| 1 | Metabolic syndrome in hypertensive patients:An unholy alliance显示文摘For many years, it has been recognized that hypertension tends to cluster with various anthropometric and metabolic abnormalities including abdominal obesity, elevated triglycerides, reduced high-density lipoprotein cholesterol, glucose intolerance, insulin resistance and hyperuricemia. This constellation of various conditions has been transformed from a pathophysiological concept to a clinical entity, which has been defined metabolic syndrome(MetS). The consequences of the MetS have been difficult to assess without commonly accepted criteria to diagnose it. For this reason, on 2009 the International Diabetes Federation, the American Heart Association and other scientific organizations proposed a unified MetS definition. The incidence of the MetS has been increasing worldwide in parallel with an increase in overweight and obesity. The epidemic proportion reached by the MetS represents a major public health challenge, because several lines of evidence showed that the MetS, even without type 2 diabetes, confers an increased risk of cardiovascular morbidity and mortality in different populations including also hypertensive patients. It is likely that the enhanced cardiovascular risk associated with MetS in patients with high blood pressure may be largely mediated through an increased prevalence of preclinical cardiovascular and renal changes, such as left ventricular hypertrophy, early carotid atherosclerosis, impaired aortic elasticity, hypertensive retinopathy and microalbuminuria. | 2 | 双侧前庭病诊断标准:Bárány学会前庭疾病分类委员会共识显示文摘Barany学会前庭疾病分类委员会制定了双侧前庭病(bilateral vestibulopathy,BVP)的诊断标准。其诊断需基于病史、床旁查体和实验室检查进行评估。BVP是一类以站立或行走时出现不稳症状、在头部运动或黑暗环境和(或)地面不平时不稳加重为特点的慢性前庭综合征。此外,患者可伴头部或身体运动诱发的视物模糊或振动幻视;在静坐或平躺时通常没有症状。双侧前庭眼反射(vestibulo-ocular reflex,VOR)功能明显下降或缺损是诊断BVP的基础。对于角前庭-眼动反射(angular VOR,aVOR)的检测,可以通过头脉冲试验(head impulse test,HIT)、视频头脉冲试验(video HIT,vHIT)和巩膜搜索线圈技术来评估高频区域的功能,温度试验检测低频区域功能,正弦谐波或阶梯模式的转椅试验评估中频区域功能。BVP诊断标准:双侧水平VOR增益均<0.6(角速度为150°/s^300°/s)和(或)温度试验反应减弱(每一侧冷水和热水刺激诱发出的眼震高峰慢相角速度(slow-phase velocity,SPV)之和<6°/s)和(或)正弦谐波转椅试验(0.1 Hz,Vmax为50°/s)水平VOR增益<0.1,相位超前>68°(时间常数<5 s)。可能的BVP诊断标准为上述典型症状和床旁HIT显示双侧水平半规管病变。以下检查可作为补充检测但目前尚未纳入在诊断标准中:(1)动态视敏度(dynamic visual acuity,DVA)(视力下降≥0.2 LogMAR);(2) Romberg试验阳性(提示前庭或本体感觉系统障碍,因此无特异性);(3)颈性和眼性前庭诱发肌源性电位(vestibular evoked myogenic potentials,VEMP)提示耳石器功能异常。目前对BVP亚型进一步细分的证据尚不充分,很难提出具有临床意义的定义。将来可根据解剖结构和频率特性对BVP的亚型进行分类:如低频或高频VOR范围内单纯半规管功能受损或单纯耳石器功能受损,其中后者较为罕见。双侧前庭病是一个临床综合征,诊断时应考虑其已知的病因(例如耳毒性药物中毒、双侧梅尼埃病、双侧前庭神经鞘瘤)。双侧前庭病还可被称为双侧前庭功能衰减、功能不足、反射消失、功能低下及功能丧失。 | 凌霞(译) 朱扬(译) 王璟(审校) 杨旭(审校) Michael Strupp Ji-Soo Kim Toshihisa Murofushi Dominik Straumann Joanna CJen Sally MRosengren Charles CDella Santina Herman Kingma | 2019 | 神经损伤与功能重建2019，14，12： | 4 |
| 3 | Altered molecular pattern of mucosal healing in Crohn's disease fibrotic stenosis显示文摘AIM: To investigate tumor necrosis factor-α (TNF-α), syndecan 1 and basic fibroblast growth factor (bFGF) balance in Crohn's disease (CD) strictures. METHODS: Our study was performed on 24 surgical specimens of CD fibrotic stenosis. Ten histological normal surgical samples were retrieved for both the large and small bowel from patients with benign conditions and healthy tissue represented control collection. Sex and age in controls did not differ from CD group. Three endoscopic biopsy specimens taken after informed consent in subjects with normal colon were also used as negative controls. TNF-α, syndecan 1 and bFGF were detected by both reverse transcriptase reverse transcriptase polymerase chain reaction after mRNA extraction (results expressed as fold-change) and immunohistochemistry.RESULTS: TNF-α did not show any significant difference between CD and control specimens (1.54 ± 1.19; P > 0.05). Very high levels of bFGF were observed in CD (11.76 ± 4.65; P < 0.001) unlike syndecan 1 which showed a moderate increase (5.53 ± 2.18; P < 0.005). analysis of variance (ANOVA) plus Student-NeumannKeuls showed: bFGF > syndecan 1 > TNF-α = control. Immunoreactivity for bFGF was observed in epithelial, stromal, endothelial cells and even in the muscular layer, whilst in normal tissue it was almost unexpressed. Syndecan 1 and TNF-α staining was confined to mucosal epithelial and stromal cells, while in controls syndecan 1 was found in its normal site, i.e. , basolateral area of the crypts and TNF-α very poorly expressed.
