1Usefulness of vonoprazan,a potassium ion-competitive acid blocker,for primary eradication of Helicobacter pylori显示文摘AIM To investigate usefulness of triple therapy with vonoprazan,a potassium ion-competitive acid blocker and antibiotics,for Helicobacter pylori(H.pylori) eradication.METHODS The H.pylori eradication rate was examined in 2507 patients(2055 undergoing primary eradication and 452 undergoing secondary eradication,excluding patients with subtotal gastrectomy) at the Japanese Red Cross Kyoto Daiichi Hospital from March 2013 to September 2015.For patients treated from March 2013 to February 2015,a proton pump inhibitor(PPI) was used to reduce acid secretion,while vonoprazan was used after March 2015.The success rates of the 2 regimens(PPI + amoxicillin + clarithromycin/metronidazole,or vonoprazan + amoxicillin + clarithromycin/metronidazole) were compared.RESULTS The success rate of primary H.pylori eradication was significantly higher in the vonoprazan group.When stratified by the underlying disease,a significant increase of the H.pylori eradication rate was observed in patients with chronic gastritis.A significantly lower H.pylori eradication rate was observed in younger patients compared to older patients in the PPI group,but there was no difference according to age in the vonoprazan group.On the otherhand,the success rate of secondary eradication was similar at approximately 90% in both groups.CONCLUSION Vonoprazan is very useful for primary eradication of H.pylori,and may become a first-line acid secretion inhibitor instead of PPIs.Shinya Yamada Takumi Kawakami Yoshikazu Nakatsugawa Takahiro Suzuki Hideki Fujii Naoya Tomatsuri Hideki Nakamura Hideki Sato Yusuke Okuyama Hiroyuki Kimura Norimasa Yoshida 2016World Journal of Gastrointestinal Pharmacology and Therapeutics2016,7,4:8
2Secondary-Atom-Doping Enables Robust Fe-N-C Single-Atom Catalysts with Enhanced Oxygen Reduction Reaction显示文摘Single-atom catalysts(SACs) with nitrogen-coordinated nonprecious metal sites have exhibited inimitable advantages in electrocatalysis.However,a large room for improving their activity and durability remains.Herein,we construct atomically dispersed Fe sites in N-doped carbon supports by secondary-atom-doped strategy.Upon the secondary doping,the density and coordination environment of active sites can be efficiently tuned,enabling the simultaneous improvement in the number and reactivity of the active site.Besides,structure optimizations in terms of the enlarged surface area and improved hydrophilicity can be achieved simultaneously.Due to the beneficial microstructure and abundant highly active FeN_5 moieties resulting from the secondary doping,the resultant catalyst exhibits an admirable half-wave potential of 0.81 V versus 0.83 V for Pt/C and much better stability than Pt/C in acidic media.This work would offer a general strategy for the design and preparation of highly active SACs for electrochemical energy devices.Xin Luo Xiaoqian Wei Hengjia Wang Wenling Gu Takuma Kaneko Yusuke Yoshida Xiao Zhao Chengzhou Zhu 2020Nano-Micro Letters2020,12,11:5
3Broccoli sprout extract induces detoxification-related gene expression and attenuates acute liver injury显示文摘AIM: To investigate the effects of broccoli sprout extract(BSEx) on liver gene expression and acute liver injury in the rat.METHODS: First, the effects of BSEx on liver gene expression were examined. Male rats were divided into two groups. The Control group was fed the AIN-76 diet, and the BSEx group was fed the AIN-76 diet containing BSEx. After a 10-d feeding period, rats were sacrificed and their livers were used for DNA microarray and realtime reverse transcription-polymerase chain reaction(RT-PCR) analyses. Next, the effects of BSEx on acute liver injury were examined. In experiments using acute liver injury models, 1000 mg/kg acetaminophen(APAP) or 350 mg/kg D-galactosamine(D-Gal N) was used to induce injury. These male rats were divided into four groups: Control, BSEx, Inducer(APAP or D-Gal N), and Inducer+BSEx. The feeding regimens were identical for the two analyses. Twenty-four hours following APAP administration via p.o. or D-Gal N administration via i.p., rats were sacrificed to determine serum aspartate transaminase(AST) and alanine transaminase(ALT) levels, hepatic glutathione(GSH) and thiobarbituric acid-reactive substances accumulation and glutathioneS-transferase(GST) activity. RESULTS: Microarray and real-time RT-PCR analyses revealed that BSEx upregulated the expression of genes related to detoxification and glutathione synthesis in normal rat liver. The levels of AST(70.91 ± 15.74 IU/m L vs 5614.41 ± 1997.83 IU/m L, P < 0.05) and ALT(11.78 ± 2.08 IU/m L vs 1297.71 ± 447.33 IU/m L, P < 0.05) were significantly suppressed in the APAP + BSEx group compared with the APAP group. The level of GSH(2.61 ± 0.75 nmol/g tissue vs 1.66 ± 0.59 nmol/g tissue, P < 0.05) and liver GST activity(93.19 ± 16.55 U/g tissue vs 51.90 ± 16.85 U/g tissue, P < 0.05) were significantly increased in the APAP + BSEx group compared with the APAP group. AST(4820.05 ± 3094.93 IU/m L vs 12465.63 ± 3223.97 IU/m L, P < 0.05) and ALT(1808.95 ± 1014.04 IU/m L vs 3936.46 ± 777.52 IU/m L, P < 0.05) levels were significantly suppressed in the D-Gal N + BSEx group compared with the D-Gal N group, but the levels of AST and ALT in the D-Gal N + BSEx group were higher than those in the APAP + BSEx group. 7Late-onset severe biliary bleeding after endoscopic pigtail plastic stent insertion显示文摘Here, we report our experience with a case of severe biliary bleeding due to a hepatic arterial pseudoaneurysm that had developed 1 year after endoscopic biliary plastic stent insertion. The patient, a 78-year-old woman, presented with hematemesis and obstructive jaundice. Ruptured hepatic arterial pseudoaneurysm was diagnosed, which was suspected to have been caused by long-term placement of an endoscopic retrograde biliary drainage(ERBD) stent. This episode of biliary bleeding was successfully treated by transarterial embolization(TAE). Pseudoaneurysm leading to hemobilia is a rare but potentially fatal complication in patients with long-term placement of ERBD.
