1From NAFLD to MAFLD:a“redefining”moment for fatty liver disease显示文摘The term non-alcoholic fatty liver disease(NAFLD)was coined in 1980 to characterize a disease similar to alcoholic fatty liver disease that developed in patients without a history of excessive alcohol intake.[1]Morphologically,NAFLD is characterized by excess fatty infiltration of the liver in the absence of known causes of liver disease(eg,alcohol,autoimmune liver disease,viral hepatitis,etc).The clinical manifestations of NAFLD(both hepatic and extrahepatic)depend on the outcome of complex interactions between its primary drivers including poor lifestyle habits and diet,a dysfunctional microbiota,genetic predisposition,and environmental cues that result in metabolic dysfunction and liver disease.However,bringing all patients with their markedly different clinical courses under the NAFLD umbrella belies its complexity and implies a homogeneous disease state that then negatively impacts clinical management and a deeper understanding of pathogenesis.With advances in current knowledge on the spectrum of fatty liver diseases,it is apparent that the fourdecade-old outdated term NAFLD can no longer serve to usefully describe a highly heterogeneous disease.The disease as we understand it today not only impacts patients who consume alcohol and those who do not,but also potentially impacts all patients with any form of liver disease,by acting as a disease modifier.[2]Kenneth IZheng Jian-Gao Fan Jun-Ping Shi Vincent Wai-Sun Wong Mohammed Eslam Jacob George Ming-Hua Zheng 2020Chinese Medical Journal2020,,19:14
2Macrophages in metabolic associated fatty liver disease显示文摘Metabolic associated fatty liver disease(MAFLD),formerly named non-alcoholic fatty liver disease is the most common liver disorder in many countries.The inflammatory subtype termed steatohepatitis is a driver of disease progression to cirrhosis,hepatocellular carcinoma,liver transplantation,and death,but also to extrahepatic complications including cardiovascular disease,diabetes and chronic kidney disease.The plasticity of macrophages in response to various environmental cues and the fact that they can orchestrate cross talk between different cellular players during disease development and progression render them an ideal target for drug development.This report reviews recent advances in our understanding of macrophage biology during the entire spectrum of MAFLD including steatosis,inflammation,fibrosis,and hepatocellular carcinoma,as well as for the extra-hepatic manifestations of MAFLD.We discuss the underlying molecular mechanisms of macrophage activation and polarization as well as cross talk with other cell types such as hepatocytes,hepatic stellate cells,and adipose tissue.We conclude with a discussion on the potential translational implications and challenges for macrophage based therapeutics for MAFLD.Jawaher Alharthi Olivier Latchoumanin Jacob George Mohammed Eslam 2020World Journal of Gastroenterology2020,26,16:5
3Biomarkers of Metabolic(Dysfunction)-associated Fatty Liver Disease:An Update显示文摘The prevalence of metabolic(dysfunction)-associated fatty liver disease(MAFLD)is rapidly increasing and affects up to two billion individuals globally,and this has also resulted in increased risks for cirrhosis,hepatocellular carcinoma,and liver transplants.In addition,it has also been linked to extrahepatic consequences,such as cardiovascular disease,diabetes,and various types of cancers.However,only a small proportion of patients with MAFLD develop these complications.Therefore,the identification of high-risk patients is paramount.Liver fibrosis is the major determinant in developing these complications.Although,liver biopsy is still considered the gold standard for the assessment of patients with MAFLD.Because of its invasive nature,among many other limitations,the search for noninvasive biomarkers for MAFLD remains an area of intensive research.In this review,we provide an update on the current and future biomarkers of MAFLD,including a discussion of the associated genetics,epigenetics,microbiota,and metabolomics.We also touch on the next wave of multiomic-based biomarkers.Jawaher Alharthi Mohammed Eslam 2022Journal of Clinical and Translational Hepatology2022,10,1:3

9Sonographic gallbladder wall thickness measurement and the prediction of esophageal varices among cirrhotics显示文摘Acute variceal bleeding in patients with liver cirrhosis and portal hypertension(PHT) is the most serious emergency complication among those patients and could have catastrophic outcomes if not timely managed. Early screening by esophago-gastro-duodenoscopy(EGD) for the presence of esophageal varices(EVs) is currently recommended by the practice guidelines for all cirrhotic patients. Meanwhile, EGD is not readily accepted or preferred by many patients. The literature is rich in studies to investigate and validate non-invasive markers of EVs prediction aiming at reducing the unneeded endoscopic procedures. Gallbladder(GB) wall thickness(GBWT) measurement has been found promising in many published research articles. We aim to highlight the validity of sonographic GBWT measurement in the prediction of EVs based on the available evidence. We searched databases including Cochrane library, Pub Med, Web of Science and many others for relevant articles. GBWT is associated with the presence of EVs in cirrhotic patients with PHT of different etiologies. The cut-off of GBWT that can predict the presence of EVs varied in the literature and ranges from 3.1 mm to 4.35 mm with variable sensitivities of 46%-90.9% and lower cutoffs in viral cirrhosis compared to non-viral, however GBWT > 4 mm in many studies is associated with acceptable sensitivity up to 90%. Furthermore, a relation was also noticed with the degree of varices and portal hypertensive gastropathy.Among cirrhotics, GBWT > 3.5 mm predicts the presence of advanced(grade Ⅲ-Ⅳ) EVs with a sensitivity of 45%, the sensitivity increased to 92% when a cut-off ≥ 3.95 mm was used in another cohort. Analysis of these results should carefully be revised in the context of ascites, hypoalbuminemia and other intrinsic GB diseases among cirrhotic patients. 18Geochemistry of magnetite in beach sands, stream sediments,and in situ magnetites in surrounding rocks at north Taiwan island显示文摘The geochemistry of magnetite provides constraints on the source(s) of the magnetite and other ore minerals. In this study, we constructed a magnetite distribution map, compared the chemical composition of magnetite present in hard rocks with that present in sand beach and stream deposits to determine magnetite’s provenance,investigated relationships among different trace elements,examined variations in the concentrations of trace elements present in magnetite, and identified the type of studied magnetite. Laser ablation inductively coupled plasma mass spectrometry(LA-ICP-MS) and inductively coupled plasma mass spectrometry/inductively coupled plasma atomic emission spectroscopy was used to analyze samples available in small and large amounts, respectively. The average magnetite compositions of beach and river samples were substantially similar to those of andesite rocks,revealing that the magnetite samples may have originated from surrounding andesite rocks;this finding is consistent with the chemical composition of the samples analyzed using LA-ICP-MS. Some of the andesite rocks found in the study area were the primary source of magnetite aggregations in the beach sands and stream sands near or slightly away from these rocks. High Ti–V magnetites lie within the fields of Fe–Ti–V bearing magnetite and titanomagnetite(ulv?spinel). These titanomagnetites may be undergoing transformation to spinel minerals such as galaxite,gahnite, magnesiochromite, and chromite, which are characterized by the high concentrations of Al, Ti, Mg, Cr,Ni, Mn, and Zn. The positive correlation between Fe and Ti, Cr and Ni, and also the negative correlation between Fe and Mg, Mn and Zn can be attributed to various magnetite phases present in the study area. V, Co, Mo, Nb, Ga, Sn,and Ta exhibited a positive correlation with Fe and a negative correlation with Al. The positive correlation between Fe and these elements indicates the enrichment of these elements in the magnetite series. We observed a negative correlation between Fe and Li, Rb, Ca, Ba, Sr, Cu,Cd, Sb, As, Sc, Y, REE, U, and Th. 19Evaluation of the chemical constituents,antioxidant and enzyme inhibitory activities of six Yemeni green coffee beans varieties显示文摘Coffee arabica L.is an economical crop cultivated in many countries including Yemen.The present study evaluated the phytochemical contents of the methanolic extracts,prepared by maceration,of six Yemeni green coffee beans varieties namely Esmaeli,Hamadi,Harazi,Mattari,Odaini and Yafei.Antioxidant(phosphomolybdenum,antiradical,reducing power and ferrous chelating),and enzyme inhibition activity of the extracts against acetylcholinesterase butyrylcholinesterase,tyrosinase,α-glucosidase,andα-amylase were also studied.Quantification of total phenols revealed that all green beans varieties were richer in their total polyphenolic(63.77-110.98 mg gallic acid equivalent per g of extract)content than their flavonoids one(2.86-5.57 mg rutin equivalent per g of extract).HPLC-MS/MS analysis of 30 selected bioactive compounds showed that all varieties had the same types of phytoconstituents with differences in their relative abundance.Caffeine(1613.89-2466.38μg/g)followed by 5-caffeoylquinic acid(1017.63-1313.39μg/g)were the dominant compounds in all varieties with caffeine more abundant in Esmaeli variety while 5-caffeoylquinic acid in Odaini variety.All varieties displayed a pronounced antioxidant property in all the in vitro assays with Odaini variety significantly(p<0.05)exerted the highest anti-DPPH radicals(253.96 mg Trolox equivalent(TE)/g),metal chelating(19.73 mg Disodium edetate equivalents/g),Cu^(2+)(441.11 mg TE/g)and Fe3+(221.04 mg TE/g)reducing activities.Furthermore,the majority of green coffee beans varieties showed comparable enzyme inhibition property with highest activity recorded against tyrosinase(39.35-46.96 mg kojic acid equivalents/g)and acetylcholinesterase(1.80-2.17 mg galanthamine equivalents/g)enzymes.In conclusion,all Yemeni green coffee beans varieties have proven to be rich source of biochemicals with beneficial impact on human health and could be of significant applications in food,pharmaceutical and cosmetics industry.Ammar Mohammed Ahmed Ali Sakina Yagi Ahmed A.Qahtan Abdurrahman A.Alatar Simone Angeloni Filippo Maggi Giovanni Caprioli Eslam M.Abdel-Salam Kouadio Ibrahime Sinan Gokhan Zengin 2022Food Bioscience2022,46,2:0
20Conventional generation emulation for power grids with a high penetration of wind power显示文摘Integrating large-scale wind plants with the electricity grids has many challenges for grid operators.Besides the variability and uncertainty of wind power,coordinating between different technologies of generation in the same grid can be considered the main problem,specifically for short-term frequency stability.Therefore,a large penetration of wind power generation in modern power grids has a risky influence on the power-system frequency.Wind-generation plants have contradictory behaviour compared to classic thermal plants,especially in active generated power-shortage events due to the variable nature of wind power.Existing experience in wind plants keeps part of the available wind power unloaded,using what are known as deloading techniques.Different deloading techniques are usually applied to emulate the thermal-plant-governor function and confirm a proper spinning reserve for any active-power shortages.These techniques decrease the generated power from wind plants continuously from maximum point tracking ones.Consequently,the practical capacity,annual generated energy and economical income of wind plants are reduced.In addition,grid-protection and control sub-schemes are set and designed according to the well-known conventional responses of thermal plants,which increase the need for thermal-plant-behaviour emulation.In this paper,instead of the usual deloading methods,a supercapacitors scheme is proposed with wind turbines to emulate the response of conventional power plants.The study discusses the technical and economic benefits of the proposed addition of supercapacitors in the wind-plant-planning phase.Restricted frequency grid-code indices are selected to evaluate studied behaviours.Simulation results of the IEEE four-generation two-area system determines the effectiveness of suggested schemes technically.The System Advisor Model(SAM)program estimates the economic benefits of a typical US study case compared with the existing wind-deloading technique.Rasha El-Azab Eslam M.Wazeer Mohamed Daowd A.M.Abdel Ghany 2021Clean Energy2021,5,1:0
