| 1 | Anterior resection for rectal carcinoma-risk factors for anastomotic leaks and strictures显示文摘AIM:To determine the incidence and factors responsible for anastomotic leaks and stricture following anterior resection(AR)and its subsequent management.METHODS:Retrospective analysis of data from 108 patients with rectal carcinoma who underwent AR or low anterior resection(LAR)to identify the various preoperative,operative,and post operative factors that might have influence on anastomotic leaks and strictures.RESULTS:There were 68 males and 40 females with an average of 47 years(range 21-75 years).The median distance of the tumor from the anal verge was 8 cm(range 3-15 cm).Sixty(55.6%)patients underwent handsewn anastomosis and 48(44.4%)were stapled.The median operating time was 3.5 h(range2.0-7.5 h).Sixteen(14.6%)patients had an anastomotic leak.Among these,11 patients required reexploration and five were managed expectantly.The anastomotic leak rate was similar in patients with and without diverting stoma(8/60,13.4%with stoma and 8/48;16.7%without stoma).In 15(13.9%)patients,resection margins were positive for malignancy.Ninteen(17.6%)patients developed anastomotic strictures at a median duration of 8 mo(range 3-20 mo).Among these,15 patients were successfully managed with per-anal dilatation.On multivariate analysis,advance age(>60 years)was the only risk factor for anastomotic leak(P=0.004).On the other hand,anastomotic leak(P=0.00),mucin positive tumor(P =0.021),and lower rectal growth(P=0.011)were found as risk factors for the development of an anastomotic stricture.CONCLUSION:Advance age is a risk factor for an anastomotic leak.An anastomotic leak,a mucin-secreting tumor,and lower rectal growth predispose patients to develop anastomotic strictures. | Ashok Kumar Ram Daga Paari Vijayaragavan Anand Prakash Rajneesh Kumar Singh Anu Behari Vinay K Kapoor Rajan Saxena | 2011 | World Journal of Gastroenterology2011,17,11: | 28 |
| 2 | Community Responses to Mountain Tourism:A Case in Bhyundar Valley,Indian Himalaya显示文摘Tourism industry has become an important part of Indian economy,and tourism development has been,however,concentrated to a few areas. Accumulation of tourism activities has reached the critical stage in most of the tourism centers,which pose serious threats to the natural environment. The Bhyundar Valley of Nanda Devi Biosphere Reserve (NDBR) in Uttarakhand State of the Indian Himalaya is a burning example of such problems. Land use/cover changes,deforestation and accumulation of garbage are some of the negative impacts of unregulated tourism in the valley. Unsustainable activities of tourists and villagers in the valley make such problems very severe. To deal with the ongoing problems and ensure community involvement in tourism management and environmental conservation,the Biosphere Reserve Authority introduced ecotourism in the form of Eco Development Committees (EDCs) in the Bhyundar Valley. Study reveals that many problems associated with tourism have been minimized to some extent with the help of EDCs but few are still prevailing in valley. | 4 | BISAP评分与Ranson评分和CT严重指数预测印度急性胰腺炎患者并发症发生率和病死率的比较研究显示文摘目的:本研究通过印度中东部地区一家三甲医院,前瞻性评估急性胰腺炎严重度床边指数(BISAP)评分预测急性胰腺炎患者病死率及疾病进展中间指标的准确性,从而为贫困人群提供一个经济、有效的预后预测方法。方法:2012-2014年间我们中心连续收治的119例急性胰腺炎患者纳入研究。起病24 h内对所有病例进行BISAP评分,同时也进行Ranson评分和CT严重指数(CTSI)评估。采用趋势和判别分析来对这三种评分系统对病死率的预测价值进行评价。通过受试者操作特征(ROC)曲线评估这三种评分系统对急性胰腺炎患者病死率及疾病进展中间指标(器官衰竭和胰腺坏死)的预测情况。结果:119例入组病例中42例(35.2%)出现器官衰竭,定义为重症急性胰腺炎,47例(39.5%)发生胰腺坏死,12例(10.1%)死亡。BISAP评分预测重症急性胰腺炎、胰腺坏死和死亡的曲线下面积(AUC)分别为0.962、0.934和0.846。Ranson评分预测重症急性胰腺炎(AUC 0.956)和死亡(AUC 0.841)的准确性稍低于BISAP评分。CTSI在预测胰腺坏死方面的准确性最高,AUC为0.958。以≥3分作为截点,BISAP评分预测病死率的敏感性和特异性分别为100%和69.2%。结论:BISAP评分简单实用,在急性胰腺炎起病24 h内即可鉴别出高危死亡风险及高危疾病进展风险的患者。这种风险分层方法可用以提高临床诊治,并有助于临床研究中的病例入组。 | Jitin Yadav Sanjay Kumar Yadav Satish Kumar Ranjan George Baxla Dipendra Kumar Sinha Pankaj Bodra Ram Chandra Besra Babu Mani Baski Om Prakash Abhinav Anand | 2016 | Gastroenterology Report2016,4,3: | 2 |

| 17 | Science Letters:Demonstration of a new biosensing concept for immunodiagnostic applications based on change in surface conductance of antibodies after biomolecular interactions显示文摘We report an important observation that the surface conductivity of antibody layer immobilized on polylysine-coated glass substrate decreases upon the formation of complex with their specific antigens. This change in conductivity has been ob- served for both monoclonal and polyclonal antibodies. The conductance of monoclonal mouse IgG immobilized on polylysine-coated glass substrate changed from 1.02×10?8 ??1 to 1.41×10?11 ??1 at 10 V when complex is formed due to the specific biomolecular interactions with rabbit anti-mouse IgG F(ab′)2. Similar behavior was observed when the same set up was tested in two clinical assays: (1) anti-Leishmania antigen polyclonal antibodies taken from Kala Azar positive patient serum interacting with Leishmania promastigote antigen, and (2) anti-p21 polyclonal antibodies interacting with p21 antigen. | 18 | Analysis of Power in an Argon Filled Pulsed Dielectric Barrier Discharge显示文摘In this paper an argon filled coaxial dielectric barrier discharge (DBD) has been studied to understand the detail of power transfer from a unipolar square pulse to plasma during discharge. A dielectric barrier discharge based diffuse pulse discharge and its electrical characteristics are investigated. A quartz coaxial DBD tube filled at different pressures is used in the experiment. A unipolar pulse voltage of different peak voltages and frequencies has been applied to the discharge electrodes for the generation of microdischarges. Two current pulses are used for two consecutive discharges per applied voltage pulse. The second discharge, which occurs at the falling flank of the voltage pulse, is induced by the charges stored on the dielectric barrier during the first discharge. It has been deduced that the power supplied to ignite the first discharge is partly stored to ignite the second discharge when the applied voltage decays. This process ultimately leads to much improved power transfer to the plasma.
