1Noninvasive methods for prediction of esophageal varices in pediatric patients with portal hypertension显示文摘AIM: To evaluate clinical and laboratory parameters for prediction of bleeding from esophageal varices (EV) in children with portal hypertension. METHODS: Retrospective study of 103 children (mean age: 10.1 ± 7.7 years), 95.1% with intrahepatic portal hypertension. All patients had no history of bleeding and underwent esophagogastroduodenoscopy for EV screening. We recorded variceal size (F1, F2 and F3), red-color signs and portal gastropathy, according to the Japanese Research Society for Portal Hypertension classification. Patients were classified into two groups: with and without EV. Seven noninvasive markers were evaluated as potential predictors of EV: (1) platelet count; (2) spleen size z score, expressed as a standard deviation score relative to normal values for age; (3)platelet count to spleen size z score ratio; (4) platelets count to spleen size (cm) ratio; (5) the clinical prediction rule (CPR); (6) the aspartate aminotransferase to platelet ratio index (APRI); and (7) the risk score. RESULTS: Seventy-one children had EV on first endoscopy. On univariate analysis, spleen size, platelets, CPR, risk score, APRI, and platelet count to spleen size z score ratio showed significant associations. The best noninvasive predictors of EV were platelet count [area under the receiver operating characteristic curve (AUROC) 0.82; 95%CI: 0.73-0.91], platelet: spleen size z score (AUROC 0.78; 95%CI: 0.67-0.88), CPR (AUROC 0.77; 95%CI: 0.64-0.89), and risk score (AUROC 0.77; 95%CI: 0.66-0.88). A logistic regression model was applied with EV as the dependent variable and corrected by albumin, bilirubin and spleen size z score. Children with a CPR < 114 were 20.7-fold more likely to have EV compared to children with CPR > 114. A risk score > -1.2 increased the likelihood of EV (odds ratio 7.47; 95%CI: 2.06-26.99). CONCLUSION: Children with portal hypertension with a CPR below 114 and a risk score greater than -1.2 are more likely to have present EV. 10The Giant Radio Array for Neutrino Detection(GRAND):Science and design显示文摘The Giant Radio Array for Neutrino Detection(GRAND)is a planned large-scale observatory of ultra-high-energy(UHE)cosmic particles,with energies exceeding 10~8 Ge V.Its goal is to solve the long-standing mystery of the origin of UHE cosmic rays.To do this,GRAND will detect an unprecedented number of UHE cosmic rays and search for the undiscovered UHE neutrinos and gamma rays associated to them with unmatched sensitivity.GRAND will use large arrays of antennas to detect the radio emission coming from extensive air showers initiated by UHE particles in the atmosphere.Its design is modular:20 separate,independent sub-arrays,each of 10000 radio antennas deployed over 10000 km^2.A staged construction plan will validate key detection techniques while achieving important science goals early.Here we present the science goals,detection strategy,preliminary design,performance goals,and construction plans for GRAND.Jaime Alvarez-Muniz Rafael Alves Batista Aswathi Balagopal V. 13Antihyperglycemic effect of Passiflora glandulosa cav. fruit rinds flour in streptozotocin-induced diabetic mice显示文摘Objective:To investigate the effect of administration of Passiflora glandulosa(P.glandulosa) fruit rinds flour on streptozotocin(STZ)-induced diabetic mice.Methods:The preliminary phytochemical screening and parameters such as centesimal composition and brine shrimp toxicity were evaluated.For in vivo study Swiss female mice were divided into four groups:NC-normal control;DC-diabetic control animals receiving saline;MET-diabetic animals receiving metformin(200 mg/kg);PFRF-diabetic animals receiving P.glandulosa fruit rinds flour(200 mg/kg).All of them were treated for 28 d.STZ was used in a single dose of 120 mg/kg to establish diabetic models.Body weight,water and food intake,fasting blood glucose were measured.Histopathological analysis of pancreas and liver were performed to evaluate STZ-induced tissue injuries.Results:Phytochemical screening showed the presence of flavanones and triterpenoids.The P.glandulosa fruit rinds flour was non-toxic by the brine shrimp test.The fruit rinds flour also reduced the loss of body weight and significantly decreased food intake in the diabetic mice.Additionally,a significant reduction in blood glucose was observed for 15 d and this was maintained on 21 d and 28 d when compared with diabetic mice.Furthermore,the P.glandulosa fruit rinds flour has a favourable effect on the histopathological changes of the pancreas in STZ induced diabetes.Conclusions:It is concluded that P.glandulosa fruit rinds flour is a natural product that contains potent antioxidant compounds and presents good prospects for the improvement of diabetic mellitus by reducing serum glucose levels.Erlandia Alves Magalhaes Queiroz Raquel Teixeira Terceiro Paim Sandra Machado Lira Jose Ytalo Gomes da Silva Carla Laine Silva Lima Marcelo Oliveira Holanda Stephen Rathinaraj Benjamin Icaro Gusmao Pinto Vieira Maria Izabel Florindo Guedes 2018Asian Pacific Journal of Tropical Medicine2018,11,9:0
14Antibody-Cytokeratin Marker 34βE12 in Prostate Cancer Detection显示文摘Introduction:The histological diagnosis of prostate cancer is commonly based on morphological patterns.The presence of malignant tissue mixed with benign tissue,or the presence of carcinoma that mimics benignity may generate difficulty in the diagnostic elucidation.Therefore,the application of immunohistochemistry contributes its diagnostic value.Objectives:To evaluate the 34βE12 marker in the detection of adenocarcinoma(ADn),atypical small acinar proliferation(ASAp),regular prostatic tissue(RPT)and regular prostatic tissue alternated by atrophy spotlights(RPTa)in transrectal biopsy guided by ultrasonography of patients with suspected prostate cancer.Method:Analysis of 34 patients who underwent ultrasound-guided transrectal biopsy with subsequent analysis by H&E staining and 34βE12 labeling for elucidation of neoplasms or diseased tissues with doubtful diagnosis.Results:The marker 34βE12 showed negativity in 100%of the neoplasms ADn,positivity in 100%of the benign prostatic tissues(RPT and RPTa);the patients with ASAp presented positivity(20%)and negativity(80%).The chi-square test(χ2)showed that there is an association(χ2=29.55 and p<0.0001)between the groups,that is,the 34βE12 marker has a significant value(p<0.0001)in the elucidation of patients with prostatic neoplasia and benign prostatic tissues.Discussion and Conclusion:With the early screening of prostate cancer in the modern era,pathologists have become increasingly challenged to diagnose small outbreaks of cancer when only a few atypical glands are present in transrectal biopsy-guided ultrasonography.The 34βE12 marker becomes an important tool in elucidating diagnoses such as ADn and ASAp.Luiz Carlos de Araújo Souza Vinícius Carvalhêdo Cunha Hugo Oliveira de Figueiredo Cavalcanti Joao Ricardo Alves Sandra Lúcia Branco Mendes Coutinho 2019Journal of Pharmacy and Pharmacology2019,7,4:0
15Evaluation of parasitemia by qPCR in patients with chronic Chagas disease treated with benznidazole显示文摘Objective:To evaluate parasitemia by qPCR in patients undergoing etiological treatment and followed in a Brazilian reference center.Methods:Parasite load was quantified by qPCR in 32 participants with chronic Chagas disease who were treated with benznidazole.Serological analyses were performed before and after the treatment and parasite loads were compared prior and 12/18 months post the treatment.Results:Thirty-two participants were recruited and treated with benznidazole,and 20 were followed-up.Adverse events(AE)were observed in 22 out of 29 participants that had safety data(76%),and dermatological alterations were the most frequently observed AE.Of the 20 participants analyzed,13 and 7 completed 12 and 18 months follow-up after the treatment,respectively.12 Months after the final treatment,Trypanosoma cruzi was detectable in 3 patients by qPCR;18 months after the final treatment,Trypanosoma cruzi was detectable per qPCR in 4 of the 7 participants.Thus,between 12 and 18 months,7 participants of the 20 initial follow-up cases showed positive qPCR,indicating treatment failures.Conclusions:qPCR can be used as an alternative method for evaluating the effectiveness of the etiological treatment of CD,and can be applied to analyze early therapeutic failures.The study showed that benznidazole therapy had limited effectiveness in treating chronic CD patients,thus emphasizing the importance of conducting continued research for developing more effective therapies and diagnosis for CD.Tycha Bianca Sabaini Pavan Eros Antônio de Almeida Rodrigo Gonçalves de Lima Jamiro da Silva Wanderley Luiz Cláudio Martins Sandra Cecília Botelho Costa Gláucia Elisete Barbosa Marcon 2023Asian Pacific Journal of Tropical Medicine2023,16,12:0
16Diagnostic Value between 1984 and 2018 of Transrectal Biopsy Guided by Ultrasonography after Radical Prostatectomy显示文摘Objectives—to determine correlation between GSs(Gleason scores)on needle biopsy and RP(radical prostatectomy),evaluating diagnostic tests on biopsy and RP within the last years,between 1984 and 2018.Method—analysis of 100 patients,diagnosed with PCa(prostate cancer)needle biopsy using 18-gauge needle,who underwent RP with lymphadenectomy and for which preoperative and postoperative GSs were available.GS group analysis used three categorization schemes for differentiation:mild,moderate and poor for the whole group and we determined SE(sensitivity),SP(specificity),PVPR(positive predictive value),negative predictive value and accuracy.Results—we found that 42%of the patients had no changes between GS on biopsy and prostatectomy,while 20%were overgraded and 38%undergraded by needle biopsy.Graduation of+1 point in GS(32%)or-1 point(17%)was the most common.Most patients were classified as moderately differentiated by biopsies(78 and 35%in scheme 1 and 2 or 3,respectively),while 43%of patients received an intermediate differentiation classification.Biopsy accuracy varied from 44 to 76%for the analysis of all three schemes.Conclusion—there are differences in correlation between GS on biopsy and on surgical specimen,and Gleason’s graduation also depends on the experience of the pathologist.We have shown that sextant biopsies using 18-gauge and a same group of pathologists showed acceptable concordance values(42%)between the GS on biopsy and prostatectomy.Luiz Carlos de Araújo Souza Vinícius Carvalhêdo Cunha Hugo Oliveira de Figueiredo Cavalcanti Joao Ricardo Alves Grimar de Oliveira Paula Sandra Lúcia Branco Mendes Coutinho 2019Journal of Pharmacy and Pharmacology2019,7,8:0
17Transcranial direct current electrical stimulation in combination with aerobic exercise:A pilot study in post-COVID-19 systemic autoimmune rheumatic patients显示文摘BACKGROUND Systemic autoimmune rheumatic diseases(SARDs)are a group of diseases with multiorgan involvement and a high prevalence of chronic pain and fatigue.Patients with SARDs and post-coronavirus disease 2019(COVID-19)syndrome experience aggravation of symptoms.In this context,it is essential to establish strategies to reduce chronic pain and fatigue and improve quality of life.AIM To assess the efficacy of transcranial direct current stimulation(tDCS)for the treatment of fatigue and pain-associated post-COVID-19 syndrome in patients with SARDs.METHODS This study included nine patients with different types of SARDs.All patients had reverse transcription-polymerase chain reaction(RT-PCR)test confirmed COVID-19 as well as significant,persistent fatigue and pain that began to worsen after infection.Anodal tDCS was administered in five daily sessions(2mA,20 min).Concomitantly,patients were involved in aerobic exercise program.All participants were evaluated using specific questionnaires and strength assessment by handgrip and physical function by timed-up-and-go test and sit-to-stand test at baseline(within one week before tDCS protocol),and one week after tDCS protocol.During all procedures,the patients’treatments remained unchanged.RESULTS The sample comprised eight women and one man with a mean age of 48.7±9.6 years.After the tDCS protocol,pain and fatigue significantly improved on the visual analog scale(P<0.05).The physical function also improved 9.5±2.7 vs 6.8±0.8(P=0.001)for timed-up-go-test and 10.3±3.7 vs 15.1±4.0(P=0.037)for sit-to-stand test.None of the patients experienced any adverse events.CONCLUSION The present study showed that tDCS in combination with aerobic exercise was effective in improving physical function,and reducing fatigue/pain in SARDs patients with post-COVID-19 syndrome.Rafael Giovani Missé Alexandre Moura dos Santos Isabela Bruna Pires Borges Marlise Sítima Mendes Simões Lorenza Rosa Silvério Bruna Lindoso Correia Ana Wook Sook Kim Aline Marques Caetano Sandra Gofinet Pasoto Carla Gonçalves Schahin Saad Diogo Souza Domiciano Clarice Tanaka Julia Maria D’Andrea Greve Abrahão Fontes Baptista Samuel Katsuyuki Shinjo 2023World Journal of Rheumatology2023,11,1:0
18SARS-CoV-2 and hearing:An audiometric analysis of COVID-19 hospitalized patients显示文摘Purpose COVID-19 associated hearing loss is still an ongoing matter of debate.No original studies exist on audiological effects of SARS-CoV-2 infection in hospitalized patients.The main objective was to determine whether SARS-CoV-2 may affect auditory function in clinically ill COVID-19 patients.Materials and methods COVID-19 patients with moderate-severe disease and without prior history of hearing abnormalities were enrolled from a tertiary referral center,and matched with controls.Participants performed an audiometric evaluation,and thresholds were compared.Results 120 ears from 60 patients were enrolled.Patients with COVID-19 showed worse mean auditory thresholds starting from 1000 Hz through higher frequencies,when compared to controls(1000 Hz:18.52±5.49 dB HL in controls vs 25.36±6.79 dB HL in COVID-19,p<0.001;2000Hz:17.50±5.57 dB HL in controls vs 21.96±7.05 dB HL in COVID-19,p=0.010;3000Hz:17.97±8.07 dB HL in controls vs 25±9.38 dB HL in COVID-19,p=0.003;4000 Hz:20.16±10.12 dB HL in controls vs 29.55±11.26 dB HL in COVID-19,p=0.001;8000 Hz:31.09±12.75 dB HL in controls vs 40.71±19.40 dB HL in COVID-19,p=0.030;Pure Tone Average:20.42±4.29 dB HL in controls vs 24.85±5.62 dB HL in COVID-19,p=0.001).Statistical significance persisted after adjusting for confounders such as age,gender and various comorbidities(p<0.05).Conclusions SARS-CoV-2 may affect hearing in COVID-19 patients with moderate-severe disease.Results are in line with the previous suggested effects of COVID-19 on auditory system.This study is expected to encourage further research on this topic.Francisco Alves de Sousa Rodrigo Pinto Costa Sandra Xará Ana Nóbrega Pinto Cecília Almeida e Sousa 2021Journal of Otology2021,16,3:0

20Metabolic and nutritional triggers associated with increased risk of liver complications in SARS-CoV-2显示文摘Obesity,diabetes,cardiovascular and respiratory diseases,cancer and smoking are risk factors for negative outcomes in severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),which can quickly induce severe respiratory failure in 5%of cases.Coronavirus disease-associated liver injury may occur during progression of SARS-CoV-2 in patients with or without pre-existing liver disease,and damage to the liver parenchyma can be caused by infection of hepatocytes.Cirrhosis patients may be particularly vulnerable to SARS-CoV-2 if suffering with cirrhosis-associated immune dysfunction.Furthermore,pharmacotherapies including macrolide or quinolone antibiotics and steroids can also induce liver damage.In this review we addressed nutritional status and nutritional interventions in severe SARS-CoV-2 liver patients.As guidelines for SARS-CoV-2 in intensive care(IC)specifically are not yet available,strategies for management of sepsis and SARS are suggested in SARS-CoV-2.Early enteral nutrition(EN)should be started soon after IC admission,preferably employing iso-osmolar polymeric formula with initial protein content at 0.8 g/kg per day progressively increasing up to 1.3 g/kg per day and enriched with fish oil at 0.1 g/kg per day to 0.2 g/kg per day.Monitoring is necessary to identify signs of intolerance,hemodynamic instability and metabolic disorders,and transition to parenteral nutrition should not be delayed when energy and protein targets cannot be met via EN.Nutrients including vitamins A,C,D,E,B6,B12,folic acid,zinc,selenium andω-3 fatty acids have in isolation or in combination shown beneficial effects upon immune function and inflammation modulation.Cautious and monitored supplementation up to upper limits may be beneficial in management strategies for SARS-CoV-2 liver patients.Rosangela Passos de Jesus Jozélio Freire de Carvalho Lucivalda Pereira Magalhães de Oliveira Carla de Magalhães Cunha Thaisy Cristina Honorato Santos Alves Sandra Tavares Brito Vieira Virginia Maria Figueiredo Allain Amador Bueno 2022World Journal of Hepatology2022,14,1:0
