1Repeat endoscopic retrograde cholangiopancreaticography after failed initial precut sphincterotomy for biliary cannulation显示文摘AIM: To investigate the outcome of repeating endoscopic retrograde cholangiopancreaticography(ERCP) after initially failed precut sphincterotomy to achieve biliary cannulation.METHODS: In this retrospective study, consecutive ERCPs performed between January 2009 and September 2012 were included. Data from our endoscopy and radiology reporting databases were analysed for use of precut sphincterotomy, biliary access rate, repeat ERCP rate and complications. Patients with initially failed precut sphincterotomy were identified.RESULTS: From 1839 consecutive ERCPs, 187(10%) patients underwent a precut sphincterotomy during the initial ERCP in attempts to cannulate a native papilla. The initial precut was successful in 79/187(42%). ERCP was repeated in 89/108(82%) of patients with failed initial precut sphincterotomy after a median interval of 4 d, leading to successful biliary cannulation in 69/89(78%). In 5 patients a third ERCP was attempted(successful in 4 cases). Overall, repeat ERCP after failed precut at the index ERCP was successful in 73/89 patients(82%). Complications after precut-sphincterotomy were observed in 32/187(17%) patients including pancreatitis(13%), retroperitoneal perforations(1%), biliary sepsis(0.5%) and haemorrhage(3%).CONCLUSION: The high success rate of biliary cannulation in a second attempt ERCP justifies repeating ERCP within 2-7 d after unsuccessful precut sphincterotomy before more invasive approaches should be considered.Michael Pavlides Ashley Barnabas Nilesh Fernandopulle Adam A Bailey Jane Collier Jane Phillips-Hughes Anthony Ellis Roger Chapman Barbara Braden 2014World Journal of Gastroenterology2014,20,36:3
2Emerging artificial intelligence applications in liver magnetic resonance imaging显示文摘Chronic liver diseases(CLDs)are becoming increasingly more prevalent in modern society.The use of imaging techniques for early detection,such as magnetic resonance imaging(MRI),is crucial in reducing the impact of these diseases on healthcare systems.Artificial intelligence(AI)algorithms have been shown over the past decade to excel at image-based analysis tasks such as detection and segmentation.When applied to liver MRI,they have the potential to improve clinical decision making,and increase throughput by automating analyses.With Liver diseases becoming more prevalent in society,the need to implement these techniques to utilize liver MRI to its full potential,is paramount.In this review,we report on the current methods and applications of AI methods in liver MRI,with a focus on machine learning and deep learning methods.We assess four main themes of segmentation,classification,image synthesis and artefact detection,and their respective potential in liver MRI and the wider clinic.We provide a brief explanation of some of the algorithms used and explore the current challenges affecting the field.Though there are many hurdles to overcome in implementing AI methods in the clinic,we conclude that AI methods have the potential to positively aid healthcare professionals for years to come.Charles E Hill Luca Biasiolli Matthew D Robson Vicente Grau Michael Pavlides 2021World Journal of Gastroenterology2021,27,40:2
3Evolving role of magnetic resonance techniques in primary sclerosing cholangitis显示文摘Development of non-invasive methods to risk-stratify patients and predict clinical endpoints have been identified as one of the key research priorities in primary sclerosing cholangitis(PSC). In addition to serum and histological biomarkers, there has been much recent interest in developing imaging biomarkers that can predict disease course and clinical outcomes in PSC.Magnetic resonance imaging/magnetic resonance cholangiopancreatography(MRI/MRCP) continue to play a central role in the diagnosis and follow-up of PSC patients. Magnetic resonance(MR) techniques have undergone significant advancement over the last three decades both in MR data acquisition and interpretation. The progression from a qualitative to quantitative approach in MR acquisition techniques and data interpretation, offers the opportunity for the development of objective and reproducible imaging biomarkers that can potentially be incorporated as an additional endpoint in clinical trials. 7MRI in liver cirrhosis显示文摘Liver magnetic resonance imaging(MRI)has seen a massive increase in its use over the last decades.Since its first description in the early 1980s,contrast‐enhanced MRI is now the gold standard for the diagnosis of hepatocellular cancer in patients with liver cirrhosis.Techniques like magnetic resonance(MR)elastography and combinations of multiple MR parameters in single scanning protocols are now available and these are opening new possibilities for the assessment of multiple aspects of disease in patients with liver cirrhosis.In this narrative review,we provide an overview of the use of MRI in patients with cirrhosis for the diagnosis of and screening for liver cancer,for the diagnosis of cirrhosis and prediction of long‐term outcomes,and for the evaluation of portal hypertension.With the multitude of techniques now available and the number of cirrhosis aspects that can be assessed by single scans make the study and application of MRI an attractive target in the search for a technique for use in this group of patients that often have to attend for multiple appointments and multiple tests,including invasive investigations.Osama Alzoubi Ahmad Arar Viraj Singh Sukru M.Erturk Ferenc Mozes Michael Pavlides 2022Portal Hypertension & Cirrhosis2022,1,1:0
