1Identification of meta-instable stress state based on experimental study of evolution of the temperature field during stick-slip instability on a 5° bending fault显示文摘Identification of the meta-instable stress state and study of its mechanism and evolution of relevant physical fields would be of great significance for determination of potential seismic risks and estimation of critical times. In laboratory experiments, that the specimen enters the meta-instable is marked by accelerated stress release. Could we use the experimental result to identify the earthquake in natural conditions? Because the observational data from one station can only reflect the stress state beneath this station, the key problem for identification of the meta-instability is how to recognize regional stress state through observational data from many stations. In this work, we choose the evolution of the temperature field over varied deformation stages during a stick-slip event on a 5 bending fault as an example, and attempt to find the response features of the physical quantity when the fault enters the meta-instable state. We discuss the characteristics of stages for the stress build-up, stress-time process deviating from linearity before instability, meta-instability, instability, and post-instability, respectively. The result shows that the fault instability slide is a conversion process from independent activities of each fault segment to synergism activity. The instability implies completion of the synergism. The stage deviating from linearity is the onset of stress release, and it is also the onset of the synergism. At the meta-instability stages, stress release becomes dominant, while the synergism tends to finish. 2International consensus statement on robotic hepatectomy surgery in 2018显示文摘The robotic surgical system has been applied in liver surgery. However,controversies concerns exist regarding a variety of factors including the safety,feasibility, efficacy, and cost-effectiveness of robotic surgery. To promote the development of robotic hepatectomy, this study aimed to evaluate the current status of robotic hepatectomy and provide sixty experts' consensus and recommendations to promote its development. Based on the World Health Organization Handbook for Guideline Development, a Consensus Steering Group and a Consensus Development Group were established to determine the topics, prepare evidence-based documents, and generate recommendations. The GRADE Grid method and Delphi vote were used to formulate the recommendations. A total of 22 topics were prepared analyzed and widely discussed during the 4 meetings. Based on the published articles and expert panel opinion, 7 recommendations were generated by the GRADE method using an evidence-based method, which focused on the safety, feasibility, indication,techniques and cost-effectiveness of hepatectomy. 3203例农村有偿献血感染HIV患者抑郁障碍及生活质量调查显示文摘目的:调查通过献血途径感染人类免疫缺陷病毒(HIV)的203例患者的情绪状况及生活质量。方法:采用贝克抑郁问卷、复合性国际诊断用检查提纲和SF-36对203例通过献血途径感染HIV的患者(HIV阳性组)及年龄、性别和受教育年限相匹配的198例HIV阴性献血者进行调查。结果:①HIV阳性组献血次数多于HIV阴性组(52.2±95.4/13.2±23.9,P<0.001),已婚者少于HIV阴性组(88.6%/ 95.5%,P=0.020),丧偶者多于HIV阴性组(10.9%/4.5%,P=0.020),失业率高于HIV阴性组(21.7%/4.0%,P<0.001),前一年全年工作时间少于HIV阴性组(5.0±3.5/7.5±3.0,P<0.001),家庭月收入低于HIV阴性组(476.3±412.7/709.9±513.7,P<0.001)。②HIV阳性组被诊断为获得性免疫缺陷综合征(AIDS)者占56.6%。HIV阳性组抑郁症的终生患病率为13.8%,HIV阴性组的终生患病率为5.1%。HIV阳性组的抑郁总分(11.3±11.3/6.6±9.6)、躯体(3.3±2.9/2.0±2.6)和非躯体因子分(8.0±8.8/4.6±7.4)均高于HIV阴性组(P<0.001)。HIV阳性组达轻度抑郁以上者占40.4%。HIV阳性组已制定自杀计划者高于HIV阴性组(8.4%/3.0%,P<0.05)。HIV阳性组总体生理健康(48.3±9.7/ 55.5±7.0)和心理健康(48.8±8.9/55.1±7.8)得分均低于HIV阴性组(P<0.001)。③HIV阳性组的贝克抑郁问卷总分与T细胞亚群CD4细胞计数无显著相关,与总体生理健康及心理健康呈显著负相关(r=- 0.46~0.68,P<0.001)。结论:HIV阳性患者生存状况较差。对HIV阳性感染者而言,抑郁给患者生活质量造成全面的影响,抑郁和自杀风险仍然是需要干预和处理的问题。宁南义 石川 于欣 陈玉芳 吴尊友 Jin Hua Vigil O Cysique LA Franklin DR Grant I Heaton RK 2008中国心理卫生杂志2008,22,7:14
4Hepatocellular carcinoma: Review of disease and tumor biomarkers显示文摘Hepatocellular carcinoma(HCC) is a common malignancy and now the second commonest global cause of cancer death. HCC tumorigenesis is relatively silent and patients experience late symptomatic presentation. As the option for curative treatments is limited to early stage cancers, diagnosis in non-symptomatic individuals is crucial. International guidelines advise regular surveillance of high-risk populations but the current tools lack sufficient sensitivity for early stage tumors on the background of a cirrhotic nodular liver. A number of novel biomarkers have now been suggested in the literature, which may reinforce the current surveillance methods. In addition, recent metabonomic and proteomic discoveries have established specific metabolite expressions in HCC, according to Warburg's phenomenon of altered energy metabolism. 6LC-MS-based lipidomic analysis in distinguishing patients with nonalcoholic steatohepatitis from nonalcoholic fatty liver显示文摘Background: Nonalcoholic fatty liver disease(NAFLD) is one of the main liver diseases, and its pathologic profile includes nonalcoholic fatty liver(NAFL) and nonalcoholic steatohepatitis(NASH). However, there is no reliable non-invasive parameter in distinguishing NASH from NAFL in clinical practice. The present study was to find a non-invasive way to differentiate these two categories of NAFLD via lipidomic analysis. Methods: Lipidomic analysis was used to determine the changes of lipid moieties in blood from 20 NAFL and 10 NASH patients with liver biopsy. Liver histology was evaluated after hematoxylin and eosin staining and Masson’s trichrome staining. The profile of lipid metabolites in correlation with steatosis, inflammation, hepatocellular necroptosis, fibrosis, and NAFLD activity score(NAS) was analyzed. Results: Compared with NAFL patients, NASH patients had higher degree of steatosis, ballooning degeneration, lobular inflammation. A total of 434 different lipid molecules were identified, which were mainly composed of various phospholipids and triacylglycerols. Many lipids, such as phosphatidylcholine(PC)(P-22:0/18:1), sphingomyelin(SM)(d14:0/18:0), SM(d14:0/24:0), SM(d14:0/22:0), phosphatidylethanolamine(PE)(18:0/22:5), PC(O-22:2/12:0), and PC(26:1/11:0) were elevated in the NASH group compared to those in the NAFL group. Specific analysis revealed an overall lipidomic profile shift from NAFL to NASH, and identified valuable lipid moieties, such as PCs [PC(14:0/18:2), PE(18:0/22:5) and PC(26:1/11:0)] or plasmalogens [PC(O-22:0/0:0), PC(O-18:0/0:0), PC(O-16:0/0:0)], which were significantly altered in NASH patients. In addition, PC(14:0/18:2), phosphatidic acid(18:2/24:4) were positively correlated with NAS;whereas PC(18:0/0:0) was correlated positively with fibrosis score. Conclusions: The present study revealed overall lipidomic profile shift from NAFL to NASH, identified valuable lipid moieties which may be non-invasive biomarkers in the categorization of NAFLD.
