1Improved quality of life in patients with gastric cancer after esophagogastrostomy reconstruction显示文摘AIM:To compare postoperative quality of life (QOL) in patients with gastric cancer treated by esophagogastrostomy reconstruction after proximal gastrectomy.METHODS: QOL assessments that included functional outcomes (a 24-item survey about treatment-specific symptoms) and health perception (Spitzer QOL Index) were performed in 149 patients with gastric cancer in the upper third of the stomach, who had received proximal gastrectomy with additional esophagogastrostomy.RESULTS: Fifty-four patients underwent reconstruction by esophagogastric anterior wall end-to-side anastomosis combined with pyloroplasty (EA group); 45 patients had reconstruction by esophagogastric posterior wall end-to-side anastomosis (EP group); and 50 patients had reconstruction by esophagogastric end-to-end anastomosis (EE group). The EA group showed the best postoperative QOL, such as recovery of body weight, less discomfort after meals, and less heart burn or belching at 6 and 24 mo postoperatively. However, the survival rates, surgical results and Spitzer QOL index were similar among the three groups.CONCLUSION: Postoperative QOL was better in the EA than EP or EE group. 2Traditional herbal medicine in preventing recurrence after resection of small hepatocellular carcinoma: a multicenter randomized controlled trial显示文摘BACKGROUND: Disease recurrence is a main challenge in treatment of hepatocellular carcinoma (HCC). There is no generally accepted method for preventing recurrence of HCC after resection. OBJECTIVE: To compare the efficacy of a traditional herbal medicine (THM) regimen and transarterial chemoembolization (TACE) in preventing recurrence in post-resection patients with small HCC. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS: This is a multicenter, open-label, randomized, controlled study, which was undertaken in five centers of China. A total of 379 patients who met the eligibility criteria and underwent randomization were enrolled in this trial. One hundred and eighty-eight patients were assigned to the THM group and received Cinobufacini injection and Jiedu Granule, and the other 191 patients were assigned to the TACE group and received one single course of TACE. MAIN OUTCOME MEASURES: Primary outcome measures were the annual recurrence rate and the time to recurrence. Incidence of adverse events was regarded as the secondary outcome measure. RESULTS: Among the 364 patients who were included in the intention-to-treat analysis, 67 patients of the THM group and 87 of the TACE group had recurrence, with a hazard ratio of 0.695 (P = 0.048). Median recurrence-free survival of the patients in the THM and TACE groups was 46.89 and 34.49 months, respectively. Recurrence rates at 1, 2 and 3 years were 17.7%, 33.0% and 43.5% for the THM group, and 28.8%, 42.5% and 54.0% for the TACE group, respectively (P = 0.026). Multivariate analysis indicated that the THM regimen had a big advantage for prolonging the recurrence-free survival. Adverse events were mild and abnormality of laboratory indices of the two groups were similar. CONCLUSION: In comparison with TACE therapy, the THM regimen was associated with diminished risk of recurrence of small-sized HCC after resection, with comparable adverse events. 3Acupuncture and regulation of gastrointestinal function显示文摘In China, acupuncture has been considered an effective method for treating gastrointestinal(GI) dysfunction diseases for thousands of years. In fact, acupuncture has gained progressive acceptance from both practitioners and patients worldwide. However, the therapeutic effects and underlying mechanisms in treating GI dysfunction have not yet been established due to a lack of systematic and comprehensive review articles. Therefore, the aim of this review is to discuss the efficacy of acupuncture as a treatment for GI dysfunction and the associated underlying mechanisms. A search of Pub Med was conducted for articles that were published over the past 10 years using the terms 'acupuncture', 'gastrointestine', and other relevant keywords. In the following review, we describe the effect and underlying mechanisms of acupuncture on GI function from the perspectives of GI motility, visceral sensitivity, the GI barrier, and the brain-gut axis. The dual regulatory effects of acupuncture may manifest by promoting gastric peristalsis in subjects with low initial gastric motility, and suppressing peristalsis in subjects with active initial motility. In addition, the regulation of acupuncture on gastric motility may be intensitydependent. 4Risk factors for lymph node metastasis and evaluation of reasonable surgery for early gastric cancer显示文摘AIM: To give the evidence for rationalizing surgical therapy for early gastric cancer with different lymph node status. METHODS: A series of 322 early gastric cancer patients who underwent gastrectomy with more than 15 lymph nodes retrieved were reviewed in this study. The rate of lymph node metastasis was calculated. Univariate and multivariate analyses were performed to evaluate the independent factors for predicting lymph node metastasis. RESULTS: No metastasis was detected in No.5, 6 lymph nodes (LN) during proximal gastric cancer total gastrectomy, and in No.10, 11p, 11d during for combined resection of spleen and splenic artery and in No.15 LN during combined resection of transverse colon mesentery. No.11p, 12a, 14v LN were proved negative for metastasis. The global metastastic rate was 14.6% for LN, 5.9% for mucosa, and 22.4% for submucosa carcinoma, respectively. The metastasis in group Ⅱ?was almost limited in No.7, 8a LN. Multivariate analysis identified that the depth of invasion, histological type and lymphatic invasion were independent risk factors for LN metastasis. No metastasis from distal cancer (≤ 1.0 cm in diameter) was detected in group Ⅱ?LN. The metastasis rate increased significantly when the diameter exceeded 3.0 cm. All tumors (≤ 1.0 cm in diameter) with LN metastasis and mucosa invasion showed a depressed macroscopic type, and all protruded carcinomas were > 3.0 cm in diameter. CONCLUSION: Segmental/subtotal gastrectomy plus D1/D1 + No.7 should be performed for carcinoma (≤ 1.0 cm in diameter, protruded type and mucosa invasion).Subtotal gastrectomy plus D2 or D1 + No.7, 8a, 9 is the most rational operation, whereas No.11p, 12a, 14v lymphadenectomy should not be recommended routinely for poorly differentiated and depressed type of submucosa carcinoma (> 3.0 cm in diameter). 5Risk factors for alcohol-related liver injury in the island population of China: A population-based case-control study显示文摘AIM:To investigate the association of alcohol dose, duration of drinking and obesity with abnormal alcohol-related liver injury indicators, the prevalence of alcohol-related liver injury in the island population of China.METHODS:Randomized multistage stratified cluster sampling from the island population of China was used in the population-based case-control study. Then interview, physical examination, laboratory assessments and ultrasonography were done. RESULTS:Daily alcohol intake ≥ 20 g, duration of drinking ≥ 5 years and obesity were closely related to alcohol-related liver injury (P < 0.05). The odds-ratio (OR) (95% CI) was 1.965 (1.122-3.442), 3.412 (1.789-6.507) and 1.887 (1.261-2.824), respectively. The prevalence rate of alcohol-related liver injury in ≥ 20 g daily alcohol intake group and < 20 g daily alcohol intake group was 37.14% and 12.06%, respectively. The prevalence rate of alcohol-related liver injury in ≥ 5 years drinking group and < 5 years drinking group was 34.44% and 8.53%, respectively. No significant dose-response relation was found between daily alcohol intake and abnormal alcohol-related liver injury indicators as well as between duration of drinking and abnormal alcohol-related liver injury indicators. There was no significant difference in the prevalence of alcohol-related liver injury between beer drinking group and yellow rice wine drinking group, hard liquor drinking group, multiple drinking group.CONCLUSION:The risk threshold of daily alcohol intake is 20 g and duration of drinking inducing alcohol-related liver injury 5 years in the island population of China. 6Validation of clinical significance of examined lymph node count for accurate prognostic evaluation of gastric cancer for the eighth edition of the American Joint Committee on Cancer(AJCC) TNM staging system显示文摘Objective: To validate the necessity of increasing the examined lymph node(ELN) count for enhancing the accuracy of prognostic evaluation of gastric cancer(GC) patients after curative gastrectomy in multiple medical centers of China.Methods: The clinicopathological data of 7,620 patients who underwent the curative resection for GC between2001 and 2011 were included to demonstrate whether the ELN count is indispensable for enhancing the accuracy of prognostic evaluation of GC patients after surgery. After a meticulous stratification by using the cut-point survival analysis, all included 7,620 patients were allocated into three groups as: less than 16(<16), between 16 and30(16-30), and more than 30(>30) ELNs. Survival differences among various subgroups of GC patients were analyzed to assess the impact of the ELN count on the stage migration in accordance with the overall survival(OS)of GC patients.Results: Survival analyses revealed that the ELN count was positively correlated with the OS(P=0.001) and was an independent prognostic predictor(P<0.01) of 7,620 GC patients. Stratum analysis showed that the accuracy of prognostic evaluation could be enhanced when the ELN count was no less than 16(≥16) for node-negative patients and >30 for node-positive patients. Stage migrations were mainly detected in the various subgroups of patients with specific pN stages as follows: pN0 with 16-30 ELNs(pN0_(16-30)) and pN0 with >30 ELNs(pN0_(>30)), pN0 with <16 ELNs(pN0_(<16)) and pN1_(>30), pN1_(<16) and pN2_(16-30), pN1_(16-30) and pN2_(>30), pN3a_(<16) and pN3b_(16-30), and pN3a_(<16) and pN3b_(>30). 7Heat and power load dispatching considering energy storage of district heating system and electric boilers显示文摘As one of promising clean and low-emission energy, wind power is being rapidly developed in China.However, it faces serious problem of wind curtailment,particularly in northeast China, where combined heat and power(CHP) units cover a large proportion of the district heat supply. Due to the inherent strong coupling between the power and the heat load, the operational flexibility of CHP units is severely restricted in winter to meet the heat supply demand, which imparts considerable stress on the wind power connection to the grid. To promote the integration of wind power and enhance the flexibility of CHP units, this paper presented a method of heat and power load dispatching by exploring the energy storage ability of electric heating boilers and district heating systems. The optimization results indicate that the proposed method can integrate additional wind power into the grid and reduce the coal consumption of CHP units over the optimized period. 102D DEM simulation of particle mixing in rotating drum: A parametric study显示文摘Mixing behaviors of equal-sized glass beads in a rotating drum were investigated by both DEM simulations and experiments. The experiments indicated that higher rotation speed can significantly enhance mixing. The particle profiles predicted by 2D DEM simulation were compared with the experimental results from a quasi-2D drum, showing inconsistency due to reduction of contacts in the single-layer 2D simulation which makes the driving friction weaker than that in the quasi-2D test, better results could be reached by specifying a higher frictional coefficient between the particles and the cylinder wall. In order to explore the influences of physical properties (density, size or friction) on mixing behavior, numerical 2D simulations were carried out systematically, in which one examined specific property being examined was exaggerated while the others were kept the same as that in the control group. The DEM simulations reveal that particle density and size are the dominating factors affecting mixing behaviors, while the effect of frictional coefficient is less significant. 11Human umbilical cord-derived mesenchymal stem cell therapy in patients with COVID-19:a phase 1 clinical trial显示文摘No effective drug treatments are available for coronavirus disease 2019(COVID-19).Host-directed therapies targeting the underlying aberrant immune responses leading to pulmonary tissue damage,death,or long-term functional disability in survivors require clinical evaluation.We performed a parallel assigned controlled,non-randomized,phase 1 clinical trial to evaluate the safety of human umbilical cord-derived mesenchymal stem cells(UC-MSCs)infusions in the treatment of patients with moderate and severe COVID-19 pulmonary disease.The study enrolled 18 hospitalized patients with COVID-19(n=9 for each group).The treatment group received three cycles of intravenous infusion of UC-MSCs(3×107 cells per infusion)on days 0,3,and 6.Both groups received standard COVID-treatment regimens.Adverse events,duration of clinical symptoms,laboratory parameters,length of hospitalization,serial chest computed tomography(CT)images,the PaO2/FiO2 ratio,dynamics of cytokines,and IgG and IgM anti-SARS-CoV-2 antibodies were analyzed.No serious UC-MSCs infusion-associated adverse events were observed.Two patients receiving UC-MSCs developed transient facial flushing and fever,and one patient developed transient hypoxia at 12 h post UC-MSCs transfusion.Mechanical ventilation was required in one patient in the treatment group compared with four in the control group.All patients recovered and were discharged.Our data show that intravenous UC-MSCs infusion in patients with moderate and severe COVID-19 is safe and well tolerated.Phase 2/3 randomized,controlled,double-blinded trials with long-term follow-up are needed to evaluate the therapeutic use of UC-MSCs to reduce deaths and improve long-term treatment outcomes in patients with serious COVID-19.Fanping Meng Ruonan Xu Siyu Wang Zhe Xu Chao Zhang Yuanyuan Li Tao Yang Lei Shi Junliang Fu Tianjun Jiang Lei Huang Peng Zhao Xin Yuan Xing Fan Ji-Yuan Zhang Jinwen Song Dawei Zhang Yanmei Jiao Limin Liu Chunbao Zhou Markus Maeurer Alimuddin Zumla Ming Shi Fu-Sheng Wang 2020Signal Transduction and Targeted Therapy2020,5,1:16

13Characteristic dysbiosis of gut microbiota of Chinese patients with diarrhea-predominant irritable bowel syndrome by an insight into the pan-microbiome显示文摘Background:Irritable bowel syndrome(IBS)is reported associated with the alteration of gut microbial composition termed as dysbiosis.However,the pathogenic mechanism of IBS remains unclear,while the studies of Chinese individuals are scarce.This study aimed to understand the concept of dysbiosis among patients with Chinese diarrhea-predominant IBS(IBS-D),as a degree of variance between the gut microbiomes of IBS-D population and that of a healthy population.Methods:The patients with IBS-D were recruited(assessed according to the Rome III criteria,by IBS symptom severity score)from the Outpatient Department of Gastroenterology of Peking University Third Hosp让al,and volunteers as healthy controls(HCs)were enrolled,during 2013.The 16S rRNA sequences were extracted from fecal samples.Ribosomal database project resources,basic local alignment search tool,and SparCC software were used to obtain the phylotype composition of samples and the internal interactions of the microbial community.Herein,the non-parametric test,Wilcoxon rank-sum test was carried out to find the statistical significance between HC and IBS-D groups.All the P values were adjusted to q values to decrease the error rate.Results:The study characterized the gut microbiomes of Chinese patients with IBS-D,and demonstrated that the dysbiosis could be characterized as directed alteration of the microbiome composition leading to greater disparity between relative abundance of two phyla,Bacteroidetes(Z=4.77,q=1.59×10^-5)and Firmicutes(Z=-3.87,q=5.83×10^-4).Moreover,it indicated that the IBS symptom features were associated with the dysbiosis of whole gut microbiome,instead of one or several certain genera even they were dominating.Two genera,Bacteroides and Lachnospiracea incertae sedis,were identified as the core genera,meanwhile,the non-core genera contribute to a larger pan-microbiome of the gut microbiome.Furthermore,the dysbiosis in patients with IBS-D was associated with a reduction of network complexity of the interacted microbial community(HC us.IBS-D:639 vs.154).The disordered metabolic functions of patients with IBS-D were identified as the potential influence of gut microbiome on the host(significant difference with q<0.01 between HC and IBS-D).Conclusions:This study supported the view of the potential influence of gut microbiome on the symptom of Chinese patients with IBS-D,and further characterized dysbiosis in Chinese patients with IBS-D,thus provided more pathological evidences for IBS-D with the further understanding of dysbiosis.Zhe Wang Cong-Min Xu Yi-Xuan Liu Xiao-Qi Wang Lu Zhang Mo Li Shi-Wei Zhu Zhong-Jie Xie Pei-Hong Wang Li-Ping Duan Huai-Qiu Zhu 2019Chinese Medical Journal2019,,8:15
14Prognosis after resection for hepatitis B virus-associated intrahepatic cholangiocarcinoma显示文摘AIM:To investigate the prognostic factors after resection for hepatitis B virus(HBV)-associated intrahepatic cholangiocarcinoma(ICC) and to assess the impact of different extents of lymphadenectomy on patient survival.METHODS:A total of 85 patients with HBV-associated ICC who underwent curative resection from January 2005 to December 2006 were analyzed.The patients were classified into groups according to the extent of lymphadenectomy(no lymph node dissection,sampling lymph node dissection and regional lymph node dissection).Clinicopathological characteristics and survival were reviewed retrospectively.RESULTS:The cumulative 1-,3-,and 5-year survival rates were found to be 60 %,18 %,and 13 %,respectively.Multivariate analysis revealed that liver cirrhosis(HR = 1.875,95%CI:1.197-3.278,P = 0.008) and multiple tumors(HR = 2.653,95%CI:1.562-4.508,P < 0.001) were independent prognostic factors for survival.Recurrence occurred in 70 patients.The 1-,3-,and 5-year disease-free survival rates were 36%,3% and 0%,respectively.Liver cirrhosis(HR = 1.919,P = 0.012),advanced TNM stage(stage Ⅲ/Ⅳ)(HR = 2.027,P < 0.001),and vascular invasion(HR = 3.779,P = 0.02) were independent prognostic factors for disease-free survival.Patients with regional lymph node dissection demonstrated a similar survival rate to patients with sampling lymph node dissection.Lymphadenectomy did not significantly improve the survival rate of patients with negative lymph node status.CONCLUSION:The extent of lymphadenectomy does not seem to have influence on the survival of patients with HBV-associated ICC,and routine lymph nodedissection is not recommended,particularly for those without lymph node metastasis.Zhen-Feng Wu Xiao-Yu Wu Nan Zhu Zhe Xu Wei-Su Li Hai-Bin Zhang Ning Yang Xue-Quan Yao Fu-Kun Liu Guang-Shun Yang 2015World Journal of Gastroenterology2015,21,3:15
15SYNERGY OF WALL SHEAR STRESS AND CIRCUMFERENTIAL STRESS IN STRAIGHT ARTERIES显示文摘The Wall Shear Stress (WSS) generated by blood flow and Circumferential Stress (CS) driven by blood pressure have been thought to play an important role in blood flow-dependent phenomena such as angiogenesis, vascular remodeling, and atherosgenesis. The WSS and CS in straight arteries were calculated by measuring the blood pressure, center-line velocity, wall thickness, and radius of vessels. The WSS and CS in the time domain were then decomposed into the amplitude and phase in the frequency domain. The CS amplitude to the WSS amplitude ratio (referred as stress amplitude ratio, Zs) and the phase difference between the CS and the WSS (referred as stress phase difference, SPA) in the frequency domain were calculated to characterize the synergy of the CS and WSS. Numerical results demonstrated that the CS is not in phase with the WSS, a time delay in the time domain or a stress phase difference in the frequency domain between the WSS and the CS exists. Theoretical analysis demonstrated that the Zs and SPA are primarily determined by the local factors (blood viscosity, local inertial effects, local geometry, local elasticity) and the input impedance of whole downstream arterial beds. 17High HIV incidence epidemic among men who have sex with men in china: results from a multi-site cross-sectional study显示文摘Background:Recent upsurge of new HIV infections among men who have sex with men(MSM)is a major concern in China.Paucity of national-level information regarding the burden and predictors of this progressive epidemic of new infections called for a multi-centric,timely and comprehensive investigation.Methods:Mixed methods were used to recruit MSM from seven cities in China between 2012 and 2013.Recent and established HIV infections were estimated by Western Blot and BED HIV-1 capture enzyme immunoassay.Syphilis and herpes simplex virus-2(HSV-2)were also tested.Results:A total of 4496 eligible MSM were recruited.The majority was aged≤35 years(77.5%),migrants(60.3%),never married(69.8%),and played receptive role in anal sex(70.5%).The HIV prevalence was 9.9%,and 41.9%were recently infected,with sensitivity/specificity adjusted HIV incidence of 8.9(95%CI:7.6-10.2)/100 Person-Years.The prevalence of history HSV-2 and syphilis were 12.5%and 8.5%,respectively.Recent HIV infection was associated with having multiple male partners(aOR=1.4,95%CI 1.1-1.9),recreational drug use(aOR=2.2,95%CI 1.6-3.0),anal bleeding(aOR=2.1,95%CI 1.4-3.0),syphilis infection(aOR=2.8,95%CI 1.9-4.3)and history HSV-2 infection(aOR=2.3,95%CI 1.5-3.3).Conclusion:High rate of recent HIV infection is potentially resulting in progressive deterioration of the overall HIV epidemic among MSM in China.Targeted interventions to address high-risk MSM including those having multiple partners,history of recreational drug use and syphilis or HSV-2 infection seemed to be the need of the hour.Jun-Jie Xu Wei-Ming Tang Hua-Chun Zou Tanmay Mahapatra Qing-Hai Hu Geng-Feng Fu Zhe Wang Lin Lu Ming-Hua Zhuang Xi Chen Ji-Hua Fu Yan-Qiu Yu Jin-Xin Lu Yong-Jun Jiang Wen-Qing Geng Xiao-Xu Han Hong Shang 2016Infectious Diseases of Poverty2016,5,1:14
18Role of gut microbiota via the gut-liver-brain axis in digestive diseases显示文摘The gut-brain axis is a bidirectional information interaction system between the central nervous system(CNS) and the gastrointestinal tract, in which gut microbiota plays a key role. The gut microbiota forms a complex network with the enteric nervous system, the autonomic nervous system, and the neuroendocrine and neuroimmunity of the CNS, which is called the microbiota-gut-brain axis. Due to the close anatomical and functional interaction of the gut-liver axis, the microbiota-gut-liver-brain axis has attracted increased attention in recent years. The microbiota-gut-liver-brain axis mediates the occurrence and development of many diseases, and it offers a direction for the research of disease treatment. 20Obesity and colorectal cancer risk: A meta-analysis of cohort studies显示文摘AIM: To evaluate the association between obesity and colorectal cancer risk. METHODS: We searched PubMed, EMBASE, and the Cochrane Library up to January 1, 2007. Cohort studies permitting the assessment of causal association between obesity and colorectal cancer, with clear definition of obesity and well-defined outcome of colorectal cancer were eligible. Study design, sample size at baseline, mean follow-up time, co-activators and study results were extracted. Pooled standardized effect sizes were calculated. RESULTS: The pooled relative risk (RR) of colorectal cancer was 1.37 (95% CI: 1.21-1.56) for overweight and obese men, 1.07 (95% CI: 0.97-1.18) for women measured by body mass index (BMI). The pooled RR for the highest vs the lowest quantiles of BMI was 1.59 (95% CI: 1.35-1.86) for men and 1.22 (95% CI: 1.08-1.39) for women at risk of colon cancer, 1.16 (95% CI: 0.93-1.46) for men and 1.23 (95% CI: 0.98-1.54) for women at risk of rectal cancer. The pooled RR for the highest vs the lowest quantiles of waist circumference was 1.68 (95% CI: 1.36-2.08) for men and 1.48 (95% CI: 1.19-1.84) for women at risk of colon cancer, 1.26 (95% CI: 0.90-1.77) for men and 1.23 (95% CI: 0.81-1.86) for women at risk of rectal cancer. The pooled RR for the highest quantiles vs the lowest quantiles of waist-to-hip ratio was 1.91 (95% CI: 1.46-2.49) for men and 1.49 (95% CI 1.23-1.81) for women at risk of colon cancer, 1.93 (95% CI: 1.19-3.13) for men and 1.20 (95% CI: 0.81-1.78) for women at risk of rectal cancer. Compared with 'normal range', the pooled RR for proximal colon cancer was 1.14 (95% CI : 0.88-1.47) for the overweight and 1.41 (95% CI: 0.66-3.01) for the obese. The pooled RR for the highest quantiles vs the lowest quantiles was 2.05 (95% CI: 1.23-3.41) with waist circumference, 1.66 (95% CI: 0.69-3.99) with waist-to-hip ratio. Compared with'normal range', the pooled RR for distal colon cancer was 1.38 (95% CI: 1.02-1.87) for the overweight and 1.23 (95% CI: 0.80-1.90) for the obese. The pooled RR for the highest quantiles vs the lowest quantiles was 1.86 (95% CI: 1.05-3.30) with waist circumference, and 1.79 (95% CI: 0.82-3.90) with waist-to-hip ratio. CONCLUSION: Obesity is a statistically significant risk factor for colorectal cancer and the relationship is more signifi cant in men than in women among different cancer subsites.
