| 1 | An oral fluoropyrimidine agent S-1 induced interstitial lung disease:A case report显示文摘A 66-year-old Japanese man with pancreatic cancer received eleven courses of gemcitabine monotherapy.The tumor responded to gemcitabine until metastatic liver tumors progressed.Subsequently,he was treated with S-1,an oral fluoropyrimidine anticancer agent,as salvage chemotherapy.Forty-two days after initiating S-1,he presented with dyspnea and fever.Chest computed tomography showed diffuse interstitial lesions with thickening of the alveolar septa and ground glass opacity.Serum KL-6 level was elevated to 1,230 U/mL and he did not use any other drugs except insulin.Thus,the development of interstitial lung disease(ILD)was considered to be due to S-1.Arterial blood oxygen pressure was 49.6 Torr in spite of oxygen administration(5 L/min).Steroid therapy improved his symptoms and the interstitial shadows on chest radiograph.Although S-1-induced ILD has mostly been reported to be mild,clinicians should be aware that S-1 has the potential to cause fatal ILD. | Hiromichi Yamane Masahide Kinugawa Shigeki Umemura Yasuhiro Shiote Kenichiro Kudo Toshimitsu Suwaki Haruhito Kamei Nagio Takigawa Katsuyuki Kiura | 2011 | World Journal of Clinical Oncology2011，2，7： | 3 |
| 2 | Endoscopic appearance of AIDS-related gastrointestinal lymphoma with c-MYC rearrangements: Case report and literature review显示文摘Acquired immune deficiency syndrome (AIDS)-related lymphoma (ARL) remains the main cause of AIDS-related deaths in the highly active anti-retroviral therapy (HAART) era. Recently, rearrangement of MYC is associated with poor prognosis in patients with diffuse large B-cell lymphoma. Here, we report a rare case of gastrointestinal (GI)-ARL with MYC rearrangements and coinfected with Epstein-Barr virus (EBV) infection presenting with various endoscopic findings. A 38-yearold homosexual man who presented with anemia and was diagnosed with an human immunodeficiency virus infection for the first time. GI endoscopy revealed multiple dish-like lesions, ulcerations, bloody spots, nodular masses with active bleeding in the stomach, erythematous flat lesions in the duodenum, and multiple nodular masses in the colon and rectum. Magnified endoscopy with narrow band imaging showed a honeycomb-like pattern without irregular microvessels in the dish-like lesions of the stomach. Biopsy specimens from the stomach, duodenum, colon, and rectum revealed diffuse large B-cell lymphoma concomitant with EBV infection that was detected by high tissue EBV-polymerase chain reaction levels and Epstein-Barr virus small RNAs in situ hybridization. Fluorescence in situ hybridization analysis revealed a fusion between the immunoglobulin heavy chain (IgH) and c-MYC genes, but not between the IgH and BCL2 loci. After 1-mo of treatment with HAART and R-CHOP, endoscopic appearance improved remarkably, and the histological features of the biopsy specimens revealed no evidence of lymphoma. However, he died from multiple organ failure on the 139 th day after diagnosis. The cause of his poor outcome may be related to MYC rearrangement. | 5 | Asymptomatic colonic metastases from primary squamous cell carcinoma of the lung with a positive fecal occult blood test显示文摘We describe a 74-year-old man with a colonic metastatic squamous cell carcinoma (SCC) from the lung. His chest X-ray revealed an abnormal shadow in the right upper lobe. Computed tomography (CT) of the chest demonstrated a large lung tumor in the right upper lobe obstructing the right upper bronchus. Bronchoscopy revealed an easy-bleeding tumor in the right upper bronchus that was diagnosed as poorly differentiated squamous cell lung carcinoma. He underwent colonoscopy because he had a positive fecal occult blood test. Colonoscopy revealed a large protruding lesion with central ulceration in the descending colon. Histological examination of the biopsy specimen obtained from the colonic lesion revealed SCC. The lesion was diagnosed as metastatic colonic SCC. He had no abdominal symptoms. He underwent chemotherapy with an infusion of cisplatin 130 mg i.v. day 1, and docetaxel hydrate 100 mg i.v. day 1, repeated every 4 wk, followed by 4 courses of chemotherapy. The primary lesion shrank by less than 10% and was judged to be 'Partial Response' (PR) after 3 courses of treatment. The patient still lived 23 wk after the diagnosis of metastatic colonic SCC.
