| 1 | Clostridium difficile and inflammatory bowel disease: Role in pathogenesis and implications in treatment显示文摘Clostridium difficile(C.difficile)is the leading cause of antibiotic associated colitis and nosocomial diarrhea.Patients with inflammatory bowel disease(IBD)are at increased risk of developing C.difficile infection(CDI),have worse outcomes of CDI-including higher rates of colectomy and death,and experience higher rates of recurrence.However,it is still not clear whether C.difficile is a cause of IBD or a consequence of the inflammatory state in the intestinal environment.The burden of CDI has increased dramatically over the past decade,with severe outbreaks described in many countries,which have been attributed to a new and more virulent strain.A parallel rise in the incidence of CDI has been noted in patients with IBD.IBD patients with CDI tend be younger,have less prior antibiotic exposure,and most cases of CDI in these patients represent outpatient acquired infections.The clinical presentation of CDI in these patients can be unique-including diversion colitis,enteritis and pouchitis,and typical findings on colonoscopy are often absent.Due to the high prevalence of CDI in patients hospitalized with an IBD exacerbation,and the prognostic implications of CDI in these patients,it is recommended to test all IBD patients hospitalized with a disease flare for C.difficile.Treatment includes general measures such as supportive care and infection control measures.Antibiotic therapy with either oral metronidazole,vancomycin,or the novel antibiotic-fidaxomicin,should be initiated as soon as possible.Fecal macrobiota transplantation constitutes another optional treatment for severe/recurrent CDI.The aim of this paper is to review recent data on CDI in IBD:role in pathogenesis,diagnostic methods,optional treatments,and outcomes of these patients. | Orna Nitzan Mazen Elias Bibiana Chazan Raul Raz Walid Saliba | 2013 | World Journal of Gastroenterology2013,19,43: | 12 |
| 2 | Advantages and disadvantages of biodegradable platforms in drug eluting stents显示文摘Coronary angioplasty with drug-eluting stent(DES)implantation is currently the most common stent procedure worldwide.Since the introduction of DES,coronary restenosis as well as the incidence of target vessel and target lesion revascularization have been significantly reduced.However,the incidence of very late stent thrombosis beyond the first year after stent deployment has more commonly been linked to DES than to baremetal stent(BMS)implantation.Several factors have been associated with very late stent thrombosis after DES implantation,such as delayed healing,inflammation,stent mal-apposition and endothelial dysfunction. | 3 | Evaluation of biostimulation and Tween 80 addition for the bioremediation of long-term DDT-contaminated soil显示文摘The bioremediation of a long-term contaminated soil through biostimulation and surfactant addition was evaluated. The concentrations of 1,1,1-trichloro-2,2-bis(4-chlorophenyl)ethane(DDT) and its metabolites 1,1-dichloro-2,2-bis(4-chlorophenyl) ethane(DDD) and1,1-dichloro-2,2-bis(4-chlorophenyl) ethylene(DDE) were monitored during an 8-week remediation process. Physicochemical characterization of the treated soil was performed before and after the bioremediation process. The isolation and identification of predominant microorganisms during the remediation process were also carried out. The efficiency of detoxification was evaluated after each bioremediation protocol. Humidity and p H and the heterotrophic microorganism count were monitored weekly. The DDT concentration was reduced by 79% after 8 weeks via biostimulation with surfactant addition(B + S) and 94.3%via biostimulation alone(B). Likewise, the concentrations of the metabolites DDE and DDD were reduced to levels below the quantification limits. The microorganisms isolated during bioremediation were identified as Bacillus thuringiensis, Flavobacterium sp., Cuprivadius sp.,Variovorax soli, Phenylobacterium sp. and Lysobacter sp., among others. Analysis with scanning electron microscopy(SEM) allowed visualization of the colonization patterns of soil particles. The toxicity of the soil before and after bioremediation was evaluated using Vibrio fischeri as a bioluminescent sensor. | 7 | Effects of underground explosions on soil and structures显示文摘Much effort has been dedicated to the study of underground explosions because they pose a major threat to people and structures below or above the ground.In this regard,it is especially important to model the propagation of blast waves in soil and their effects on structures.The main phenomena caused by underground explosive detonation that must be addressed are crater or camouflet formation,shock wave and elastic–plastic wave propagation in soil,and soil-structure interaction.These phenomena can be numerically simulated using hydrocodes,but much care must be taken to obtain reliable results.The objective of this study is to analyze the ability of a hydrocode and simple soil models that do not require much calibration to approximately reproduce experimental and empirical results related to different buried blast events and to provide general guidelines for the simulation of this type of phenomena.In this regard,crater formation,soil ejecta,blast wave propagation in soil,and their effects on structures below and above the ground are numerically simulated using different soil models and parameters;the results are analyzed.The properties of soil have a significant effect on structures,the ejecta,and the propagation of shock waves in soil.Thus,the model of the soil to study these phenomena must be carefully selected.However their effect on the diameter of a crater is insignificant. | Daniel Ambrosini Bibiana Luccioni | 2020 | Underground Space2020,5,4: | 2 |
