1Transitions of the Understanding and Definition of Primary Glaucoma显示文摘Objective:In previous decades,glaucoma has been primarily attributed to elevated intraocular pressure (IOP),but this has gradually been replaced by the development of optic neuropathy as the central concept of glaucoma in developed countries.However,there still remain strong controversies in the definition of glaucoma in China.In this current review,we are going to discuss these controversies and elaborate on the historical transitions of the definition of glaucoma both in China and developed countries.Furthermore,we will briefly describe the 'ocular-cranial pressure gradient'theory and discuss the relationship between glaucoma and degenerative diseases of the central nervous system (CNS)in order to show the complex pathogenesis of glaucoma and the importance for the modification to the definition of glaucoma. Data Sources:We performed a comprehensive search in both PubMed and SinoMed using the following keywords:(a)'primary glaucoma'and 'guideline,'(b)'ocular-cranial pressure gradient,'and (c)'glaucoma,''Alzheimer's disease,'and 'Parkinson's disease.' The literature search included the related articles with no restrictions on publication date. Study Selection:The primary references were Chinese and English articles including (a)original guidelines and expert consensuses of primary glaucoma,(b)reviews focusing on the differences between various versions of these guidelines and consensuses,and (c)papers about ocular-cranial pressure gradient theory and the relationship between glaucoma and CNS degenerative diseases. Results:The definitions and classifications of both primary open-angle glaucoma and primary angle-closure glaucoma differ between Chinese glaucoma consensuses and international primary glaucoma guidelines.Chinese definitions and classifications put more emphasis on the IOP,while international guidelines put more emphasis on the presence of optic neuropathy.The ocular-cranial pressure gradient theory and the research on the relationship between glaucoma and CNS degenerative diseases have provided new directions for exploring the pathogenesis of glaucoma. Conclusions:As regards the definition and classification of primary glaucoma,we find that there are still some discrepancies between Chinese expert consensuses and international guidelines.Glaucoma is a disease with complex etiologies,while its common characteristic is a specific optic neuropathy.The current definition and understanding of glaucoma is an ongoing and evolving process,reflecting our latest available evidence on its pathogenesis.Chinese ophthalmology community may need to update our guidelines,accommodating these latest developments.Si-An Liu Zhen-Ni Zhao Nan-Nan Sun Ying Han Jeremy Chen Zhi-Gang Fan 2018Chinese Medical Journal2018,,23:6
2Tumour suppressor HLJ1: A potential diagnostic, preventive and therapeutic target in non-small cell lung cancer显示文摘Lung cancer is the leading cause of cancer-related mortality throughout the world. Non-small cell lung cancer(NSCLC) accounts for 85% of all diagnosed lung cancers. Despite considerable progress in the diagnosis and treatment of the disease, the overall 5-year survival rate of NSCLC patients remains lower than 15%. The most common causes of death in lung cancer patients are treatment failure and metastasis. Therefore, developing novel strategies that target both tumour growth and metastasis is an important and urgent mission for the next generation of anticancer therapy research. Heat shock proteins(HSPs), which are involved in the fundamental defence mechanism for maintaining cellular viability, are markedly activated during environmen-tal or pathogenic stress. HSPs facilitate rapid cell division, metastasis, and the evasion of apoptosis in cancer development. These proteins are essential players in the development of cancer and are prime therapeutic targets. In this review, we focus on the current understanding of the molecular mechanisms responsible for HLJ1's role in lung cancer carcinogenesis and progression. HLJ1, a member of the human HSP 40 family, has been characterised as a tumour suppressor. Research studies have also reported that HLJ1 shows promising dual anticancer effects, inhibiting both tumour growth and metastasis in NSCLC. 7Objective assessment of bradykinesia in Parkinson’s disease using evolutionary algorithms:clinical validation显示文摘Background:There is an urgent need for developing objective,effective and convenient measurements to help clinicians accurately identify bradykinesia.The purpose of this study is to evaluate the accuracy of an objective approach assessing bradykinesia in finger tapping(FT)that uses evolutionary algorithms(EAs)and explore whether it can be used to identify early stage Parkinson’s disease(PD).Methods:One hundred and seven PD,41 essential tremor(ET)patients and 49 normal controls(NC)were recruited.Participants performed a standard FT task with two electromagnetic tracking sensors attached to the thumb and index finger.Readings from the sensors were transmitted to a tablet computer and subsequently analyzed by using EAs.The output from the device(referred to as'PD-Monitor')scaled from−1 to+1(where higher scores indicate greater severity of bradykinesia).Meanwhile,the bradykinesia was rated clinically using the Movement Disorder Society-Sponsored Revision of the Unified Parkinson’s Disease Rating Scale(MDS-UPDRS)FT item.Results:With an increasing MDS-UPDRS FT score,the PD-Monitor score from the same hand side increased correspondingly.PD-Monitor score correlated well with MDS-UPDRS FT score(right side:r=0.819,P=0.000;left side:r=0.783,P=0.000).Moreover,PD-Monitor scores in 97 PD patients with MDS-UPDRS FT bradykinesia and each PD subgroup(FT bradykinesia scored from 1 to 3)were all higher than that in NC.Receiver operating characteristic(ROC)curves revealed that PD-Monitor FT scores could detect different severity of bradykinesia with high accuracy(≥89.7%)in the right dominant hand.Furthermore,PD-Monitor scores could discriminate early stage PD from NC,with area under the ROC curve greater than or equal to 0.899.Additionally,ET without bradykinesia could be differentiated from PD by PD-Monitor scores.A positive correlation of PD-Monitor scores with modified Hoehn and Yahr stage was found in the left hand sides.Conclusions:Our study demonstrated that a simple to use device employing classifiers derived from EAs could not only be used to accurately measure different severity of bradykinesia in PD,but also had the potential to differentiate early stage PD from normality.Chao Gao Stephen Smith Michael Lones Stuart Jamieson Jane Alty Jeremy Cosgrove Pingchen Zhang Jin Liu Yimeng Chen Juanjuan Du Shishuang Cui Haiyan Zhou Shengdi Chen 2018Translational Neurodegeneration2018,7,1:3

13Effectiveness, safety, and drug sustainability of biologics in elderly patients with inflammatory bowel disease: A retrospective study显示文摘BACKGROUND Biologic therapy resulted in a significant positive impact on the management of inflammatory bowel disease(IBD) however data on the efficacy and side effects of these therapies in the elderly is scant.AIM To evaluate retrospectively the drug sustainability, effectiveness, and safety of the biologic therapies in the elderly IBD population.METHODS Consecutive elderly(≥ 60 years old) IBD patients, treated with biologics [infliximab(IFX), adalimumab(ADAL), vedolizumab(VDZ), ustekinumab(UST)] followed at the McGill University Inflammatory Bowel Diseases Center were included between January 2000 and 2020.Efficacy was measured by clinical scores at 3, 6-9 and 12-18 mo after initiation of the biologic therapy. Patients completing induction therapy were included. Adverse events(AEs) or serious AE were collected during and within three months of stopping of the biologic therapy.RESULTS We identified a total of 147 elderly patients with IBD treated with biologicals during the study period, including 109 with Crohn’s disease and 38 with ulcerative colitis. Patients received the following biologicals: IFX(28.5%), ADAL(38.7%), VDZ(15.6%), UST(17%). The mean duration of biologic treatment was 157.5(SD = 148) wk. Parallel steroid therapy was given in 34% at baseline,19% at 3 mo, 16.3% at 6-9 mo and 6.5% at 12-18 mo. The remission rates at 3, 6-9 and 12-18 mo were not significantly different among biological therapies. Kaplan-Meyer analysis did not show statistical difference for drug sustainability(P = 0.195), time to adverse event(P = 0.158) or infection rates(P = 0.973) between the four biologics studied. The most common AEs that led to drug discontinuation were loss of response, infusion/injection reaction and infection.CONCLUSION Current biologics were not different regarding drug sustainability, effectiveness, and safety in the elderly IBD population. 20Strengthened Regulation of the Onset of the South China Sea Summer Monsoon by the Northwest Indian Ocean Warming in the Past Decade显示文摘Traditionally,a delayed(early)onset of the South China Sea summer monsoon(SCSSM)has been observed to follow a warm(cold)El Niño-Southern Oscillation(ENSO)event in winter,supporting high seasonal predictability of SCSSM onset.However,the empirical seasonal forecasting skill of the SCSSM onset,solely based on ENSO,has deteriorated since 2010.Meanwhile,unexpected delayed onsets of the SCSSM have also occurred in the past decade.We attribute these changes to the Northwest Indian Ocean(NWIO)warming of the sea surface.The NWIO warming has teleconnections related to(1)suppressing the seasonal convection over the South China Sea,which weakens the impacts of ENSO on SCSSM onset and delays the start of SCSSM,and(2)favoring more high-frequency,propagating moist convective activities,which enhances the uncertainty of the seasonal prediction of SCSSM onset date.Our results yield insight into the predictability of the SCSSM onset under the context of uneven ocean warming operating within the larger-scale background state of global climate change.Yang AI Ning JIANG Weihong QIAN Jeremy Cheuk-Hin LEUNG Yanying CHEN 2022Advances in Atmospheric Sciences2022,39,6:1
