| 1 | Multimodal imaging evaluation in staging of rectal cancer显示文摘Rectal cancer is a common cancer and a major cause of mortality in Western countries.Accurate staging is essential for determining the optimal treatment strategies and planning appropriate surgical procedures to control rectal cancer.Endorectal ultrasonography(EUS)is suitable for assessing the extent of tumor invasion,particularly in early-stage or superficial rectal cancer cases.In advanced cases with distant metastases,computed tomography(CT)is the primary approach used to evaluate the disease.Magnetic resonance imaging(MRI)is often used to assess preoperative staging and the circumferential resection margin involvement,which assists in evaluating a patient’s risk of recurrence and their optimal therapeutic strategy.Positron emission tomography(PET)-CT may be useful in detecting occult synchronous tumors or metastases at the time of initial presentation.Restaging after neoadjuvant chemoradiotherapy(CRT)remains a challenge with all modalities because it is difficult to reliably differentiate between the tumor mass and other radiation-induced changes in the images.EUS does not appear to have a useful role in post-therapeutic response assessments.Although CT is most commonly used to evaluate treatment responses,its utility for identifying and following-up metastatic lesions is limited.Preoperative high-resolution MRI in combination with diffusion-weighted imaging,and/or PET-CT could provide valuable prognostic information for rectal cancer patients with locally advanced disease receiving preoperative CRT.Based on these results,we conclude that a combination of multimodal imaging methods should be used to precisely assess the restaging of rectal cancer following CRT. | Suk Hee Heo Jin Woong Kim Sang Soo Shin Yong Yeon Jeong Heoung-Keun Kang | 2014 | World Journal of Gastroenterology2014，20，15： | 16 |
| 2 | Prevalence of clonorchiasis in patients with gastrointestinal disease: A Korean nationwide multicenter survey显示文摘AIM: To investigate prevalence of Clonorchis sinensis in patients with gastrointestinal symptoms, and the relation of the infection to hepatobiliary diseases in 26 hospitals in Korea.METHODS: Consecutive patients who had been admitted to the Division of Gastroenterology with gastrointestinal symptoms were enrolled from March to April 2005. Of those who had been diagnosed with clonorchiasis, epidemiology and correlation between infection and hepatobiliary diseases were surveyed by questionnaire.RESULTS: Of 3080 patients with gastrointestinal diseases, 396 (12.9%) had clonorchiasis and 1140 patients (37.2%) had a history of eating raw freshwater fish. Of those with a history of raw freshwater fish ingestion, 238 (20.9%) patients had clonorchiasis. Cholangiocarcinoma was more prevalent in C. sinensis-infected patients than non-infected patients [34/396 (8.6%) vs 145/2684 (5.4%), P = 0.015]. Cholangiocarcinoma and clonorchiasis showed statistically significant positive cross-relation (P = 0.008). Choledocholithiasis, cholecystolithiasis, cholangitis, hepatocellular carcinoma, and biliary pancreatitis did not correlate with clonorchiasis.CONCLUSION: Infection rate of clonorchiasis was still high in patients with gastrointestinal diseases in Korea, and has not decreased very much during the last two decades. | 3 | MRI of gastric carcinoma:Results of T and N-staging in an in vitro study显示文摘AIM:To determine the accuracy of 1.5-T magnetic resonance imaging (MRI) in the evaluation of gastric wall invasion and perigastric lymph node metastasis in gastric adenocarcinoma.METHODS: Twenty resected gastric specimens containing 20 tumors were studied with a 1.5-T MR system using a commercial head surface coil. MR scanning was performed with a T1 weighted image (TR/TE = 500/20), and a T2 weighted image (TR/TE=2500/90). MR findings were compared with pathologic findings.RESULTS: A T1-weighted image demonstrated three layers in the normal gastric wall. All of the gastric tumors were well demonstrated by lesions and location. In a MRI findings of gastric wall invasion, there was 1 case of T1, 7 of T2, 11 of T3. Pathologic results of resected specimens included 3 cases of pT1, 4 of pT2, and 12 of pT3. The accuracy of T staging with MRI was 74% (14 of 19). MRI findings of lymph node me-tastasis included 6 cases of N0, 13 cases of N1. | 5 | Electrical Stimulation of Auricular Acupressure for Dry Eye:A Randomized Controlled-Clinical Trial显示文摘Objective: To evaluate the clinical efficacy of electrical stimulation(ES) of auricular acupressure on reducing the ocular symptoms and signs before and after treatment for dry eye. Methods: The inclusion criteria were the tear film break-up time(TFBUT) below 5 s and a Schirmer test-Ⅰ below 5 mm in dry eyes with ocular symptoms for at least 6 months. Subjects were randomized into a treatment group(50 cases) with continuous low frequency ES under auricular acupressure at acupoints and a no ES under auricular acupressure(no-ES, control group, 50 cases) on the same acupoints. Auricular acupressure were stimulated with ES at 4 master points of both ears, which were performed twice a week for 4 weeks at each point for 30 s. The ocular symptoms, the TFBUT, and Schirmer test-Ⅰ were evaluated before and after this procedure. Results: There were significantly better scores in TFBUT(P=0.032), the Schirmer test-Ⅰ(P=0.044) and ocular symptoms(P=0.029) at 3 months post-treatment in the treatment group than in the control group. The total effective rate in the treatment group was accomplished in 41(82%) of the 50 cases of dry eye. | 7 | Alterations of Gefitinib Pharmacokinetics by Co-administration of Herbal Medications in Rats显示文摘Objective: To evaluate the potential pharmacokinetic interactions of the anticancer agent gefitinib(Iressa?) and the oriental medications Guipi Decoction(归脾汤, GPD, Guibi-tang in Korean) and Bawu Decoction(八物汤, BWD, Palmul-tang in Korean). Methods: Methylcellulose(MC, control), GPD(1,200 mg/kg), or BWD(6,000 mg/kg) was orally administered to rats either as a single dose or multiple doses prior to gefitinib administration. To examine the effects of a single dose of the herbal medicines, gefitinib(10 mg/kg) was orally administered after 5 min or 1 h of MC or the herbal medicine pretreatments. To examine the effects of the multiple doses of the herbal medicines, gefitinib(10 mg/kg) was orally administered following 7 consecutive days of the administration of MC or each herbal medicine. The plasma concentrations of gefitinib were determined with liquid chromatography-tandem mass spectrometry assay. The plasma concentration-time profiles of gefitinib were analyzed with a noncompartmental analysis. Results: Gefitinib was rapidly absorbed and showed a monoexponential decline with an elimination half-life of 3.7–4.1 h. The pharmacokinetics of gefitinib was not affected by GPD pretreatment. However, a significantly lower maximum plasma concentration(Cmax, P<0.05) and area under the curve(P<0.05), and a delayed time to reach Cmax(Tmax, P<0.01) were observed in both single-and multipledose BWD-pretreated rats compared with the control rats. Conclusions: BWD and not GPD might delay and interfere with gefitinib absorption.
