1Chinese Stroke Center Alliance:a national effort to improve healthcare quality for acute stroke and transient ischaemic attack:rationale,design and preliminary findings显示文摘Background In June 2015,the Chinese Stroke Association(CSA)initiated the Chinese Stroke Center Alliance(CSCA)to establish the national hospital-based stroke care quality assessment and improvement platform.This article outlines its objectives,operational structure,patient population,quality improvement(QI)intervention tools,data elements,data collection methodology and current patient and hospital data.Methods The CSCA is a national,hospital-based,multicentre,voluntary,multifaceted intervention and continuous QI initiative.This multifaceted intervention includes stroke centre development,written care protocols,workshops and a monitoring/feedback system of evidencebased performance measures.The data coordinating centre of the CSCA resides at the China National Clinical Research Center for Neurological Diseases,Beijing Tiantan Hospital.results As of July 2017,1576 hospitals in China have contributed detailed clinical information to serve as a benchmark for the stroke care quality of 433264 patients with acute stroke/transient ischaemic attacks(TIA),including 352572(81.38%)acute ischaemic stroke,30362(7.01%)TIA,42080(9.71%)spontaneous intracranial haemorrhage,5505(1.27%)subarachnoid haemorrhage and 2745(0.63%)not specified stroke.Conclusion The CSCA programme is designed to establish a continuous national stroke registry and help healthcare providers develop stroke centres and treat patients in a consistent manner in accordance with accepted national guidelines and,ultimately,improve patient outcomes.It supports the CSA mission to reduce stroke burden in China.Yongjun Wang Zixiao Li Yilong Wang Xingquan Zhao Liping Liu Xin Yang Caiyun Wang Hongqiu Gu Fuying Zhang Chunjuan Wang Ying Xian David Z Wang Qiang Dong Anding Xu Jizong Zhao Chinese Stroke Center Alliance investigators 2018Stroke & Vascular Neurology2018,3,4:38
2Necessity and indications of invasive treatment for Budd-Chiari syndrome显示文摘BACKGROUND:The development of collaterals in Budd-Chiari syndrome has been described and these collaterals play an important role in the presentation of this disease.These collaterals are diagnostic and their use in management strategy has never been evaluated.This study aimed to investigate the indications,feasibility and necessity of invasive treatment for patients with Budd-Chiari syndrome and to determine whether such a strategy is necessary for optimal management.METHODS:Twenty-nine patients who had been treated at our unit were enrolled in this study.Based on physical and biochemical examination,and hemodynamic compensation by collaterals,18 patients underwent radiological intervention (group A),while the other 11 had no invasive treatment (group B).The related hemodynamic parameters were acquired when percutaneous angiography was performed.RESULTS:In group A,all patients underwent successfully inferior vena cava (IVC) balloon angioplasty with or without stenting.Four patients also underwent hepatic vein angioplasty.In these patients,the mean IVC pressure before and after treatment was statistically different (29.3±9.2 vs 15.1±4.6 mmHg,P<0.01).The mean IVC pressure was much lower in group B than in group A (12.9±2.4 vs 29.3±9.2 mmHg,P<0.01),but there was no difference from that of the patients after radiological treatment (12.9±2.4 vs 15.1±4.6 mmHg,P>0.05).Median follow-up was 32.3 months (mean 21.3 months;range 3-61 months).In the course of follow-up,the patients in group A survived with good systemic status except for re-stenosis in one patient who underwent re-canalization of the IVC.In group B,10 patients had good systemic status except one patient who had a meso-caval shunt because of deterioration.CONCLUSIONS:The rationale of 'early diagnosis and early treatment' is not suitable for all patients with Budd-Chiari syndrome.Satisfactory survival can be achieved in some patients without invasive treatment,who are completely compensated by rich collaterals.Nonetheless,a positive treatment procedure should be performed if the patient's situation worsens in the course of regular follow-up.Department of General Surgery (Fu Y,Sun YL,Ma XX,Xu PQ,Feng LS,Tang Z and Luo CH),Institute of Hepatic Vascular Disease (Sun YL),Department of Radiological Intervention (Guan S and Wang ZW),First Affiliated Hospital,Zhengzhou University School of Medicine,Zhengzhou 450052,China 2011Hepatobiliary & Pancreatic Diseases International2011,10,3:10
3Magnetic resonance evaluation of transplanted mesenchymal stem cells after myocardial infarction in swine显示文摘Objectives To trace and evaluate intracoronary transplanted mesenchymal stem cells(MSCs) labeled with superparamagnetic iron oxide(SPIO) by using magnetic resonance imaging(MRI) in a swine model of myocardial infarction (MI).Methods MSCs were transfected with a lentiviral vector carrying the gene encoding green fluorescent protein (GFP) and labeled in vitro with SPIO.Two weeks after MI, swine were randomized to intracoronary transplantation of dual -labeled MSCs(n = 10),MSCs-GFP(n = 10) and saline(n = 5).MRI examination was performed with a 1.5T clinical scanner at 24 hours,3 weeks and 8 weeks after cells transplantation. Signal intensity(SI) changes,cardiac function and MI size were measured using MRI.Correlation between MR findings and histomorphologic findings was also investigated. Results The labeling efficiency at a combination of 25μg Fe/ml SPIO and 0.8 pi/ml Lipofectamine 2000 reached 100%.SPIO labeling did not affect GFP fluorescence and dual-labeling did not affect cell proliferation(P>0.05). Multipotentiality was not affected especially for cardiomyocyte-like cells differentiation.Cardiac cell marker of a-MHC and actinin were positively expressed by immunofluorescence staining after induction.SI on T2 * WI decreased substantial- ly in the interventricular septum 24 hours after injection of MSCs.The intensity of hypo-intense signals appeared to increase throughout the later time points.Changes in SI at 24 hours,3 weeks and 8 weeks were 52.98%±10.74%,21.53%±5.40%and 6.23%±2.01%,respectively(P<0.01).DE-MRI demonstrated both dual-labeled MSCSs and MSCs-GFP could dramatically reduce the size of MI and improve cardiac function. Histological data revealed that prussian blue stain-positive cells were found mainly in the border zone which also showed green fluorescence but negative for macrophage marker(CD68).Gross pathologic examination revealed that engrafted MSCs dramatically reduce the extent of necrotic myocardium and promote the regeneration of new,contractile myocardium along the subendocardial surface of the MI. 4Novel Manufacturing Technology of Hyperbolic Plates for Shipbuilding显示文摘On the basis of the principle of dieless hydrobulging technology, a novel hydrobulging technology for manufacturinghyperbolic plates is proposed. First, a toroidal pipe elbow or a partial toroidal pipe elbow is formed, then the single hydrobulgedstructure can be cut up into some desired plates. 5Nitrite-derived nitric oxide by xanthine oxidoreductase protects the liver against ischemia- reperfusion injury显示文摘It was demonstrated that xanthine oxidoreductase (XOR), during ischemia, catalyzes the formation of nitric oxide (NO) from nitrite (NO2-) and this NO2--derived NO protects the isolated perfused rat heart against the damaging effects of ischemia-reperfusion (I/R) when conventional nitric oxide synthase (NOS) -dependent NO production is impaired. Liver is one of the organs with the highest XOR concentration. This study was designed to determine whether NO2--derived NO by XOR protects liver against I/R injury in vivo. For its minute amounts and active reactivity, NO can not be detected directly in real time in vivo by this time. We have to prove the above hypothesis indirectly. METHODS:Wistar rats were pretreated with saline, NOS inhibitor L-NAME (10 mg/kg intravenously), XOR inhibitor allopurinol (1.5 mg/kg orally), L-NAME +allopurinol and NO scavenger carboxy-PTIO (0.6 mg/kg intravenously) respectively (12 animals per group). And then, they were subjected to total liver ischemia for 40 minutes followed by reperfusion. Blood samples and liver tissues were obtained for analysis after 3 hours of reperfusion. Survival was also investigated. RESULTS:Allopurinol-treated animals exhibited further increased serum alanine aminotransferase (ALT) levels and liver myeloperoxidase (MPO) activities, but further decreased liver adenosine triphosphate (ATP) stores after I/R compared to saline-treated counterparts (830.5±108.3 U/L, 56.5±11.0 U/mg protein and 1.93±0.47 μmol/g vs. 505.8± 184.2 U/L, 41.5±10.2 U/mg protein and 3.05±0.55 μmol/g respectively, P<0.01, P<0.05 and P<0.01 respectively). The hepatocyte injury was further exacerbated and the overall survival rate was significantly decreased after I/R in animals given by allopurinol compared to those pretreated by saline (P<0.05). L-NAME and allopurinol co-treated animals exhibited more severe liver injury (P<0.05 and P<0.01) and a further decreased overall survival rate (P<0.05) compared to L-NAME or allopurinol alone-treated counterparts, but they were not different from carboxy-PTIO treated animals (P>0.05). 7DEBRIS FLOW CONTROL BY USING SLIT DAMS显示文摘Slit dams have been shown to be effective in trapping coarse sediment and retarding the debris flow. It is necessary to establish the controlling efficiency and the systematic procedures for designing slit dams, as well as reasonable sites for their location. Using the principle of mass conservation and laboratory experiments, this paper develops the sediment flow-out ratio and sediment concentration ratio to evaluate the effectiveness of the slit dam, and confirms that sediment concentration ratio increases with an increase in relative spacing and slit density. The sediment flow-out ratio depends on the sediment concentration ratio and sediment concentration before passing through a slit dam. For sediment concentration before passing through a slit dam, based upon Bagnold’s concept of the dispersive stress generated by grain collisions in flow and bed shear stress, a semi-empirical relationship is obtained. Compared with other researchers’ studies, this study is found to be in excellent conformity with theirs, when taking 0.1 as the empirical coefficient. Besides, height, spacing and location factors affecting a slit dam design for debris flow mitigation are established herein.
