1Practice guidelines for the pathological diagnosis of primary liver cancer: 2015 update显示文摘In 2010, a panel of Chinese pathologists reported the first expert consensus for the pathological diagnosis of primary liver cancers to address the many contradictions and inconsistencies in the pathological characteristics and diagnostic criteria for PLC. Since then considerable clinicopathological studies have been conducted globally, prompting us to update the practice guidelines for the pathological diagnosis of PLC. In April 18, 2014, a Guideline Committee consisting of 40 specialists from seven Chinese Societies(including Chinese Society of Liver Cancer, Chinese Anti-Cancer Association; Liver Cancer Study Group, Chinese Society of Hepatology, Chinese Medical Association; Chinese Society of Pathology, Chinese Anti-Cancer Association; Digestive Disease Group, Chinese Society of Pathology, Chinese Medical Association; Chinese Society of Surgery, Chinese Medical Association; Chinese Society of Clinical Oncology, Chinese Anti-Cancer Association; Pathological Group of Hepatobiliary Tumor and Liver Transplantation, Chinese Society of Pathology, Chinese Medical Association) was created for the formulation of the first guidelines for the standardization of the pathological diagnosis of PLC, mainly focusing on the following topics: gross specimen sampling, concepts and diagnostic criteria of small hepatocellular carcinoma(SHCC), microvascular invasion(MVI), satellite nodules,and immunohistochemical and molecular diagnosis. The present updated guidelines are reflective of current clinicopathological studies, and include a novel 7-point baseline sampling protocol, which stipulate that at least four tissue specimens should be sampled at the junction of the tumor and adjacent liver tissues in a 1:1 ratio at the 12, 3, 6 and 9 o'clock reference positions. For the purposes of molecular pathological examination, at least one specimen should be sampled at the intratumoral zone, but more specimens should be sampled for tumors harboring different textures or colors. Specimens should be sampled at both adjacent and distant peritumoral liver tissues or the tumor margin in order to observe MVI, satellite nodules and dysplastic foci/nodules distributed throughout the background liver tissues. Complete sampling of whole SHCC ≤ 3 cm should be performed to assess its biological behavior, and in clinical practice, therapeutic borders should be also preserved, even in SHCC. The diagnostic criteria of MVI and satellite nodules, immunohistochemical panels, as well as molecular diagnostic principles, such as clonal typing, for recurrent HCC and multinodule HCC were also proposed and recommended. The standardized process of pathological examination is aimed at ensuring the accuracy of pathological PLC diagnoses as well as providing a valuable frame of reference for the clinical assessment of tumor invasive potential, the risk of postoperative recurrence, long-term survival, and the development of individualized treatment regimens. 4Combinations of Hd2 and Hd4 genes determine rice adaptability to Heilongjiang Province, northern limit of China显示文摘Heading date is a key trait in rice domestication and adaption, and a number of quantitative trait loci(QTLs)have been identified. The rice(Oryza sativa L.) cultivars in the Heilongjiang Province, the northernmost region of China,have to flower extremely early to fulfill their life cycle.However, the critical genes or different gene combinations controlling early flowering in this region have not been determined. QTL and candidate gene analysis revealed that Hd2/Ghd7.1/Os PRR37 plays a major role in controlling rice distribution in Heilongjiang. Further association analysis with a collection of rice cultivars demonstrated that another three major QTL genes(Hd4/Ghd7, Hd5/DTH8/Ghd8, and Hd1)also participate in regulating heading date under natural long day(LD) conditions. Hd2/Ghd7.1/Os PRR37 and Hd4/Ghd7 are two major QTLs and function additively. With the northward rice cultivation, the Hd2/Ghd7.1/Os PRR37 and Hd4/Ghd7 haplotypes became non-functional alleles. Hd1 might be non-functional in most Heilongjiang rice varieties,implying that recessive hd1 were selected during local rice breeding. Non-functional Hd5/DTH8/Ghd8 is very rare, but constitutes a potential target for breeding extremely early flowering cultivars. 8Comparison of the autofluorescence bronchoscope and the white light bronchoscope in airway examination显示文摘Background and Objective: The sensitivity and accuracy of white light bronchoscopy (WLB) in airway examination are low. Autofluorescence bronchoscope (AFB) can determine early lesions in bronchial mucosa more sensitively, but it has seldom performed in China. To assess the clinical value of the AFB in airway examination, we compared the sensitivity and specificity of the AFB and WLB in detecting cancer of the airway mucosa. Methods: Between September 2009 and May 2010, bronchoscope examinations using both the AFB and WLB were performed on 136 patients, 95 men and 41 women with a median age of 61.5 years (ranged from 25 to 84 years). There were 46 lesions located in the central airway, 84 in the peripheral lung parenchyma, and 6 in the mediastinal region. All patients received local and general anesthesia and were subsequently examined with the WLB and AFB in tandem. All procedures were completed safely. Abnormal visual findings were recorded, and biopsies of the affected regions were collected for pathologic examination. Results: Of 241 regions sampled for biopsy, 76 sites contained malignant lesions, whereas 165 sites contained benign lesions. The AFB detected 72 of the 76 malignant lesions, but the WLB detected only 50. The sensitivities of the AFB and WLB were 94.7% and 65.8%, respectively, and the specificities were 57.0% and 83.6%, respectively. The negative predictive values of the AFB and WLB were 95.9% and 84.1%, respectively. 10Three-dimensional Culture of Human Airway Epithelium in Matrigel for Evaluation of Human Rhinovirus C and Bocavirus Infections显示文摘Objective Newly identified human rhinovirus C(HRV-C) and human bocavirus(HBoV) cannot propagate in vitro in traditional cell culture models; thus obtaining knowledge about these viruses and developing related vaccines are difficult. Therefore, it is necessary to develop a novel platform for the propagation of these types of viruses. Methods A platform for culturing human airway epithelia in a three-dimensional(3 D) pattern using Matrigel as scaffold was developed. The features of 3 D culture were identified by immunochemical staining and transmission electron microscopy. Nucleic acid levels of HRV-C and HBoV in 3 D cells at designated time points were quantitated by real-time polymerase chain reaction(PCR). Levels of cytokines, whose secretion was induced by the viruses, were measured by ELISA. Results Properties of bronchial-like tissues, such as the expression of biomarkers CK5, ZO-1, and PCK, and the development of cilium-like protuberances indicative of the human respiration tract, were observed in 3 D-cultured human airway epithelial(HAE) cultures, but not in monolayer-cultured cells. Nucleic acid levels of HRV-C and HBoV and levels of virus-induced cytokines were also measured using the 3 D culture system. 11The heterogeneity of islet autoantibodies and the progression of islet failure in type 1 diabetic patients显示文摘Type 1 diabetes mellitus is heterogeneous in many facets. The patients suffered from type 1 diabetes present several levels of islet function as well as variable number and type of islet-specific autoantibodies. This study was to investigate prevalence and heterogeneity of the islet autoantibodies and clinical phenotypes of type 1 diabetes mellitus; and also discussed the process of islet failure and its risk factors in Chinese type 1 diabetic patients. A total of 1,291 type 1 diabetic patients were enrolled in this study. Demographic information was collected. Laboratory tests including mixed-meal tolerance test, human leukocyte antigen alleles, hemoglobin A1 c, lipids, thyroid function and islet autoantibodies were conducted. The frequency of islet-specific autoantibody in newly diagnosed T1 DM patients(duration shorter than half year) was 73% in East China. According to binary logistic regressions, autoantibody positivity, longer duration and lower Body Mass Index were the risk factors of islet failure. As the disease developed, autoantibodies against glutamic acid decarboxylase declined as well as the other two autoantibodies against zinc transporter 8 and islet antigen 2. The decrease of autoantibodies was positively correlated with aggressive beta cell destruction. Autoantibodies can facilitate the identification of classic T1 DM from other subtypes and predict the progression of islet failure. 12Anatomical variation of infra-pyloric artery origination: A prospective multicenter observational study (IPA-Origin)显示文摘Objective: Infra-pyloric artery(IPA) is an important anatomical landmark in treatment of gastric cancer and is the key vessel for pylorus-preserving gastrectomy and subgroup of infra-pyloric lymph nodes. However, its anatomical variation is not thoroughly understood. Our study aimed to clarify the origination of the IPA.Methods: We did this prospective, multicenter, open-label, observational study at gastric surgery departments of34 hospitals in China. Gastric cancer patients aged 18 years or older and scheduled to undergo elective total or distal gastrectomy were assigned. During the surgery, IPA dissecting and exposing the origination point with photographs or video clips were required. The primary outcome was the origination of the IPA. Analysis of variance, χ~2 tests and Fisher's tests were used to analyze the differences between groups. The study is registered at Clinicaltrials.gov(No. NCT03071237).Results: Between May 8 and July 31, 2017, 429 patients were assigned for the study, and 419(97.7%) patients had the IPA dissected and recorded through photograph or video and were included in the primary outcome analysis. The median age was 62 years old, and 73.7% were male. Among the patients, 78.5% received laparoscopic surgery. Single IPA origination was identified in 398(95.0%) patients, including gastroduodenal artery(GDA) in154(36.8%) patients, anterior superior pancreaticoduodenal artery(ASPDA) in 130(31.0%) patients, and right gastroepiploic artery(RGEA) in 114(27.2%) patients. Fifteen(3.6%) patients were identified with multiple IPA and 6(1.4%) patients were identified as IPA absence. The differences in the distribution of surgical approach(P=0.003) and geographic area(P=0.030) were statistically significant. 13Is delayed gastric emptying so terrible after pylorus-preserving pancreaticoduodenectomy? Prevention and management显示文摘AIM: To explore some operative techniques to prevent the occurrence of delayed gastric emptying (DGE) after pylorus-preserving pancreaticoduodenectomy (PPPD). METHODS: One hundred and eighty-six patients in a single medical center who accepted PPPD were retrospectively studied. The incidence of DGE was investigated and the influence of some operative techniques on the prevention of DGE was analyzed. RESULTS: During the operative process of PPPD, the methods of detached drainage of pancreatic fluid and bile and gastric fistulization were used. Postoperatively, six patients suffered DGE among the 186 cases; the incidence was 3.23% (6/186). One of them was complicated with intraabdominal infection at the same time, and two with pancreatic leakage. CONCLUSION: Appropriate maneuvers during operation are essential to avoid postoperative DGE in PPPD. The occurrence of DGE is avoidable. 15Changes in soil carbon sequestration and soil respiration following afforestation on paddy fields in north subtropical China显示文摘Aims Although many studies have reported net gains of soil organic carbon(SOC)after afforestation on croplands,this is uncertain for Chinese paddy rice croplands.Here,we aimed to evaluate the effects of affores-tation of paddy rice croplands on SOC sequestration and soil respiration(R).Such knowledge would improve our understanding of the efctive-ness of various land use options on greenhouse gas mitigation in China.Methods The investigation was conducted on the Chongming Island,north subtropical China.Field sites were reclaimed from coastal salt marshes in the 1960s,and soils were homogeneous with simple land use:histories.SOC stocks and R,levels were monitored over one year in a paddy rice cropland,an evergreen and a deciduous broad-leaved plantation established on previous paddy fields and a reference fal-low land site never cultivated.Laboratory incubation of soil under fast-changing temperatures was used to compare the temperature sensitivity(Q10)of SOC decomposition across land uses.Important Findings After 15-20 years of afforestation on paddy fields,SOC concentra-tion only slightly increased at the depth of 0--5 cm but decreased in deeper layers,which resulted in a net loss of SOC stock in the top 40cm.Seasonal increase of sOC was observed during the rice-growing period in croplands but not in afforested soils,suggest-ing a stronger SOC sequestration by paddy rice cropping.However,SOC sequestered under cropping was more labile,as indicated by its higher contents of dissolved organic carbon and microbial bio-mass.Also,paddy soils had higher annual R,than afforested soils;R,abruptly increased after paddy fields were drained and plowed and remained distinctively high throughout the dry farming period.Laboratory incubation revealed that paddy soils had a much higher Q10 of SOC decomposition than afforested soils.Given that tem-perature was the primary controller of R,in this region,it was con-cluded that despite the stronger SOC sequestration by paddy rice cropping,its SOC was less stable than in afforested systems and might be more easily released into the atmosphere under global warming.Jun Cui Rongjuan Zhang Naishun Bu Huabin Zhang Boping Tang Zhaolei Li Lifeng Jiang Jiakuan Chen Changming Fang 2013Journal of Plant Ecology2013,6,3:4

17Sensitivity and vulnerability of water resources in the arid Shiyang River Basin of Northwest China显示文摘The sensitivity and vulnerability of water resources to climate change is difficult to assess. In this study, we used a conceptual hydrologic model to investigate the sensitivity of streamflow to climate change. We also proposed a framework to evaluate the vulnerability of water resources in arid regions. We applied this framework to a case study of the Shiyang River Basin in Northwest China. Results showed that the precipitation and streamflow in Shiyang River Basin exhibited no significant trends of change from 1956 to 2010. In the past five decades, however, the temperature increased significantly by 0.37°C per decade. According to the sensitivity assessment, a 10% increase in precipitation and a 1°C increase in temperature altered mean annual streamflow by averages of 14.6% and –0.5%, respectively, from 1988 to 2005. In the 2000s, the calculated vulnerability of water resources in Shiyang River Basin was more than 0.95, indicating severe vulnerability. The increase in the amount of precipitation and the implementation of water-saving measures can reduce the vulnerability of water resources in the future; if precipitation increases by 10% per decade and the use of irrigation water decreases by 15% in the 2030s, the evaluated value of water resources vulnerability will be reduced to 0.79. However, the region remains highly vulnerable. 18Anti-BCMA CAR T-cell therapy CT103A in relapsed or refractory AQP4-IgG seropositive neuromyelitis optica spectrum disorders:phase 1 trial interim results显示文摘Chimeric antigen receptor(CAR)T-cell therapy that targets B-cell maturation antigen(BCMA)have great potentials in autoimmune diseases and could be novel therapeutics for relapsed/refractory neuromyelitis optica spectrum disorder(NMOSD).To evaluate the safety and efficacy of the CT103A,a self-developed BCMA-targeting CAR construct against BCMA,in patients with AQP4-IgG seropositive NMOSD,an ongoing,investigator-initiated,open-label,single-arm,phase 1 clinical trial is conducted at our center.In total,12 patients were administered with a CAR-BCMA infusion.Ten of the 12 patients dosed were women(83.3%),with a median age of 49.5 years(range,30-67).were The most common events of grade 3 or higher were hematologic toxic effects.Seven patients(58%)developed infections,but no grade 4 infections occurred.Cytokine release syndrome was reported in all patients with only events of grade 1 or 2 observed.During the follow-up of a median 5.5 months,11 patients had no relapse;all patients generally reported improvement in disabilities and quality-of-life outcomes;11 patients’AQP-4 antibodies in serum showed a downward trend by the cutoff date.CAR T-cell expansion was associated with responses,and persisted more than 6 months post-infusion in 17%of the patients.In summary,CAR T-cell therapy shows a manageable safety profile and therapeutic potentials for patients with relapsed/refractory AQP4-IgG seropositive NMOSD.Another expansion phase is currently underway to determine the safety and efficacy of CAR T-BCMA infusion in patients with other neuro-inflammatory diseases.Chuan Qin Dai-Shi Tian Luo-Qi Zhou Ke Shang Liang Huang Ming-Hao Dong Yun-Fan You Jun Xiao Ying Xiong Wen Wang Hao Pang Jing-Jing Guo Song-Bai Cai Di Wang Chun-Rui Li Min Zhang Bi-Tao Bu Wei Wang 2023Signal Transduction and Targeted Therapy2023,8,2:4

20Emerging Organic Contaminants in Chinese Surface Water:Identification of Priority Pollutants显示文摘The occurrence and impacts of emerging organic contaminants(EOCs)in the aquatic environment have gained widespread attention over the past two decades.Due to large number of potential contaminants,monitoring campaigns,treatment plants,and proposed regulations should preferentially focus on specific pollutants with the highest potential for ecological and human health effects.In the present study,a multi-criteria screening approach based on hazard and exposure potentials was developed for prioritization of 405 unregulated EOCs already present in Chinese surface water.Hazard potential,exposure potential,and risk quotients for ecological and human health effects were quantitatively analyzed and used to screen contaminants.The hazard potential was defined by contaminant persistence,bioaccumulation,ecotoxicity,and human health effects;similarly,the exposure potential was a function of contaminant concentration and detection frequency.In total,123 compounds passed the preselection process,which involved a priority index equal to the normalized hazard potential multiplied by the normalized exposure potential.Based on the prioritization scheme,11 compounds were identified as top-priority,and 37 chemicals were defined as high-priority.The results obtained by the priority index were compared with four other prioritization schemes based on exposure potential,hazard potential,or risk quotients for ecological effects or human health.The priority index effectively captured and integrated the results from the more simplistic prioritization schemes.Based on identified data gaps,four uncertainty categories were classified to recommend:①regular monitoring,derivation of environmental quality standards,and development of control strategies;②increased monitoring;③fortified hazard assessment;and④increased efforts to collect occurrence and toxicity data.Overall,20 pollutants were recommended as priority EOCs.The prioritized list of contaminants provides the necessary information for authoritative regulations to monitor,control,evaluate,and manage the risks of environmentally-relevant EOCs in Chinese surface water.Mengmeng Zhong Tielong Wang Wenxing Zhao Jun Huang Bin Wang Lee Blaney Qingwei Bu Gang Yu 2022Engineering2022,8,4:3
