1Hepatitis B virus genotypes:an overview显示文摘BACKGROUND:Hepatitis B virus(HBV)infection is a major cause of mortality and morbidity globally.The quest continues to identify viral factors that influence disease progression and severity as well as responses to treatment of HBV infection.Based on variations in HBV,the virus has been divided into a number of genotypes. DATA SOURCES:Review of published literature on HBV genotypes. RESULTS:HBV genotypes are likely to be important in determining the severity and progression of HBV-induced liver disease as well as responses to different anti-viral agents. 2A review on promising phytochemical, nutritional and glycemic control studies on Moringa oleifera Lam. in tropical and sub-tropical regions显示文摘Plants have provided sources to find novel compounds. These plants are being used as therapeutic purposes since the birth of mankind. The traditional healers normally utilize medicinal plants as crude drugs while scientists using the folk claim as guides to explore medicinal plants. Moringa oleifera is a famous edible plant having therapeutic and nutritive values. The present study was designed to cumulate the research data regarding to what extent, phytochemical, nutritional and glycemic control studies has been explored using its different extracts. The articles indicated that the powder, aqueous, methanol and ethanol extracts of Moringa oleifera(leaves, pods, seeds, stem and root bark) have significant therapeutic herbal potential to treat diabetes mellitus. Collectively, the mechanism behind is intestinal glucose inhibition, insulin release as well as decrease in insulin resistance probably regeneration of b-cells of pancreas, increase in glutathione and reduction in malondialdehyde. Conclusively, this article give descriptive information about antidiabetic effect, claimed marker compounds and proposed antihyperglycemic mechanism of a single plant. 4PI3K signaling pathway targeting by using different molecular approaches to treat cancer显示文摘Recent evidence of research has been proposed that the phosphoinositide 3-kinase(PI3K) pathway is noticeable target for searching novel anticancer agents. The phosphoinositide 3-kinase(PI3K) is accountable for harmonizing a diverse range of cell functions, such as transcription, proliferation, cell survival, cell growth, degranulation, vesicular trafficking and cell migration, which are mostly involved in carcinogenesis. Particularly, PI3K-mediated signaling molecules and its effects on gene expression contribute to tumorigenesis. PI3Ks generally are grouped into three distinct classes: Ⅰ, Ⅱ and Ⅲ according to their structure and function. The class IA of PI3K includes an alpha, beta or delta p110 catalytic subunit(p110α, p110β, or p110γ), which are associated with the activation of RTKs. Mutations in PIK3CA, the gene encoding the p110α catalytic subunit of PI3K, have just been recognized as novel mechanisms of inducing oncogenic PI3K signaling. Therefore, the class IA PI3K is the only one of most evidently implicated in cancer. The PI3K pathway is mostly mutated in more cancer patients compared with normal person, making it an eyecatching molecular target for analyses based on inhibitor molecule. In this article, we highlighted the signaling effects and regulation pathway of PI3K involved in the development and survival of tumor cells. 5Hepatitis E virus is a leading cause of acute-on-chronic liver disease:experience from a tertiary centre in Bangladesh显示文摘BACKGROUND:Acute-on-chronic liver failure(ACLF) is common in Bangladesh.Acute viral E hepatitis is sporadically encountered in this country each year,with a rising incidence in the rainy season.This study aimed to identify the etiology of ACLF in Bangladesh. METHODS:In this retrospective study,69 ACLF patients were included.They presented to our department at the Bangabandhu Sheikh Mujib Medical University in Dhaka.History of diseases was recorded and appropriate investigations were conducted in all patients. RESULTS:Acute hepatitis E virus(HEV)infection was positive in 21.7%(15/69)of the patients,while 14.5% (10/69)had septicemia.Upper gastrointestinal tract hemorrhage was seen in 4.3%of the patients(3/69),while another 4.3%(3/69)had a positive history for alcohol or drugs.None of the patients tested positive for hepatitis A virus infection and no evidence of hepatitis B virus flare was found in any patient.No specific cause for ACLF could be identified. 6Epidemiology of hepatitis B virus in Bangladeshi general population显示文摘BACKGROUND:Hepatitis B virus(HBV)is encountered sporadically the year round in Bangladesh.It results in a wide range of liver diseases,with asymptomatic acute hepatitis at one end to hepatocellular carcinoma(HCC)at the other end of the spectrum. METHODS:All 1018 individuals of different age groups and sex with varied religious,educational and social backgrounds were tested for HBsAg by ELISA.The positive samples were further tested by ELISA for HBeAg.Before testing,blood samples were preserved at-20℃.The study was conducted in a semi-urban location on the outskirts of Dhaka. RESULTS:Of the 1018 individuals,5.5%tested positive for HBsAg.None were tested positive for anti-HCV.Among the HBsAg-positive population,58.93%were HBeAg- positive and the rest 41.07%HBeAg-negative.There was a male predominance and those who were tested positive were mostly between 16 and 50 years of age.Major risk factors for exposure to HBV appeared to be injudicious use of injectable medications,treatment by unqualified, traditional practitioners,mass-vaccination against cholera and smallpox,barbers and body piercing. 7Effects of vitamin E on stroke: a systematic review with meta- analysis and trial sequential analysis显示文摘There are several previous studies on the association of vitamin E with prevention of stroke but the findings remain controversial.We have conducted a systematic review,meta-analysis together with trial sequential analysis of randomised controlled trials to evaluate the effect of vitamin E supplementation versus placebo/no vitamin E on the risk reduction of total,fatal,non-fatal,haemorrhagic and ischaemic stroke.Relevant studies were identified by searching online databases through Medline,PubMed and Cochrane Central Register of Controlled Trials.A total of 18 studies with 148016 participants were included in the analysis.There was no significant difference in the prevention of total stroke(RR(relative risk)=0.98,95% CI 0.92-1.04,p=0.57),fatal stroke(RR=0.96,95% CI 0.77-1.20,p=0.73)and non-fatal stroke(RR=0.96,95% CI 0.88-1.05,p=0.35).Subgroup analyses were performed under each category(total stroke,fatal stroke and non-fatal stroke)and included the following subgroups(types of prevention,source and dosage of vitamin E and vitamin E alone vs control).The findings in all subgroup analyses were statistically insignificant.In stroke subtypes analysis,vitamin E showed significant risk reduction in ischaemic stroke(RR=0.92,95% CI 0.85-0.99,p=0.04)but not in haemorrhagic stroke(RR=1.17,95%CI 0.98-1.39,p=0.08).However,the trial sequential analysis demonstrated that more studies were needed to control random errors.Limitations of this study include the following:trials design may not have provided sufficient power to detect a change in stroke outcomes,participants may have had different lifestyles or health issues,there were a limited number of studies available for subgroup analysis,studies were mostly done in developed countries,and the total sample size for all included studies was insufficient to obtain a meaningful result from meta-analysis.In conclusion,there is still a lack of statistically significant evidence of the effects of vitamin E on the risk reduction of stroke.Nevertheless,vitamin E may offer some benefits in the prevention of ischaemic stroke and additional well-designed randomised controlled trials are needed to arrive at a definitive finding.PROSPERO registration number:CRD42020167827.Hong Chuan Loh Renly Lim Kai Wei Lee Chin Yik Ooi Deik Roy Chuan Irene Looi Yuen Kah Hay Nurzalina Abdul Karim Khan 2021Stroke & Vascular Neurology2021,6,1:2
8Recent Progress,Challenges,and Prospects in Two‑Dimensional Photo‑Catalyst Materials and Environmental Remediation显示文摘The successful photo-catalyst library gives significant information on feature that affects photo-catalytic performance and proposes new materials.Competency is considerably significant to form multi-functional photo-catalysts with flexible characteristics.Since recently,two-dimensional materials(2DMs)gained much attention from researchers,due to their unique thickness-dependent uses,mainly for photo-catalytic,outstanding chemical and physical properties.Photo-catalytic water splitting and hydrogen(H2)evolution by plentiful compounds as electron(e−)donors is estimated to participate in constructing clean method for solar H2-formation.Heterogeneous photocatalysis received much research attention caused by their applications to tackle numerous energy and environmental issues.This broad review explains progress regarding 2DMs,significance in structure,and catalytic results.We will discuss in detail current progresses of approaches for adjusting 2DMs-based photo-catalysts to assess their photo-activity including doping,hetero-structure scheme,and functional formation assembly.Suggested plans,e.g.,doping and sensitization of semiconducting 2DMs,increasing electrical conductance,improving catalytic active sites,strengthening interface coupling in semiconductors(SCs)2DMs,forming nano-structures,building multi-junction nano-composites,increasing photo-stability of SCs,and using combined results of adapted approaches,are summed up.Hence,to further improve 2DMs photo-catalyst properties,hetero-structure design-based 2DMs’photo-catalyst basic mechanism is also reviewed.Karim Khan Ayesha Khan Tareen Muhammad Aslam Rizwan Ur Rehman Sagar Bin Zhang Weichun Huang Asif Mahmood Nasir Mahmood Kishwar Khan Han Zhang Zhongyi Guo 2020Nano-Micro Letters2020,12,12:2

13Viral load speaks little about toll on liver显示文摘BACKGROUND:Bangladesh is situated in the intermediate prevalence region of hepatitis B virus (HBV).The lifetime risk of acquiring HBV infection in Bangladesh is greater than 40%.It has been estimated that this virus is responsible for 10%-35%cases of acute viral hepatitis,35.7%cases of fulminant hepatic failure, 33.3%-40.5%cases of chronic hepatitis and 46.8%cases of hepatocellular carcinoma in Bangladesh.The aim of this study is to compare the correlation between HBV DNA load and grade and stage of liver disease in patients with chronic hepatitis B(CHB). METHODS:Percutaneous liver biopsies done in 159 CHB patients revealed 62.9%(100 patients)had wild type HBV infection and the rest 37.1%(59)had pre-core/core promoter mutant HBV infection.HBV DNA load was measured using PCR in all patients. RESULTS:In the wild type CHB group,97%(97 patients) had moderate to high HBV DNA load and 3%(3)had low to moderate HBV DNA.In the pre-core/core promoter mutant group,74.6%(44 patients)had moderate to high HBV DNA and the rest 25.4%(15)had low to moderate HBV DNA.The patients with moderate to high HBV DNA of the patients with wild type CHB,78.4%(76 patients) had minimal to mild chronic hepatitis(HAI-NI 0-8)and 21.6%(21)had moderate to severe chronic hepatitis(HAI- NI 9-18).66.6%(2 patients)and 33.3%(1)patients with low to moderate HBV DNA load had minimal to mild and moderate to severe chronic hepatitis respectively.In the moderate to high HBV DNA group,77.3%(75 patients) patients had minimal to moderate fibrosis(HAI-F 0-2)and 22.7%(22)(HAI-F 3-4)had severe fibrosis to cirrhosis.These figures were 33.3%(1 patient)and 66.6% (2)respectively in the patients with low to moderate HBV DNA load.On the other hand in case of patients with pre-core/core promoter mutant type CHB,in the moderate to high HBV DNA group,79.5%(35 patients)had minimal to mild chronic hepatitis(HAI-NI 0-8)and 20.5%(9)had moderate to severe chronic hepatitis(HAI-NI 9-18).93.3% (14)and 6.7%(1)patients with low to moderate HBV DNA load had minimal to mild and moderate to severe chronic hepatitis respectively.In the moderate to high HBV DNA group,68.2%(30 patients)had minimal to moderate fibrosis(HAI-F 0-2)and 31.8%(14)(HAI-F 3-4)had severe fibrosis to cirrhosis.These figures were 86.7%(13)and 13.3%(2)respectively in patients with low to moderate HBV DNA load.
