1Differential diagnosis of left-sided abdominal pain:Primary epiploic appendagitis vs colonic diverticulitis显示文摘AIM:To investigate the clinical characteristics of left primary epiploic appendagitis and to compare them with those of left colonic diverticulitis.METHODS:We retrospectively reviewed the clinical records and radiologic images of the patients who presented with left-sided acute abdominal pain and had computer tomography(CT) performed at the time of presentation showing radiological signs of left primary epiploic appendagitis(PEA) or left acute colonic diverticulitis(ACD) between January 2001 and December 2011. A total of 53 consecutive patients were enrolled and evaluated. We also compared the clinical characteristics,laboratory findings,treatments,and clinical results of left PEA with those of left ACD.RESULTS:Twenty-eight patients and twenty-five patients were diagnosed with symptomatic left PEA and ACD,respectively. The patients with left PEA had focal abdominal tenderness on the left lower quadrant(82.1%). On CT examination,most(89.3%) of the patients with left PEA were found to have an oval fatty mass with a hyperattenuated ring sign. In cases of left ACD,the patients presented with a more diffuse abdominal tenderness throughout the left side(52.0%vs 14.3%; P = 0.003). The patients with left ACD had fever and rebound tenderness more often than those with left PEA(40.0% vs 7.1%,P = 0.004; 52.0% vs14.3%,P = 0.003,respectively). 2Correlation of fatty liver and abdominal fat distribution using a simple fat computed tomography protocol显示文摘AIM: To evaluate the relationship between hepatic fat infiltration and abdominal fat volume by using computed tomography (CT).METHODS: Three hundred and six patients who visited our obesity clinic between November 2007 and April 2008 underwent fat protocol CT scans.The age range of the patients was 19 to 79 years and the mean age was 49 years.The male to female ratio was 116:190.Liver and spleen attenuation measurements were taken with three regions of interests (ROIs) from the liver and two ROIs from the spleen.Hepatic attenuation indices (HAIs) were measured as follows: (1) hepatic parenchymal attenuation (CTLP);(2) liver to spleen attenuation ratio (LS ratio);and (3) difference between hepatic and splenic attenuation (LSdif).Abdominal fat volume was measured using a 3 mm slice CT scan starting at the level of the umbilicus and was automatically calculated by a workstation.Abdominal fat was classified into total fat (TF),visceral fat (VF),and subcutaneous fat (SF).We used a bivariate correlation method to assess the relationship between the three HAIs and TF,VF,and SF.RESULTS: There were significant negative correlations between CTLP,LS ratio,and LSdif with TF,VF,and SF,respectively.The CTLP showed a strong negative correlation with TF and VF (r = -0.415 and -0.434,respectively,P < 0.001).The correlation between CTLP and SF was less significant (r = -0.313,P < 0.001).CONCLUSION: Fatty infiltration of the liver was correlated with amount of abdominal fat and VF was more strongly associated with fatty liver than SF.Seonah Jang Chang Hee Lee Kyung Mook Choi Jongmee Lee Jae Woong Choi Kyeong Ah Kim Cheol Min Park 2011World Journal of Gastroenterology2011,17,28:5
3Effects of feeding a diet containing Gymnema sylvestre extract: attenuating progression of obesity in C57BL/6J mice显示文摘Objective: To investigate the effect of Gymnema sylvestre extract(GS) on initial anti-obesity, liver injury, and glucose homeostasis induced by a high-fat diet(HFD). Methods: The dry powder of GS was extracted with methanol, and gymnemic acid was identified by high performance liquid chromatography as deacyl gymnemic acid. Male C57BL/6J mice that fed on either a normal diet, normal diet containing 1 g/kg GS(CON+GS) HFD, or HFD containing 1.0 g/kg GS(HFD+GS) for 4 weeks were used to test the initial anti-obesity effect of GS. Body weight gain and food intake, and serum levels about lipid and liver injury markers were measured. Histopathology of adipose tissue and liver stained with hematoxylin and eosin(H&E) and oil-red O were analyzed. After 4 weeks of GS extract feeding, intraperitoneal glucose tolerance test(IPGTT) was performed. Results: The methanol extracts of GS exerted significant anti-obesity effects in HFD+GS group. They decreased body weight gain, a lower food and energy efficiency ratio, and showed lower serum levels of total cholesterol(TC), triglyceride(TG), low-density lipoprotein(LDL)-cholesterol, very-low density lipoprotein(VLDL)-cholesterol and leptin compared with the HFD group. The decreases of abdominal as well as epididymal fat weight and adipocyte hypertrophy, lipid droplets in liver, and serum levels of aspartate aminotransferase(AST) and alanine transaminase(ALT) were also observed. The CON+GS group showed an effect of glucose homeostasis compared to the CON group. 5A case of hypereosinophilic syndrome presenting with intractable gastric ulcers显示文摘We report a rare case of hypereosinophilic syndrome(HES)presenting with intractable gastric ulcers.A 71-year-old man was admitted with epigastric pain.Initial endoscopic findings revealed multiple,active gastric ulcers in the gastric antrum.He underwent Helicobacter pylori(H pylori)eradication therapy followed by proton pump inhibitor(PPI)therapy.However,follow-up endoscopy at 4,6,10 and 14 mo revealed persistent multiple gastric ulcers without significant improvement.The proportion of his eosinophil count increased to 43%(total count:7903/mm 3).Abdominal-pelvic and chest computed tomography scans showed multiple small nodules in the liver and both lungs.The endoscopic biopsy specimen taken from the gastric antrum revealed prominent eosinophilic infiltration,and the liver biopsy specimen also showed eosinophilic infiltration in the portal tract and sinusoid.A bone marrow biopsy disclosed eosinophilic hyperplasia as well as increased cellularity of 70%.The patient was finally diagnosed with HES involving the stomach,liver,lung,and bone marrow.When gastric ulcers do not improve despite H pylori eradication and prolonged PPI therapy,infiltrative gastric disorders such as HES should be considered.Tae Young Park Chang Hwan Choi Suh Yoon Yang InSoo Oh In-Do Song Hyun Woong Lee Hyung Joon Kim Jae Hyuk Do Sae Kyung Chang Ah Ra Cho Young Joo Cha 2009World Journal of Gastroenterology2009,15,48:2
