| 1 | Aberrant gene methylation in the peritoneal fluid is a risk factor predicting peritoneal recurrence in gastric cancer显示文摘AIM:To investigate whether gene methylation in the peritoneal fluid (PF) predicts peritoneal recurrence in gastric cancer patients.METHODS: The gene methylation of CHFR (checkpoint with forkhead and ring finger domains), p16, RUNX3 (runt-related transcription factor 3), E-cadherin, hMLH1 (mutL homolog 1), ABCG2 (ATP-binding cassette, sub-family G, member 2) and BNIP3 (BCL2/adenovirus E1B 19 kDa interacting protein 3) were analyzed in 80 specimens of PF by quantitative methylation-specific polymerase chain reaction (PCR). Eighty patients were divided into 3 groups; Group A (n=35):the depth of cancer invasion was less than the muscularis propria; Group B (n=31): the depth of cancer invasion was beyond the muscularis propria. Both group A and B were diagnosed as no cancer cells in peritoneal cytology and histology; Group C (n=14): disseminated nodule was histologically diagnosed or cancer cells were cytologically defi ned in the peritoneal cavity.RESULTS: The positive rates of methylation in CHFR, E-cadherin and BNIP3 were significantly different among the 3 groups and increased in order of group A, B and C (0%,0% and 21% in CHFR, P<0.05; 20%, 45% and 50% in E-cadherin, P<0.05;26%,35% and 71% in BNIP3, P<0.05). In addition, the multigene methylation rate among CHFR, E-cadherin and BNIP3 was correlated with group A, B and C (9%,19% and 57%, P<0.001). Moreover, the prognosis was analyzed in group B, excluding 3 patients who underwent a non-curative resection. | 2 | Epithelial Cell Apoptosis and Lung Remodeling显示文摘Lung epithelium is the primary site of lung damage in various lung diseases.Epithelial cell apoptosis has been considered to be initial event in various lung diseases.Apoptosis signaling is classically composed of two principle pathways.One is a direct pathway from death receptor ligation to caspase cascade activation and cell death.The other pathway triggered by stresses such as drugs,radiation,infectious agents and reactive oxygen species is mediated by mitochondria.Endoplasmic reticulum has also been shown to be the organelle to mediate apoptosis. Epithelial cell death is followed by remodeling processes,which consist of epithelial and fibroblast activation, cytokine production,activation of coagulation pathway,neoangiogenesis,re-epithelialization and fibrosis. Abdominal ultrasonography and CT scan confirmed the diagnosis of a gallstone, as well as,situs inversus; the liver and gallbladder on the left side and the spleen on the right. The biliary system was thought to be left-right reversal, mirror image in the view of drip infusion cholangiogram and MRI. | 5 | Prevention of aspiration of gastric contents during attempt in tracheal intubation in the semi-lateral and lateral positions显示文摘BACKGROUND:Pulmonary aspiration of gastric contents during tracheal intubation is a lifethreatening complication in emergency patients.Rapid sequence intubation is commonly performed to prevent aspiration but is not associated with low risk of intubation related complications.Although it has been considered that aspiration can be prevented in the lateral position,few studies have evaluated the ability to prevent aspiration.Moreover,this position is not always a favorable position for tracheal intubation.If aspiration can be prevented in a clinically relevant semi-lateral position,it may be advantageous.We assessed the ability to prevent aspiration in the lateral position and various degrees of the semi-lateral position using a vomiting-regurgitation manikin model.METHODS:A manikin's head was placed in the neutral,simple extension,or sniffing position.The amount of aspirated saline into the bronchi during simulated vomiting was measured at semilateral position angles of 0°to 90° in 10° increments.The difference in the vertical height between the mouth corner and the inferior border of the vocal cord was measured radiologically at each semilateral position in the three head-neck positions.RESULTS:Pulmonary aspiration was prevented at the ≥70°,≥80°,and 90° semi-lateral positions in the neutral,simple extension,and sniffing positions,respectively.The mouth was lower than the vocal cord in the semi-lateral position in which aspiration was prevented.CONCLUSION:The lateral or excessive semi-lateral position was necessary to protect the lung from aspiration in the head-neck positions commonly used for tracheal intubation.Prevention of aspiration was difficult within clinically relevant semi-lateral positions. | Ichiro Takenaka Kazuyoshi Aoyama | 2016 | World Journal of Emergency Medicine2016,7,4: | 7 |

| 11 | Nonlinear Vibration Induced by the Water-film Whirl and Whip in a Sliding Bearing Rotor System显示文摘Many industrial applications and experiments have shown that sliding bearings often experience fluid film whip due to nonlinear fluid film forces which can cause rotor-stator rub-impact failures. The oil-film whips have attracted many studies while the water-film whips in the water lubricated sliding bearing have been little researched with the mechanism still an open problem. The dynamic fluid film forces in a water sliding bearing are investigated numerically with rotational, whirling and squeezing motions of the journal using a nonlinear model to identify the relationships between the three motions. Rotor speed-up and slow-down experiments are then conducted with the rotor system supported by a water lubricated sliding bearing to induce the water-film whirl/whip and verify the relationship. The experimental results show that the vibrations of the journal alternated between increasing and decreasing rather than continuously increasing as the rotational speed increased to twice the first critical speed, which can be explained well by the nonlinear model. The radial growth rate of the whirl motion greatly affects the whirl frequency of the journal and is responsible for the frequency lock in the water-film whip. Further analysis shows that increasing the lubricating water flow rate changes the water-film whirl/whip characteristics, reduces the first critical speed, advances the time when significant water-film whirling motion occurs, and also increases the vibration amplitude at the bearing center which may lead to the rotor-stator rub-impact. | 12 | Rational arrangement of measuring shear wave speed in the liver显示文摘BACKGROUND Shear wave speed has been widely applied to quantify a degree of liver fibrosis. However, there is no standardized procedure, which makes it difficult to utilize the speed universally. AIM To provide procedural standardization of shear wave speed measurement. METHODS Point shear wave elastography (pSWE) was measured in 781 patients, and twodimensional shear wave elastography (2dSWE) was measured on the same day in 18 cases. Regions-of-interest were placed at 12 sites, and the median and robust coefficient-of-variation (CVR) were calculated. A residual sum-of-square (Σdi2) was computed for bootstrap values of 1000 iterations in 18 cases with each assumption of 1 to 12 measurements. The proportion of the Σdi2 (%Σdi2) was calculated as the ratio of Σdi2 to pSWE after converting it based on the correlation between pSWE and 2dSWE. RESULTS The CVR showed a significantly broader distribution in the left lobe (P < 0.0001),and the smallest CVR in the right anterior segment that covered 95% cases was 40.4%. pSWE was significantly higher in the left lobe than in the right lobe (1.63 ± 0.78 m/s vs 1.61 ± 0.78 m/s, P = 0.0004), and the difference between the lobes became further discrete when the subjects were limited to the cases with a CVR less than 40.4% in any segment (1.76 ± 0.80 m/s vs 1.70 ± 0.82 m/s, P < 0.0001). The highest values of the CVR in every 0.1 m/s interval were plotted in convex upward along pSWE and peaked at 1.93 m/s. pSWE and 2dSWE were significantly correlated (P < 0.0001, r = 0.95). In 216000 resamples from 18 cases, the %Σdi2 of 12 sites was 8.0% and gradually increased as the acquisition sites decreased to reach a significant difference with a %Σdi2 of 7 sites (P = 0.027). | 16 | Macrophage colony-stimulating factor expressed in non-cancer tissues provides predictive powers for recurrence in hepatocellular carcinoma显示文摘AIM To investigate the role of macrophage colony-stimulating factor(M-CSF) in patients with hepatocellular carcinoma(HCC) after surgery. METHODS Expression of M-CSF, distribution of M2 macrophages(Mφs), and angiogenesis were assessed in the liver, including tumors and peritumoral liver tissues. The prognostic power of these factors was assessed. Mouse isolated hepatic Mφs or monocytes were cultured with media containing M-CSF. The concentration of vascular endothelial growth factor(VEGF) in media was assessed. Furthermore, the role of the M-CSF-matured hepatic Mφs on proliferation of the vascular endothelial cell(VEC) was investigated. RESULTS A strong correlation between the expressions of M-CSF and CD163 was observed in the peritumoral area. Also, groups with high density of M-CSF, CD163 or CD31 showed a significantly shorter time to recurrence(TTR) than low density groups. Multivariate analysis revealedthe expression of M-CSF or hepatic M2Mφs in the peritumoral area as the most crucial factor responsible for shorter TTR. Moreover, the expression of M-CSF and hepatic M2Mφs in the peritumoral area had better predictable power of overall survival. Values of VEGF in culture media were significantly greater in the hepatic Mφs compared with the monocytes. | 17 | Helianthus tuberosus(Jerusalem artichoke) tubers improve glucose tolerance and hepatic lipid profile in rats fed a high-fat diet显示文摘Objective:To analyze the effects of feeding Helianthus tuberosus(HT) tubers on glucose tolerance and lipid profile in rats fed a high-fat diet(HFD). Methods:A normal HFD or HFD including 10 w/w% HT tubers(HFD + HT) was fed to F334/Jcl rats. After 10 weeks,organ weights,glucose tolerance,and lipid profile were analyzed. Results:The body weight,liver weight,and epidermal fat content in the HFD group were higher than those of the normal group,and similar to those of the HFD + HT group. The oral glucose tolerance test at 10 weeks revealed that the blood glucose level 30 minutes after beginning the test in the HFD + HT group was significantly lower than that in the HFD group. Liver triglyceride and total cholesterol levels in the HFD + HT group were significantly lower than those in the HFD group. Fecal triglyceride and total cholesterol levels in the HFD + HT group were higher than those in the HFD group. Histological analyses revealed that fat and glycogen accumulation increased in the HFD group,but decreased in the HFD + HT group.
