| 1 | Effects of electroacupuncture and Chinese kidney-nourishing medicine on polycystic ovary syndrome in obese patients显示文摘OBJECTIVE: To explore the effect of electroacupuncture and Chinese kidney-nourishing medicine on insulin (INS), adiponectin (APN), leptin (LEP), and glucolipid metabolism of obese patients with polycystic ovary syndrome (PCOS). METHODS: Sixty-seven obese PCOS patients were randomly divided into two groups. Thirty-three patients in the acupuncture-medicine group were treated three times a week with electroacupuncture at the Tianshu (ST 25), Zhongwan (CV 12), Qihai (CV 6), Sanyinjiao (SP 6), Geshu (BL 17), and Cil-iao (BL 32) acupoints. They also took the Chinese drug, Tiankui capsule, for 3 months as a course of treatment. Point-taking and treatment in the electroacupuncture group of 34 patients was the same as those in the acupuncture-medicine group. We observed and compared the changes in the obesity-related indexes of body weight (BW), body mass index (BMI), and waist-hip ratio (WHR), as well as fasting plasma glucose (FPG), fasting insulin (FINS), APN, and LEP. RESULTS: BW, BMI, WHR, and FINS decreased and insulin sensitivity index (ISI) and APN were higher in the acupuncture-medicine group than in the electroacupuncture group (P<0.01). There was no obvious difference in LEP between the two groups (P>0.05). CONCLUSION: Acupuncture combined with medicine is better than just electroacupuncture for obese PCOS patients by improving obesity-related indexes, insulin sensitivity, and APN level. | 2 | Effect of moxa-burning heat stimulating Liangmen(ST 21) and Zusanli(ST 36) on proliferation and apoptosis signaling proteins in rats with stress-induced gastric ulcer显示文摘OBJECTIVE: To observe the effect of moxa-burning heat stimulating acupoints of Liangmen(ST 21)and Zusanli(ST 36) on the proliferation and apoptosis signaling proteins in rats with stress-induced gastric ulcer.METHODS: Forty rats were randomly divided into four groups: negative control(NC), ulcer control(UC), acupoints of stomach meridian(ASM), and acupoints control(AC). The acute gastric ulcer model was established by bound and water immersion.Rats in NC and UC groups didn't receive any moxa-burning heat stimulating treatment, while rats in ASM and AC groups were treated with buringmoxa heat stimulating the acupoints of Liangmen(ST 21) and Zusanli(ST 36) and their controlled points, respectively. Rats in all groups were sacrificed after 12 consecutive days treatment. Visceral hypersensitivity is closely associated with irritable bowel syndrome. Mild moxibustion can relieve chronic visceral hyperalgesia in rats with irritable bowel syndrome. We hypothesized that prokineticin-1 and prokineticin receptor-1 is the key target in the mechanism. This study established chronic visceral hyperalgesia rat models by colorectal distention. Protein and mRNA expression of prokineticin-1 and prokineticin receptor-1 were determined by immunohisto-chemical method and fluorescence quantitative-PCR, respectively, and were found to be signifi-cantly increased in visceral hyperalgesic rats. | 5 | Efficacy of herb-partitioned moxibustion at Qihai(CV 6) and bilateral Tianshu(ST 25) on colonic damage and the TLR4/NF-κB signaling pathway in rats with Crohn's disease显示文摘OBJECTIVE: To observe the effect of stimulating Qihai(CV 6) and bilateral Tianshu(ST 25) with herb-partitioned moxibustion(HPM) in rats with Crohn's disease(CD), and to investigate the possible anti-inflammatory mechanism of HPM.METHODS: Forty rats were randomly divided into four groups(n = 10 rats per group): normal control(NC), model control(MC), mesalamine(MES), and HPM. The CD rat model was established in the MC,MES, and HPM groups by administering a mixture of trinitrobenzenesulfonic acid and alcohol via enema. The HPM group received HPM on Qihai(CV 6)and bilateral Tianshu(ST 25), while the MES group received intragastric mesalamine. Colonic histomorphological scores, and serum concentrations of tumor necrosis factor α(TNF-α) and interleukin 1β(IL-1β) were assessed to evaluate the effects of HPM on colonic reparation and anti-inflammation.The expressions of Toll-like receptor 4(TLR-4), nuclear factor κB inhibitor α(IκB-α), IκB kinase α/β(IKKα/β), and NF-κB p65 were further analyzed to investigate the regulatory effects of the interventions on the TLR4/NF-κB pathway.RESULTS: CD rats showed inflammatory colonic damage and increased serum concentrations of TNF-α and IL-1β. The expressions of TLR4, IKKα/β,and NF-κB p65 in the colons of CD rats were significantly increased compared with the NC group,while the expression of IκBα(a key negative regulator of NF-κB p65) was decreased. HPM significantly mitigated colonic damage and reduced the serum concentrations of TNF-α and IL-1β. HPM downregulated the expressions of TLR4, IKKα/β, and NF-κB p65 in the colon, and upregulated the expression of IκBα. The effects of HPM in CD rats were similar to those of mesalamine.CONCLUSION: HPM alleviates colonic inflammation in CD rats. | 6 | Electroacupuncture at Tianshu (ST 25) for diarrhea-predominant irritable bowel syndrome using positron emission tomography Changes in visceral sensation center显示文摘Previous studies have demonstrated that electroacupuncture therapy is effective in the treatment of irritable bowel syndrome. However, the precise mechanism of this therapy is unknown. The present study served to investigate the effects of electroacupuncture therapy on treatment of patients with diarrhea-predominant irritable bowel syndrome (IBS). We compared brain activation maps based on the changes of cerebral glucose metabolism obtained by 18-fluorodeoxyglucose positron emission tomography scanning under three conditions: resting, rectal balloon distension and rectal balloon distension plus electroacupuncture. Under the resting condition, compared with healthy controls, IBS patients displayed an increasing regional cerebral metabolic rate of glucose over a wide range: bilateral superior temporal gyrus, right middle occipital gyrus, superior frontal gyrus and bilateral middle frontal gyrus. However, there was no significant activity in the visceral pain center. Compared with the resting condition, under the rectal balloon distension condition, patients with IBS had a greater regional cerebral metabolic rate of glucose in the prefrontal cortex, left anterior cingulate cortex, postcentral gyrus, precentral gyrus and temporal gyrus. Under the rectal balloon distension plus electroacupuncture condition, stimulation by electroacupuncture at Tianshu (ST 25) manifested a decreased regional cerebral metabolic rate of glucose in the left cingulate gyrus, right insula, right caudate nucleus, fusiform gyrus and hippocampal gyrus. | 7 | Influence of herb-partitioned moxibustion at Qihai (CV6) and bilateral Tianshu (ST25) and Shangjuxu (ST37) acupoints on toll-like receptors 4 signaling pathways in patients with ulcerative colitis显示文摘OBJECTIVE: To evaluate the efficacy of herb-partitioned moxibustion(HPM) at Qihai(CV6), Tianshu(ST25) and Shangjuxu(ST37) acupoints in relieving symptoms and the immune regulation of HPM on the toll-like receptors 4(TLR4) signaling pathway in ulcerative colitis(UC) patients.METHODS: A randomized, single-blind study was conducted 63 patients to receive HPM or sham HPM treatment. The efficacy outcomes included scores of the Mayo, Baron, inflammatory bowel disease questionnaire(IBDQ), self-rating depression scale(SDS), self-rating anxiety scale(SAS). HE staining was used to observe the histopathological changes of the colon. The expression of inflammatory cytokines and TLR4 signaling pathway related molecules were determined by enzyme-linked immunosorbent assay and immunohistochemistry.RESULTS: Baron, SDS, SAS scores were significantly decreased in moxibustion group(P < 0.05), IBDQ score was significantly greater in the moxibustion group than in the sham moxibustion group(P < 0.05). Histopathology of mucosal biopsies showed that both two groups improved in mucosa after treatment. | 8 | Effects of electroacupuncture on c-Fos expression in the spinal cord and brain of rats with chronic visceral hypersensitivity显示文摘BACKGROUND:Visceral hypersensitivity is the main cause of irritable bowel syndrome.c-Fos is a marker of visceral hypersensitivity in the central nervous system.Electroacupuncture can relieve chronic visceral hypersensitivity in rats,but the mechanism is still unknown. OBJECTIVE:To identify c-Fos expression in the spinal cord and cerebral cortex of rats with chronic visceral hypersensitivity,and to test the effects of electroacupuncture on pain sensitivity in rats with chronic visceral hypersensitivity. DESIGN,TIME AND SETTING:A randomized controlled animal experiment was performed at the Animal Experimental Center,Shanghai University of Traditional Chinese Medicine,from January to April,2007. MATERIALS:A total of 24 neonatal,male,Sprague Dawley rats,aged five days old,were equally and randomly assigned into a normal group,a model group,and an electroacupuncture group. Rabbit anti-rat c-Fos antibody and Evision secondary antibody kits(Sigma,USA),diaminobenzidine kit(Dako,Denmark),and an LD202H electroacupuncture apparatus(Huawei,Beijing,China) were used in this study. METHODS:Neonatal rats from the model and electroacupuncture groups were used to establish rat models of chronic visceral hypersensitivity by the saccule stimulation method.After model establishment,0.25 mm diameter electric needles were inserted into Tianshu(ST 25) and Shangjuxu(ST37) at a depth of approximately 0.5 cm,with an square wave(alternating current frequency at 100/20 Hz,amplitude ranged 0.2-0.6 ms,intensity at 1 mA) once for 20 minutes,once a day,for seven days.Rats in the normal and model groups were not treated. MAIN OUTCOME MEASURES:Following 7 days of treatment,c-Fos expression in the spinal cord and cerebral cortex was detected by immunohistochemistry.After the first electroacupuncture treatment,abdominal withdrawal reflex scores were investigated to evaluate the pain threshold for chronic visceral hypersensitivity in rats. RESULTS:Visceral hypersensitivity increased c-Fos staining(P<0.05),and electroacupuncture significantly decreased the number of these cells to near normal levels(P>0.05).Abdominal withdrawal reflex scores were significantly lower in the electroacupuncture and normal groups than in the model group(P<0.05) and were similar between the electroacupuncture and normal groups (P>0.05). | 9 | Interventional effect of electroacupuncture combined with medicine on monoamine neurotransmitters in hypothalamus of rats with ulcerative colitis显示文摘BACKGROUND: Ulcerative colitis (UC) is conventionally treated with sulfasalazine and other aminosalicylic acids. The symptoms of UC can be rapidly controlled, but high recurrence, severe adverse reactions and other shortages exist commonly. Whether electroacupuncture combined with medicine can make up these shortages remains unclear. OBJECTIVE: This study was to observe the regulatory effect of electroacupuncture combined with medicine on monoamine neurotransmitter in hypothalamus of rats with ulcerative colitis, and to analyze the pathogenesis of UC and the action pathway of electroacupuncture combined with medicine. DESIGN: A randomized controlled observation. SETTING: Shanghai Institute of Acupuncture and Meridian. MATERIALS: Thirty involved male SD rats of clean grade, weighing (200±20)g, were provided by the Experimental Animal Center, Shanghai University of Traditional Chinese Medicine. Sulfasalazine was produced in the Shanghai Sanwei Pharmaceutical Co., Ltd [certification No. (1995)002083]. METHODS: This study was carried out in the State Laboratory (grade 3) for Acupuncture and Immunology, Shanghai Institute of Acupuncture and Meridian. The involved 30 rats were randomized into 5 groups: normal group, model group, electroacupuncture group, medicine group and electroacupuncture combined with medicine group, with 6 rats in each group. Rats in the latter 4 groups were prepared into models of UC. In the electroacupuncture group, Zusanli(shuang) point was selected. Electro-acupuncture apparatus (G6805Ⅱ type) was connected to the point and used to stimulate it with continuous wave, frequency of 2 Hz, electrical intensity 4 mA, 20 minutes a day, for 14 days successively. In the medicine group, rats were intragastrically administrated with sulfasalazine, twice a day, 3 mL once, for 14 days successively. In the electroacupuncture combined with medicine group, rats were treated with electroacupuncture and medicine simultaneously as described in the previous two groups. Rats in the model group and normal control group were untouched except for being fixed. MAIN OUTCOME MEASURES: The levels of noradrenaline acid tartrate (NE), 3-methoxy-4 hydroxyphenylglycol (MHPG), dopamine hydrochloride (DA), 3,4-dihydroxyphenylacetic acid (DOPAC), 5-hydroxytryptamine (5-HT) hydrochloride, homovanillic acid (HVA) and 5-hydroxyindole acetic acid (5-HIAA) of rats in each group were detected by high-performance liquid chromatography (HPLC)-electrochemical method. RESULTS: All the 30 involved rats entered into the stage of final analysis. ①Detection of NE and MHPG levels: After treatment, NE level in the hypothalamus of rats was significantly higher in the electroacupuncture combined with medicine group, electroacupuncture group and medicine group was all significantly higher than that in the model group, respectively (P < 0.01). After treatment, MHPG level in the metabolite of NE was very significantly higher in the electroacupuncture group than in the medicine group (P < 0.01). ②Detection of DA and DOPAC levels: After treatment, DA level in the hypothalamus of rats was very significantly higher in the medicine group than in the model group (P < 0.01). ③Detection of 5-HT, HVA and 5-HIAA levels: After treatment, 5-HIAA level in the hypothalamus of rats was significantly higher in the medicine group than in the model group (P < 0.05). CONCLUSION: The abnormal synthesis and metabolism of monoamine neurotransmitter in the central nervous system is one of pathogenesis of UC. Nine programs were designed.Subjects were assigned to four subgroups according to age and gender; each subject underwent one of the nine programs randomly. We selected Tianshu(ST 25, bilateral) and Qihai(CV 6). The scores of symptoms and the IBS Quality of Life(IBS-QOL) were evaluated after the final treatment.RESULTS: After treatment, clinical symptoms of all patients were significantly improved(P = 0.001).Quality of life also improved significantly(P < 0.05).For all the participants, frequency was the dominating factor regarding symptoms(P = 0.01). Duration was the dominating factor for quality of life(P =0.0001). The best combination for improving symptoms in 30-40-year-old male and female patients was two courses of 10 min each once a day, and for41-50-year-old male and female patients it was three courses of 10 min(males) or 30 min(females)twice a day. The best combination for quality of life was three courses of 20 min once a day. No interaction was found between frequency and course, or frequency and duration.CONCLUSION: Warming moxibustion Tianshu(ST25, bilateral) and Qihai(CV 6) is a promising therapy for D-IBS. | 16 | Herb-partitioned moxibustion regulated the miRNA expression profile in the thyroid tissues of rats with experimental autoimmune thyroiditis显示文摘OBJECTIVE:To observe the effect of herb-partitioned moxibustion(HPM)on the miRNA expression profile of thyroid tissue in experimental autoimmune thyroiditis(EAT)rats.METHODS:Rats were randomly divided into normal control(NC)group,EAT model(EAT)group,HPM group and western medicine(Med)group.EAT model rats were prepared by a combined immunization with complete and incomplete Freund’s adjuvant emulsified with porcine thyroglobulin and iodine.Rats in the HPM group were treated with HPM,while rats in the Med group were treated with levothyrocine(1μg/2 m L)by gavage.HE staining was used to observe the pathological morphological changes of thyroid tissue,ELISAs was uaed to detect the serum concentrations of TGAb,TPOAb,FT3,FT4,TSH.We then performed high-throughput mi RNA sequencing to analyse the mi RNA expression profiles in the thyroid tissues,followed by a bioinformatics analysis.RT-q PCR was used to verify the identified differentially expressed mi RNAs.RESULTS:HPM improved the thyroid tissue morphology and reduced serum TPOAb,TGAb,TSH concentration in EAT rats(P<0.05),but with no obvious effect on FT3 and FT4 concentration.While the TSH,FT3 and FT4 concentration was significantly changed in the Med group(P<0.01 or P<0.05)compared with that of EAT group.Sequencing results showed that a total of 17 mi RNAs were upregulated,and 4 were downregulated in the EAT rats,in which the expression levels of mi R-346 and mi R-331-5 p were reversed by HPM.The target genes of the mi RNAs that regulated by HPM wereassociated with a variety of immune factors and immune signals.RT-q PCR verification showed that the expression of mi RNA-346 and mi RNA-331-5 p was consistent with the sequencing results.CONCLUSIONS:HPM could regulate the the expression of mi RNA-346 and mi RNA-331-5 p,then act on their target genes to immune and inflammation-related pathways,which may be one of the mechanisms of HPM on EAT rats. | WANG Zhaoqin CUI Yunhua HUANG Yan WU Huangan ZHAO Jimeng HE Jinsen LIU Huirong ZHENG Handan WU Luyi HU Zhihai LIU Yanan | 2021 | Journal of Traditional Chinese Medicine2021,41,5: | 1 |
