| 1 | 多国的纵向前瞻性研究调查膝骨关节炎的当前治疗状况显示文摘目的评估有症状的膝骨关节炎(OA)患者经镇痛治疗后疼痛缓解不充分(IPR)的出现率,并调查这些IPR患者的特点。方法此前瞻性非干预研究在六个欧洲国家进行,纳入了经医生诊断为OA且局部用或口服止痛药物至少14天的50岁及以上患者。使用简易疼痛量表(BPI)和WOMAC对膝盖疼痛和功能进行评估。使用12项的简易表对生活质量(Qo L)进行评估。IPR指在BPI问题5中的10分疼痛评分中平均得分>4的状况。结果共纳入1187名患者,68%为女性,平均年龄(68±9)岁;639名(54%)患者存在IPR。患者对BPI平均疼痛问卷的回答与其对WOMAC疼痛分量表的反应相关性良好(Spearman r=0.64,P<0.001)。多变量逻辑回归中,女性[校正比值比(adj OR)=1.90,95%CI(1.46,2.48)]、双膝OA[adj OR=1.48,95%CI(1.15,1.90)]、较高BMI、较长的OA病程、抑郁或糖尿病患者有着更高几率的IPR发生率。较之非IPR患者,IPR患者更易出现生活质量恶化、更大幅度膝功能丧失及更多疼痛干预。结论 IPR常见于需要镇痛治疗的膝OA患者,且与大幅度的膝功能丧失和生活质量受损相关。对于本研究中表明的那些有IPR风险的患者,其镇痛治疗尤须注重。试验注册号:https://clinicaltrials.gov/(NCT01294696)。 | Philip G.
