| 2 | Development of an educational cartoon to prevent worm infections in Chinese schoolchildren显示文摘Background:With more than two billion people infected worldwide,soil-transmitted helminths(STH)are the most widespread infections.To date,STH control efforts rely predominantly on recurrent mass drug administration(MDA),which does not prevent reinfection.Additional public health measures including novel health educational tools are required for more sustained integrated control of STH.We describe the development of an educational cartoon video(The Magic Glasses)targeting STH infections in Chinese schoolchildren and its pilot testing in China.We applied an extensive community-based mixed methods approach involving input from the target group of 9–10 year old schoolchildren and key informants,such as teachers,doctors and parents,in order to identify potential STH infection risks in the study area and to formulate key messages for the cartoon.The development of the educational cartoon included three major steps:formative research,production,and pilot testing and revision.Results:We found that most adults and approximately 50%of the schoolchildren were aware of roundworm(Ascaris)infection,but knowledge of transmission,prevention and treatment of STH was poor.Observations in the study area showed that unhygienic food practices,such as eating raw and unwashed fruit or playing in vegetable gardens previously fertilised with human faeces,posed major STH infection risks.Conclusions:It was crucial to assess the intellectual,emotional,social and cultural background of the target population prior to video production in order to integrate the key messages of the cartoon into everyday situations.Overall,our strategy for the development of the cartoon and its incorporation into a health education package proved successful,and we provide a summary of recommendations for the development of future educational videos based on our experiences in China. | Franziska A Bieri Li-Ping Yuan Yue-Sheng Li Yong-Kang He Andrew Bedford Robert S Li Feng-Ying Guo Sheng-Ming Li Gail M Williams Donald P McManus Giovanna Raso Darren J Gray | 2013 | Infectious Diseases of Poverty2013，2，1： | 2 |

| 4 | Outcomes of submucosal(T1b) esophageal adenocarcinomas removed by endoscopic mucosal resection显示文摘AIM To investigate the outcomes and recurrences of p T1 b esophageal adenocarcinoma(EAC) following endoscopic mucosal resection(EMR) and associated treatments.METHODS Patients undergoing EMR with pathologically confirmed T1 b EAC at two academic referral centers were retrospectively identified.Patients were divided into 4 groups based on treatment following EMR:Endoscopic therapy alone(group A),endoscopic therapy with either chemotherapy,radiation or both(group B),surgicalresection(group C) or no further treatment/lost to follow-up(<12 mo)(group D).Pathology specimens were reviewed by a central pathologist.Follow-up data was obtained from the academic centers,primary care physicians and/or referring physicians.Univariate analysis was performed to identify factors predicting recurrence of EAC.RESULTS Fifty-three patients with T1 b EAC underwent EMR,of which 32(60%) had adequate follow-up ≥ 12 mo(median 34 mo,range 12-103).There were 16 patients in group A,9 in group B,7 in group C and 21 in group D.Median follow-up in groups A to C was 34 mo(range 12-103).Recurrent EAC developed overall in 9 patients(28%) including 6(38%) in group A(median:21 mo,range:6-73),1(11%) in group B(median:30 mo,range:30-30) and 2(29%) in group C(median 21 mo,range:7-35.Six of 9 recurrences were local;of the 6 recurrences,5 were treated with endoscopy alone.No predictors of recurrence of EAC were identified.CONCLUSION Endoscopic therapy of T1 b EAC may be a reasonable strategy for a subset of patients including those either refusing or medically unfit for esophagectomy. | Darren D Ballard Neel Choksi Jingmei Lin Eun-Young Choi B Joseph Elmunzer Henry Appelman Douglas K Rex Hala Fatima William Kessler John M DeWitt | 2016 | World Journal of Gastrointestinal Endoscopy2016，8，20： | 2 |

| 17 | Outcomes following liver transplantation in intensive care unit patients显示文摘AIM:To determine feasibility of liver transplantation in patients from the intensive care unit (ICU) by estimating graft and patient survival.METHODS:This single center retrospective study included 39 patients who had their first liver transplant directly from the intensive care unit and 927 non-ICU patients who were transplanted from hospital ward or home between January 2005 and December 2010.RESULTS:In comparison to non-ICU patients,ICU patients had a higher model for end-stage liver disease (MELD) at transplant (median:37 vs 20,P < 0.001).Fourteen out of 39 patients (36%) required vasopressor support immediately prior to liver transplantation (LT) with 6 patients (15%) requiring both vasopressin and norepinephrine.Sixteen ICU patients (41%) were ventilator dependent immediately prior to LT with 9 patients undergoing percutaneous tracheostomy prior to transplantation.Twenty-five ICU patients (64%) required dialysis preoperatively.At 1,3 and 5 years after LT,graft survival was 76%,68% and 62% in ICU patients vs 90%,81% and 75% in non-ICU patients.Patient survival at 1,3 and 5 years after LT was 78%,70% and 65% in ICU patients vs 94%,85% and 79% in non-ICU patients.When formally comparing graft survival and patient survival between ICU and nonICU patients using Cox proportional hazards regression models,both graft survival [relative risk (RR):1.94,95%CI:1.09-3.48,P=0.026] and patient survival (RR:2.32,95%CI:1.26-4.27,P=0.007) were lower in ICU patients vs non-ICU patients in single variable analysis.These findings were consistent in multivariable analysis.Although not statistically significant,graft survival was worse in both patients with cryptogenic cirrhosis (RR:3.29,P=0.056) and patients who received donor after cardiac death (DCD) grafts (RR:3.38,P=0.060).These findings reached statistical significance when considering patient survival,which was worse for patients with cryptogenic cirrhosis (RR:3.97,P=0.031) and patients who were transplanted with DCD livers (RR:4.19,P=0.033).Graft survival and patient survival were not significantly worse for patients on mechanical ventilation (RR:0.91,P=0.88 in graft loss;RR:0.69,P=0.56 in death) or patients on vasopressors (RR:1.06,P=0.93 in graft loss;RR:1.24,P=0.74 in death) immediately prior to LT.Trends toward lower graft survival and patient survival were observed for patients on dialysis immediately before LT,however these findings did not approach statistical significance (RR:1.70,P=0.43 in graft loss;RR:1.46,P=0.58 in death).CONCLUSION:Although ICU patients when compared to non-ICU patients have lower survivals,outcomes are still acceptable.Pre-transplant ventilation,hemodialysis,and vasopressors were not associated with adverse outcomes. | Lena Sibulesky Michael G Heckman C Burcin Taner Juan M Canabal Nancy N Diehl Dana K Perry Darren L Willingham Surakit Pungpapong Barry G Rosser David J Kramer Justin H Nguyen | 2013 | World Journal of Hepatology2013，5，1： | 1 |
