1Identification of a novel coronavirus causing severe pneumonia in human:a descriptive study显示文摘Background:Human infections with zoonotic coronaviruses(CoVs),including severe acute respiratory syndrome(SARS)-CoV and Middle East respiratory syndrome(MERS)-CoV,have raised great public health concern globally.Here,we report a novel batorigin CoV causing severe and fatal pneumonia in humans.Methods:We collected clinical data and bronchoalveolar lavage(BAL)specimens from five patients with severe pneumonia from Wuhan Jinyintan Hospital,Hubei province,China.Nucleic acids of the BAL were extracted and subjected to next-generation sequencing.Virus isolation was carried out,and maximum-likelihood phylogenetic trees were constructed.Results:Five patients hospitalized from December 18 to December 29,2019 presented with fever,cough,and dyspnea accompanied by complications of acute respiratory distress syndrome.Chest radiography revealed diffuse opacities and consolidation.One of these patients died.Sequence results revealed the presence of a previously unknownβ-CoV strain in all five patients,with 99.8%to 99.9%nucleotide identities among the isolates.These isolates showed 79.0%nucleotide identity with the sequence of SARS-CoV(GenBank NC_004718)and 51.8%identity with the sequence of MERS-CoV(GenBank NC_019843).The virus is phylogenetically closest to a bat SARS-like CoV(SL-ZC45,GenBank MG772933)with 87.6%to 87.7%nucleotide identity,but is in a separate clade.Moreover,these viruses have a single intact open reading frame gene 8,as a further indicator of bat-origin CoVs.However,the amino acid sequence of the tentative receptor-binding domain resembles that of SARS-CoV,indicating that these viruses might use the same receptor.Conclusion:A novel bat-borne CoV was identified that is associated with severe and fatal respiratory disease in humans.Li-Li Ren Ye-Ming Wang Zhi-Qiang Wu Zi-Chun Xiang Li Guo Teng Xu Yong-Zhong Jiang Yan Xiong Yong-Jun Li Xing-Wang Li Hui Li Guo-Hui Fan Xiao-Ying Gu Yan Xiao Hong Gao Jiu-Yang Xu Fan Yang Xin-Ming Wang Chao Wu Lan Chen Yi-Wei Liu Bo Liu Jian Yang Xiao-Rui Wang Jie Dong Li Li Chao-Lin Huang Jian-Ping Zhao Yi Hu Zhen-Shun Cheng Un-Lin Liu Zhao-Hui Qian Chuan Qin Qi Jin Bin Cao Jian-Wei Wang 2020Chinese Medical Journal2020,,9:99
2Epalrestat protects against diabetic peripheral neuropathy by alleviating oxidative stress and inhibiting polyol pathway显示文摘Epalrestat is a noncompetitive and reversible aldose reductase inhibitor used for the treatment of diabetic neuropathy. This study assumed that epalrestat had a protective effect on diabetic peripheral nerve injury by suppressing the expression of aldose reductase in peripheral nerves of diabetes mellitus rats. The high-fat and high-carbohydrate model rats were established by intraperitoneal injection of streptozotocin. Peripheral neuropathy occurred in these rats after sustaining high blood glucose for 8 weeks. At 12 weeks after streptozotocin injection, rats were intragastrically administered epalrestat 100 mg/kg daily for 6 weeks. Transmission electron microscope revealed that the injuries to myelinated nerve fibers, non-myelinated nerve fibers and Schwann cells of rat sciatic nerves had reduced compared to rats without epalrestat administuation. Western blot assay and immunohistochemical results demonstrated that after intervention with epalrestat, the activities of antioxidant enzymes such as superoxide dismutase, catalase and glutathione peroxidase gradually increased, but aldose reductase protein expression gradually diminished. 4TIPS improves liver transplantation-free survival in cirrhotic patients with refractory ascites:An updated meta-analysis显示文摘AIM:To compare the liver transplantation-free(LTF)survival rates between patients who underwent transjugular intrahepatic portosystemic shunts(TIPS)and those who underwent paracentesis by an updated meta-analysis that pools the effects of both number of deaths and time to death.METHODS:MEDLINE,EMBASE,and the Cochrane Library were searched from the inception to October2012.LTF survival,liver transplantation,liver diseaserelated death,non-liver disease-related death,recurrent ascites,hepatic encephalopathy(HE)and severe HE,and hepatorenal syndrome were assessed as outcomes.LTF survival was estimated using a HR with a95%CI.Other outcomes were estimated using OR with95%CIs.Sensitivity analyses were performed to assess the effects of potential outliers in the studies according to the risk of bias and the study characteristics.RESULTS:Six randomized controlled trials with 390patients were included.In comparison to paracentesis,TIPS significantly improved LTF survival(HR=0.61,95%CI:0.46-0.82,P<0.001).TIPS also significantly decreased liver disease-related death(OR=0.62,95%CI:0.39-0.98,P=0.04),recurrent ascites(OR=0.15,95%CI:0.09-0.24,P<0.001)and hepatorenal syndrome(OR=0.32,95%CI:0.12-0.86,P=0.02).However,TIPS increased the risk of HE(OR=2.95,95%CI:1.87-4.66,P=0.02)and severe HE(OR=2.18,95%CI:1.27-3.76,P=0.005).CONCLUSION:TIPS significantly improved the LTF survival of cirrhotic patients with refractory ascites and decreased the risk of recurrent ascites and hepatorenal syndrome with the cost of increased risk of HE compared with paracentesis.Further studies are warranted to validate the survival benefit of TIPS in clinical practice settings.Ming Bai Xing-Shun Qi Zhi-Ping Yang Man Yang Dai-Ming Fan Guo-Hong Han 2014World Journal of Gastroenterology2014,20,10:29

6Shunting branch of portal vein and stent position predict survival after transjugular intrahepatic portosystemic shunt显示文摘AIM:To evaluate the effect of the shunting branch of the portal vein(PV)(left or right)and the initial stent position(optimal or suboptimal)of a transjugular intrahepatic portosystemic shunt(TIPS).METHODS:We retrospectively reviewed 307 consecu5tive cirrhotic patients who underwent TIPS placement for variceal bleeding from March 2001 to July 2010 at our center.The left PV was used in 221 patients and the right PV in the remaining 86 patients.And,224 and83 patients have optimal stent position and sub-optimal stent positions,respectively.The patients were followed until October 2011 or their death.Hepatic encephalopathy,shunt dysfunction,and survival were evaluated as outcomes.The difference between the groups was compared by Kaplan-Meier analysis.A Cox regression model was employed to evaluate the predictors.RESULTS:Among the patients who underwent TIPS to the left PV,the risk of hepatic encephalopathy(P=0.002)and mortality were lower(P<0.001)compared to those to the right PV.Patients who underwent TIPS with optimal initial stent position had a higher primary patency(P<0.001)and better survival(P=0.006)than those with suboptimal initial stent position.The shunting branch of the portal vein and the initial stent position were independent predictors of hepatic encephalopathy and shunt dysfunction after TIPS,respectively.And,both were independent predictors of survival.CONCLUSION:TIPS placed to the left portal vein with optimal stent position may reduce the risk of hepatic encephalopathy and improve the primary patency rates,thereby prolonging survival.Ming Bai Chuang-Ye He Xing-Shun Qi Zhan-Xin Yin Jian-Hong Wang Wen-Gang Guo Jing Niu Jie-Lai Xia Zhuo-Li Zhang Andrew C Larson Kai-Chun Wu Dai-Ming Fan Guo-Hong Han 2014World Journal of Gastroenterology2014,20,3:26

9Keyword Extraction Based on tf/idf for Chinese News Document显示文摘Keyword extraction is an important research topic of information retrieval. This paper gave the specification of key- words in Chinese news documents based on analyzing linguistic characteristics of news documents and then proposed a new key- word extraction method based on tf/idf with multi-strategies. The approach selected candidate keywords of uni-, bi-and tri-grams, and then defines the features according to their morphological characters and context information. Moreover, the paper proposed several strategies to amend the incomplete words gotten from the word segmentation and found unknown potential keywords in news documents. Experimental results show that our proposed method can significantly outperform the baseline method. We also applied it to retrospective event detection. 14Clinical trial with traditional Chinese medicine intervention ''tonifying the kidney to promote liver regeneration and repair by affecting stem cells and their microenvironment'' for chronic hepatitis B-associated liver failure显示文摘AIM:To study the clinical efficacy of traditional Chinese medicine(TCM)intervention'tonifying the kidney to promote liver regeneration and repair by affecting stem cells and their microenvironment'('TTK')for treating liver failure due to chronic hepatitis B.METHODS:We designed the study as a randomized controlled clinical trial.Registration number of Chinese Clinical Trial Registry is Chi CTR-TRC-12002961.A total of 144 patients with liver failure due to infection with chronic hepatitis B virus were enrolled in this randomized controlled clinical study.Participants were randomly assigned to the following three groups:(1)a modern medicine control group(MMC group,36patients);(2)a'tonifying qi and detoxification'('TQD')group(72 patients);and(3)a'tonifying the kidney to promote liver regeneration and repair by affecting stem cells and their microenvironment'('TTK')group(36patients).Patients in the MMC group received general internal medicine treatment;patients in the'TQD'group were given a TCM formula'tonifying qi and detoxification'and general internal medicine treatment;patients in the'TTK'group were given a TCM formula of'TTK'and general internal medicine treatment.All participants were treated for 8 wk and then followed at 48 wk following their final treatment.The primaryefficacy end point was the patient fatality rate in each group.Measurements of various virological and biochemical indicators served as secondary endpoints.The one-way analysis of variance and the t-test were used to compare patient outcomes in the different treatment groups.RESULTS:At the 48-wk post-treatment time point,the patient fatality rates in the MMC,'TQD',and'TTK'groups were 51.61%,35.38%,and 16.67%,respectively,and the differences between groups were statistically significant(P<0.05).However,there were no significant differences in the levels of hepatitis B virus DNA or prothrombin activity among the three groups(P>0.05).Patients in the'TTK'group had significantly higher levels of serum total bilirubin compared to MMC subjects(339.40μmol/L±270.09μmol/L vs 176.13μmol/L±185.70μmol/L,P=0.014).Serum albumin levels were significantly increased in both the'TQD'group and'TTK'group as compared with the MMC group(31.30 g/L±4.77g/L,30.72 g/L±2.89 g/L vs 28.57 g/L±4.56 g/L,P<0.05).There were no significant differences in levels of alanine transaminase among the three groups(P>0.05).Safety data showed that there was one case of stomachache in the'TQD'group and one case of gastrointestinal side effect in the'TTK'group.CONCLUSION:Treatment with'TTK'improved the survival rates of patients with liver failure due to chronic hepatitis B.Additionally,liver tissue was regenerated and liver function was restored.Han-Min Li Zhi-Hua Ye Jun Zhang Xiang Gao Yan-Ming Chen Xin Yao Jian-Xun Gu Lei Zhan Yang Ji Jian-Liang Xu Ying-He Zeng Fan Yang Lin Xiao Guo-Guang Sheng Wei Xin Qi Long Qing-Jing Zhu Zhao-Hong Shi Lian-Guo Ruan Jia-Yao Yang Chang-Chun Li Hong-Bin Wu Sheng-Duo Chen Xin-La Luo 2014World Journal of Gastroenterology2014,20,48:20
15Development of a class-specific polyclonal antibody-based indirect competitive ELISA for detecting fluoroquinolone residues in milk显示文摘Modified 1-ethyl-3-(3-dimethylaminopropyl)carbodiimide(EDC)method was employed to synthesize the artificial antigen of norfloxacin(NOR),and New Zealand rabbits were used to produce anti-NOR polyclonal antibody(pAb).Based on the checkerboard titration,an indirect competitive enzyme-linked immunosorbent assay(icELISA) standard curve was established.This assay was sensitive and had a working range from 0.12 to 68.40 ng/ml,with the half maximal inhibitory concentration(IC50)and limit of detection(LOD)values of 2.7 ng/ml and 0.06 ng/ml,respectively.The produced pAb exhibited high cross-reactivity to fluoroquinolones(FQs)tested,and the IC50 values to enoxacin,ciprofloxacin,and pefloxacin were 3.1,3.4,and 4.1 ng/ml,respectively.It also indicated that the concentrations of NaOH and methanol in assay buffer should not be higher than 10%and 30%.When spiked in milk at 5,20,and 50 ng/ml,the recoveries for NOR,enoxacin,ciprofloxacin,and pefloxacin ranged 90.5%-98.0%,84.0%-95.2%,94.0%-106.0%,and 89.5%-100.0%,respectively.The results suggest that this class-specific pAb-based icELISA could be utilized for the primary screening of FQ residues in animal-original products.Guo-ying FAN Ruo-song YANG Jin-qing JIANG Xin-yao CHANG Jun-jie CHEN Yong-hua QI Shi-xiu WU Xue-feng YANG 2012Journal of Zhejiang University-Science B(Biomedicine & Biotechnology)2012,13,7:20

17Diagnostic value of gamma-glutamyltransferase/aspartate aminotransferase ratio, protein induced by vitamin K absence or antagonist II, and alpha-fetoprotein in hepatitis B virus-related hepatocellular carcinoma显示文摘BACKGROUND Researchers have investigated the diagnostic value of protein induced by vitamin K absence or antagonist II (PIVKA-II) and alpha-fetoprotein (AFP) in hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC), and obtained abundant clinical diagnostic data. However, PIVKA-II and AFP have unsatisfactory specificity and sensitivity in the diagnosis of early-stage HBV-related HCC. Gamma-glutamyltransferase (γ-GT) and aspartate aminotransferase (AST) are common biomarkers for evaluating liver function, and we hypothesized that the γ-GT/AST ratio in combination with PIVKA-II and AFP would improve the diagnosis of early-stage HBV-related HCC. AIM To evaluate the diagnostic value of γ-GT/AST ratio alone or in combination with PIVKA-II and AFP in HBV-related HCC. METHODS Serum levels of γ-GT, AST, PIVKA-II, and AFP were detected and analysed in 176 patients with HBV-related HCC and in 359 patients with chronic hepatitis B. According to tumour size and serum level of HBV DNA, HBV-related HCC patients were divided into the following categories: Early-stage HCC patients, HCC patients, HBV DNA positive (HBV DNA+) HCC patients, and HBV DNA negative (HBV DNA-) HCC patients. Receiver-operating characteristic (ROC) curves were used to analyse and compare the diagnostic value of the single and combined detection of various biomarkers in different types of HBV-related HCC. RESULTS Tumour size was positively correlated with serum levels of PIVKA-II and AFP in HCC patients (r = 0.529, aP < 0.001 and r = 0.270, bP < 0.001, respectively), but there was no correlation between tumour size and the γ-GT/AST ratio (r = 0.073, P = 0.336). The areas under the receiver-operating characteristic curves (AUROCs) of the γ-GT/AST ratio in early-stage HCC patients, HBV DNA+ HCC patients and HBV DNA- HCC patients were not significantly different from that in the total HCC patients (0.754, 0.802, and 0.705 vs 0.779, respectively;P > 0.05). When PIVKA-II was combined with the γ-GT/AST ratio in the diagnosis of earlystage HCC, HCC, and HBV DNA+ HCC, the AUROCs of PIVKA-II increased, with values of 0.857 vs 0.835, 0.925 vs 0.913, and 0.958 vs 0.954, respectively. When AFP was combined with the γ-GT/AST ratio in the diagnosis of early-stage HCC, HCC, HBV DNA+ HCC, and HBV DNA- HCC, the AUROCs of AFP increased, with values of 0.757 vs 0.621, 0.837 vs 0.744, 0.868 vs 0.757, and 0.840 vs 0.828, respectively. 18Minimally invasive surgery alone compared with intensity-modulated radiotherapy for primary stage I nasopharyngeal carcinoma显示文摘Background:The National Comprehensive Cancer Network guidelines recommend intensity-modulated radiotherapy(IMRT)as the primary curative treatment for newly diagnosed nasopharyngeal carcinoma(NPC),but the radiation-related complications and relatively high medical costs remain a consequential burden for the patients.Endoscopic nasopharyngectomy(ENPG)was successfully applied in recurrent NPC with radiation free and relatively low medical costs.In this study,we examined whether ENPG could be an effective treatment for localized stage I NPC.Methods:Ten newly diagnosed localized stage I NPC patients voluntarily received ENPG alone from June 2007 to September 2017 in Sun Yat-sen University Cancer Center.Simultaneously,the data of 329 stage I NPC patients treated with IMRT were collected and used as a reference cohort.The survival outcomes,quality of life(QOL),and medical costs between two groups were compared.Results:After a median follow-up of 59.0 months(95%CI 53.4-64.6),no death,locoregional recurrence,or distant metastasis was observed in the 10 patients treated with ENPG.The 5-year overall survival,local relapse-free survival,regional relapse-free survival,and distant metastasis-free survival among the ENPG-treated patients was similar to that among the IMRT-treated patients(100%vs.99.1%,100%vs.97.7%,100%vs.99.0%,100%vs.97.4%,respectively,P>0.05).In addition,compared with IMRT,ENPG was associated with decreased total medical costs($4090.42±1502.65 vs.$12620.88±4242.65,P<0.001)and improved QOL scores including dry mouth(3.3±10.5 vs.34.4±25.8,P<0.001)and sticky saliva(3.3±10.5 vs.32.6±23.3,P<0.001).Conclusions:ENPG alone was associated with promising long-term survival outcomes,low medical costs,and satisfactory QOL and might therefore be an alternative strategy for treating newly diagnosed localized stage I NPC patients who refused radiotherapy.However,the application of ENPG should be prudent,and prospective clinical tri-als were needed to further verify the results.You-Ping Liu Xing Lv Xiong Zou Yi-Jun Hua Rui You Qi Yang Le Xia Shao-Yan Guo Wen Hu Meng-Xia Zhang Si-Yuan Chen Mei Lin Yu-Long Xie Li-Zhi Liu Rui Sun Pei-Yu Huang Wei Fan Xiang Guo Ming-Huang Hong Ming-Yuan Chen 2019Cancer Communications2019,39,1:17

20Selection of treatment modalities for Budd-Chiari Syndrome in China:A preliminary survey of published literature显示文摘AIM:To evaluate the frequency of use of various treatment modalities for Budd-Chiari syndrome(BCS)in China by conducting a preliminary survey of relevant literature.METHODS:All papers regarding the treatment of BCS in Chinese patients were identified by searching PubMed,Chinese Scientific and Technological Journal,and China National Knowledge Infrastructure databases.Data regarding the number of BCS patients treated with different treatment modalities over time were collected.The proportions of BCS patients undergoing various treatment modalities were calculated.RESULTS:Overall,300 of 3005 papers initially retrieved were included.These papers included 23352BCS patients treated with different treatment modalities.The treatment modalities include surgery(n=8625),interventional treatment(n=13940),surgery combined with interventional treatment(n=363),medical therapy(n=277),other treatments(n=91),and no treatment(n=56).After 2005,the number of BCS patients treated with surgery was drastically decreased,but the number of BCS patients who underwent interventional treatment was almost maintained.Shunt surgery was the most common type of surgery(n=3610).Liver transplantation was rarely employed(n=2).Balloon angioplasty with or without stenting was the most common type of interventional treatment(n=13747).Transjugular intrahepatic portosystemic shunt was rarely employed(n=81).CONCLUSION:Selection of treatment modalities for BCS might be different between China and Western countries.Further work should be necessary to establish a unanimous therapeutic strategy for BCS in China.Xing-Shun Qi Wei-Rong Ren Dai-Ming Fan Guo-Hong Han 2014World Journal of Gastroenterology2014,20,30:16
