| 14 | 内源性CD34+脂肪干细胞在提高人类脂肪移植效果持久性中的优势:一项异种移植模型显示文摘背景与目的脂肪移植是一项很有前景的软组织填充增大技术,但移植疗效持久性存在高度不可预知性,而且影响移植成活率的因素尚未明确。因为具有分化的潜能和释放生长因子的能力,脂肪来源干细胞(adipose-derivedstemcells,ADSCs)可在移植修复过程中起关键作用。我们旨在确定ADSCs在人体脂肪中呈现的生物学特性是否与移植体积的体内效果保持存在关联。方法利用标准的离心分离技术对从8例人体受术者获取的脂肪抽吸物进行处理,然后将其注射至6周龄无胸腺裸鼠躯体两侧。测量移植物的质量和体积,于移植后8周进行包含针对血管的CD31^+染色的组织学评估。从各供体脂肪抽吸物中分离获取基质血管细胞群(stromalvascularfraction,SVF),利用多参数流式细胞术对其表面标记物进行分析,并对其增值、分化能力和正常含氧量/低含氧量血管内皮细胞生长因子(vas—cularendothelialgrcwlhfactor,VEGF)的分泌情况进行实验研究。结果从不同供体中获取的SVF,其CD34^+祖细胞含量百分率存在较大差异,其平均值为21.3%±15.O%(X±s)。SVF细胞的增殖速率和成脂潜能在不同供体间存在较为中等差异。在小鼠异种移植研究中,移植后8周体积保存的量约为36%~68%,总体平均值为52%±11%。SVF中CD34^+祖细胞的含量与脂肪移植效果持久性方面存在较强的关联(P〈0.05)。结论在不同患者之间,其脂肪组织的内在生物学特性存在差异。特别是SVF中CD34^+祖细胞的浓度可能是影响脂肪移植持久性的因素之一。 | Brian J.
