| 1 | Inflammatory bowel disease imaging:current practice andfuture directions显示文摘The purpose of this paper is to evaluate the role of imaging in inflammatory bowel disease(IBD), including detection of extraluminal complications and extraintestinal manifestations of IBD, assessment of disease activity and treatment response, and discrimination of inflammatory from fibrotic strictures. IBD is a chronic idiopathic disease affecting the gastrointestinal tract that is comprised of two separate, but related intestinal disorders; Crohn's disease and ulcerative colitis. The paper discusses, in detail the pros and cons of the different IBD imaging modalities that need to be considered in order to optimize the imaging and clinical evaluation of patients with IBD. Historically, IBD evaluation of the bowel has included imaging to assess the portions of the small bowel that are inaccessible to optical endoscopic visualization. This traditionally was performed using barium fluoroscopic techniques; however, cross-sectional imaging techniques(computed tomography and magnetic resonance imaging) are being increasingly utilized for IBD evaluation because they can simultaneously assess mural and extramural IBD manifestations. Recent advances in imaging technology, that continue to improve the ability of imaging to noninvasively follow disease activity and treatment response, are also discussed. | 4 | Controversies in the pathological assessment of colorectal cancer显示文摘Pathologic assessment of colorectal cancer specimens plays an essential role in patient management,informing prognosis and contributing to therapeutic decision making.The tumor-node-metastasis(TNM)staging system is a key component of the colorectal cancer pathology report and provides important prognostic information.However there is significant variation in outcome of patients within the same tumor stage.Many other histological features such as tumor budding,vascular invasion,perineural invasion,tumor grade and rectal tumor regression grade that may be of prognostic value are not part of TNM staging.Assessment of extramural tumor deposits and peritoneal involvement contributes to TNM staging but there are some difficulties with the definition of both of these features.Controversies in colorectal cancer pathology reporting include the subjective nature of some of the elements assessed,poor reporting rates and reproducibility and the need for standardized examination protocols and reporting.Molecular pathology is becoming increasingly important in prognostication and prediction of response to targeted therapies but accurate morphology still has a key role to play in colorectal cancer pathology reporting. | Aoife Maguire Kieran Sheahan | 2014 | World Journal of Gastroenterology2014,20,29: | 5 |
| 5 | Quantitative study of prostate cancer using three dimensional fiber tractography显示文摘AIM:To investigate feasibility of a quantitative study of prostate cancer using three dimensional(3D)fiber tractography.METHODS:In this institutional review board approved retrospective study,24 men with biopsy proven prostate cancer underwent prostate magnetic resonance imaging(MRI)with an endorectal coil on a 1.5 T MRI scanner.Single shot echo-planar diffusion weighted images were acquired with b = 0.600 s/mm^2,six gradient directions.Open-source available software Track Vis and its Diffusion Toolkit were used to generate diffusion tensor imaging(DTI)map and 3D fiber tracts.Multiple 3D spherical regions of interest were drawn over the areas of tumor and healthy prostatic parenchyma to measure tract density,apparent diffusion coefficient(ADC)and fractional anisotropy(FA),which were statistically analyzed.RESULTS:DTI tractography showed rich fiber tract anatomy with tract heterogeneity.Mean tumor region and normal parenchymal tract densities were 2.53 and 3.37 respectively(P < 0.001).In the tumor,mean ADC was 0.0011 × 10^(-3 )mm^2/s vs 0.0014 × 10^(-3) mm^2/s in the normal parenchyma(P < 0.001).The FA values for tumor and normal parenchyma were 0.2047 and 0.2259 respectively(P = 0.3819).CONCLUSION:DTI tractography of the prostate is feasible and depicts congregate fibers within the gland.Tract density may offer new biomarker to distinguish tumor from normal tissue. | Sandeep Hedgire Alexey Tonyushkin Aoife Kilcoyne Jason A Efstathiou Peter F Hahn Mukesh Harisinghani | 2016 | World Journal of Radiology2016,8,4: | 4 |
| 6 | Gut-associated lymphoid tissue or so-called “dome” carcinoma of the colon: Review显示文摘AIM To present a comprehensive review of the etiology, clinical features, macroscopic and pathological findings, and clinical significance of Gut-associated lymphoid tissue or 'dome' carcinoma of the colon.METHODS The English language medical literature on gut-or gastrointestinal-associated lymphoid tissue(GALT) or 'dome' carcinoma of the colon was searched and appraised.RESULTS GALT/dome-type carcinomas of the colon are thought to arise from the M-cells of the lymphoglandular complex of the intestine. They are typically asymptomatic and have a characteristic endoscopic plaque-or 'dome'-like appearance. Although the histology of GALT/dome-type carcinomas displays some variability, they are characterized by submucosal localization, a prominent lymphoid infiltrate with germinal center formation, tumor-infiltrating lymphocytes, absence of desmoplasia, and dilated glands lined by columnar epithelial cells with bland nuclear features and cytoplasmic eosinophilia.
