| 2 | Comparison of imaging-based and pathological dimensions in pancreatic neuroendocrine tumors显示文摘AIM To establish the ability of magnetic resonance(MR) and computer tomography(CT) to predict pathologic dimensions of pancreatic neuroendocrine tumors(Pan NET) in a caseload of a tertiary referral center.METHODS Patients submitted to surgery for Pan NET at the Surgical Unit of the Pancreas Institute with at least 1 preoperative imaging examination(MR or CT scan) from January 2005 to December 2015 were included and data retrospectively collected. Exclusion criteria were: multifocal lesions, genetic syndromes, microadenomas or mixed tumors, metastatic disease and neoadjuvant therapy. Bland-Altman(BA) and Mountain-Plot(MP) statistics were used to compare size measured by each modality with the pathology size. Passing-Bablok(PB) regression analysis was used to check the agreement between MR and CT.RESULTS Our study population consisted of 292 patients. Seventy-nine(27.1%) were functioning Pan NET. The mean biases were 0.17 ± 7.99 mm, 1 ± 8.51 mm and 0.23 ± 9 mm, 1.2 ± 9.8 mm for MR and CT, considering the overall population and the subgroup of non-functioning-Pan NET, respectively. Limits of agreement(LOA) included the vast majority of observations, indicating a good agreement between imaging and pathology. The MP further confirmed this finding and showed that the two methods are unbiased with respect to each other. Considering ≤ 2 cm non-functioning-Pan NET, no statistical significance was found in the size estimation rate of MR and CT(P = 0.433). | 3 | Serum chromogranin-A in hepatocellular carcinoma: Diagnostic utility and limits显示文摘AIM: The utility of serum alpha-fetoprotein (α-FP) for the detection of hepatocellular carcinoma (HCC) is questionable.High serum levels of chromogranin-A (CgA) have recently been reported in HCC. Impaired hepatic, renal, and heart functions influence circulating CgA. The aim of this study was to assess sensitivity and specificity of serum CgA as a marker of HCC in patients with liver cirrhosis (LC).METHODS: Serum CgA levels were measured by RIA in 339 patients of which 54 HCC, 132 LC, 45 chronic hepatitis (CH), 27 chronic heart failure (CHF), 36 chronic renal failure (CRF), 45 chronic inflammatory bowel disease (IBD)as disease controls and in 75 healthy controls. Patients with liver disease or IBD and concomitant renal and/or heart failure were excluded. Pearson correlation, nonparametric combination test and confidence interval analysis were used for statistical analysis.RESULTS: Serum CgA above normal values (100 ng/mL)were found in 83% of HCC patients, in 48% of LC patients,in 20% of CH patients, in 33% of IBD patients, in 92% of CRF patients, in 100% of CHF patients, and in none of the healthy controls. The mean CgA values in HCC (769±1 046), in LC (249±369), in CH (87±94), in CRF (1 390±1 401), in CHF (577±539), in IBD (146±287) were significantly higher than those in healthy controls (48±18).HCC patients had higher CgA values (P<0.01) than LC,CH, and IBD patients but did not differ from those with CRF or CHF. The 95% CI for the mean (250-1 289 ng/mL)in HCC patients was selected as a CgA range and the lower value of such range was assumed as cut-off.Sensitivity and specificity of CgA, calculated in relation to the cut-off in patients with cirrhosis and HCC, were respectively 61% (CI 48-73%) and 82% (CI 75-88%).Serum α-FP values were >200 ng/mL in 21% of the HCC patients and in none of the LC patients. | 4 | Replication of interleukin 23 receptor and autophagy-related 16-like 1 association in adult-and pediatric-onset inflammatory bowel disease in Italy显示文摘AIM: To investigate gene variants in a large Italian inflammatory bowel disease (IBD) cohort, and to analyze the correlation of sub-phenotypes (including age at diagnosis) and epistatic interaction with other IBD genes. METHODS: Total of 763 patients with Crohn's disease (CD, 189 diagnosed at age < 19 years), 843 with ulcerative colitis (UC, 179 diagnosed <19 years), 749 healthy controls, and 546 healthy parents (273 trios) were included in the study. The rs2241880 [autophagy-related 16-like 1 (ATG16L1)], rs11209026 and rs7517847 [interleukin 23 receptor (IL23R)], rs2066844, rs2066845, rs2066847 (CARD15), rs1050152 (OCTN1), and rs2631367 (OCTN2) gene variants were genotyped. RESULTS: The frequency of G allele of ATG16L1 SNP (Ala197Thr) was increased in patients with CD compared with controls (59% vs 54% respectively) (OR = 1.25, CI = 1.08-1.45, P = 0.003), but not in UC (55%). The frequency of A and G (minor) alleles of Arg381Gln, rs11209026 and rs7517847 variants of IL23R were reduced significantly in CD (4%, OR = 0.62, CI = 0.45-0.87, P = 0.005; 28%, OR = 0.64, CI = 0.55-0.75, P < 0.01), compared with controls (6% and 38%, respectively). The A allele (but not G) was also reduced signifi cantly in UC (4%, OR = 0.69, CI = 0.5-0.94, P = 0.019). No association was demonstrated with sub-phenotypes and interaction with CARD15 , and OCTN1/2 genes, although both gene variants were associated with pediatric-onset disease. | 5 | Transcriptome reprogramming,epigenetic modifications and alternative splicing orchestrate the tomato root response to the beneficial fungus Trichoderma harzianum显示文摘Beneficial interactions of rhizosphere microorganisms are widely exploited for plant biofertilization and mitigation of biotic and abiotic constraints.To provide new insights into the onset of the roots–beneficial microorganisms interplay,we characterised the transcriptomes expressed in tomato roots at 24,48 and 72 h post inoculation with the beneficial fungus Trichoderma harzianum T22 and analysed the epigenetic and post-trascriptional regulation mechanisms.We detected 1243 tomato transcripts that were differentially expressed between Trichoderma-interacting and control roots and 83 T.harzianum transcripts that were differentially expressed between the three experimental time points.Interaction with Trichoderma triggered a transcriptional response mainly ascribable to signal recognition and transduction,stress response,transcriptional regulation and transport.In tomato roots,salicylic acid,and not jasmonate,appears to have a prominent role in orchestrating the interplay with this beneficial strain.Differential regulation of many nutrient transporter genes indicated a strong effect on plant nutrition processes,which,together with the possible modifications in root architecture triggered by ethylene/indole-3-acetic acid signalling at 72 h post inoculation may concur to the well-described growth-promotion ability of this strain.Alongside,T.harzianum-induced defence priming and stress tolerance may be mediated by the induction of reactive oxygen species,detoxification and defence genes.A deeper insight into gene expression and regulation control provided first evidences for the involvement of cytosine methylation and alternative splicing mechanisms in the plant–Trichoderma interaction.A model is proposed that integrates the plant transcriptomic responses in the roots,where interaction between the plant and beneficial rhizosphere microorganisms occurs. | Monica De Palma Maria Salzano Clizia Villano Riccardo Aversano Matteo Lorito Michelina Ruocco Teresa Docimo Anna Lisa Piccinelli Nunzio D’Agostino Marina Tucci | 2019 | Horticulture Research2019,6,1: | 2 |

| 18 | Clinical significance of glycemic parameters on venous thromboembolism risk prediction in gastrointestinal cancer显示文摘AIM To investigate the possible predictive role of routinely used glycemic parameters for a first venous thromboembolism(VTE) episode in gastrointestinal(GI) cancer ambulatory patients-with or without clinically diagnosed type 2 diabetes(T2D) or obesity-treated with chemotherapy.METHODS Pre-treatment fasting blood glucose,insulin,glycated hemoglobin(Hb A1c) and homeostasis model of risk assessment(HOMA) were retrospectively evaluated in a cohort study of 342 GI cancer patients. Surgery was performed in 142(42%) patients with primary cancer,30(21%) and 112(79%) of whom received neoadjuvant and adjuvant therapies,respectively. Firstline chemotherapy was administered in 200(58%) patients with metastatic disease. The study outcome was defined as the occurrence of a first symptomatic or asymptomatic VTE episode during active treatment.RESULTS Impaired glucose tolerance(IGT) or T2 D were diagnosed in 30% of GI cancer patients,while overweight/obesity had an incidence of 41%. VTE occurred in 9.4% of patients(7% of non-diabetic non-obese),especially in those with a high ECOG score(P = 0.025). No significant association was found between VTE incidence and T2 D,obesity,different tumor types,metastatic disease,Khorana class of risk,or different anti-cancer drugs,although VTE rates were substantially higher in patients receiving bevacizumab(17% vs 8%,P = 0.044). Conversely,all glucose metabolic indexes were associated with increased VTE risk at ROC analysis. Multivariate Cox proportional analyses confirmed that HOMA index(HR = 4.13,95%CI: 1.63-10.5) or fasting blood glucose(HR = 3.56,95%CI: 1.51-8.39) were independent predictors of VTE occurrence during chemotherapy.CONCLUSION The results here reported demonstrate that evaluating glucose metabolic asset may allow for VTE risk stratification in GI cancer,helping to identify chemotherapy-treated patients who might benefit from thromboprophylaxis.
