| 1 | Differential diagnosis of left-sided abdominal pain:Primary epiploic appendagitis vs colonic diverticulitis显示文摘AIM:To investigate the clinical characteristics of left primary epiploic appendagitis and to compare them with those of left colonic diverticulitis.METHODS:We retrospectively reviewed the clinical records and radiologic images of the patients who presented with left-sided acute abdominal pain and had computer tomography(CT) performed at the time of presentation showing radiological signs of left primary epiploic appendagitis(PEA) or left acute colonic diverticulitis(ACD) between January 2001 and December 2011. A total of 53 consecutive patients were enrolled and evaluated. We also compared the clinical characteristics,laboratory findings,treatments,and clinical results of left PEA with those of left ACD.RESULTS:Twenty-eight patients and twenty-five patients were diagnosed with symptomatic left PEA and ACD,respectively. The patients with left PEA had focal abdominal tenderness on the left lower quadrant(82.1%). On CT examination,most(89.3%) of the patients with left PEA were found to have an oval fatty mass with a hyperattenuated ring sign. In cases of left ACD,the patients presented with a more diffuse abdominal tenderness throughout the left side(52.0%vs 14.3%; P = 0.003). The patients with left ACD had fever and rebound tenderness more often than those with left PEA(40.0% vs 7.1%,P = 0.004; 52.0% vs14.3%,P = 0.003,respectively). | 2 | MRI of magnetically labeled mesenchymal stem cells in hepatic failure model显示文摘AIM:To track intravascularly transplanted mesenchymal stem cells (MSCs) labeled with superparamagnetic iron oxide (SPIO) by using magnetic resonance imaging (MRI) in an experimental rabbit model of hepatic failure.METHODS:Human MSCs labeled with FDA-approved SPIO particles (Feridex) were transplanted via the mes-enteric vein into rabbits (n=16) with carbon tetrachloride-induced hepatic failure.Magnetic resonance (MR) examinations were performed with a 3.0 T clinical scanner immediately before and 2 h and 1,3,and 7 d after transplantation.Signal intensity (SI) changes on T2weighted MRI were measured,and correlation between MR findings and histomorphologic findings was also investigated.RESULTS:SI on T2-weighted MRI decreased significantly in the liver 2 h after injection of human MSCs and returned gradually to the levels found before injection in 7 d.Changes in SI in the liver at 2 h,1,3,and 7 d were 41.87% ± 9.63%,10.42% ± 4.3%,5.12% ± 1.9%,3.75% ± 1.2%,respectively (P < 0.001).Histologic analyses confirmed the presence of MSCs in the liver,localized mainly in the sinusoids in early period (2 h and 1 d) and concentrated to the border zone in late period (3 and 7 d).The number of iron-positive cells in the liver at 2 h and on 1,3 and 7 d after transplantation was 29.2 ± 4.8,10.1 ± 3.7,6.7 ± 2.2,and 5.8 ± 2.1,respectively (P=0.013).CONCLUSION:Intravascularly injected SPIO-labeled MSCs in an experimental rabbit model of hepatic failure can be detected and followed with MRI. | Kyu Ri Son Se Young Chung Hyo-Cheol Kim Hoe Suk Kim Seung Hong Choi Jeong Min Lee Woo Kyung Moon | 2010 | World Journal of Gastroenterology2010，16，44： | 4 |

| 10 | Ileal polypoid lymphangiectasia bleeding diagnosed and treated by double balloon enteroscopy显示文摘Intestinal lymphangiectasia is a rare disease characterized by focal or diffuse dilated enteric lymphatics with impaired lymph drainage.It causes protein-losing enteropathy and may lead to gastrointestinal bleeding.Commonly,lymphangiectasia presents as whitish spots or specks.To our knowledge,small bowel bleeding resulting from polypoid intestinal lymphangiectasia has not been reported.Here,we report a rare case of active bleeding from the small bowel caused by polypoid lymphangiectasia with a review of the relevant literature.An 80-year-old woman was hospitalized for melena.Esophagogastroduodenoscopy could not identify the source of bleeding.Subsequent colonoscopy showed fresh bloody material gushing from the small bowel.An abdominal-pelvic contrast-enhanced computed tomography scan did not reveal any abnormal findings.Video capsule endoscopy showed evidence of active and recent bleeding in the ileum.To localize the bleeding site,we performed double balloon enteroscopy by the anal approach.A small,bleeding,polypoid lesion was found in the distal ileum and was successfully removed using endoscopic snare electrocautery. | Min Seon Park Beom Jae Lee Dae Hoe Gu Jeung-Hui Pyo Kyeong Jin Kim Yun Ho Lee Moon Kyung Joo Jong-Jae Park Jae Seon Kim Young-Tae Bak | 2013 | World Journal of Gastroenterology2013，19，45： | 1 |

| 16 | Procedure-related musculoskeletal symptoms in gastrointestinal endoscopists in Korea显示文摘AIM: To determine the prevalence and risk factors of work-related musculoskeletal disorders in gastrointestinal endoscopists in Korea. METHODS: A survey of musculoskeletal symptoms, using a self-administered questionnaire, was conducted on 55 endoscopists practicing in general hospitals or health promotion centers. RESULTS: Forty-nine (89.1%) endoscopists reported musculoskeletal pain on at least one anatomic location and 37 (67.3%) endoscopists complained of pain at rest. Twenty-six (47.3%) endoscopists had severe musculoskeletal pain defined as a visual analogue score greater than 5.5. Factors related to the development of severe pain were (1) standing position during upper endoscopy, (2) specific posture/ habit during endoscopic procedures, and (3) multiple symptomatic areas. Finger pain was more common in beginners, whereas shoulder pain was more common in experienced endoscopists. Sixteen percent of symptomatic endoscopists have modified their practice or reduced the number of endoscopic examinations. Only a few symptomatic endoscopists had sought professional consultation with related specialists. CONCLUSION: The prevalence of musculoskeletal pain in endoscopists is very high. The location of pain was different between beginners and experienced endoscopists.
