8生命早期室外空气污染暴露与婴幼儿呼吸系统健康、耳部感染和湿疹:INMA研究显示文摘[背景]产前和生命早期可能是空气污染对婴幼儿健康造成有害影响的重要窗口期。[目的]选择西班牙出生队列中2199名婴儿,研究他们在孕期和出生第1年空气污染暴露与年龄为12~18个月时的呼吸系统疾病、耳部感染和湿疹之间的关联。[方法]父母提供医生诊断的呼吸道感染(LRTI)、喘息、湿疹和耳部感染等信息。使用经时间调整的土地利用回归模型,估计个体二氧化氮(NO2)和苯的暴露情况。采用对数二项式回归模型和联合随机效应meta分析,评估4个研究地点空气污染暴露对健康结果的影响。[结果]在孕期,NO2均值每增加10μg/m3与LRT[I相对危险度(RR)=1.05,95%CI:0.98~1.12]和耳部感染(RR=1.18,95%CI:0.98~1.41)相关联。NO2每增加1个四分位距(IQR),LRTI的RR为1.08(95%CI:0.97~1.21),耳部感染的RR为1.31(95%CI:0.97~1.76)。与NO2相比,苯暴露均值每增加1个IQR,其与LRTI具有类似的关联性(RR=1.06,95%CI:0.94~1.19),与耳部感染的关联性稍弱(RR=1.17,95%CI:0.93~1.46)。母亲孕期大多数时间在家里的婴幼儿,其关联性稍强。由于出生第1年的空气污染暴露与产前暴露高度相关,所以无法分辨各个暴露时期的相对重要性。[结论]研究结果表明生命早期环境空气污染暴露可能增加婴幼儿上、下呼吸道感染的风险。Inmaculada Aguilera Marie Pedersen Raquel Garcia-Esteban Ferran Ballester Mikel Basterrechea Ana Esplugues Ana Fernández-Somoano Aitana Lertxundi Adonina Tardón Jordi Sunyer 2013环境与职业医学2013,30,6:0
