| 1 | Robotic surgery in pediatric urology显示文摘While robotic surgery has shown clear utility and advantages in the adult population,its role in pediatrics remains controversial.Pediatric-sized robotic instruments and equipment are not readily available yet,so certain modifications can be made in order to make robotic surgery successful in children.While the cost of robotic surgery remains high compared to open procedures,patients experience greater satisfaction and quality of life with robotic surgery.Robotic pyeloplasty is a standard of care in older children,and has even been performed in infants and re-do surgery.Other robotic procedures performed in children include heminephroureterectomy,ureteroureterostomy,ureteral reimplantation,urachal cyst excision,bladder diverticulectomy,and bladder reconstructive procedures such as augmentation,appendicovesicostomy,antegrade continence enema,bladder neck reconstruction and sling,as well as other procedures.Robotic surgery has also been used in oncologic cases such as partial nephrectomy and retroperitoneal lymph node dissection.Future improvements in technology with production of pediatric-sized robotic instruments,along with increases in robotictrained pediatric urologists and surgeon experience along each’s learning curve,will help to further advance the field of robotic surgery in pediatric urology. | Adam Howe Zachary Kozel Lane Palmer | 2017 | Asian Journal of Urology2017,4,1: | 6 |

| 3 | Natural orifice transluminal endoscopic surgery in urology: The Chinese experience显示文摘Objective:To describe the Chinese experience of natural orifice transluminal endoscopic surgery(NOTES)in urology.Methods:From December 2008 to May 2017,35 animal experiments and 305 clinical surgeries of NOTES or natural orifices specimen extractions(NOSE)were performed in China.The animal experiments included five kidney biopsies,24 nephrectomies and six partial nephrectomies.The clinical surgeries included 12 transvaginal NOSE(TV-NOSE),266 hybrid transvaginal NOTES(TV-NOTES)and 27 pure TV-NOTES.The TV-NOSE procedure was performed in five transumbilical laparoendoscopic single-site(U-LESS)nephrectomies,four suprapubic-assisted laparoendoscopic single-site surgery(SA-LESS)nephroureterectomies,and three laparoscopic radical cystectomies.The hybrid TV-NOTES procedure included 210 nephrectomies,31 adrenalectomies,eight nephroureterectomies,13 partial nephrectomies,and four heminephrectomies.The pure TV-NOTES procedure included five renal cyst decortications and 22 nephrectomies.Results:A total of 29 animal experiments were successfully performed.One partial nephrectomy was converted to standard laparoscopic surgery.Two kidney biopsies and two nephrectomies were unsuccessful.A total of 297 clinical surgeries were successfully performed.Six patients who underwent hybrid TV-NOTES were converted to open surgery.Two patients who underwent pure TV-NOTES were converted to SA-LESS.There were 22 major complications,16 occurred intraoperatively and six postoperatively.The mean visual analog score(VAS)of 48 h after the operation was 2.5 points in TV-NOSE,2.3 points in hybrid TV-NOTES and 1.7 points in pure TV-NOTES.The mean follow-up of 50.6(3.0-87.0)months showed that all patients were in good condition.The umbilicus scars were nearly invisible in TV-NOSE and hybrid TV-NOTES.The vaginal incision healed well.Conclusions:TV-NOSE and TV-NOTES are feasible,safe,and effective with little injury,low pain,fast recovery,and good cosmetic outcomes in properly selected patients.They are worth consideration for urological clinical practice. | Xiaofeng Zou Guoxi Zhang Tianpeng Xie Yuanhu Yuan Rihai Xiao Gengqing Wu Xiaoning Wang Hui Xu Folin Liu Yuting Wu Yunfeng Liao Quanliang Liu Yinghao Sun Bo Yang Linhui Wang Chuanliang Xu Xiaofeng Gao | 2020 | Asian Journal of Urology2020,7,1: | 5 |
| 4 | Intraoperative laparoscopic complications for urological cancer procedures显示文摘AIM: To structure the rate of intraoperative complications that requires an intraoperative or perioperative resolution. METHODS: We perform a literature review of Medline database. The research was focused on intraoperative laparoscopic procedures inside the field of urological oncology. General rate of perioperative complications in laparoscopic urologic surgery is described to be around 12.4%. Most of the manuscripts published do not make differences between pure intraoperative, intraoperative with postoperative consequences and postoperative complications. RESULTS: We expose a narrative statement of complications, possible solutions and possible preventions for most frequent retroperitoneal and pelvic laparoscopic surgery. We expose the results with the following order: retroperitoneal laparoscopic surgery(radical nephrectomy, partial nephrectomy, nephroureterectomy and adrenalectomy) and pelvic laparoscopic surgery(radical prostatectomy and radical cystectomy).CONCLUSION: Intraoperative complications vary from different series. | 5 | Comprehensive urodynamics: Being devoted to clinical urologic practice显示文摘As a combined electrophysiological system for evaluating the lower urinary tract(LUT), comprehensive urodynamics(UDS) aims at duplicating patient's micturition process, either normal or abnormal, and further seeking for possible causative origin, either neurogenic or non-neurogenic, in order to guide treatment. Through thorough analysis, some so-called cutoff values, for example, bladder outlet obstruction(BOO) degree or dyssynergic degree between the detrusor and sphincter, could be gained; however, in most cases, their qualitative description, such as stress urinary incontinence, idiopathic detrusor underactivity(DUA), detrusor overactivity(IDO), low compliance, and idiopathic sphincter overactivity(ISO), is more preferable and important. In aged neurologically intact male patients with symptoms of the LUT(LUTS) including benign prostatic hyperplasia, a combined UDS system, which coupled BOO with compliance, was constructed. The patients may be categorized into one of the seven subgroups, including equivocal or mild BOO with sphincter synergia with or without IDO(pattern A), equivocal or mild BOO with ISO(B), classic BOO with sphincter synergia(C) or ISO(D), BOO with only low compliance(E), BOO with both DUA and low compliance(F), and potential BOO with DUA(G). | 6 | Professionalism and patient education in urologic surgery显示文摘Medical professionalism provides the guidelines that govern the patient-physician relationship. This implicit contract requires that patients be informed before making decisions regarding their medical care. Educating patients about diagnostic and treatment decisions is critical to an informed decision-making process. Shared decision-making is a recent paradigm shift in patient education that allows patients to make decisions based both on the counsel of their physicians and according to their own preferences and values. This approach moves away from previous models that focused on physicians or third-party payers as the arbiters of diagnostic and treatment choices. Urologic surgeons have been at the forefront of shared decision-making research and continue to promote this concept in the most recent American Urological Association Guideline on Detection of Prostate Cancer. Unfortunately, the feefor-service financial structure that predominates in the United States' health care system provides a disincentive for shared decision-making. By promoting patient volume rather than time spent with patients, this system rewards physicians who spend less time educating patients about diagnostic and treatment options. | 7 | Laparoscopic single site surgery: Experience in pediatric urology显示文摘Laparoendoscopic single-site surgery(LESS) has been developed to benefit patients by enabling surgeons to perform scarless surgery. In this review we aimed to summarize and critically analyze the available evidence on the current status and future prospects for LESS in pediatric urology, with special emphasis on our experience with LESS in children. The clinical data available clearly demonstrate that LESS can safely and effectively be performed in a variety of pediatric urology settings. As clinical experience increases, expanding indications are expected to be documented and the efficacy of the procedure to improve. So far, the quality of evidence of all available studies remains low; mostly being small case series or case-control studies from selected centers. Thus, the only objective benefit of LESS remains improved cosmetic outcome. Prospective randomized studies are awaited to determine which LESS procedures will be established and which are unlikely to stand the test of time. Technological advances hold promise to minimize the challenging technical nature of scarless surgery. | 8 | Reconstructive surgery in Peyronie's disease: What's new?显示文摘Surgical treatment of Peyronie's disease(PD) is still a challenge and a gold standard approach does not exist; however the main goal is to straight penile shaft, and to restore penetrative and coital capacity. The less invasive approach aims to correct curvature without intervening directly on the fibrous plaque while the more complex 'corporoplasty' applies specific geometric criteria and uses different autologous and heterologous grafts. Each approach has its pros and cons and decisionmaking should be tailored to the individual patient's expectations. Other surgical options include different use of patches to cover the tunica albuginea defect, with the choice depending on the surgeon's personal experience. Despite the wide range of autologous(buccal mucosa, vein, dermis, etc.) and heterologous grafts(bovine pericardium, swine intestinal submucosa, porcine dermis, etc.) none currently represents the real 'gold standard' because the data are extremely variable and frequently not representative. Several factors seem to favor buccal mucosa grafts over inert biocompatible materials: as vital tissue, buccal mucosa tends to heal rapidly, immediately integrating with the surrounding albuginea tissue. This translates into a more rapid resumption of spontaneous erections(after 3/4 d) and sexual activity and into a reduced risk of curvature relapse and erectile dysfunction after surgery. Another advantage of the buccal mucosa graft is its low cost. In conclusion, despite the recent development of some exciting new surgical techniques we are still unable to deliver a definitive take-home message about reconstructive surgery in PD because the majority of the studies reported insufficient data. | 9 | Urological manifestations and treatment of the primary systemic vasculitides显示文摘The primary systemic vasculitides(PSV) are a group of rare inflammatory disorders affecting blood vessels of varying size and multiple organs. Urological manifestations of PSV are uncommon. Testicular vasculitis is the most commonly reported finding and is associated with Polyarteritis Nodosa(PAN), Henoch-Sch?nleinPurpura(HSP), anti-neutrophil cytoplasm antibody associated Vasculitides(AAV), Giant Cell Arteritis(GCA) and Kawasaki disease. Prostatic vasculitis has been reported in association with GCA and AAV. Ureteric involvement has been noted in PAN, HSP and AAV. Other urogenital manifestations of PSV include genital ulceration and bladder dysfunction in Beh?ets Disease and haematuria which is commonly seen in many of the PSV. Finally, therapies used to treat the PSV, especially cyclophosphamide, are associated with urological sideeffects including haemorrhagic cystitis and urothelial malignancy. The aim of this review is to examine how the urological system is involved in the PSV. Each PSV is examined in turn, with a brief clinical description of the disease followed by a description of the urological manifestations and management. Identification of urological manifestations of PSV is important as in many cases symptoms may improve with immunosuppressive therapy, avoiding the need for invasive surgery. | 12 | Urological and genital surgery in ancient Egypt显示文摘Background:Ancient Egypt might be considered the cradle of medicine.The modern literature is somewhat too enthusiastic regarding the procedures given an Egyptian origin.The aim of the current paper is to briefly analyze the claims regarding urological and genital surgery in Egypt,in order to decide what the Egyptian actually do,and what has incorrectly been ascribed to them.Methods:The original sources as well as the modern literature was reviewed regarding surgery in ancient Egypt.Results:There is only one source indicating a urological procedure for medical indications in the Egyptian material.The Ebers papyrus can be interpreted as describing a surgical treatment for hydrocele.The sources are more abundant regarding male circumcision,while female circumcision is mainly documented from a later period.The suggestions that castration and lithotomy were performed are based on a lack of understanding of the sources.Conclusion:The ancient Egyptians did possibly treat hydrocele with a minor surgical procedure,but there are no indications in the sources that other urological procedures were performed.Circumcisions were common,but were not performed on a medical indication.These findings are in line with the general level of Egyptian surgery. | Patric Blomstedt | 2023 | History & Philosophy of Medicine2023,5,1: | 0 |

We report the occurrence of a classical seminoma in a 92-year-old man,who is currently the oldest patient with this histology reported in literature. He presented with a painful,swollen testis. Scrotal ultrasound scan revealed a testicular mass. A left inguinal orchidectomy was carried out and histological examination confirmed the diagnosis of a classical seminoma. Further staging by computerised tomography revealed pulmonary lesions suspicious of metastases. | 19 | A systematic review of dedicated models of care for emergency urological patients显示文摘Objective:To systematically evaluate the spectrum of models providing dedicated resources for emergency urological patients(EUPs).Methods:A search of Cochrane,Embase,Medline and grey literature from January 1,2000 to March 26,2019 was performed using methods pre-published on PROSPERO.Reporting followed Preferred Reporting Items for Systematic Review and meta-analysis guidelines.Eligible studies were articles or abstracts published in English describing dedicated models of care for EUPs,which reported at least one secondary outcome.Studies were excluded if they examined pathways dedicated only to single presentations,such as torsion,or outpatient solutions,such as rapid access clinics.The primary outcome was the spectrum of models.Secondary outcomes were time-to-theatre,length of stay,complications and cost.Results:Seven studies were identified,totalling 487 patients.Six studies were conference abstracts,while one study was of full-text length but published in grey literature.Four distinct models were described.These included consultant urologists allocated solely to the care of EUPs(“Acute Urological Unit”)or dedicated registrars or operating theatres(“Hybrid structures”).In some services,EUPs bypassed emergency department assessment and were referred directly to urology(“Urological Assessment Unit”)or were managed by other dedicated means.Allocating services to EUPs was associated with reduced time-to-theatre,length of stay and hospital cost,and improved supervision of junior medical staff.Conclusion:Multiple dedicated models of care exist for EUPs.Low-level evidence suggests these may improve outcomes for patients,staff and hospitals.Higher quality studies are required to explore patient outcomes and minimum requirements to establish these models. | Ned Kinnear Matheesha Herath Dylan Barnett Derek Hennessey Christopher Dobbins Tarik Sammour James Moore | 2021 | Asian Journal of Urology2021,8,3: | 0 |
