| 1 | Can we predict which patients with community-acquired pneumonia are likely to have positive blood cultures?显示文摘BACKGROUND: Blood cultures (BC ) are commonly ordered during the initial assessment ofpatients with community-acquired pneumonia (CAP), yet their yield remains low. Selective use of BCwould allow the opportunity to save healthcare resources and avoid patient discomfort. The studywas to determine what demographic and clinical factors predict a greater likelihood of a positiveblood culture result in patients diagnosed with CAP.METHODS: A structured retrospective systematic chart audit was performed to comparerelevant demographic and clinical details of patients admitted with CAP, in whom blood culture resultswere positive, with those of age, sex, and date-matched control patients in whom blood cultureresults were negative.RESULTS: On univariate analysis, eight variables were associated with a positive BC result.After logistic regression analysis, however, the only variables statistically significantly associatedwith a positive BC were WBC less than 4.5 x 109/L [likelihood ratio (LR): 7.75, 95% CI=2.89-30.39], creatinine 〉106 !mol/L (LR: 3.15, 95%CI=1.71-5.80), serum glucose〈6.1 mmol/L (LR: 2.46,95%CI=1.14-5.32), and temperature 〉 38 °C (LR: 2.25, 95% CI =1.21-4.20). A patient with all of thesevariables had a LR of having a positive BC of 135.53 (95% CI=25.28-726.8) compared to patientswith none of these variables.CONCLUSIONS: Certain clinical variables in patients with CAP admitted to hospitals doappear to be associated with a higher probability of a positive yield of BC, with combinations of thesevariables increasing this likelihood. | 13 | Effect of different intensities of physical activity on cardiometabolic markers and vascular and cardiac function in adult rats fed with a high-fat high-carbohydrate diet显示文摘Background: Physical activity(PA) and diet are 2 lifestyle factors that affect cardiometabolic risk. However, data on how a high-fat highcarbohydrate(HFHC) diet influences the effect of different intensities of PA on cardiometabolic health and cardiovascular function in a controlled setting are yet to be fully established. This study investigated the effect of sedentary behavior, light-intensity training(LIT), and high-intensity interval training(HIIT) on cardiometabolic markers and vascular and cardiac function in HFHC-fed adult rats.Methods: Twelve-week-old Wistar rats were randomly allocated to 4 groups(12 rats/group): control(CTL), sedentary(SED), LIT, and HIIT.Biometric indices, glucose and lipid control, inflammatory and oxidative stress markers, vascular reactivity, and cardiac electrophysiology of the experimental groups were examined after 12 weeks of HFHC-diet feeding and PA interventions.Results: The SED group had slower cardiac conduction(p = 0.0426) and greater thoracic aortic contractile responses(p < 0.05) compared with the CTL group. The LIT group showed improved cardiac conduction compared with the SED group(p = 0.0003), and the HIIT group showed decreased mesenteric artery contractile responses compared with all other groups and improved endothelium-dependent mesenteric artery relaxation compared with the LIT group(both p < 0.05). The LIT and HIIT groups had lower visceral(p = 0.0057 for LIT, p = 0.0120 for HIIT)and epididymal fat(p < 0.0001 for LIT, p = 0.0002 for HIIT) compared with the CTL group.Conclusion: LIT induced positive adaptations on fat accumulation and cardiac conduction, and HIIT induced a positive effect on fat accumulation,mesenteric artery contraction, and endothelium-dependent relaxation. No other differences were observed between groups. | 18 | Evaluating long-term attachment of two different endoclips in the human gastrointestinal tract显示文摘AIM:To evaluate the long-term attachment of two types of endoclips in the human gastrointestinal tract. METHODS:In this prospective observational study,endoclips were placed and followed-up during endoscopies or using fluoroscopic images as part of a pro s pective feasibility study evaluating external beam radiotherapy (EBRT,wk 1-3) followed by high dose rate brachytherapy (HDRBT with an endoluminal app licator once a week for 3 wk,wk 9-11) in medically inoperablerectal cancer patients. Initially,the type and number of endoclips were chosenrandomly and later refined to 1 Resolution clip (Microvasive) proximal and 2 Quickclips (Olympus) distal to the tumor. Nine consecutive patients from between September 2007 and August 2008 were analyzed. Retention rates were evaluated over three different observational periods [period 1:pre-HDRBT (wk-2-8),period 2:during HD RBT (wk 9-11) and period 3:post-HDRBT (wk 1216)]. RESULTS:In this study,a total of 44 clips were placed during endoscopy,either at the beginning or at the end of period 1. The Resolution clip had a higher overall retention rate than the Quickclip (P = 0.01). After a median period of 81 d after placement (in period 1),longterm retention rates for the Resolution clip and Quickclip clip were 67% and 35% respectively.
