1Non-invasive determination of hepatic steatosis by acoustic structure quantification from ultrasound echo amplitude显示文摘AIM:To use leptin-deficient(ob/ob) mice with demonstrated differences in steatosis levels to test a new diagnostic method using the acoustical structure quantification(ASQ) mode and the associated analytical parameter,'focal disturbance ratio'(FD-ratio).METHODS:Nine ob/ob mice,at 5,8,and 12 wk of age(n = 3 in each age group),were used as models for hepatic steatosis.Echo signals obtained from ultrasonography in the mice were analyzed by ASQ,which uses a statistical analysis of echo amplitude to estimate inhomogeneity in the diagnostic region.FD-ratio,as calculated from this analysis,was the focus of the present study.FD-ratio and fat droplet areas and sizes were compared between age groups.RESULTS:No fibrosis or inflammation was observed in any of the groups.The fat droplet area significantly(P < 0.01) increased with age from 1.25% ± 0.28% at 5 wk to 31.07% ± 0.48% at 8 wk to 51.69% ± 3.19% at 12 wk.The median fat droplet size also significantly(P < 0.01) increased with age,from 1.33(0.55-10.52) m at 5 wk,2.82(0.61-44.13) m at 8 wk and 6.34(0.66-81.83) m at 12 wk.The mean FD-ratio was 0.42 ± 0.11 at 5 wk,0.11 ± 0.05 at 8 wk,and 0.03 ± 0.02 at 12 wk.The FD-ratio was significantly lower at 12 wk than at 5 wk and 8 wk(P < 0.01).A significant negative correlation was observed between the FD-ratio and either the fat droplet area(r =-0.7211,P = 0.0017) or fat droplet size(r =-0.9811,P = 0.0052).CONCLUSION:This tool for statistical analysis of signals from ultrasonography using the FD-ratio can be used to accurately quantify fat in vivo in an animal model of hepatic steatosis,and may serve as a quantitative biomarker of hepatic steatosis.Hidekatsu Kuroda Keisuke Kakisaka Naohisa Kamiyama Takayoshi Oikawa Mio Onodera Kei Sawara Kanta Oikawa Ryujin Endo Yasuhiro Takikawa Kazuyuki Suzuki 2012World Journal of Gastroenterology2012,18,29:15
2Visualizing the hepatic vascular architecture using superb microvascular imaging in patients with hepatitis C virus: A novel technique显示文摘AIM: To identify the hepatic vascular architecture of patients with hepatitis C virus(HCV) using superb microvascular imaging(SMI) and investigate the use of SMI in the evaluation of liver fibrosis.METHODS: SMI was performed in 100 HCV patients. SMI images were classified into five types according to the vascular pattern, and these patterns were compared with the fibrosis stage. Moreover, the images were analyzed to examine vascularity by integrating the number of SMI signals in the region of interest ROI [number of vascular trees(VT)]. The number of VT, fibrosis stage, serum parameters of liver function, and CD34 expression were investigated.RESULTS: There was a significant difference between SMI distribution pattern and fibrosis stage(P < 0.001). The mean VT values in each of the fibrosis stages were as follows: 26.69 ± 7.08 in F0, 27.72 ± 9.32 in F1, 36.74 ± 9.23 in F2, 37.36 ± 5.32 in F3, and 58.14 ± 14.08 in F4. The VT showed excellent diagnostic ability for F4 [area under the receiver operator characteristic(AUROC): 0.911]. The VT was significantly correlated with the CD34 labeling index(r = 0.617, P < 0.0001).CONCLUSION: SMI permitted the detailed delineation of the vascular architecture in chronic liver disease. 3Hepatic steatosis as a possible risk factor for the development of hepatocellular carcinoma after eradication of hepatitis C virus with antiviral therapy in patients with chronic hepatitis C显示文摘AIM: To elucidate risk factors contributing to the development of hepatocellular carcinoma (HCC) among patients with sustained viral response (SVR) after interferon (IFN) treatment and to examine whether HCV-RNA still remained in the liver of SVR patients who developed HCC. METHODS: Two-hundred and sixty-six patients, who achieved SVR, were enrolled in this study. We retrospectively reviewed clinical, viral and histological features of the patients, and examined whether the development of HCC depends on several clinical variables using Kaplan-Meier Method. RT-PCR was used to seek HCV-RNA in 3 out of 7 patients in whom liver tissue was available for molecular analysis. RESULTS: Among the enrolled 266 patients with SVR, HCC developed in 7 patients (7/266; 2.6%). We failed to detect HCV-RNA both in cancer and non-cancerous liver tissue in all three patients. The cumulative incidence for HCC was significantly different depending on hepatic fibrosis (F3-4) (P = 0.0028), hepatic steatosis (Grade 2-3) (P = 0.0002) and age (≥ 55) (P = 0.021) at the pre-interferon treatment. 4Change in arterial tumor perfusion is an early biomarker of lenvatinib efficacy in patients with unresectable hepatocellular carcinoma显示文摘BACKGROUND Lenvatinib is one of the first-line tyrosine kinase inhibitors used for unresectable hepatocellular carcinoma(HCC). In the present study, we evaluated the potential of early changes in the time-intensity curve(TIC) of arterial phase on contrastenhanced ultrasound(CEUS) as early imaging biomarkers of lenvatinib efficacy.AIM To evaluate the potential of the early changes in the TIC of CEUS as early imaging biomarkers of lenvatinib efficacy in patients with unresectable HCC.METHODS We analyzed 20 consecutive patients with unresectable HCC treated with lenvatinib from March to November 2018. Tumor response at 8 wk was assessed by computed tomography using the modified Response Evaluation Criteria in Solid Tumors(m RECIST). CEUS was performed at baseline before treatment(Day 0) and on day 7(Day 7), and the images were analyzed in the arterial phase for 20 seconds after the contrast agent arrived at the target tumor. Three perfusion parameters were extracted from the TICs: the slope of wash-in(Slope),time to peak(TTP) intensity, and the total area under the curve(AUC) during wash-in. The rate of change in the TIC parameters between Day 0 and Day 7 was compared between treatment responders and non-responders based on m RECIST.RESULTS The rate of change for all TIC parameters showed significant differences between the responders(n = 9) and non-responders(n = 11)(Slope, P = 0.025; TTP, P =0.004; and AUC, P = 0.0003). 5Transcatheter arterial chemoembolization with a fine-powder formulation of cisplatin for hepatocellular carcinoma显示文摘AIM:To evaluate the efficacy of transcatheter arterial chemoembolization(TACE) using a suspension of a fine-powder formulation of cisplatin(DDPH) for hepatocellular carcinoma(HCC).METHODS:The study population was comprised of 164 patients who were treated by TACE alone.Of these patients,76 underwent TACE using a suspension of DDPH in lipiodol(LPD)(DDPH group),and the remaining 88 underwent TACE with an emulsion of doxorubicin(ADM) with LPD(ADM group).We compared the DDPH group with the ADM group in terms of the objective early response rate,progression free survival(PFS) and overall survival(OS).RESULTS:The objective early response rate in the DDPH group was significantly higher than that in the ADM group(54% vs 24%,P < 0.001).The PFS rate in the DDPH group was also significantly higher than that in the ADM group(P < 0.001).Moreover,the OS in the DDPH group was significantly longer than that in the ADM group(P = 0.002).Although the incidence rate of nausea or vomiting in the DDPH group was higher than that in the ADM group,the ADM group showed a higher incidence rate of the adverse events of hepatic arterial damage and leucopenia.No other serious complications were observed in either group.CONCLUSION:We conclude that TACE using a suspension of DDPH in LPD could be a useful treatment for HCC.Kazuhiro Kasai Akira Ushio Kei Sawara Yasuhiro Miyamoto Yukiho Kasai Kanta Oikawa Hidekatsu Kuroda Yasuhiro Takikawa Kazuyuki Suzuki 2010World Journal of Gastroenterology2010,16,27:6
6Resected case of eosinophilic cholangiopathy presenting with secondary sclerosing cholangitis显示文摘Eosinophilic cholangiopathy is a rare condition characterized by eosinophilic infiltration of the biliary tract and causes sclerosing cholangitis.We report a patient with secondary sclerosing cholangitis with eosinophilic cholecystitis.A 46-year-old Japanese man was admitted to our hospital with jaundice. Computed tomography revealed dilatation of both the intrahepatic and extrahepatic bile ducts,diffuse thickening of the wall of the extrahepatic bile duct,and thickening of the gallbladder wall.Under the diagnosis of lower bile duct carcinoma,he underwent pyloruspreserving pancreatoduodenectomy and liver biopsy. On histopathological examination,conspicuous fibrosis was seen in the lower bile duct wall.In the gallbladder wall,marked eosinophilic infiltration was seen.Liver biopsy revealed mild portal fibrosis.He was diagnosed as definite eosinophilic cholecystitis with sclerosing cholangitis with unknown etiology.The possible etiology of sclerosing cholangitis was consequent fibrosis from previous eosinophilic infiltration in the bile duct.The clinicopathological findings of our case and a literature review indicated that eosinophilic cholangiopathy could cause a condition mimicking primary sclerosing cholangitis(PSC).Bile duct wall thickening in patientswith eosinophilic cholangitis might be due to fibrosis of the bile duct wall.Eosinophilic cholangiopathy might be confused as PSC with eosinophilia.Fumihiko Miura Takehide Asano Hodaka Amano Masahiro Yoshida Naoyuki Toyota Keita Wada Kenichiro Kato Tadahiro Takada Junichi Fukushima Fukuo Kondo Hajime Takikawa 2009World Journal of Gastroenterology2009,15,11:4

8IgG4-unrelated type 1 autoimmune pancreatitis显示文摘A 50-year-old male was referred to our hospital for the evaluation of hyperproteinemia.Fluorodeoxyglucose positron emission tomography revealed high fluorodeoxyglucose uptake in the pancreas,bilateral lacrimal glands,submandibular glands,parotid glands,bilateral pulmonary hilar lymph nodes,and kidneys.Laboratory data showed an elevation of hepatobiliary enzymes,renal dysfunction,and remarkably high immunoglobulin(Ig) G levels,without elevated serum IgG4.Abdominal computed tomography revealed swelling of the pancreatic head and bilateral kidneys.Endoscopic retrograde cholangiopancreatography showed an irregular narrowing of the main pancreatic duct in the pancreatic head and stricture of the lower common bile duct.Histological examination by endoscopic ultrasonography-guided fine-needle aspiration revealed findings of lymphoplasmacytic sclerosing pancreatitis without IgG4-positive plasma cells.Abnormal laboratory values and the swelling of several organs were improved by the treatment with steroids.The patient was diagnosed as having type 1 autoimmune pancreatitis(AIP) based on the International Consensus Diagnostic Criteria.Therefore,we encountered a case of compatible type 1 AIP without elevated levels of serum IgG4 or IgG4-positive plasma cells.This case suggests that AIP phenotypes are not always associated with IgG4.Eriko Nakano Atsushi Kanno Atsushi Masamune Naoki Yoshida Seiji Hongo Shin Miura Tetsuya Takikawa Shin Hamada Kiyoshi Kume Kazuhiro Kikuta Morihisa Hirota Keisuke Nakayama Fumiyoshi Fujishima Tooru Shimosegawa 2015World Journal of Gastroenterology2015,21,33:3
9Pancreatic duct drainage using EUS-guided rendezvous technique for stenotic pancreaticojejunostomy显示文摘The patient was a 30-year-old female who had undergone excision of the extrahepatic bile duct and Rouxen-Y hepaticojejunostomy for congenital biliary dilatation at the age of 7.Thereafter,she suffered from recurrent acute pancreatitis due to pancreaticobiliary maljunction and received subtotal stomach-preserving pancreaticoduodenectomy.She developed a pancreatic fistula and an intra-abdominal abscess after the operation.These complications were improved by percutaneous abscess drainage and antibiotic therapy.How ever,upper abdominal discomfort and the elevation of serum pancreatic enzymes persisted due to stenosis from the pancreaticojejunostomy.Because we could not accomplish dilation of the stenosis by endoscopic retrograde cholangiopancreatography,we tried an endoscopic ultrasonography(EUS) guided rendezvous technique for pancreatic duct drainage.After transgastric puncture of the pancreatic duct using an EUS-fine needle aspiration needle,the guidewire was inserted into the pancreatic duct and finally reached to the jejunum through the stenotic anastomosis.We changed the echoendoscope to an oblique-viewing endoscope,then grasped the guidewire and withdrew it through the scope.The stenosis of the pancreaticojejunostomy was dilated up to 4 mm,and a pancreatic stent was put in place.Though the pancreatic stent was removed after three months,the patient remained symptomfree.Pancreatic duct drainage using an EUS-guided rendezvous technique was useful for the treatment of a stenotic pancreaticojejunostomy after pancreaticoduodenectomy.Tetsuya Takikawa Atsushi Kanno Atsushi Masamune Shin Hamada Eriko Nakano Shin Miura Hiroyuki Ariga Jun Unno Kiyoshi Kume Kazuhiro Kikuta Morihisa Hirota Hiroshi Yoshida Yu Katayose Michiaki Unno Tooru Shimosegawa 2013World Journal of Gastroenterology2013,19,31:3
10Therapeutic efficacy of transarterial chemo-embolization with a fine-powder formulation of cisplatin for hepatocellular carcinoma显示文摘AIM:To evaluate the efficacy of transarterial chemoembolization(TACE) using a suspension of a finepowder formulation of cisplatin(DDPH) in lipiodol(LPD) in the treatment of hepatocellular carcinoma(HCC).METHODS:The subjects were 262 HCC patients treated with TACE using a DDPH-LPD suspension.The DDPH-LPD suspension was prepared by mixing 50 mg of DDPH into 10 mL of LPD.TACE was repeated when treated lesions relapsed and/or new hepatic lesions were detected.These patients received additional TACE using the same agent.TACE was repeated until complete regression of the tumor was obtained.The primary efficacy endpoint of the current study was the objective early response rate.Secondary efficacy endpoints were progression-free survival(PFS) and overall survival.RESULTS:The objective early response rate was 43.6%.Cumulative PFS rates were 56.7% at 6 mo,23.1% at 12 mo,13.4% at 18 mo,and 10.5% at 24 mo.The median PFS was 6.6 mo.Cumulative survival rates were 90.6% at 6 mo,81.9% at 12 mo,70.5% at 24 mo,and 58.8% at 36 mo.Median survival time was 46.6 mo.All adverse reactions were controllable by temporary suspension of treatment.No serious complications or treatment-related deaths were observed.CONCLUSION:TACE using a suspension of DDPH in LPD may be a useful treatment for HCC.Kazuhiro Kasai Akira Ushio Yukiho Kasai Kei Sawara Yasuhiro Miyamoto Kanta Oikawa Yasuhiro Takikawa Kazuyuki Suzuki 2013World Journal of Gastroenterology2013,19,14:3
