| 1 | China Stroke Statistics 2019:A Report From the National Center for Healthcare Quality Management in Neurological Diseases,China National Clinical Research Center for Neurological Diseases,the Chinese Stroke Association,National Center for Chronic and Non-communicable Disease Control and Prevention,Chinese Center for Disease Control and Prevention and Institute for Global Neuroscience and Stroke Collaborations显示文摘China faces the greatest challenge from stroke in the world.The death rate for cerebrovascular diseases in China was 149.49 per 100000,accounting for 1.57 million deaths in 2018.It ranked third among the leading causes of death behind malignant tumours and heart disease.The age-standardised prevalence and incidence of stroke in 2013 were 1114.8 per 100000 population and 246.8 per 100000 person-years,respectively.According to the Global Burden of Disease Study 2017,the years of life lost(YLLs)per 100000 population for stroke increased by 14.6%;YLLs due to stroke rose from third highest among all causes in 1990 to the highest in 2017.The absolute numbers and rates per 100000 population for all-age disability-adjusted life years(DALYs)for stroke increased substantially between 1990 and 2017,and stroke was the leading cause of all-age DALYs in 2017.The main contributors to cerebrovascular diseases include behavioural risk factors(smoking and alcohol use)and pre-existing conditions(hypertension,diabetes mellitus,dyslipidaemia and atrial fibrillation(AF)).The most prevalent risk factors among stroke survivors were hypertension(63.0%-84.2%)and smoking(31.7%-47.6%).The least prevalent was AF(2.7%-7.4%).The prevalences for major risk factors for stroke are high and most have increased over time.Based on the latest national epidemiological data,26.6%of adults aged≥15 years(307.6 million adults)smoked tobacco products.For those aged≥18 years,age-adjusted prevalence of hypertension was 25.2%;adjusted prevalence of hypercholesterolaemia was 5.8%;and the standardised prevalence of diabetes was 10.9%.For those aged≥40 years,the standardised prevalence of AF was 2.31%.Data from the Hospital Quality Monitoring System showed that 3010204 inpatients with stroke were admitted to 1853 tertiary care hospitals during 2018.Of those,2466785(81.9%)were ischaemic strokes(ISs);447609(14.9%)were intracerebral haemorrhages(ICHs);and 95810(3.2%)were subarachnoid haemorrhages(SAHs).The average age of patients admitted was 66 years old,and nearly 60%were male.A total of 1555(0.1%),2774(0.6%)and 1347(1.4%)paediatric strokes(age<18 years)were identified among IS,ICH and SAH,respectively.Over one-third(1063892(35.3%))of the patients were covered by urban resident basic medical insurance,followed by urban employee basic medical insurance(699513(23.2%))and new rural cooperative medical schema(489361(16.3%)).The leading risk factor was hypertension(67.4%for IS,77.2%for ICH and 49.1%for SAH),and the leading comorbidity was pneumonia or pulmonary infection(10.1%for IS,31.4%for ICH and 25.2%for SAH).In-hospital death/discharge against medical advice rate was 8.3%for stroke inpatients,ranging from 5.8%for IS to 19.5%for ICH.The median and IQR of length of stay was 10.0(7.0-14.0)days,ranging from 10.0(7.0-13.0)in IS to 14.0(8.0-22.0)in SAH.Data from the Chinese Stroke Center Alliance demonstrated that the composite scores of guideline-recommended key performance indicators for patients with IS,ICH and SAH were 0.77±0.21,0.72±0.28 and 0.59±0.32,respectively. | Yong-Jun Wang Zi-Xiao Li Hong-Qiu Gu Yi Zhai Yong Jiang Xing-Quan Zhao Yi-Long Wang Xin Yang Chun-Juan Wang Xia Meng Hao Li Li-Ping Liu Jing Jing Jing Wu An-Ding Xu Qiang Dong David Wang Ji-Zong Zhao On behalf of China Stroke Statistics 2019 Writing Committee | 2020 | Stroke & Vascular Neurology2020,5,3: | 181 |

| 12 | Asymmetric flow effect in a horizontal natural ventilated tunnel with different aspect ratios under the influence of longitudinal fire locations显示文摘This paper has analyzed the asymmetric flow effect of fire-induced thermal flow in a horizontal tunnel under the natural ventilation condition by conducting large eddy simulations (LES). The key objective is to reveal and to have a better understanding of the asymmetric flow effect caused by the upstream and downstream tunnel length difference. The mechanism behind it can be explained based on the conservation of mass and dynamic force analysis on the smoke and fresh air. The strength of the asymmetric flow effect is characterized by the mass flow rate of the induced longitudinal flow (net mass flow rate of a cross-section). An empirical correlation to predict the induced longitudinal mass flow rate is proposed. Furthermore, the law of smoke and air flow distribution within a horizontal tunnel is established. The proportion of smoke (or air) flowing out (or coming in) through the opening increase (or decrease) linearly with the increasing distance between that opening to the fire location. The variation of the air flow with the longitudinal fire location in a tunnel is more sensitive than the smoke flow. | 14 | Smoking under hypoxic conditions: a potent environmental risk factor for inflammatory and autoimmune diseases显示文摘Autoimmune disease management presents a significant challenge to medical science. Environmental factors potentially increase the risk of developing inflammatory and autoimmune diseases, such as multiple sclerosis, rheumatoid arthritis, and lupus. Among various environmental stresses, cigarette smoke and hypoxia have both been reported to lead to an enhanced risk of inflammatory and autoimmune diseases. In this review, we shed light on all reported mechanisms whereby cigarette smoke and a hypoxic environment can induce inflammatory and autoimmune diseases and discuss how hypoxic conditions influence the cigarette smoke-induced threat of inflammatory and autoimmune disease development. Cigarette smoke and hypoxia both lead to increased oxidative stress and production of reactive oxygen species and other free radicals, which have various effects including the generation of autoreactive pro-inflammatory T cells and autoantibodies, reductions in T regulatory(Treg) cell activity, and enhanced expression of pro-inflammatory mediators [e.g., interleukin-6(IL-6), interleukin-4(IL-4) and interleukin-8(IL-8)]. Accordingly, smoking and hypoxic environments may synergistically act as potent environmental risk factors for inflammatory and autoimmune diseases. To our knowledge, no studies have reported the direct association of cigarette smoke and hypoxic environments with the risk of developing inflammatory and autoimmune diseases. | 16 | Long-term Trends and Predictors of Smoking Behaviors among Men Following First-ever Ischemic Stroke显示文摘Continued smoking following stroke is associated with adverse outcomes including increased risk of mortality and secondary stroke. The aim of this study was to examine the long-term trends in smoking behaviors and factors associated with smoking relapse among men who survived their first-ever stroke. Data collection for this longitudinal study was conducted at baseline through face-toface interviews and follow-up was completed every 3 months via telephone, beginning in 2010 and continuing through 2014. Cox proportional hazard regression models were used to identify predictors of smoking relapse behavior. At baseline, 372 male patients were recruited into the study. Totally, 155(41.7%) of these patients stopped smoking for stroke, and 61(39.3%) began smoking again within 57 months after discharge with an increasing trend in the number of cigarettes smoked per day. Exposure to environmental tobacco smoke at places outside of home and work(such as bars, restaurants)(HR, 2.34; 95% CI, 1.04–5.29, P=0.04), not having a spouse(HR, 0.12; 95% CI, 0.04–0.36; P=0.0002) and smoking at least 20 cigarettes per day before stroke(HR, 2.42; 95% CI, 1.14–5.14, P=0.02) were predictors of smoking relapse. It was concluded that environmental tobacco smoke is an important determinant of smoking relapse among men who survive their first stroke. | 19 | Syndecan 4-c-ros oncogene 1 fusion as a mechanism of acquired resistance in epidermal growth factor receptor mutant lung adenocarcinoma显示文摘To the Editor:A 37-year-old Chinese woman,a nonsmoker,presented with adenocarcinoma of the left lung(stage IV)[Figure 1A],malignant pleural effusion,and multiple bone metastases in December 2014.Deoxyribonucleic acid sequencing of the tumor biopsy revealed an epidermal growth factor receptor(EGFR)exon 19 deletion,and treatment with gefitinib was consequently initiated in December 2014.There were no treatmentrelated side effects except for a grade 1 rash,and stable disease was achieved.Unfortunately,the patient did not experience long-term benefits and developed disease progression after only two months.A follow-up examination showed that extracranial disease and a small intracranial lesion had developed gradually.The patient then received whole-brain radiotherapy(30 Gy in ten fractions)and four cycles of single-agent pemetrexed in April 2015.Stable disease was achieved,which coincided with an improvement in the patient’s cough,shortness of breath,and general condition.The chemotherapy course was changed to two cycles of pemetrexed combined with cisplatin.A partial response(PR)was achieved,and the patient was treated with 14 cycles of pemetrexed alone as maintenance chemotherapy.The follow-up evaluations showed the patient’s condition to be stable;however,the patient’s disease progressed after 23 months.A computed tomography(CT)scan in March 2017 revealed that both the lung lesion and the malignant pleural effusion had increased in size[Figure 1B].The subsequent biopsy specimen was subjected to next-generation sequencing(NGS)and a Syndecan 4-c-ros oncogene 1(SDC4-ROS1)rearrangement was detected[Figure 1C].The patient then received crizotinib in April 2017,and a PR was achieved[Figure 1D].A CT scan done in August 2018 indicated the progression of the primary lesion in the left lung and malignant pleural effusion.However,the growth of the remaining lesions remained stable. | You-Cai Zhu Chun-Wei Xu Qu-Xia Zhang Wen-Xian Wang Lei Lei Wu Zhuang | 2019 | Chinese Medical Journal2019,,24: | 1 |
| 20 | Smoking influences outcome in patients who had thrombolysed ischaemic stroke:the ENCHANTED study显示文摘Background and purpose As studies vary in defining the prognostic significance of smoking in acute ischaemic stroke(AIS),we aimed to determine the relation of smoking and key outcomes in patient participants who had thrombolysed AIS of the international quasi-factorial randomised Enhanced Control of Hypertension and Thrombolysis Stroke Study(ENCHANTED).Methods Post-hoc analyses of ENCHANTED,an international quasi-factorial randomised evaluation of intravenous alteplase-dose comparison and levels of blood pressure control in patients who had thrombolysed AIS.Multivariable logistic regression models with inverse probability of treatment weighting(IPTW)propensity scores were used to determine associations of self-reported smoking status and clinical outcomes,according to 90-day modified Rankin Scale(mRS)scores and symptomatic intracerebral haemorrhage(sICH).Results Of 4540 patients who had an AIS,there were 1008(22.2%)current smokers who were younger and predominantly male,with more comorbidities of hypertension,coronary artery disease,atrial fibrillation and diabetes mellitus,and greater baseline neurological impairment,compared with non-smokers.In univariate analysis,current smokers had a higher likelihood of a favourable shift in mRS scores(OR 0.88,95%CI 0.77 to 0.99;p=0.038)but this association reversed in a fully adjusted model with IPTW(adjusted OR 1.15,95%CI 1.04 to 1.28;p=0.009).A similar trend was also apparent for dichotomised poor outcome(mRS scores 2–6:OR 1.18,95%CI 1.05 to 1.33;p=0.007),but not with the risk of sICH across standard criteria.Conclusion Smoking predicts poor functional recovery in patients who had thrombolysed AIS.Trial registration number NCT01422616. | Lingli Sun Lili Song Jie Yang Richard I Lindley Thompson Robinson Pablo M Lavados Candice Delcourt Hisatomi Arima Bruce Ovbiagele John Chalmers Craig S Anderson Xia Wang | 2021 | Stroke & Vascular Neurology2021,6,3: | 1 |
