| 4 | Antiviral treatment for chronic hepatitis B: Safety, effectiveness, and prognosis显示文摘The goal of chronic hepatitis B (CHB) therapy is to improve the patient prognosis through the sustained inhibition of viral replication. However, due to the uncertainty and potentially unlimited duration of the treatment course, nucleus(t)ide analogue (NA) resistance and safety, financial costs and patient compliance, different endpoints of antiviral treatment have been proposed in CHB prevention and treatment guidelines. Different treatment endpoints are closely associated with the safety of drug withdrawal and improvements in prognosis. Antiviral treatment suppresses HBV DNA replication, drug withdrawal leads to relapse, and long-term treatment causes drug safety and resistance issues. Although hepatitis B e antigen seroconversion based on HBV DNA inhibition is considered as “a satisfactory endpoint”, drug withdrawal still leads to high relapse rates. Hepatitis B surface antigen (HBsAg) clearance is the “ideal endpoint” in terms of the safety of drug withdrawal and improvements in prognosis. However, the HBsAg clearance rate is low using the conventional single drug treatment and fixed course regimens. Recently, the application of an “optimized antiviral treatment strategy” has improved the HBsAg clearance rate, and make an “ideal endpoint” possible. At the first clustering stage, the weighted K-means is performed to produce clusters with homogeneous data. At the second predicting stage, each fuzzy neural network is carried out on each cluster and the results from all fuzzy neural networks are combined to be the final result of the hybrid method. | 7 | Risk assessment model for endpoints of ischemic stroke: A study protocol for a registry study显示文摘OBJECTIVE: To evaluate the risk of ischemic stroke endpoints by establishing risk assessment models that combine Traditional Chinese Medicine(TCM)and modern medicine indicators.METHODS/DESIGN: The proposed study is a registry-based participant survey conducted in seven hospitals nationwide in China. After obtaining informed consent, 3000 patients diagnosed with ischemic stroke will be recruited. One-year follow-ups will be performed on-site in hospitals and by telephone to track endpoint events. Comparative analysis of the prevalence of endpoint events and other TCM or modern medicine features in different groups will be conducted using frequency analysis and χ2tests, and the results will be expressed as composition ratios. Comparative analysis of quantitative scores and related patterns or symptoms will be conducted using a rank-sum test. Correlation analysis of endpoint events and TCM or modern medicine factors will be performed using a multivariate Cox proportional hazard model.DISCUSSION: Previous reports have described modern medicine indicator-based risk assessment models for ischemic stroke endpoint events, but no such studies have included TCM features. | 8 | Improved Method and Application of EMD Endpoint Continuation Processing for Blasting Vibration Signals显示文摘In order to deal with the non-stationary characteristics of blasting vibration signals and the end issue in the empirical mode decomposition(EMD), an improved endpoint continuation method is proposed. First, the linear continuation method of extreme points is used to determine the extremum of the signal endpoint fast. Secondly, the extreme points of transition section outside the signal ends are obtained by a mirror continuation method of extreme points, and then the envelope and continuation curve of the transition section of the signal are constructed. Lastly, the sinusoid of the stationary section outside the signal is constructed to achieve the continuation curve from the transition section to the stationary section. Based on the 'singular extreme points' phenomenon of blasting vibration signal, the negative maxima and positive minimum are eliminated, then the maximum and minimum are guaranteed to appear at intervals. Thus,the number of iterations is reduced and the instability of EMD decomposition is improved. The calculation formula of amplitude, cycle and initial phase are given for the transition section and stationary section outside the signal. | 11 | Opportunities and challenges of clinical trials in cardiology using composite primary endpoints显示文摘In clinical trials, the primary efficacy endpoint often corresponds to a so-called 'composite endpoint'. Composite endpoints combine several events of interest within a single outcome variable. Thereby it is intended to enlarge the expected effect size and thereby increase the power of the study. However, composite endpoints also come along with serious challenges and problems. On the one hand, composite endpoints may lead to difficulties during the planning phase of a trial with respect to the sample size calculation, asthe expected clinical effect of an intervention on the composite endpoint depends on the effects on its single components and their correlations. This may lead to wrong assumptions on the sample size needed. Too optimistic assumptions on the expected effect may lead to an underpowered of the trial, whereas a too conservatively estimated effect results in an unnecessarily high sample size. On the other hand, the interpretation of composite endpoints may be difficult, as the observed effect of the composite does not necessarily reflect the effects of the single components. Therefore the demonstration of the clinical efficacy of a new intervention by exclusively evaluating the composite endpoint may be misleading. In addition to being the most sensitive to killing, cells in this age-interval were also shown to be the most sensitive to radiation-induced neoplastic transformation. In this work. C3H mouse cells, designated 10T1/2, were irradiated with fission-spectrum neutrons and assayed in vitro via the endpoint focus formation on a monolayer of normal cells. From these observations, a biophysical model was developed to explain the anomalous finding that the frequency of transformation by low doses was enhanced when the exposure was protracted. In contrast to transformation by X- and y-rays where repair and kinetics during exposure play dominant roles, with radiations like reactor neutrons and x-particles repair has a minimal effect; only cell kinetics acts significantly to modify transformation due to protracted doses.
