| 1 | Update in management of hepatocellular carcinoma in Eastern population显示文摘Hepatocellular carcinoma(HCC) is one of the commonest malignant tumours in the East. Although the management of HCC in the West is mainly based on the Barcelona Clinic for Liver Cancer staging, it is considered too conservative by Asian countries where the number of HCC patients is huge. Scientific and clinical advances were made in aspects of diagnosis, staging, and treatment of HCC. HCC is well known to be associated with cirrhosis and the treatment of HCC must take into account the presence and stage of chronic liver disease. The major treatment modalities of HCC include:(1) surgical resection;(2) liver transplantation;(3) local ablation therapy;(4) transarterial locoregional treatment; and(5) systemic treatment. Among these, resection, liver transplantation and ablation therapy for small HCC are considered as curative treatment. Portal vein embolisation and the associating liver partition with portal vein ligation for staged hepatectomy may reduce dropout in patients with marginally resectable disease but the midterm and long-term results are still to be confirmed. Patient selection for the best treatment modality is the key to success of treatment of HCC. | 3 | Phase Ⅰ/Ⅱ randomized controlled trial of autologous bone marrow-derived mesenchymal stem cell therapy for chronic stroke显示文摘AIM To examine the safety and efficacy of mesenchymal stem cell(MSC) therapy for intracerebral haemorrhage with neurological dysfunctions for a year.METHODS MSC were ex vivo expanded from 29 mL(17-42 mL) autologous bone marrow. Patients were randomized to have two intravenous injections of autologous MSC or placebos in four weeks apart. Neurological functions and clinical outcomes were monitored before treatment and at 12^(th), 16^(th), 24^(th), 36^(th) and 60^(th) week upon completion of^(th)e treatment. RESULTS A mean of 4.57 × 10~7(range: 1.43 × 10~7-8.40 × 10~7) MSC per infusion was administered accounting to 8.54 × 10~5(2.65 × 10~5-1.45 × 10~6) per kilogram body weight in two occasions. There was neither adverse event at time of administration nor sign of de novo tumour development among patients after monitoring for a year post MSC therapy. Neuro-restoration and clinical improvement in terms of modified Barthel index, functional independence measure and extended Glasgow Outcome Scale were evident among patients having MSC therapy compared to patients receiving placebos. | 5 | Atrophy of the left dorsolateral prefrontal cortex is associated with poor performance in verbal fluency in elderly poststroke women显示文摘This study aimed to investigate the association between atrophy in the prefrontal cortex with executive function and verbal fluency in elderly male and female patients poststroke.Thirty elderly female patients with non-aphasic ischemic stroke aged ≥ 60 years and 30 age-matched non-aphasic male patients with ischemic stroke were recruited.Automatic magnetic resonance imaging segmentation was used to assess the volume of the whole prefrontal cortex,along with its subdivisions:anterior cingulate cortex,orbitofrontal cortex and dorsolateral prefrontal cortex.The Semantic Verbal Fluency Test was administered at 3 and 15 months poststroke.At 3 months poststroke,left dorsolateral prefrontal cortex volume was significantly correlated with Verbal Fluency Test score in female patients only(partial coefficient = 0.453,P = 0.045),after controlling for age,education,diabetes,neurological deficit,white matter lesions volume,as well as the location and volume of infarcts.At 15 months poststroke,there remained a significant association between the left dorsolateral prefrontal cortex volume and Verbal Fluency Test(partial coefficient = 0.661,P = 0.001) and between the left prefrontal cortex volume and Verbal Fluency Test(partial coefficient = 0.573,P = 0.004) in female patients after the same adjustments.These findings indicate that atrophy of the left dorsolateral prefrontal cortex contributes to the impairment of verbal fluency in elderly female patients with stroke.Sex differences may be present in the neuropsychological mechanisms of verbal fluency impairment in patients with stroke. | Yang-Kun Chen Wei-Min Xiao Defeng Wang Lin Shi Winnie CW Chu Vincent CT Mok Ka Sing Wong Gabor S Ungvari Wai Kwong Tang | 2013 | Neural Regeneration Research2013，8，4： | 2 |
