all time points (P < 0.05), and higher in BN groups than SAP group at 1 h (P < 0.05). The NF-κBp65 protein level was higher in SAP groups than NC groups at 1 h, 3 h, and 6 h (P < 0.01), and 2 h, 12 h, and 24 h (P < 0.05), higher in BN groups than NC groups at all time points (P < 0.05), and lower in BN groups than SAP groups at 1 h, 3 h, and 6 h (P < 0.05). CONCLUSION: The expression of NF-κBp65 in pancreatic tissues is dynamically changed and the changes play an important role in pathogenesis of SAP. 9Mechanism and dose-effect of Ginkgolide B on severe acute pancreatitis of rats显示文摘AIM:To determine the optimal dosage and mechanism of Ginkgolide B(BN52021) on severe acute pancreatitis(SAP) of rats.METHODS:Seventy male Wistar rats were randomly divided into seven groups(10 for each group).Shamoperation group(SO),SAP model group(SAP),dimethyl sulfoxide(DMSO) contrast group(DMSO),and groups treated with 2.5 mg/kg BN52021(BN1),5 mg/kg BN52021(BN2),10 mg/kg BN52021(BN3),and 20 μg/kg Sandostatin(SS).The SAP model was established in Wistar rats by injecting 5% sodium taurocholate retrogradely into the common bilio-pancreatic duct.The rats of SO,DMSO and BN52021 were injected with 0.9% NaCl,0.5% DMSO and BN52021 through femoral vein 15 min after the operation.The SS group was injected with Sandostatin subcutaneously.All rats were anaesthetized at 6 h after operation,and venous blood was collected to determine the levels of serum amylase and phospholipase A2(PLA2),and pancreas tissue was harvested and stained.RESULTS:There was no significant difference between the SAP and DMSO groups in serum amylase level,PLA2,ascites and pathologic score,but significant difference was found in SAP/DMSO groups compared with those in SO group(P < 0.05) and the levels of serum amylase,PLA2,ascites,and pathologic score were lower in the BN1,BN2,BN3 and SS groups than in the SAP and DMSO groups(P < 0.05).However,among BN1,BN2,BN3 and SS groups,BN2 had the best effect in decreasing the levels of serum amylase and PLA2(P < 0.05).Expression of platelet activating factor(PAF) receptor(PAFR) mRNA and protein showed no significant difference between the SAP and DMSO groups,or among BN1,BN2,BN3 and SS groups,but there was remarkable difference between SAP/DMSO group and SO group(P < 0.05),and expression of PAFR mRNA and protein was higher in the BN1,BN2,BN3 and SS groups than in the SAP and DMSO groups(P < 0.05).PAFR expression was observed in the nucleus and cytoplasm of pancreatic islet cells in Wistar rats by immunohistochemistry.CONCLUSION:By iv injection,5 mg/kg of BN52021 is the optimal dosage for SAP rats.BN52021 may inhibit the interaction/binding of PAF with PAFR.Run-Li Ji Shi-Hai Xia, Yao Di Wei Xu 2011World Journal of Gastroenterology2011,17,17:9

12低分子肝素对伴高脂血症急性坏死性胰腺炎大鼠PAF、ET-1/NO的影响显示文摘目的:研究低分子肝素(LMWH)治疗伴高脂血症性急性胰腺炎(HL-AP)大鼠微循环障碍的作用.方法:SD大鼠80只,高脂饲料喂养4wk建立高脂血症模型后,随机分成3组:假手术(S)组,n=16;NS对照(N)组,n=32;低分子肝素(L)组,n=32.N组与L组大鼠建立ANP模型,S组大鼠仅开腹翻动胰腺.L组造模后分别于0h,6h,12h及18h开始使用LMWH皮下注射.N、S组同时点同方法用生理盐水对照,24h后抽门静脉检测血小板活化因子(PAF)、内皮素-1(ET-1)/一氧化氮(NO)浓度并行电镜观察胰腺组织微循环结构.结果:L组PAF、ET-1/NO浓度分别为5.9250mmol/L±0.6113mmol/L及3.5368±0.26;N组为7.4059mmol/L±0.4281mmol/L及4.1697±0.08;SO组为4.4950mmol/L±0.2628mmol/L及2.5133±0.20;L组PAF、ET-1/NO浓度均低于N组但高于SO组,有统计学意义;L组中0h及6h开始用LMWH的大鼠PAF、ET-1/NO浓度低于12h及18h组,有统计学意义.电镜观察L组胰腺病变程度轻于N组.结论:高脂血症急性胰腺炎大鼠存在胰腺微循环障碍;LMWH可以改善高脂血症大鼠SAP胰腺微循环障碍,并且早期应用优于晚期应用.王湘英 卢义展 张兴文 骆永富 王强 2012世界华人消化杂志2012,20,10:7

14PAAF与ASPAAF诱导的肠损伤比较的实验研究显示文摘目的探讨胰腺炎相关性腹水(PAAF)与急性化脓性腹膜炎相关性腹水(ASPAAF)诱导大鼠肠损伤的作用及其可能机理。方法以PAAF和ASPAAF分别诱导的SD大鼠肠损伤模型为研究对象,将48只SD大鼠随机均分为3组。对照组腹腔内注射生理盐水8ml/只;PAAF组腹腔内注射PAAF8ml/只,ASPAAF组腹腔内注射ASPAAF8ml/只。分别于注射后6h和12h分批处死大鼠,每时间点8只。测定血清内毒素和TNF-α水平,同时测定小肠组织中Na+-K+-ATP酶活性及TNF-α水平;于光镜下观察肠黏膜损伤情况并作等级比较。结果注射后6h和12h,PAAF组和ASPAAF组血清内毒素和TNF-α水平,小肠组织中TNF-α水平以及肠黏膜损伤程度均较对照组增高(P<0.05),而Na+-K+-ATP酶活性则显著降低(P<0.05);ASPAAF组血清内毒素和TNF-α水平,小肠组织中TNF-α水平以及肠黏膜损伤程度又均较PAAF组增高(P<0.05),Na+-K+-ATP酶活性则显著降低(P<0.05)。结论PAAF及ASPAAF均能引起大鼠肠损伤,且ASPAAF引起的损伤更为严重。刘震 程若川 方向 张跃天 赵海剑 李俊 陈玉祥 2008中国普外基础与临床杂志2008,15,8:6
15血清多种细胞因子同时检测对急性胰腺炎的诊断价值显示文摘目的探讨血清多种细胞因子同时检测对急性胰腺炎(AP)早期诊断和病情评估的价值。方法选择符合AP诊断标准的患者48例,依据CT诊断结果分为轻症急性胰腺炎(MAP)组30例和重症急性胰腺炎(SAP)组18例,另选择30例健康体检者作为对照组。采用酶联免疫吸附试验(EuSA)方法定量检测患者血清中多种细胞因子水平。结果MAP组血清白细胞介素(IL)-1、IL-10和肿瘤坏死因子(TNF).d水平分别为(25.00±1.92)、(59.78±4.51)、(55.31±8.54)ng/L,显著高于对照组的(10.08士2.65)、(1.80±0.66)、(18.72±7.84)ng/L,差异有统计学意义(P〈0.05);SAP组血清IL-1、IL-6、IL.8、TNF-a和血小板活化因子(PAF)水平分别为(93.27±3.98)、(397.84±13.05)、(93.32±3.40)、(181.94±7.54)、(284.53±7.88)ng/L,显著高于MAP组[(25.00±1.92)、(34.12±4.96)、(13.06±1.86)、(55.31±8.54)、(175.25±30.15)ng/L],差异有统计学意义(P〈0.05)。结论血清多种细胞因子的检测对AP的早期诊断和鉴别MAP与SAP具有重要的价值。金培声 任宗海 叶方鹏 应卫星 2013中国医师进修杂志2013,36,25:6

18奥曲肽联合前列地尔治疗急性胰腺炎的Meta分析显示文摘目的:系统评价奥曲肽联合前列地尔治疗急性胰腺炎的安全性及有效性.方法:计算机检索Pub Med、中国生物医学文献数据库、维普、中国知网)、万方数据库,查找有关应用奥曲肽联合前列地尔治疗急性胰腺炎的随机对照试验(randomized controlled trials,RCT),并追索纳入文献的参考文献,检索时限:建库至2015-10.由2名评价者独立选择试验、提取资料和评估方法学质量后,采用Rev Man5.2软件进行Meta分析.结果:最终纳入18个研究,共1277例受试者.Meta分析结果显示:奥曲肽与前列地尔联用在以下方面均优于常规治疗+奥曲肽治疗,且差异有统计学意义:总有效率(RR=1.20,95%CI:1.15-1.26),腹部症状缓解时间[SMD=-2.68,95%CI:-3.11-(-2.25)],腹部体征缓解时间[SMD=-2.54,95%CI:-2.79-(-2.30)],血淀粉酶恢复正常时间[SMD=-1.95,95%CI:-2.15-(-1.76)],并发症发生率(R R=0.30,95%C I:0.13-0.69),平均住院时间[S M D=-8.95,95%CI:-13.61-(-4.29)].结论:本Meta分析结果提示,奥曲肽与前列地尔联用优于单用奥曲肽治疗,为急性胰腺炎的治疗提供了一个新的选择.但本研究纳入研究的样本量小、质量偏低,上述结论尚需要更多高质量的研究验证.曾俊 李林青 程洁 王双 陈欣 姜政 2016世界华人消化杂志2016,24,20:5
19血清脂肪酶、白细胞介素-6及血小板活化因子对内镜逆行胰胆管造影术后胰腺炎早期预测及诊断价值研究显示文摘目的探讨血清脂肪酶(LPS)、白细胞介素-6(IL-6)及血小板活化因子(PAF)对内镜逆行胰胆管造影术后胰腺炎(PEP)的早期临床预测及诊断价值。方法选取自2015年10月至2017年9月马鞍山市人民医院收治的因胆总管结石行经内镜逆行胰胆管造影术(ERCP)及经内镜取石术的120例患者为研究对象,根据ERCP术后淀粉酶情况,有无腹痛、恶心等胰腺炎临床症状,以及是否符合胰腺炎影像学检查等将其分为正常组(n=66)、高淀粉酶血症组(n=30)及PEP组(n=24)。比较术前、ERCP术后4 h及术后24 h时各组患者淀粉酶、LPS、IL-6、PAF含量,以及联合LPS、IL-6、PAF检测对PEP诊断的灵敏度与特异度。结果术后4、24 h,高淀粉酶血症组和PEP组中LPS、IL-6、PAF及淀粉酶均较术前明显升高,且高于同期正常组,组间比较,差异有统计学意义(P<0.05)。ERCP术后4 h,LPS、IL-6、PAF联合检测的买灵敏度、特异度均高于各项指标单独检测(P<0.05)。结论血清LPS、IL-6、PAF在PEP患者术后均明显升高,可作为PEP早期诊断标志物;三者联合检测可有效提高PEP早期诊断的灵敏度和特异度,对指导临床早期治疗具有一定的参考价值。沈颖洲 王旷靖 黄锦 沈琳 唐郡 2018临床军医杂志2018,46,7:5
