2Early warning system for shallow landslides using rainfall threshold and slope stability analysis显示文摘A combined cluster and regression analysis were performed for the first time to identify rainfall threshold that triggers landslide events in Amboori, Kerala, India. Amboori is a tropical area that is highly vulnerable to landslides. The 2, 3, and 5-day antecedent rainfall data versus daily rainfall was clustered to identify a cluster of critical events that could potentially trigger landslides. Further, the cluster of critical events was utilized for regression analysis to develop the threshold equations. The 5-day antecedent(xvariable) vs. daily rainfall(y-variable) provided the best fit to the data with a threshold equation of y = 80.7-0.1981 x. The intercept of the equation indicates that if the 5-day antecedent rainfall is zero, the minimum daily rainfall needed to trigger the landslide in the Amboori region would be 80.7 mm. The negative coefficient of the antecedent rainfall indicates that when the cumulative antecedent rainfall increases, the amount of daily rainfall required to trigger monsoon landslide decreases. The coefficient value indicates that the contribution of the 5-day antecedent rainfall is~20% to the landslide trigger threshold. The slope stability analysis carried out for the area, using Probabilistic Infinite Slope Analysis Model(PISA-m), was utilized to identify the areas vulnerable to landslide in the region. The locations in the area where past landslides have occurred demonstrate lower Factors of Safety(FS) in the slope stability analysis. 5Neuroprotective measures in children with traumatic brain injury显示文摘Traumatic brain injury(TBI) is a major cause of death and disability in children. Severe TBI is a leading cause of death and often leads to life changing disabilities in survivors. The modern management of severe TBI in children on intensivecare unit focuses on preventing secondary brain injury to improve outcome. Standard neuroprotective measures are based on management of intracranial pressure(ICP) and cerebral perfusion pressure(CPP) to optimize the cerebral blood flow and oxygenation, with the intention to avoid and minimise secondary brain injury. In this article, we review the current trends in management of severe TBI in children, detailing the general and specific measures followed to achieve the desired ICP and CPP goals. We discuss the often limited evidence for these therapeutic interventions in children, extrapolation of data from adults, and current recommendation from paediatric guidelines. 6Selective CDK inhibitors:promising candidates for future clinical traumatic brain injury trials显示文摘Traumatic brain injury induces secondary injury that contributes to neuroinflammation, neuronal loss, and neurological dysfunction. One important injury mechanism is cell cycle activation which causes neuronal apoptosis and glial activation. The neuroprotective effects of both non-selective(Flavopiridol) and selective(Roscovitine and CR-8) cyclin-dependent kinase inhibitors have been shown across multiple experimental traumatic brain injury models and species. Cyclin-dependent kinaseinhibitors, administered as a single systemic dose up to 24 hours after traumatic brain injury, provide strong neuroprotection-reducing neuronal cell death, neuroinflammation and neurological dysfunction. 12Rectal ulcers and massive bleeding after hemorrhoidal band ligation while on aspirin显示文摘Endoscopic hemorrhoidal band ligation is a well-established nonoperative method for treatment of bleeding internal hemorrhoids(grade 1 to 3). It is a safe and effective technique with a high success rate. Complications with this procedure are uncommon. Although rectal ulceration due to band ligation is a rare complication, it can cause life-threatening hemorrhage especially when patients are on medications which impair hemostasis like aspirin or non steroidal antiinflammatory drugs. We present 2 cases of massive lower gastro-intestinal bleeding in patients who had a band ligation procedure performed 2 wk prior to the presentation and were on aspirin at home. Both the patients were hemodynamically unstable requiring resuscitation. They required platelet and blood transfusions and were found to have rectal ulcers on colonoscopy done subsequently. The rectal ulcers corresponded to the site of band ligation. The use of aspirin by these patients would have caused defects in the hemostasis and may have predisposed them to massive bleeding in the presence of rectal ulcers occurring after the band ligation procedure. Managing aspirin before and after the ligation may be difficult especially since adequate guidelines are unavailable.
