1Challenges in the diagnosis and treatment of gestational trophoblastic neoplasia worldwide显示文摘Gestational trophoblastic neoplasia(GTN) is a rare tumor that originates from pregnancy that includes invasive mole, choriocarcinoma(CCA), placental site trophoblastic tumor and epithelioid trophoblastic tumor(PSTT/ETT). GTN presents different degrees of proliferation, invasion and dissemination, but, if treated in reference centers, has high cure rates, even in multi-metastatic cases.The diagnosis of GTN following a hydatidiform molar pregnancy is made according to the International Federation of Gynecology and Obstetrics(FIGO)2000 criteria: four or more plateaued human chorionic gonadotropin(hCG)concentrations over three weeks; rise in hCG for three consecutive weekly measurements over at least a period of 2 weeks or more; and an elevated but falling hCG concentrations six or more months after molar evacuation. However,the latter reason for treatment is no longer used by many centers. In addition,GTN is diagnosed with a pathological diagnosis of CCA or PSTT/ETT. For staging after a molar pregnancy, FIGO recommends pelvic-transvaginal Doppler ultrasound and chest X-ray. In cases of pulmonary metastases with more than 1cm, the screening should be complemented with chest computed tomography and brain magnetic resonance image. Single agent chemotherapy, usually Methotrexate(MTX) or Actinomycin-D(Act-D), can cure about 70% of patients with FIGO/World Health Organization(WHO) prognosis risk score ≤ 6(low risk), reserving multiple agent chemotherapy, such as EMA/CO(Etoposide,MTX, Act-D, Cyclophosphamide and Oncovin) for cases with FIGO/WHO prognosis risk score ≥ 7(high risk) that is often metastatic. Best overall cure rates for low and high risk disease is close to 100% and > 95%, respectively. The management of PSTT/ETT differs and cure rates tend to be a bit lower. 2Acute cholestatic hepatitis caused by amoxicillin/ clavulanate显示文摘Amoxicillin/clavulanate is a synthetic penicillin that is currently commonly used,especially for the treatment of respiratory and cutaneous infections.In general,it is a well-tolerated oral antibiotic.However,amoxicillin/clavulanate can cause adverse effects,mainly cutaneous,gastrointestinal,hepatic and hematologic,in some cases.Presented here is a case report of a 63-yearold male patient who developed cholestatic hepatitis after recent use of amoxicillin/clavulanate.After 6 wk of prolonged use of the drug,he began to show signs of cholestatic icterus and developed severe hyperbilirubinemia(total bilirubin>300 mg/L).Diagnostic investigation was conducted by ultrasonography of the upper abdomen,serum tests for infection history,laboratory screening of autoimmune diseases,nuclear magnetic resonance(NMR)of the abdomen with bile duct-NMR and transcutaneous liver biopsy guided by ultrasound.The duration of disease was approximately 4 mo,with complete resolution of symptoms and laboratory changes at the end of that time period.Specific treatment was not instituted,only a combination of anti-emetic(metoclopramide)and cholestyramine for pruritus.Daniel Oliveira Beraldo Joanderson Fernandes Melo Alexandre Vidal Bonfim Andrei Alkmim Teixeira Ricardo Alkmim Teixeira André Loyola Duarte 2013World Journal of Gastroenterology2013,19,46:2

5First-line cisplatin,docetaxel,and cetuximab for patients with recurrent or metastatic head and neck cancer:A multicenter cohort study显示文摘BACKGROUND The targeted therapy cetuximab[directed at the epidermal growth factor receptor(EGFR)]in combination with 5-fluorouracil and platinum-based chemotherapy(the EXTREME regimen)has shown substantial efficacy for patients with recurrent or metastatic squamous cell carcinoma of the head and neck(R/M SCCHN).Thus,this scheme has been established as the preferred first-line option for these patients.However,more recently,a new strategy combining platinum,taxanes,and cetuximab(the TPEx regimen)has demonstrated similar efficacy with a more favorable toxicity profile in clinical trials.AIM To evaluate the safety and efficacy of the TPEx scheme as first-line therapy in advanced SCCHN in a multicenter cohort study.METHODS This retrospective multicenter cohort study included patients with histologically confirmed recurrent or metastatic SCCHN treated with first-line TPEx at five medical centers in Argentina between January 1,2017 and April 31,2020.Chemotherapy consisted of four cycles of docetaxel,cisplatin,and cetuximab followed by cetuximab maintenance therapy.Clinical outcomes and toxicity profiles were collected from medical charts.Treatment response was assessed by the investigator in accordance with Response Evaluation Criteria in Solid Tumors(version 1.1).Adverse events were graded according to the National Cancer Institute Common Terminology Criteria for Adverse Events(version 4.0).RESULTS Twenty-four patients were included.The median age at diagnosis was 58 years(range:36-77 years).The majority of patients(83.3%)received at least four chemotherapy cycles in the initial phase.In the included group,the overall response rate was 62.5%,and 3 patients achieved a complete response(12.5%).The median time to response was 2.4 mo[95% confidence interval(CI):1.3-3.5].With a median follow-up of 12.7 mo(95%CI:8.8-16.6),the median progression-free survival(PFS)was 6.9 mo(95%CI:6.5-7.3),and the overall survival rate at 12 mo was 82.4%.Patients with documented tumor response showed a better PFS than those with disease stabilization or progression[8.5 mo(95%CI:5.5-11.5)and 4.5 mo(95%CI:2.5-6.6),respectively;P=0.042].Regarding the safety analysis,two-thirds of patients reported at least one treatment-related adverse event,and 25% presented grade 3 toxicities.Of note,no patient experienced grade 4 adverse events.CONCLUSION TPEx was an adequately tolerated regimen in our population,with low incidence of grade 3-4 adverse events.The median PFS were consistent with those in recent reports of clinical trials evaluating this treatment combination.This regimen may be considered an attractive therapeutic strategy due to its simplified administration,decreased total number of chemotherapy cycles,and treatment tolerability.Agustin Falco Mariano Leiva Albano Blanco Guido Cefarelli Andres Rodriguez Juan Melo Federico Cayol Manglio Miguel Rizzo Alejandro Sola Hernan Rodriguez Montani Matias Chacon Diego Enrico Federico Waisberg 2022World Journal of Clinical Oncology2022,13,2:1

13Effectiveness of different biochars in remediating a salt-affected Luvisol in Northeast Brazil显示文摘Biochar has been proposed as a soil amendment to reclaim salt-affected soils.However,the evaluation of the different biochar types in these soils is essential due to the significant variation among biochar feedstocks.In this study,biochars from sugarcane bagasse(SB),orange bagasse(OB),and corncob(CB)were applied alone and in the presence of gypsum(G)to test their potential to remediate a highly degraded saline-sodic soil in Brazil´s Northeast region.For that,we conducted a laboratory column-leaching experiment and a greenhouse study.After the column-leaching test,we analyzed the sodium adsorption ratio(SAR),the electrical conductivity of the saturation extract(ECe),the concentrations of major ions,and the exchangeable sodium percentage(ESP).Maize plants were grown in the leached soil in a greenhouse.Sugarcane-and corncob-derived biochars were more efficient than gypsum in reducing the soil salinity,bringing the indicators below threshold values,and optimizing soil reclamation time.Soil ECe,SAR,and ESP were reduced to 3.42 dS m−1,1.64(mmolc dm−3)0.5 and 4.86%,respectively,in the SB treatment;and to 3.19 dS m−1,0.88(mmolc dm−3)0.5 and 2.53%,respectively,in the CB treatment.Orange bagasse biochar did not effectively reduce the salinity indicators.All biochar treatments improved seed germination and plant growth.The SB,CB,SBG,and CBG treatments increased plant height(478–558%),shoot mass(783–983%),and root mass(386–500%),respectively.Therefore,the application of SB and CB is an effective alternative to remediate saline-sodic soils and to reduce the impact of soil salinity in the environment.Wallace Melo dos Santos Maria Isidoria Silva Gonzaga JoséAiron da Silva AndréQuintão de Almeida JoséCarlos de Jesus Santos Thomas Antonio Silva Gonzaga Idamar da Silva Lima EloáMoura Araújo 2021Biochar2021,3,2:0
14CXCL9 and CXCL10 display an agedependent profile in Chagas patients:a cohort study of aging in Bambui,Brazil显示文摘Background:Chagas disease is endemic in Latin America and still represents an important public health problem in the region.Chronic cardiomyopathy is the most significant chronic form due to its association with morbidity and mortality.The last decade has seen increasing evidence that inflammatory cytokines and chemokines are responsible for the generation of inflammatory infiltrate and tissue damage,with chronic chagasic cardiomyopathy patients presenting a pro-inflammatory immune response.Although studies have evaluated the role of chemokines in experimental T.cruzi infection,few have addressed their systemic profile,especially for human infection and in aging populations.The present work aimed to use the data from a large population based study of older adults,conducted in an endemic area for Chagas disease,to examine the association between serum levels of cytokines and chemokines,T.cruzi infection and electrocardiogram(ECG)abnormality.Methods:The present work evaluated serum levels of CCL2,CXCL9,CXCL10,CCL5,CXCL8,IL-1β,IL-6,TNF,IL-12 and IL-10 by Flow Cytometric Bead Array assay(CBA)and the results expressed in pg/ml.The baseline survey started in January 1st 1997,with 1284 participants of an aged population-based cohort.Participants signed an informed consent at baseline and at each subsequent visit and authorized death certificate and medical records verification.Results:Our results demonstrated that Chagas disease patients had higher serum levels of CXCL9,CXCL10 and IL-1βand lower serum levels of CCL5 than non-infected subjects.Moreover,our data demonstrated that CXCL9 and CXCL10 increased in an age-dependent profile in Chagas disease patients.Conclusion:Together,this study provided evidences that serum biomarkers increase along the age continuum and may have potential implications for establishing clinical management protocols and therapeutic intervention in Chagas disease patients.Fernanda Fortes de Araújo Karen Cecília Lima Torres Sérgio Viana Peixoto Antonio Luiz Pinho Ribeiro Juliana Vaz Melo Mambrini Vitor Bortolo Rezende Maria Luiza Lima Silva Antônio Ignácio Loyola Filho Andréa Teixeira-Carvalho Maria Fernanda Lima-Costa Olindo Assis Martins-Filho 2020Infectious Diseases of Poverty2020,9,3:0

18Carbonates and organic matter in soils characterized by reflected energy from 350-25000 nm wavelength显示文摘The soil carbon pool which is the sum of soil organic carbon(SOC)and soil inorganic carbon(SIC)is the second largest active store of carbon after the oceans and it is an important component of the global carbon cycle.Hence,accurate estimation of SOC and SIC as important carbon reservoirs in terrestrial ecosystems using fast,inexpensive and non-destructive methods is crucial for planning different climate change policies.The aim of the current research was to examine the effectiveness of Vis-NIR(visible and near-infrared spectroscopy:350-2500 nm)and MIR(mid-infrared spectroscopy:4000-400 cm-1)to characterize and estimate soil organic matter(SOM)and carbonates as main components of soil carbon stocks in Juneqan,Charmahal va Bakhtiari,Iran.To do so,a total of 548 soil samples from this area were collected(October 2015)and analyzed in laboratory(August 2017).In order to develop models capable of predicting SOM and carbonates content,seven spectral preprocessing methods comprising Absorbance(Abs),De-trending(Det),Continuum removal(CR),Savitzky-Golay derivatives(SGD),standard normal variate transformation(SNV),multiplicative scatter correction(MSC)and Normalization by range(NBR)were conducted along with five multivariate methods including Random Forest(RF),Partial Least-Squares Regression(PLSR),Artificial Neural Network(ANN),Support Vector Machine(SVM)and Gaussian Process Regression(GPR).The content of carbonates caused spectral reflectance intensity to augment on several ranges of spectrum and strong absorption feature at 2338 nm in the Vis-NIR and 714,850,870,1796,2150 and 2510 cm-1 in the MIR spectra range.SOM absorbed energy in several ranges,but also showed specific peaks in MIR.Both facts are associated with the structure of carbonates and SOM and its interaction with energy.The best combination of preprocessing and calibration models for carbonates quantification in Vis-NIR spectra was Det/PLSR(R2=0.74,RPD=2.19,RMSE=6.45).For SOM,it was Det/PLSR(R2=0.82,RPD=2.41,RMSE=0.75).The Det/RF(R2=0.87,RPD=2.44,RMSE=0.66)for the quantification of SOM and MSC/RF(R2=0.84,RPD=2.84,RMSE=5.50)for carbonates in MIR spectra range showed the greatest results.The stronger occurrence of spectral bands in MIR as well as the specificity of the absorption features indicated that this range produced better predictions.The obtained results highlighted the significant role of soil spectroscopy technique in predicting SOC and soil carbonates as key components of soil carbon stocks in the study area.Therefore,this technique can be used as a more cost-effective,time saving and nondestructive alternative to traditional methods of soil analysis.Najmeh ASGARI Shamsollah AYOUBI Jose Alexandre Melo DEMATTê AndréCarnieletto DOTTO 2020Journal of Mountain Science2020,17,7:0
