12018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 3A Comparative Analysis of Environmental Quality Assessment Methods for Heavy Metal-Contaminated Soils显示文摘Four assessment methods(two pollution indexes and two fuzzy mathematical models) were employed to investigate the environmental quality of four soils around a ferroalloy plant in Nanjing City.Environmental quality was assessed as class IV(moderately polluted) for each soil with single-factor index method,and was identified to be classes IV,III(slightly polluted),III,and III for soils A,B,C,and D,respectively,with the comprehensive index model.In comparison with the single-factor index method,the comprehensive index model concerned both dominant parameter and average contribution of all factors to the integrated environmental quality.Using the two fuzzy mathematical methods(single-factor deciding and weighted average models),the environmental risks were determined to be classes IV,III,II(clean),and II for soils A,B,C,and D,respectively.However,divergence of the membership degree to each pollution class still occurred between the two methods.In fuzzy mathematical methods,membership functions were used to describe the limits between different pollution degrees,and different weights were allocated for the factors according to pollution contribution.Introduction of membership degree and weight of each factor to fuzzy mathematical models made the methods more reasonable in the field of environmental risk assessment.LI Wei-Xin ZHANG Xu-Xiang WU Bing SUN Shi-Lei CHEN Yan-Song PAN Wen-Yang ZHAO Da-Yong CHENG Shu-Pei 2008Pedosphere2008,18,3:29

7Percutaneous ethanol injection, radiofrequency and their combination in treatment of hepatocellular carcinoma显示文摘AIM: To evaluate the therapeutic effect and the indication of percutaneous ethanol injection (PEI),radiofrequency ablation (RFA) and their combination in treatment of hepatocellular carcinoma (HCC).METHODS: Two hundred and fifty-five patients with HCC received treatment of PEI, RFA or their combination.Group1 (< 3 cm in diameter, n=85) was treated with PEI,group2 (< 3 cm in diameter, n=153) with RFA. Group3(>3 cm in diameter, n=86) was divided into two groups.Group 3a (n=34) was treated with RFA, while group 3b(n=52) was treated with RFA for 2 wk after transcatheter arterial chemoembolization or PEI. Contrast-enhanced sonography was performed for 61 patients before and after RFA. Liver function and serum alpha-fetoprotein (AFP) were measured for all patients. Changes of the lesions on ultrasound and contrast-enhanced CT/MRI were evaluated for assessing the therapeutic responses.The 1-, 2-, 3- and 5-year survival rates were recorded after treatment.RESULTS: In group 1, the complete necrosis rate of lesions after 1 mo was 77.6% (66/85). The level of AFP declined conspicuously after 1 mo. The 1-, 2-, 3- and 5-year survival rate after treatment was 80.0% (52/65),60.4% (32/53), 52.5% (21/40) and 33.3% (7/21),respectively. In group 2, the complete necrosis rate of lesions after 1 moh was 92.2% (141/153). The level of AFP decreased conspicuously after 1 mo. The 1-, 2-,3- and 5-year survival rate after treatment was 94.6%(88/93), 73.2% (52/71), 63.5% (33/52) and 46.4%(13/28), respectively. In group 3a, the complete necrosis rate of lesions after 1 mo was 23.5% (8/34). AFP dropped down to the normal level in only one patient after 1 mo. The 1-, 2- and 3-year survival rate after treatment was 47.6% (10/21), 42.9% (6/14) and 27.3%(3/11), respectively. Only one patient was still alive after 5 years. In group 3b, the complete necrosis rate of lesions after 1 mo was 57.7% (30/52). The level of AFP decreased after 1 mo. The 1-, 2-, 3- and 5-year survival rate after treatment was 68.6% (24/35), 46.2% (12/26),36.8% (7/19) and 27.3% (3/11), respectively.CONCLUSION: The therapeutic effect of RFA on small HCC is better than that of PEI. Small HCC is the optimal indication of RFA. 9Review of single-camera stereo-digital image correlation techniques for full-field 3D shape and deformation measurement显示文摘Single-camera stereo-digital image correlation(stereo-DIC) techniques have gained increasing attentions and demonstrated excellent prospects in the experimental mechanics community owing to their prominent advantages of cost-effectiveness,compactness, and the avoidance of the complicated camera synchronization. Using additional optical devices, e.g. a diffraction grating, a bi-prism or a set of planar mirrors, pseudo stereo images of a test sample surface can be recorded with a single camera.By correlating these stereo images using DIC, full-field three-dimensional(3 D) shape and deformation can be retrieved. This review comprehensively summarizes the historical development, methodologies, strengths and weaknesses of the diffraction grating-based, prism-based, four-mirror-adaptor-based single-camera stereo-DIC techniques, and the recently proposed novel full-frame single color camera-based stereo-DIC technique for full-field 3 D shape and deformation measurement. The optical arrangements, principles and calibration procedures of these single-camera stereo-DIC techniques are described in detail. 12A nano-cocrystal strategy to improve the dissolution rate and oral bioavailability of baicalein显示文摘Baicalein(BE) is one of the main active flavonoids representing the variety of pharmacological effects including anticancer, anti-inflammatory and cardiovascular protective activities, but it's very low solubility, dissolution rate and poor oral absorption limit the therapeutic applications. In this work, a nano-cocrystal strategy was successfully applied to improve the dissolution rate and bioavailability of BE. Baicalein-nicotinamide(BE-NCT) nanococrystals were prepared by high pressure homogenization and evaluated both in vitro and in vivo. Physical characterization results including scanning electron microscopy, dynamic light scattering, powder X-ray diffraction and differential scanning calorimetry demonstrated that BE-NCT nano-cocrystals were changed into amorphous state with mean particle size of 251.53 nm. In the dissolution test, the BE-NCT nano-cocrystals performed 2.17-fold and 2.54-fold enhancement than BE coarse powder in FaSSIF-V2 and FaSSGF. Upon oral administration, the integrated AUC0-t of BE-NCT nano-cocrystals(6.02-fold) was significantly higher than BE coarse powder(1-fold), BE-NCT cocrystals(2.87-fold) and BE nanocrystals(3.32-fold). 13Effect of caffeic acid phenethyl ester on proliferation and apoptosis of hepatic stellate cells in vitro显示文摘AIM: To investigate the role of nuclear factor-κB (NF-κB)inhibitor caffeic acid phenethy1 ester (CAPE) in the proliferation, collagen synthesis and apoptosis of hepatic stellate cells (HSCs) of rats. METHODS: The HSCs from rats were isolated and cultured in Dulbecco's Modified Eagle's Medium (DMEM) and treated with CAPE. The proliferation and collagen synthesis of HSCs were determined by 3H-TdR and 3H-proline incorporation respectively, and the expression of type Ⅰ, Ⅲ procollagen genes was further explored byin situ hybridization. Apoptosis cell indices (AIs) were examined using terminal deoxynucleotidyl transferase- mediated DIG-dUTP nick end labeling (TUNEL). RESULTS: Tn activated HSC in culture, CAPE significantly inhibited 3H-TdR and 3H-proline incorporation by HSCs at concentrations of 5 μmol/L and 10 μmol/L respectively. CAPE also reduced the type I procollagen gene expression (P<0.05)at higher concentration. Apoptosis of HSC was induced by CAPE and the AIs were time-and dose-dependently increased from 2.82+0.73 % to 7.66±1.25 % at 12 h (P<0.01) and from 3.15±0.88 % to 10.6L±2.88 % at 24 h (P<0.01). 16Prioritizing natural-selection signals from the deep-sequencing genomic data suggests multi-variant adaptation in Tibetan highlanders显示文摘Human genetic adaptation to high altitudes(>2500 m)has been extensively studied over the last few years,but few functional adaptive genetic variants have been identified,largely owing to the lack of deep-genome sequencing data available to previous studies.Here,w e build a list of putative adaptive variants,including 63 missense,7 loss-of-function;1,298 evolutionarily conserved variants and 509 expression quantitative traits loci.Notably,the top signal of selection is located in TMEM 247,a transmembrane protein-coding gene.The Tibetan version of TMEM 247 harbors one high-frequency(76.3%)missense variant,rsl 16983452(c.248C>T;p.Ala83Val);with the T allele derived from archaic ancestry and carried by>94%of Tibetans but absent or in low frequencies(<3%)in non-Tibetan populations.The rsl 16983452-T is strongly and positively correlated with altitude and significantly associated with reduced hemoglobin concentration(p=5.78×10^-5),red blood cell count(p=5.72×10^-7)and hematocrit(p=2.57×10^-6).In particular,TMEM247-rs 116983452 shows greater effect size and better predicts the phenotypic outcome than any E P A S1 variants in association with adaptive traits in Tibetans.Modeling the interaction between TM£M247-rsl 16983452 and EPAS1 variants indicates weak but statistically significant epistatic effects.Our results support that multiple variants may jointly deliver the fitness of the Tibetans on the plateau,where a complex model is needed to elucidate the adaptive evolution mechanism.Lian Deng Chao Zhang Kai Yuan Yang Gao Yuwen Pan Xueling Ge Yaoxi He Yuan Yuan Yan Lu Xiaoxi Zhang Hao Chen Haiyi Lou Xiaoji Wang Dongsheng Lu Jiaojiao Liu Lei Tian Qidi Feng Asifullah Khan Yajun Yang Zi-Bing Jin Jian Yang Fan Lu Jia Qu Longli Kang Bing Su Shuhua Xu 2019National Science Review2019,6,6:9
17Experimental study of an insert and its influence on churning losses in a high-speed electro-hydrostatic actuator pump of an aircraft显示文摘The speed of an Electro-Hydrostatic Actuator(EHA) pump can recently reach 20000 r/min, and its churning losses increase obviously with an increasing speed, which results in low efficiency and thus increasing heat in aircraft EHA systems. In order to reduce churning losses at high speeds, more attention should be given to the design of an insert. In this paper, the effect of an insert with different design parameters on churning losses is investigated through Computational Fluid Dynamics(CFD) simulation and experiments by calculating the difference between churning losses torques of the test pump with and without the insert based on a high-speed churning losses test rig.Analytical results show that the gap between the insert and the cylinder is critical for churning losses reduction. It is found that the churning losses of the test pump can be reduced with a decreasing gap between the cylinder block and the insert at high speeds. This is because the insert can decrease the turbulence occurrence at high speeds. 18GCH1 plays a role in the high-altitude adaptation of Tibetans显示文摘Tibetans are well adapted to high-altitude hypoxia. Previous genome-wide scans have reported many candidate genes for this adaptation, but only a few have been studied. Here we report on a hypoxia gene(GCH1, GTP-cyclohydrolase I), involved in maintaining nitric oxide synthetase(NOS) function and normal blood pressure, that harbors many potentially adaptive variants in Tibetans. We resequenced an 80.8 kb fragment covering the entire gene region of GCH1 in 50 unrelated Tibetans. Combined with previously published data, we demonstrated many GCH1 variants showing deep divergence between highlander Tibetans and lowlander Han Chinese. Neutrality tests confirmed a signal of positive Darwinian selection on GCH1 in Tibetans. Moreover, association analysis indicated that the Tibetan version of GCH1 was significantly associated with multiple physiological traits in Tibetans, including blood nitric oxide concentration, blood oxygen saturation, and hemoglobin concentration. 20Strength and deformation behaviors of bedded rock mass under bolt reinforcement显示文摘The mechanism of bolt support is an important topic in mining engineering and slope treatment. The artificial material and loading system were self-developed to study the influence of bedding cohesion and bolt number on the anchoring behavior of bedded rock mass. The results show that, both peak strength and elasticity modulus increase gradually with the increase of bedding cohesion and bolt number. The axial stress–strain curve of bedded rock mass under the reinforcement of bolts presents the features of strain-softening and secondary strengthening. Finally, anchoring behavior of bedded rock mass with different bolt numbers was simulated by using FLAC3 D numerical program and the results were compared with the experimental results.
