| 1 | Endoscopy and polyps-diagnostic and therapeutic advances in management显示文摘Despite multiple efforts aimed at early detection through screening, colon cancer remains the third leading cause of cancer-related deaths in the United States, with an estimated 51000 deaths during 2013 alone. The goal remains to identify and remove benign neoplastic polyps prior to becoming invasive cancers. Polypoid lesions of the colon vary widely from hyperplastic, hamartomatous and inflammatory to neoplastic adenomatous growths. Although these lesions are all benign, they are common, with up to one-quarter of patients over 60 years old will develop pre-malignant adenomatous polyps. Colonoscopy is the most effective screening tool to detect polyps and colon cancer, although several studies have demonstrated missed polyp rates from 6%-29%, largely due to variations in polyp size. This number can be as high as 40%, even with advanced (> 1 cm) adenomas. Other factors including sub-optimal bowel preparation, experience of the endoscopist, and patient anatomical variations all affect the detection rate. Additional challenges in decision-making exist when dealing with more advanced, and typically larger, polyps that have traditionally required formal resection. | 3 | Nationwide trends and predictors of inpatient mortality in 83884 transjugular intrahepatic portosystemic shunt显示文摘AIM: To evaluate and validate the national trends and predictors of in-patient mortality of transjugular intrahepatic portosystemic shunt(TIPS) in 15 years.METHODS: Using the National Inpatient Sample which is a part of Health Cost and Utilization Project, we identified a discharge-weighted national estimate of 83884 TIPS procedures performed in the United States from 1998 to 2012 using international classification of diseases-9 procedural code 39.1. The demographic, hospital and co-morbility data were analyzed using a multivariant analysis. Using multi-nominal logistic regression analysis, we determined predictive factors related to increases in-hospital mortality. Comorbidity measures are in accordance to the Comorbidity Software designed by the Agency for Healthcare Research and Quality.RESULTS: Overall, 12.3% of patients died during hospitalization with downward trend in-hospitalmortality with the mean length of stay of 10.8 ± 13.1 d. Notable, African American patients(OR = 1.809 vs Caucasian patients, P < 0.001), transferred patients(OR = 1.347 vs non-transferred, P < 0.001), emergency admissions(OR = 3.032 vs elective cases, P < 0.001), patients in the Northeast region(OR = 1.449 vs West, P < 0.001) had significantly higher odds of inhospital mortality. Number of diagnoses and number of procedures showed positive correlations with in-hospital death(OR = 1.249 per one increase in number of procedures). Patients diagnosed with acute respiratory failure(OR = 8.246), acute kidney failure(OR = 4.359), hepatic encephalopathy(OR = 2.217) and esophageal variceal bleeding(OR = 2.187) were at considerably higher odds of in-hospital death compared with ascites(OR = 0.136, P < 0.001). Comorbidity measures with the highest odds of in-hospital death were fluid and electrolyte disorders(OR = 2.823), coagulopathy(OR = 2.016), and lymphoma(OR = 1.842).CONCLUSION: The overall mortality of the TIPS procedure is steadily decreasing, though the length of stay has remained relatively constant. | 9 | Levels and correlates of 24-hour movement behaviors among South Koreans:Results from the Korea National Health and Nutrition Examination Surveys,2014 and 2015显示文摘Purpose.This study aimed to examine the levels and correlates of 24-h movement behaviors(i.e.,physical activity,sedentary time,and sleep),and different patterns of these behaviors in a nationally representative sample of South Koreans aged 12 years and older.This study also aimed to examine the sociodemographic correlates of 24?h movement behaviors stratified by age groups.Methods.Self-reported,repeated cross-sectional data from 10,708 participants in the 2014 and 2015 Korea National Health and Nutrition Examination surveys were used.Key variables included moderate-to-vigorous intensity physical activity,muscular strengthening exercises,walking,active transportation,sedentary time,and sleep.Sociodemographic variables included age,sex,household income,area of residence,and education level.Descriptive statistics by sex and age as well as general linear models by age group were performed.Results.The proportions of individuals meeting the moderate-to-vigorous intensity physical activity,muscular strengthening exercises,and sleep guidelines were,respectively,21.6%,22.1%,and 32.5%in male youth;6.9%,4.5%,and 22.8%in female youth;55.5%,30.8%,and 54.0%in male adults;48.8%,14.4%,and 57.6%in female adults;44.0%,30.6%,and 45.5%in male older adults;and 29.5%,8.9%,and 37.3%in female older adults.The proportions of individuals showing the most ideal combinations of 24-h movement behaviors were only 3.2%in youth,0.4%in adults,and 0 in older adults.Universally,older age,female sex,or living in metro Seoul were associated with unfavorable patterns of 24-h movement behaviors across different age groups.However,the associations of income and education with movement behaviors were mixed across age groups.Conclusion.Overall,the proportion of South Koreans with a healthy 24-h movement behavior pattern is low.The sociodemographic correlates of different types of 24-h movement behaviors should be considered when designing targeted interventions for the promotion of healthy active living for South Koreans. | Eun-Young,Lee Valerie Carson Justin Y.Jeon John C.Spence Mark S.Tremblay | 2019 | Journal of Sport and Health Science2019，8，4： | 2 |
