1Modern approach to cholecysto-choledocholithiasis显示文摘Gallstones and common bile duct calculi are found to be associated in 8%-20% of patients, leading to possible life-threatening complications, such as acute biliary pancreatitis, jaundice and cholangitis. The gold standard of care for gallbladder calculi and isolated common bile duct stones is represented by laparoscopic cholecystectomy and endoscopic retrograde cholangiopancreatography, respectively, while a debate still exists regarding how to treat the two diseases at the same time. Many therapeutic options are also available when the two conditions are associated, including many different types of treatment, which local professionals often administer. The need to limit maximum discomfort and risks for the patients, combined with the economic pressure of reducing costs and utilizing resources, favors single-step procedures. However, a multitude of data fail to strongly demonstrate the superiority of any technique(including a two or multi-step approach), while rigorous clinical trials that include so many different types of treatment are still lacking, and it is most likely unrealistic to conduct them in the future. 4Difficult biliary cannulation: Historical perspective, practical updates, and guide for the endoscopist显示文摘Despite improvements in endoscopic technologies and accessories, development of advanced endoscopy fellowship programs, and advances in ancillary imaging techniques, biliary cannulation in endoscopic retrograde cholangiopancreatography(ERCP) can still be unsuccessful in up to 20% of patients, even in referral centers. Once cannulation has been deemed to be difficult, the risk of post-ERCP pancreatitis and technical failure inherently increases. A number of factors, including endoscopist experience and patient anatomy, have been associated with difficult biliary cannulation, but predicting a case of difficult cannulation a priori is often not possible. Numerous techniques such as pancreatic guidewire and stenting, early pre-cut, and rendezvous may be employed when standard approaches fail. Data regarding the rate of success and adverse events of these techniques have been variable, though most studies suggest that pancreatic duct stenting generally reduces the rate of post-ERCP pancreatitis in instances of difficult biliary cannulation. 5A preliminary study on dead geostationary satellite removal显示文摘The collision between satellites IRIDIUM 33 and COSMOS 2251 indicated that the clash of two on-orbit satellites was becoming an inevitable reality. Our calculation with the two-line orbit element by NORAD showed that some two geostationary satellites had approached very close in July 2009. Therefore, more attention should be given to avoid such collisions. This paper analyzes the orbital long-term variation of a dead satellite drifting in the geostationary orbit. Also, the negative effects posed by dead satellites upon the on-orbit operational geostationary satellites are studied. Then the paper proposes a novel idea to launch a satellite sweeper whose purpose is to collect the on-orbit dead satellites and help them de-orbit to a 'graveyard'. The satellite sweeper consists of a parent satellite and a child satellite. The child satellite collects a dead satellite and transfers it to a higher orbit. The parent satellite stationed in the geostationary orbit is in charge of refueling the child satellite. The strategy of maneuver and rendezvous is presented and a series of formulas are derived. The analysis results show that our method to clean the geostationary orbital zone is practical and fuel-saving. 6endoscopic ultrasonography-guided biliary drainage:Who,when,which,and how?显示文摘Both endoscopic ultrasonography(EUS)-guided choledochoduodenostomy( EUS- CDS) and EUS-guided hepaticogastrostomy(EUS-HGS) are relatively well established as alternatives to percutaneous transhepatic biliary drainage(PTBD). Both EUSCDS and EUS-HGS have high technical and clinical success rates(more than 90%) in high-volume centers. Complications for both procedures remain high at 10%-30%. Procedures performed by endoscopists who have done fewer than 20 cases sometimes result in severe or fatal complications. When learning EUSguided biliary drainage(EUS-BD), we recommend a mentor's supervision during at least the first 20 cases. For inoperable malignant lower biliary obstruction, a skillful endoscopist should perform EUS-BD before EUS-guided rendezvous technique(EUS-RV) and PTBD. We should be select EUS-BD for patients having altered anatomy from malignant tumors before balloon-enteroscope-assisted endoscopic retrograde cholangiopancreatography, EUS-RV, and PTBD. If both EUS-CDS and EUS-HGS are available, we should select EUS-CDS, according to published data. 7Comprehensive review on EUS-guided biliary drainage显示文摘Feasibility of endoscopic retrograde cholangiopancreatography(ERCP) for biliary drainage is not always applicable due to anatomical alterations or to inability to access the papilla. Percutaneous transhepatic biliary drainage has always been considered the only alternative for this indication. However,endoscopic ultrasonography-guided biliary drainage represents a valid option to replace percutaneous transhepatic biliary drainage when ERCP fails. According to the access site to the biliary tree,two kinds of approaches may be described: the intrahepatic and the extrahepatic. Endoscopic ultrasonography-guided rendezvous transpapillary drainage is performed where the second portion of the duodenum is easily reached but conventional ERCP fails. The recent introduction of self-expandable metal stents and lumen-apposing metal stents has improved this field. However,the role of the latter is still controversial. Echoendoscopic transmural biliary drainage can be challenging with potential severe adverse events. 8Optimal impulsive ellipse-to-circle coplanar rendezvous显示文摘The problem of ellipse-to-circle coplanar rendezvous with chaser spacecraft in low eccentricity was investigated in this paper.With reference frame established in the centroid of the target spacecraft,the process of ellipse-to-circle coplanar rendezvous was described by the relative equations based on cy-lindrical reference frame,and then the solutions and distributions of optimal rendezvous models of a kind of close ellipse-to-circle coplanar rendezvous were provided.The simulation results showed that the guidance law based on the optimal rendezvous model in this research has good performance,and that the distributions of optimal rendezvous models of ellipse-to-circle coplanar rendezvous with the chaser spacecraft in low eccentricity are similar,albeit with slight difference,to those of rendezvous between close circular orbits.The work in this paper is a useful extension to Prussing's optimal ren-dezvous theory between close circular orbits.CHEN ChangQing XIE YongChun 2009Science China(Technological Sciences)2009,52,5:2
9Electrohydraulic lithotripsy and rendezvous nasal endoscopic cholangiography for common bile duct stone: A case report显示文摘BACKGROUND In patients with large stones in the common bile duct(CBD),advanced treatment modalities are generally needed.Here,we present an interesting case of a huge CBD stone treated with electrohydraulic lithotripsy(EHL)by the percutaneous approach and rendezvous endoscopic retrograde cholangiography(ERC)using a nasal endoscope.CASE SUMMARY A 91-year-old woman underwent ERC for a symptomatic large CBD stone with a diameter of 50 mm.She was referred to our institution after the failure of lithotomy by ERC,and after undergoing percutaneous transhepatic biliary drainage.We attempted to fragment the stone by transhepatic cholangioscopy using EHL.However,the stones were too large and partly soft clay-like for lithotripsy.Next,we attempted lithotomy with ERC and cholangioscopy by the rendezvous technique using a nasal endoscope and achieved complete lithotomy.No complication was observed at the end of this procedure.CONCLUSION Cholangioscopy by rendezvous technique using a nasal endoscope is a feasible and safe endoscopic method for removing huge CBD stones.Koichi Kimura Kensuke Kudo Tomoharu Yoshizumi Takeshi Kurihara Shohei Yoshiya Yohei Mano Kazuki Takeishi Shinji Itoh Noboru Harada Toru Ikegami Tetsuo Ikeda 2019World Journal of Clinical Cases2019,7,10:2
10Saturated Adaptive Output-Constrained Control of Cooperative Spacecraft Rendezvous and Docking显示文摘This paper investigates the robust relative pose control for spacecraft rendezvous and docking with constrained relative pose and saturated control inputs.A barrier Lyapunov function is used to ensure the constraints of states,so that the computational singularity of the inverse matrix in control command can be avoided,while a linear auxiliary system is introduced to handle with the adverse effect of actuator saturation.The tuning rules for designing parameters in control command and auxiliary system are derived based on the stability analysis of the closed-loop system.It is proved that all closed-loop signals always keep bounded,the prescribed constraints of relative pose tracking errors are never violated,and the pose tracking errors ultimately converge to small neighborhoods of zero.Simulation experiments validate the performance of the proposed robust saturated control strategy.Liang Sun 2019IEEE/CAA Journal of Automatica Sinica2019,6,6:2
11Endoscopic salvage therapy after failed biliary cannulation using advanced techniques:A concise review显示文摘Therapeutic endoscopic retrograde cholangiopancreatography(ERCP)begins with successful biliary cannulation.However,it is not always be successful.The failure of the initial ERCP is attributed to two main aspects:the papilla/biliary orifice is endoscopically accessible,or it is inaccessible.When the papilla/biliary orifice is accessible,bile duct cannulation failure can occur even with advanced cannulation techniques,including double guidewire techniques,transpancreatic sphincterotomy,needle-knife precut papillotomy,or fistulotomy.There is currently no consensus on the next steps of treatment in this setting.Therefore,this review aims to propose and discuss potential endoscopic options for patients who have failed ERCP due to difficult bile duct cannulation.These options include interval ERCP,percutaneous-transhepatic-endoscopic rendezvous procedures(PTE-RV),and endoscopic ultrasound-assisted rendezvous procedures(EUS-RV).The overall success rate for interval ERCP was 76.3%(68%-79% between studies),and the overall adverse event rate was 7.5%(0-15.9% between studies).The overall success rate for PTE-RV was 88.7%(80.4%-100%between studies),and the overall adverse event rate was 13.2%(4.9%-19.2% between studies).For EUS-RV,the overall success rate was 82%-86.1%,and the overall adverse event rate was 13%-15.6%.Because interval ERCP has an acceptably high success rate and lower adverse event rate and does not require additional expertise,facilities,or other specialists,it can be considered the first choice for salvage therapy.EUS-RV can also be considered if local experts are available.For patients in urgent need of biliary drainage,PTE-RV should be considered.Yung-Kuan Tsou Kuang-Tse Pan Mu Hsien Lee Cheng-Hui Lin 2022World Journal of Gastroenterology2022,28,29:1

13Two-dimensional launch window method to search for launch opportunities of interplanetary missions显示文摘Accurate and highly efficient approaches to obtain mission opportunities are still the goals of mission planners of interplanetary explorations.The search for launch opportunities not only determines the specified launch window of the mission but also presents the performance requirements for the interplanetary probe and its launch vehicle.An effective method, namely the two-dimensional launch window method, is developed from a completely new perspective to determine all the launch opportunities of the mission in this research.For a fixed launch time,the method to determine all the time windows in the dimension of Time-of-Flight(TOF) is firstly proposed and these time windows represent all the launch opportunities for the given launch time.And then, the two-dimensional launch window method is proposed, which computes the time windows in both the launch time and TOF dimensions to achieve all launch opportunities of the mission.Numerical examples are provided to demonstrate the accuracy and high efficiency of the method.Compared with the widely-used pock-chop plot method, the proposed method reduces the computational time by two orders of magnitude for the same search precision, and thus is especially suitable for the cases involving rapid, high-precision, and/or large-scale searches for mission opportunities.Junhong DUAN Yuefeng LIU 2020Chinese Journal of Aeronautics2020,33,3:1
14Surgical treatment of a rare complication after endoscopic stent placement for anastomotic leakage after esophageal resection显示文摘The best approach to achieve cure in esophageal cancer is a combination of chemo-radiation and surgery. However, complications occur in half of patients. The current report, reports a rare but severe complication: Complete obstruction of the esophagus, induced by preoperative chemo-radiation therapy. Normally, strictures are treated by repeated dilatations, however, in case of complete obstruction, the perforation rate of standard blind anterograde wire access and dilation is severely increased. In order to minimize the risk of perforations, the rendezvous technique was introduced. This technique involves an anterograde approach in combination with a retrograde approach in order to open and dilatate the esophagus. While technical success rates between 83% and 100% have been reported in literature, data on clinical outcomes are scarcer. The limited amount of studies available claim that success was achieved in almost half of patients. 15Combined radiologic and endoscopic treatment(using the “rendezvous technique”) of a biliary fistula following left hepatectomy显示文摘Despite the ongoing decrease in the frequency of complications after hepatectomy, biliary fistulas still occur and are associated with high morbidity and mortality rates. Here, we report on an unusual technique for managing biliary fistula following left hepatectomy in a patient in whom the right posterior segmental duct joined the left hepatic duct. The biliary fistula was treated with a combined radiologic and endoscopic procedure based on the 'rendezvous technique'.
