| 2 | Current management of spontaneous intracerebral haemorrhage显示文摘Intracerebral haemorrhage(ICH)is the most devastating and disabling type of stroke.Uncontrolled hypertension(HTN)is the most common cause of spontaneous ICH.Recent advances in neuroimaging,organised stroke care,dedicated Neuro-ICUs,medical and surgical management have improved the management of ICH.Early airway protection,control of malignant HTN,urgent reversal of coagulopathy and surgical intervention may increase the chance of survival for patients with severe ICH.Intensive lowering of systolic blood pressure to<140 mm Hg is proven safe by two recent randomised trials.Transfusion of platelets in patients on antiplatelet therapy is not indicated unless the patient is scheduled for surgical evacuation of haematoma.In patients with small haematoma without significant mass effect,there is no indication for routine use of mannitol or hypertonic saline(HTS).However,for patients with large ICH(volume>30 cbic centmetre)or symptomatic perihaematoma oedema,it may be beneficial to keep serum sodium level at 140–150 mEq/L for 7–10 days to minimise oedema expansion and mass effect.Mannitol and HTS can be used emergently for worsening cerebral oedema,elevated intracranial pressure(ICP)or pending herniation.HTS should be administered via central line as continuous infusion(3%)or bolus(23.4%).Ventriculostomy is indicated for patients with severe intraventricular haemorrhage,hydrocephalus or elevated ICP.Patients with large cerebellar or temporal ICH may benefit from emergent haematoma evacuation.It is important to start intermittent pneumatic compression devices at the time of admission and subcutaneous unfractionated heparin in stable patients within 48 hours of admission for prophylaxis of venous thromboembolism.There is no benefit for seizure prophylaxis or aggressive management of fever or hyperglycaemia.Early aggressive comprehensive care may improve survival and functional recovery. | Cyrus K Dastur Wengui Yu | 2017 | Stroke & Vascular Neurology2017,2,1: | 18 |

| 5 | No association between elevated homocysteine levels and carotid atherosclerosis in a rural population in China显示文摘Objectives:We wish to determine if homocysteine(Hcy)is an independent risk factor for carotid atherosclerosis in a rural Chinese population.Methods:2291 individuals(1016 men and 1275 women),aged 64.6±7.4 years,from Lvliang,China participated in this study.Tests performed included carotid artery ultrasound scan and blood analysis,to measure Hcy levels and other blood components.Results:The mean serum Hcy level was 24.7±18.0μmol/L.The overall detection rate of carotid atherosclerotic lesions was 76.3%and the detection rate of plaque was 48.6%.Participants were divided into 4 groups based on their Hcy levels,<14.49,14.49–19.43,19.44–28.30,and≥28.30μmol/L.The relative risk of carotid atherosclerosis for each quartile as compared with the risk for the lowest quartile was estimated as the OR derived from the logistic regression coefficients.After adjusting for age and gender,the OR of carotid atherosclerosis in the third and fourth quartiles of Hcy were 1.219(95%CI 0.922 to 1.612)and 1.156(95%CI 0.859 to 1.555;p>0.05),respectively.After controlling for demographic variables(age,gender,current smoker,fasting blood glucose,low-density lipoprotein and systolic blood pressure)the OR of carotid plaque(s)in the third and fourth quartiles were 1.246(95%CI 0.967 to 1.606)and 1.259(95%CI 0.963 to 1.646;p>0.05).Conclusions:The mean value of Hcy among participants in this study was much higher than that previously reported.However,no significant correlation between elevated Hcy and carotid atherosclerosis was found. | Yang Li Li Wang Wei Zhang Yalan Fang Xiaoyuan Niu | 2016 | Stroke & Vascular Neurology2016,1,4: | 13 |

| 7 | Association of elevated hs-CRP and multiple infarctions with outcomes of minor stroke or TIA: subgroup analysis of CHANCE randomised clinical trial显示文摘Background and purpose The relationship of high-sensitive C-reactive protein(hs-CRP)levels and infarction numbers with the prognosis of stroke is uncertain.This study evaluated the association of different hs-CRP levels and infarction numbers with the prognosis of acute minor ischaemic stroke or transient ischaemic attack(TIA).Methods A subset of 807 patients with both hs-CRP measurement and baseline MRI was included from the Clopidogrel in High-risk Patients with Acute Non-disabling Cerebrovascular Events trial.The primary efficacy outcome was the occurrence of an ischaemic stroke at the 1-year follow-up.Infarction numbers were classified as multiple acute infarctions(MAIs),single acute infarction and no acute infarction(NAI).The association between different hs-CRP levels with different infarction numbers and the risk of any outcome was analysed using multivariable Cox regression models.Results Among the 807 patients,84(10.4%)patients had a recurrent ischaemic stroke within 1 year.After adjustment for conventional confounding factors,patients with both elevated hs-CRP levels and MAIs were associated with approximately 4.7-fold of risk of ischaemic stroke within 1 year(16.7% vs 3.5%,HR 4.68,95% CI 1.54 to 14.23,p=0.007),compared with those with non-elevated hs-CRP levels and NAI.Similar results were observed for the composite events.Conclusions Combined elevated hs-CRP levels and MAIs may increase 1-year stroke risk stratification efficiency in patients with minor ischaemic stroke or TIA compared with using those markers alone,which indicated that the combination of inflammatory and imaging markers might improve the effectiveness of risk stratification concerning minor ischaemic stroke or TIA. | Guangyao Wang Jing Jing Jiejie Li Yuesong Pan Hongyi Yan Xia Meng Xingquan Zhao Liping Liu Hao Li David Z Wang Yongjun Wang Yilong Wang | 2021 | Stroke & Vascular Neurology2021,6,1: | 11 |

| 10 | Inflammation-associated microsatellite alterations:Mechanisms and significance in the prognosis of patients with colorectal cancer显示文摘Microsatellite alterations within genomic DNA frameshift as a result of defective DNA mismatch repair(MMR). About 15% of sporadic colorectal cancers(CRCs) manifest hypermethylation of the DNA MMR gene MLH1, resulting in mono-and di-nucleotide frameshifts to classify it as microsatellite instability-high(MSI-H) and hypermutated, and due to frameshifts at coding microsatellites generating neo-antigens, produce a robust protective immune response that can be enhanced with immune checkpoint blockade. More commonly, approximately 50% of sporadic nonMSI-H CRCs demonstrate frameshifts at di-and tetra-nucleotide microsatellites to classify it as MSIlow/elevated microsatellite alterations at selected tetranucleotide repeats(EMAST) as a result of functional somatic inactivation of the DNA MMR protein MSH3 via a nuclear-to-cytosolic displacement. The trigger for MSH3 displacement appears to be inflammation and/or oxidative stress, and unlike MSI-H CRC patients, patients with MSI-L/EMAST CRCs show poor prognosis. These inflammatory-associated microsatellite alterations are a consequence of the local tumor microenvironment, and in theory, if the microenvironment is manipulated to lower inflammation, the microsatellite alterations and MSH3 dysfunction should be corrected. | 11 | Creep damage characterization of UNS N10003 alloy based on a numerical simulation using the Norton creep law and Kachanov–Rabotnov creep damage model显示文摘The calculation of inelastic creep damage is important for the structural integrity evaluation of the elevated temperature structure in a thorium molten salt reactor(TMSR). However, a creep damage theory model and numerical simulation method have not been proposed for the key materials(UNS N10003 alloy) in the TMSR. In this study, creep damage characterization of UNS N10003 alloy is investigated using the Norton creep law and Kachanov–Rabotnov(K–R) creep damage model. First, the creep experimental data of the UNS N10003 alloy at 650 °C were adopted to fit the material constants of the two models. Then, the creep damage behavior of the UNS N10003 alloy was analyzed and discussed under uniaxial and multi-axial stress states. The results indicated that the K–R creep damage model is more suitable for the UNS N10003 alloy than the Norton model. Finally, the numerical simulation method was developed by a user-defined UMAT subroutine and subsequently verified through a finite element analysis(FEA). The FEA results were in agreement with the theoretical solutions. | 12 | Glycogenic hepatopathy:A narrative review显示文摘Glycogenic hepatopathy(GH) is a rare complication of the poorly controlled diabetes mellitus characterized by the transient liver dysfunction with elevated liver enzymes and associated hepatomegaly caused by the reversible accumulation of excess glycogen in the hepatocytes. It is predominantly seen in patients with longstanding type 1 diabetes mellitus and rarely reported in association with type 2 diabetes mellitus. Although it was first observed in the pediatric population, since then, it has been reported in adolescents and adults with or without ketoacidosis. The association of GH with hyperglycemia in diabetes has not been well established. One of the essential elements in the pathophysiology of development of GH is the wide fluctuation in both glucose and insulin levels. GH and non-alcoholic fatty liver disease(NAFLD) are clinically indistinguishable, and latter is more prevalent in diabetic patients and can progress to advanced liver disease and cirrhosis. Gradient dual-echo MRI can distinguish GH from NAFLD; however, GH can reliably be diagnosed only by liver biopsy. Adequate glycemic control can result in complete remission of clinical, laboratory and histological abnormalities. There has been a recent report of varying degree of liver fibrosis identified in patients with GH. Future studies are required to understand the biochemical defects underlying GH, noninvasive, rapid diagnostic tests for GH, and to assess the consequence of the fibrosis identified as severe fibrosis may progress to cirrhosis. Awareness of this entity in the medical community including specialists is low. | 16 | A new approach on necking constitutive relationships of ductile materials at elevated temperatures显示文摘A new method is presented to determine the full-range, uniaxial constitutive relationship of materials by tensile tests on funnel specimens with small curvature radius and finite element analysis(FEA). An iteration method using FEA APDL(ANSYS parametric design language) programming has been developed to approach the necking constitutive relationship of materials. Test results from SAE 304 stainless steel at room temperature show that simulated load vs displacement curve,diameter at funnel root vs displacement curve, and funnel deformation contours are close to modeled results. | 17 | Risk factors of hyperprolactinemia induced by risperidone and olanzapine and their correlations with plasma glucose and lipids显示文摘Background Hyperprolactinemia is a common adverse reaction in patients with schizophrenia who take antipsychotic drugs;it often leads to treatment noncompliance in patients and has an adverse effect on their prognosis.Aims This study aimed to explore the risk factors of elevated prolactin(PRL)caused by risperidone(RIS)and olanzapine(OLZ)and the relationship between PRL and fasting plasma glucose and lipids.Methods Patients with schizophrenia were divided into two groups:264 patients who were taking RIS and 175 patients who were taking OLZ.These two groups were further divided according to serum PRL levels:an elevated PRL group(>30ng/mL)and a normal PRL group(PRL<30ng/mL).The demographics,medication dosage,fasting plasma glucose,total cholesterol and triglycerides were compared in the two groups.Logistic regression analysis was performed to explore the risk factors of elevated PRL levels.Results Compared with the OLZ group,the RIS group had a greater number of patients with elevated PRL(155/264 vs 58/175).Either the RIS or the OLZ group,the proportion of elevated PRL was greater in female patients(RIS:x^2=6.76,p=0.009;OLZ:x^2=12.98,p<0.001)and with higher doses of the related drugs(RIS:U=-3.73,p<0.001;OLZ:U=-2.31,p=0.021).In patients taking RIS,the elevated PRL subgroup took the drug for a longer period(U=-2.76,p=0.006)and had lower triglyceride levels(U=2.76,p=0.006).In patients taking OLZ,the elevated PRL subgroup had lower fasting plasma glucose levels(U=2.29,p=0.022).Logistic regression analysis showed that gender,dose and fasting glucose levels were significantly associated with elevated PRL levels(RIS:p=0.001,OLZ:p<0.001;RIS:p<0.001;OLZ:p=0.003;RIS:p=0.020,OLZ:p=0.001,respectively).Conclusion Compared with OLZ,RIS had a greater effect on PRL in patients with schizophrenia,and in patients with schizophrenia taking RIS or OLZ,gender and dose were significantly correlated with the PRL value.Moreover,the plasma glucose level of the group with elevated PRL was lower than that of the group with normal PRL.The results also showed that high serum PRL may be associated with a favourable glucose metabolic profile in patients with schizophrenia taking RIS or OLZ.Further studies are warranted to confirm this association. | Sidi He Wen Juan Yu Xiaoliang Wang Lei Zhang Nan Zhao Guanjun Li Yi Feng Shen Huafang Li | 2020 | General Psychiatry2020,33,4: | 5 |
| 18 | Diabetes fuels periodontal lesions via GLUT1-driven macrophage inflammaging显示文摘Hyperglycemia induces chronic low-grade inflammation(inflammaging),which is a newly identified contributor to diabetes-related tissue lesions,including the inflammatory bone loss in periodontitis.It is also a secondary senescent pattern mediated by an increased burden of senescent cells and senescence-associated secretory phenotype(SASP).Macrophage is a key SASPspreading cell and may contribute to the maintenance of SASP response in the periodontal microenvironment.Using a transgenic diabetic model(BLKS/J-Lepr^(db)/lepr^(db)mice)we identified striking senescence of the periodontium in young(18-wk)-diabetic mice accompanied by amassed p16^(+)-macrophages and enhanced early SASP response.Exposed to high glucose in vitro,bone marrow-derived macrophage(BMDM)revealed a strong GLUT1 m RNA response driving the elevated-glucose uptake.GLUT1 is a representative and facilitative glucose transporter in macrophages with potential roles in hyperglycemia-induced inflammation.In this study,both GLUT1 and the downstream GTPase Rheb expression upregulated in the gingiva of diabetic mice with impaired condition.Furthermore,SASP release and p16/p21 signaling were proven to be triggered by m TOR phosphorylation in BMDM and antagonized by restricting glucose uptake in GLUT1^(-/-)BMDM.Taken together,our findings suggest that elevated-GLUT1 sensor responded to high glucose is important for macrophage senescence and SASP response,generated as a result of hyperglycemia,and it is a potential molecular mechanism for the exacerbation of periodontitis in diabetes. | Qian Wang Lulingxiao Nie Pengfei Zhao Xinyi Zhou Yi Ding Qianming Chen Qi Wang | 2021 | International Journal of Oral Science2021,13,3: | 5 |
