| 1 | Non-invasive versus invasive mechanical ventilation for respiratory failure in severe acute respiratory syndrome显示文摘Background Severe acute respiratory syndrome is frequently complicated by respiratory failure requiringventilatory support.We aimed to compare the efficacy of non-invasive ventilation against invasive mechanicalventilation treating respiratory failure in this disease.Methods Retrospective analysis was conducted on all respiratory failure patients identified from the HongKong Hospital Authority Severe Acute Respiratory Syndrome Database.Intubation rate, mortality and secondaryoutcome of a hospital utilizing non-invasive ventilation under standard infection control conditions ( NIVHospital) were compared against13 hospitals using solely invasive ventilation (IMVHospitals).Multiple logisticregression analyses with adjustments for confounding variables were performed to test for association betweenoutcomes and hospital groups.Results Both hospital groups had comparable demographics and clinical profiles, but NIV Hospital (42patients) had higher lactate dehydrogenase ratio and worse radiographic score on admission and ribavirin-corticosteroid commencement.Compared to IMV Hospitals (451 patients), NIV Hospital had lower adjustedodds ratios for intubation (0.36, 95% CI 0.164 -0.791,P=0.011) and death (0.235, 95% CI 0.077 -0.716,P=0.011), and improved earlier after pulsed steroid rescue.There were no instances of transmission ofsevere acute respiratory syndrome among health care workers due to the use of non-invasive ventilation.Conclusion Compared to invasive mechanical ventilation, non-invasive ventilation as initial ventilatory supportfor acute respiratory failure in the presence of severe acute respiratory syndrome appeared to be associated withreduced intubation need and mortality. | Loretta YC Yam Alfred YF Chan Thomas MT Cheung Eva LH Tsui Jane CK Chan Vivian CW Wong | 2005 | Chinese Medical Journal2005，，17： | 3 |
| 2 | 全球城市景观的概念辨析与整合——一个空间分析的框架(英文)显示文摘Current global urbanisation processes are leading to new forms of massive urban constellations. The conceptualisations and classifications of these, however, are often ambiguous, overlap or lag behind in scientific literature. This article examines whether there is a common denominator to define and delimitate–and ultimately map–these new dimensions of cityscapes. In an extensive literature review we analysed and juxtaposed some of the most common concepts such as megacity, megaregion or megalopolis. We observed that many concepts are abstract or unspecific, and for those concepts for which physical parameters exist, the parameters are neither properly defined nor used in standardised ways. While understandably concepts originate from various disciplines, the authors identify a need for more precise definition and use of parameters. We conclude that often, spatial patterns of large urban areas resemble each other considerably but the definitions vary so widely that these differences may surpass any inconsistencies in the spatial delimitation process. In other words, today we have tools such as earth observation data and Geographic Information Systems to parameterise if clear definitions are provided. This appears not to be the case. | 11 | PillCamColon2 after incomplete colonoscopy-A prospective multicenter study显示文摘AIM To evaluate the ability of PillCamColon2 to visualize colonic segments missed by incomplete optical colonoscopy(OC) and to assess the diagnostic yield.METHODS This prospective multicentre study included 81 patients from nine centres who underwent second-generation colon capsule endoscopy(CCE) following incomplete OC performed by an experienced gastroenterologist(> 1000 colonoscopies). Patients with stenosis were excluded. According to patient preferences, CCE was performed the following day(protocol A) after staying on clear liquids and 0.75 L Moviprep in the morning or within 30 d after new split-dose Moviprep(protocol B). Boosts consisted of 0.75 L and 0.25 L Moviprep, and phospho-soda was given as a rescue if the capsule was not excreted after seven hours.RESULTS Seventy-four patients were analysed(51% of them in group A; 49% in group B). Bowel cleansing was adequate in 67% of cases, and CCE could visualize colonic segments missed by incomplete colonoscopy in 90% of patients under protocol A and 97% of patients under protocol B(P = 0.35, n.s.). Significant polyps including adenocarcinoma were detected in 24% of cases. Detection rates for all polyps and significant polyps per patient were similar in both protocols. Polyps were found predominantly in the right colon(86%) in segments that were not reached by OC. Extracolonic findings-such as reflux esophagitis, suspected Barrett esophagus, upper GI-bleeding, gastric polyps, gastric erosions and angiectasia-were detected in eight patients. Pill Cam Colon2 capsule was retained in the ileum of one patient(1.4%) without symptoms and removed during an uneventful resection for unknown Crohn's disease that was diagnosed as the cause of anemia, which was the indication for colonoscopy. CCE was well tolerated.
