2Indirect traumatic optic neuropathy显示文摘Indirect traumatic optic neuropathy(ITON) refers to optic nerve injury resulting from impact remote to the optic nerve. The mechanism of injury is not understood, and there are no confirmed protocols for prevention, mitigation or treatment. Most data concerning this condition comes from case series of civilian patients suffering blunt injury, such as from sports- or motor vehicle-related concussion, rather than military-related ballistic or blast damage. 4Intravascular large B-cell lymphoma presenting with altered mental status: A case report显示文摘BACKGROUND Intravascular large B-cell lymphoma(IVLBCL)is a rare and aggressive subtype of non-Hodgkin lymphoma with a varied presentation and no pathognomonic findings.Early diagnosis is critical to altering the disease course as early treatment with chemoimmunotherapy is required to prevent a rapidly fatal outcome.Strategies including improved awareness of this clinical entity through publication of cases with unique presentations are essential to prompt consideration of IVLBCL early in the disease workup.Here,we present a case of IVLBCL presenting with altered mental status and systemic organ dysfunction.CASE SUMMARY A 61-year-old male patient presented with flu-like symptoms and a high fever.He experienced rapid clinical deterioration with liver,kidney failure,and shock despite rapid antibiotic administration and supportive care.A broad infectious workup was negative.Intracranial imaging revealed nonspecific changes to the corpus callosum suspicious for vasculitis.Renal biopsy was non-diagnostic.After further progression of his symptoms,the family elected to withdraw care and the patient died shortly thereafter.Post-mortem analysis revealed clear multi-organ involvement by IVLBCL,prompting re-examination of the ante-mortem renal biopsy that also identified IVLBCL involvement.CONCLUSION IVLBCL is a rare disease.Communication with specialties and early biopsy is critical to establishing the diagnosis and initiating therapy.Christopher Robert D’Angelo Kimberly Ku Jessica Gulliver Julie Chang 2019World Journal of Clinical Oncology2019,10,12:2

15Disrupted Maturation of Prefrontal Layer 5 Neuronal Circuits in an Alzheimer’s Mouse Model of Amyloid Deposition显示文摘Mutations in genes encoding amyloid precursor protein(APP)and presenilins(PSs)cause familial forms of Alzheimer’s disease(AD),a neurodegenerative disorder strongly associated with aging.It is currently unknown whether and how AD risks affect early brain development,and to what extent subtle synaptic pathology may occur prior to overt hallmark AD pathology.Transgenic mutant APP/PS1 over-expression mouse lines are key tools for studying the molecular mechanisms of AD pathogenesis.Among these lines,the 5XFAD mice rapidly develop key features of AD pathology and have proven utility in studying amyloid plaque formation and amyloidβ(Aβ)-induced neurodegeneration.We reasoned that transgenic mutant APP/PS1 over-expression in 5XFAD mice may lead to neurodevelopmental defects in early cortical neurons,and performed detailed synaptic physiological characterization of layer 5(L5)neurons from the prefrontal cortex(PFC)of 5XFAD and wild-type littermate controls.L5 PFC neurons from 5XFAD mice show early APP/Aβimmunolabeling.Whole-cell patch-clamp recording at an early post-weaning age(P22–30)revealed functional impairments;although 5XFAD PFC-L5 neurons exhibited similar membrane properties,they were intrinsically less excitable.In addition,these neurons received smaller amplitude and frequency of miniature excitatory synaptic inputs.These functional disturbances were further corroborated by decreased dendritic spine density and spine head volumes that indicated impaired synapse maturation.Slice biotinylation followed by Western blot analysis of PFC-L5 tissue revealed that 5XFAD mice showed reduced synaptic AMPA receptor subunit GluA1 and decreased synaptic NMDA receptor subunit GluN2A.Consistent with this,patch-clamp recording of the evoked L23>L5 synaptic responses revealed a reduced AMPA/NMDA receptor current ratio,and an increased level of AMPAR-lacking silent synapses.These results suggest that transgenic mutant forms of APP/PS1 overexpression in 5XFAD mice leads to early developmental defects of cortical circuits,which could contribute to the age-dependent synaptic pathology and neurodegeneration later in life.Chang Chen Jing Wei Xiaokuang Ma Baomei Xia Neha Shakir Jessica K.Zhang Le Zhang Yuehua Cui Deveroux Ferguson Shenfeng Qiu Feng Bai 2023Neuroscience Bulletin2023,39,6:0
16Helper T Cell(CD4^(+))Targeted Tacrolimus Delivery Mediates Precise Suppression of Allogeneic Humoral Immunity显示文摘Antibody-mediated rejection(ABMR)is a major cause of dysfunction and loss of transplanted kidney.The current treatments for ABMR involve nonspecific inhibition and clearance of T/B cells or plasma cells.However,the prognosis of patients following current treatment is poor.T follicular helper cells(Tfh)play an important role in allograft-specific antibodies secreting plasma cell(PC)development.Tfh cells are therefore considered to be important therapeutic targets for the treatment of antibody hypersecretion disorders,such as transplant rejection and autoimmune diseases.Tacrolimus(Tac),the primary immunosuppressant,prevents rejection by reducing T cell activation.However,its administration should be closely monitored to avoid serious side effects.In this study,we investigated whether Tac delivery to helper T(CD4^(+))cells using functionalized mesoporous nanoparticles can block Tfh cell differentiation after alloantigen exposure.Results showed that Tac delivery ameliorated humoral rejection injury in rodent kidney graft by suppressing Tfh cell development,PC,and donor-specific antibody(DSA)generation without causing severe side effects compared with delivery through the drug administration pathway.This study provides a promising therapeutic strategy for preventing humoral rejection in solid organ transplantation.The specific and controllable drug delivery avoids multiple disorder risks and side effects observed in currently used clinical approaches.Jia Shen Chang Liu Pengpeng Yan Meifang Wang Luying Guo Shuaihui Liu Jianghua Chen Jessica MRosenholm Hongfeng Huang Rending Wang Hongbo Zhang 2022Research2022,,3:0
