| 1 | Long-term results of endoscopic balloon dilatation of lower gastrointestinal tract strictures in Crohn's disease:A prospective study显示文摘AIM:To examine the long-term results of endoscopic treatment in a prospective study conducted over a period of 10 years,1997 to January 2007.METHODS:A total of 25 patients(20 female and five male:aged 18-75 years),with at least one symptom of stricture not passable with the standard colonoscope and with a confirmed scarred Crohn's stricture of the lower gastrointestinal tract,were included in the study.The main symptom was abdominal pain.The endoscopic balloon dilatation was performed with an 18 mm balloon under endoscopic and radiological control.RESULTS:Eleven strictures were located in the colon,13 at the anastomosis after ileocecal resection,three at the Bauhin valve and four in the ileum.Four patients had two strictures and one patient had three strictures.Of the 31 strictures,in 30 was balloon dilatation successful in a single endoscopic session,so that eventually the strictures could be passed easily with the standard colonoscope.In one patient with a long stricture of the ileum involving the Bauhin valve and an additional stricture of the ileum which were 15 cm apart,sufficient dilatation was not possible.This patient therefore required surgery.Improvement of abdominal symptoms was achieved in all cases which had technically successful balloon dilatation,althoughin one case perforation occurred after dilatation of a recurrent stricture.Available follow-up was in the range of 54-118 mo(mean of 81 mo).The relapse rate over this period was 46%,but 64% of relapsing strictures could be successfully dilated again.Only in four patients was surgery required during this follow-up period.CONCLUSION:We conclude from these initial results that endoscopic balloon dilatation,especially for short strictures in Crohn's disease,can be performed with reliable success.Perforation is a rare complication.It is our opinion that in the long-term,the relapse rate is probably higher than after surgery,but usually a second endoscopic treatment can be performed successfully,leading to a considerable success rate of the endoscopic procedure. | Klaus Stienecker Daniel Gleichmann Ulrike Neumayer H Joachim Glaser Carolin Tonus | 2009 | World Journal of Gastroenterology2009，15，21： | 9 |

| 3 | Gastroesophageal reflux disease:A review of surgical decision making显示文摘Gastroesophageal reflux disease(GERD) is a very common disorder with increasing prevalence. It is estimated that up to 20%-25% of Americans experience symptoms of GERD weekly. Excessive reflux of acidic often with alkaline bile salt gastric and duodenal contents results in a multitude of symptoms for the patient including heartburn, regurgitation, cough, and dysphagia. There are also associated complications of GERD including erosive esophagitis, Barrett's esophagus, stricture and adenocarcinoma of the esophagus. While first line treatments for GERD involve mainly lifestyle and non-surgical therapies, surgical interventions have proven to be effective in appropriate circumstances. Anti-reflux operations are aimed at creating an effective barrier to reflux at the gastroesophageal junction and thus attempt to improve physiologic and mechanical issues that may be involved in the pathogenesis of GERD. The decision for surgical intervention in the treatment of GERD, moreover, requires an objective confirmation of the diagnosis. Confirmation is achieved using various preoperative evaluations including: ambulatory p H monitoring, esophageal manometry, upper endoscopy(esophagogastroduodenoscopy) and barium swallow. Upon confirmation of the diagnosis and with appropriate patient criteria met, an antireflux operation is a good alternative to prolonged medical therapy. Currently, minimally invasive gastroesophageal fundoplication is the gold standard for surgical intervention of GERD. Our review outlines the many factors that are involved in surgical decisionmaking. | 5 | Medicinal properties of Angelica archangelica root extract: Cytotoxicity in breast cancer cells and its protective effects against in vivo tumor development显示文摘Objective: Although Angelica archangelica is a medicinal and aromatic plant with a long history of use for both medicinal and food purposes, there are no studies regarding the antineoplastic activity of its root.This study aimed to evaluate the cytotoxicity and antitumor effects of the crude extract of A. archangelica root(CEAA) on breast cancer.Methods: The cytotoxicity of CEAA against breast adenocarcinoma cells(4T1 and MCF-7) was evaluated by a 3-(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl-2-H-tetrazolium bromide(MTT) assay. Morphological and biochemical changes were detected by Hoechst 33342/propidium iodide(PI) and annexin V/PI staining. Cytosolic calcium mobilization was evaluated in cells staining with FURA-4 NW. Immunoblotting was used to determine the effect of CEAA on anti-and pro-apoptotic proteins(Bcl-2 and Bax, respectively).The 4 T1 cell-challenged mice were used for in vivo assay.Results: Using ultra-high-performance liquid chromatography–mass spectrometry analysis, angelicin, a constituent of the roots and leaves of A. archangelica, was found to be the major constituent of the CEAA evaluated in this study(73 mg/mL). The CEAA was cytotoxic for both breast cancer cell lines studied but not for human fibroblasts. Treatment of 4 T1 cells with the CEAA increased Bax protein levels accompanied by decreased Bcl-2 expression, in the presence of cleaved caspase-3 and cytosolic calcium mobilization, suggesting mitochondrial involvement in breast cancer cell death induced by the CEAA in this cell line. No changes on the Bcl-2/Bax ratio were observed in CEAA-treated MCF7 cells. Gavage administration of the CEAA(500 mg/kg) to 4T1 cell-challenged mice significantly decreased tumor growth when compared with untreated animals.Conclusion: Altogether, our data show the antitumor potential of the CEAA against breast cancer cells in vitro and in vivo. | 16 | ^(11)C-PK11195 plasma metabolization has the same rate in multiple sclerosis patients and healthy controls:a cross-sectional study显示文摘^(11)C-PK11195 is a positron emitter tracer used for Positron Emission Tomography(PET)imaging of innate immune cell activation in studies of neuroinflammatory diseases.For the image quantitative analysis,it is necessary to quantify the intact fraction of this tracer in the arterial plasma during imaging acquisition(plasma intact fraction).Due to the complexity and costs involved in this analysis it is important to evaluate the real necessity of individual analysis in each 11C-PK11195 PET imaging acquisition.The purpose of this study is to compare 11CPK11195 plasma metabolization rate between healthy controls and multiple sclerosis(MS)patients and evaluate the interference of sex,age,treatment,and disease phenotype in the tracer intact fraction measured in arterial plasma samples.11C-PK11195 metabolization rate in arterial plasma was quantified by high performance liquid chromatography in samples from MS patients(n=50)and healthy controls(n=23)at 20,45,and 60 minutes after 11C-PK11195 injection.Analyses were also stratified by sex,age,treatment type,and MS phenotype.The results showed no significant differences in the metabolization rate of healthy controls and MS patients,or in the stratified samples.In conclusion,11C-PK11195 metabolization has the same rate in patients with MS and healthy controls,which is not affected by sex,age,treatment,and disease phenotype.Thus,these findings could contribute to exempting the necessity for tracer metabolization determination in all 11C-PK11195 PET imaging acquisition,by using a population metabolization rate average.The study procedures were approved by the Ethics Committee for Research Projects Analysis of the Hospital das Clinicas of the University of Sao Paulo Medical School(approval No.624.065)on April 23,2014. | Aline Morais de Souza Milena Sales Pitombeira Larissa Estessi de Souza Fabio Luiz Navarro Marques Carlos Alberto Buchpiguel Caroline Cristiano Real Daniele de Paula Faria | 2021 | Neural Regeneration Research2021，16，12： | 1 |
